Multiple myeloma is one of the most common haematologic malignancies. Currently there are numerous studies looking for new prognostic markers in multiple myeloma. The most important of them are the markers related to proliferative activity of neoplastic cells or to size of tumor mass. The subject of this paper are the results obtained from investigation of some such laboratory markers in a group of patients with monoclonal gammopathies diagnosed at our department in the last 3 years. We analyzed blood and bone marrow samples from 51 patients with new diagnosed monoclonal gammopathies, 14 of them were patients with monoclonal gammopathy of undetermined significance and 37 patients had multiple myeloma. 17 patients with multiple myeloma were treated by high-dose chemotherapy regimen. We assessed significance of selected laboratory markers for differential diagnosis of monoclonal gammopathies and for monitoring of activity of multiple myeloma. Among the investigated parameters, we verified the significance of cell cycle analysis of bone marrow plasmatic population and of the determination of the number of circulating myeloma cells in differential diagnosis of monoclonal gammopathies. In our opinion, the determination of soluble CD138, beta 2-microglobulin and neopterin serum levels can be also recommended as helpful markers for a solution of this problem. Except of beta 2-microglobulin serum level we did not find statistically significant correlation with activity of multiple myeloma in any of the investigated parameters.

Immunological characterization of semen and male fertility are discussed. The pH of seminal plasma is very similar in groups of fertile and infertile men. Nonspecific immunological factors such as lysozyme, C3c, alpha-1 antitrypsin and beta-2 microglobulin were significantly higher in 14% of infertile men. Seminal antibodies agglutinating spermatozoa are frequently observed in IgG and IgA isotypes. High levels of seminal anti-phospholipid antibodies in seminal plasma could influence epitopes on the endometrium, oocyte or early embryo in patients with repeated unexplained spontaneous miscarriages.

Immunosensors for the detection of human beta 2-microglobulin (B2M) were prepared by immobilisation of covalently crosslinked assemblies containing various numbers of molecular layers of monoclonal antibody against B2M (anti-B2M) on the surface of a Ta2O5 grating coupler sensor. The immobilisation procedure consisted of repeated successive adsorption of anti-B2M and dextran sulfate (DS) followed by glutaraldehyde (GA) crosslinking of anti-B2M and washing out DS. The flexibility of the resulting anti-B2M networks was evaluated from the sensor response to the reversible expansion and contraction of the networks induced by changing pH of the ambient solution. A decreased GA concentration and the use of a higher-molecular-mass DS increased the network flexibility. The sensor sensitivity to B2M increased with increasing flexibility of the antibody networks and with increasing number of anti-B2M molecular layers, indicating that B2M can penetrate inside the antibody network.

BACKGROUND: Some authors found in Paget's disease bone and in osteoporosis elevated serum levels of beta-2-microglobulin. The objective of the present work was to assess whether in patients with a different activity of Paget's disease of bone the serum values of beta-2-microglobulin before treatment and after 1,6 and 12 month's treatment with pamidronate can serve as a marker of bone metabolism and whether pamidronate treatment has an impact on the serum concentration of beta-2-microglobulin. METHODS AND RESULTS: 35 patients with Paget's disease of bone (25 men and 10 women, mean age 64 and 72 years resp.) were divided into three groups with a different activity of the disease based on the values with a different activity of the disease based on the values of the bone isoenzyme of alkaline phosphatase in serum. In all serum concentrations of beta-2- microglobulin were examined before and during treatment with pamidronate. The control group was formed by five healthy men and five healthy women, mean age 65 years, and a special group of weight drug addicts dependent on opiates (5 men and 3 women, mean age 26 years). For assessment of beta-2-microglobulin in serum the enzymic immunoassay and fluorescence detection were employed, using Abbott, IMX, MEIA system kits. The serum concentrations of the bone isoenzyme of alkaline phosphatase were assessed by the method used at the Third Medical Clinic of the First Medical Faculty Charles University Prague (8). For statistical processing the non-parametric Friedman test was used to assess the significance of differences between means (p < 0.01) and simultaneous testing at the 5% level of significance. Serum concentrations of beta-2-microglobulin in patients with Paget's disease of bone with a low activity before treatment and after 1, 6 and 12 months of pamidronate treatment were 2.02 +/- 0.9, 2.10 +/- 1.2, 1.9 +/- 1.1 and 1.98 +/- 1.4 ng/l. In patients with a medium activity of the disease the values were 1.80 +/- 0.5, 1.78 +/- 1.02, 2.2 +/- 1.1 and 1.50 +/- 0.7 ng/l. The values in patients with a high activity of the disease were 1.9 +/- 0.9, 2.02 +/- 1.2, 2.40 +/- 0.8 and 2.45 +/- 1.01 ng/l. The values of beta2-microglobulin did not differ when compared with normal values (p < 0,1). CONCLUSIONS: In patients with Paget's disease bone regardless of the different activity of the disease before treatment or after short-term or prolonged pamidronate treatment no differences were revealed in the serum levels of beta-2-microglobulin. Pamidronate treatment did not influence the concentration of beta2-microglobulin and assessment of beta2-microglobulin in Paget's disease bone is no asset.

Sjögren's syndrome is a chronic inflammatory disease which is believed to be immunologically mediated. It is marked by inflammation and destruction of the salivary glands and lacrimal glands. We have found significantly higher serum levels of sICAM-1 and beta-2-microglobulin in both primary Sjögren's syndrome patients (429.1 +/- 114.33 ng/ml, 4.4 +/- 1.5 ug/ml resp.) and secondary Sjögren's syndrome patients (424 +/- 140.73 ng/ml, 4.4 +/- 2.67 ug/ml resp.) in comparison with healthy controls (189.1 +/- 42.34 ng/ml, 1.3 +/- 0.38 ug/ml resp.). The serum level of beta-2-microglobulin was significantly correlated with the serum level of sICAM-1 in secondary Sjögren's syndrome patients' group.

BACKGROUND: Infrequent studies, having been published so far, have brought few, partly contradictory, results on the importance of soluble receptor of Interleukin-6 (sIL-6R) in serum in patients with multiple myeloma (MM). The aim of the study was to find out, if there are applicable relations between the levels of sIL-6R and the values of selected clinical and biochemical indices and the levels of some cytokines in serum. METHODS AND RESULTS: The authors analyzed a cohort of 50 patients suffering from MM (age median 64 years, range 34-83 years, ratio of men versus women 1.2:1.0). The soluble IL-6R and other cytokines were examined by ELISA assay, clinical stages of the disease were evaluated according to Durie-Salmon and Bataille. Increased values of sIL-6R (> 90 ng/ml) were in 28 (56%) of patients, median value was 87.4 (range 28.3-220 ng/l). Correlation analysis (Pearson test, p < 0.05) proved a statistically significant relation between the levels of sIL-6R and the values of beta 2-microglobulin (S-B2M) and thymidine kinase (S-TK) in serum. There was no relation to the erythrocyte sedimentation velocity, haemoglobin levels, calcium, urea, creatinine, lactate dehydrogenase, albumin, monoclonal immunoglobulin, C-reactive protein, ferritin, IL-1, IL-2, sIL-2R, IL-6, TNF-alfa, and the occurrence of myeloma plasmocytes in bone marrow. No differences were found between the patients with normal values of S-B2M (< 3 ng/ml) and plasmocytes in bone marrow (< 5%) in comparison with the groups of patients with increased values (3-6, > 6 ng/ml and > 20%, respectively). There was no statistically significant relation of the levels of sIL-6R to clinical stages of MM (1-3) and the degree of activity of the disease. CONCLUSIONS: Increased values of sIL-6R, detected in more than a half of the cohort, were, with the exception of S-B2M, S-TK and massive infiltration of bone marrow by myeloma plasmocytes, in no applicable relation to selected biochemical indices, to the level of some cytokines and to the degree of advancement and activity of the disease.

The early diagnosis of renal allograft dysfunction in the immediate posttransplantation period is very important. The prospective study was undertaken in 60 patients after kidney transplantation. The significance of blood and urine levels of glomerular and tubular proteins was studied for early diagnosis of allograft dysfunction. The authors evaluate some glomerular (CRP, transferrin, immunoglobulins) and tubular (alfa 1 microglobulin and beta 2 microglobulin) proteins for differential diagnosis of acute tubular necrosis and acute rejection of transplanted kidney.

The authors elaborated a method of radial immunodiffusion for assessment of beta 2-microglobulin serum and cerebrospinal fluid levels in patients. The method is based on a modified procedure of staining and decolouration. The antibody produced by USOL Co. which was used for the purpose was not modified. Comparison of 90 sera and 27 cerebrospinal fluid samples where also the reference method ELISA Pharmacia was used revealed almost absolute agreement, the correlation coefficient was 0.98. The method is used for clinical monitoring in patients with myelomas and renal insufficiencies.

The aim of this crossover clinical study was to gain basic information on the hemocompatibility and effectiveness of recently developed high-flux membranes made of cuprammonium rayon with ultrafiltration coefficients of 10, 17, and 19 ml/mm Hg/h (S12W, SU12W, and SS12W dialyzers, respectively), and to identify any possible differences from a conventional membrane made of the same material with an ultrafiltration coefficient of 6 ml/mm Hg/h (C12W dialyzer). All the tested membranes led to an abrupt drop in leukocyte count in the initial phase of hemodialysis. In high-flux membranes, C5a anaphylatoxin would pass into the dialysate, but mean C5a anaphylatoxin concentrations in the dialysate were lower by orders of magnitude than its plasma concentrations, which behaved, in high- and low-flux membranes alike, typically of those made of nonsubstituted cellulose with no intermembrane differences. As judged by the concentrations of the thrombin-antithrombin III complex, the coagulation system was activated--again, without differences between membranes. The reduction rates for urea, creatinine, and phosphates were comparable for all the tested membranes. Compared with baseline, the post-dialysis serum concentrations of beta 2-microglobulin in high-flux membranes, unlike the low-flux membrane, were significantly lower. We conclude that there are no significant differences between the tested high- and low-flux membranes made of cuprammonium rayon in the monitored hemocompatibility parameters, and that high-flux membranes are capable of reducing serum beta 2-microglobulin concentrations.

In a series of 2038 paraproteinemic sera, 9 IgD paraproteins (0.44%) were found. Clinical diagnosis of all the 9 was multiple myeloma. One patient had an IgD-lambda plasmacytoma which developed into plasmocellular leukemia. Our series of IgD paraproteins consisted of 6 females and 3 males with a mean age of 58.5 years. The mean concentrations of IgD paraproteins were 6.7 +/- 4.26 g/l and the mean proteinuria was 5.1 +/- 4.76 g/24 h. Eight IgD paraproteins had light chains lambda and one had light chains kappa.
