Cleft lip and cleft palate are among the most common congenital defects of the head. The treatment of clefts is centralized, multidisciplinary, and involves a plastic surgeon, orthodontist, anesthesiologist, clinical speech therapist, and other specialists. While the incidence of cleft lip and cleft palate remains approximately unchanged, the approach to their treatment is evolving. Modern treatment methods increasingly include 3D printing and the associated therapeutic and educational possibilities.

Bilateral complete cleft lip and palate (BCLP) is the most severe of the common orofacial clefts and is associated with the greatest deformity during development. The aim of this study was to use geometric morphometrics to evaluate palatal shape and size variability in patients with BCLP in comparison to nonclefted Czech boys. The variability of palatal size and shape in BCLP patients was greater when compared with the nonclefted population. Though palate variability in BCLP was wide, nonclefted palatal shapes were generally different (a high, wide and vaulted palate) and fell almost outside the range of BCLP variability. The palatal size and shape of BCLP patients (range from 12.1 to 16.5 years) was not correlated with age. A comparison of the mean shapes of the clefted and nonclefted groups showed that the BCLP palate is flatter and narrower. The most notable size difference was found in the area between the maxilla and premaxilla. This phenomenon is associated with the persisting separation of the premaxilla from the rest of the palate. The shape of the palatal configuration of the premaxilla and adjacent area was concave in the nonclefted group and convex in BCLP patients.

This five-year prospective study demonstrates prosthetic treatment by multidisciplinary therapy: surgeon, orthodontist, and prosthodontist. 10 patients volunteered for the study (a group of 7 men and 3 women at an average age of 33.2 years). 10 obturators, 49 fixed dentures were inserted to the upper jaw. Based on ADA (American Dental Association) recommendation a special card was prepared containing relevant information on the patients. Clinical assessments were carried out in accordance with the US Public Health Service System. In a 5-year period only 50.0% of restorations were excellent, receiving 100% alpha rating. The marginal ridge contour and adaptation of obturator achieved 60.0% alpha rating. The anatomic form of dental arch was destroyed in 50.0%. The presence of caries was not detected. Six teeth were extracted due to periodontal disease. The general contour of the restoration followed the overall contour of the fixed denture in 95.9%. Plaque accumulation was found in 50%. The colour match of crowns was darker and translucent in 27.7%, but discoloration of removable denture was seen in 30.0%. The three case reports demonstrate the long-term stability of treatment (from alpha to charlie evaluation). Attachment retention, fixed and removable denture with metal base are the first method of choice, due to acceptable long-term stability.

The multidisciplinary care of cleft defects in the Prague Department of Plastic Surgery has a long tradition. The authors describe the team care of patients including the timing of the surgery as well as the conservative therapy.

The Department of Plastic Surgery, Prague, provides comprehensive care to children and families where cleft lip and palate occur, which goes under the name of preventive family regimen. Apart from the treatment alone, this regimen is the concern of a number of specialists whose aim is to eliminate the parents' sense of "guilt", and to try, with the aid of the family, to create a situation which would improve the condition for the child's socialization. The geneticist is in charge of the prenatal preventive regimen which consists in the recommendation for the dated conception in an attempt to influence the baby's sex prior to conception, in eliminating potential internal and infectious (TORCH) etiological factors as well as potential teratogenic agents within the family. Gynecological care is aimed at uterine hypoplasia, atypia, gynaecological inflammation and hormonal deviations. Two months prior to conception and in the 1st trimester, it is recommended to keep to a diet rich in proteins, mineral substances, trace elements, vitamins and folic acid. The authors point to the potential risks of this method (impaired prenatal selection, increased fertility of risk families). At present, it is not possible to make an objective assessment of the results of the preventive pre-conception regimen due to the unrepresentative selection of the group under observation.
