A group of 118 patients, aged 1-73 years, suffering from giardiasis in their gastrointestinal tract was studied in the period of 1988-1990. The analysis consisted in determination of frequency, sex distribution, age categories and incidence within the period of 12 months. Patients older than 15 years were asked about the possible sources of infection, subjective and objective symptoms and therapy. The information obtained for our region were compared with data reported in literature.

The relationship between giardiasis and ABO blood groups was studied in a group of 136 children from Eastern Slovakia, with a special attention to ethnic specifics of gipsy children. The study showed no statistically significant differences in the distribution of ABO blood groups between the subgroup of Giardia-infected gipsy children and the gipsy population minority living in the area. On the other hand, the ABO distribution pattern in subgroup of non-gipsy children infected with Giardia intestinalis was found to differ from that in the general non-gipsy Eastern Slovakian population by the absence of blood group AB among children with giardiasis. There was indicated a tendency towards an increased frequency of blood group B in subgroup of Giardia-infected gipsy children and towards group A in subgroup of nongipsy children with giardiasis, but the differences were not statistically significant.

Here, we report a case of direct zoonotic transmission of giardiasis between a pet chinchilla and a human. Microscopic and molecular examinations of stool samples from a child and samples of chinchilla droppings revealed cysts/DNA of Giardia intestinalis. The transmission from the chinchilla to the child has been confirmed as coprophagous after the 1-year-old toddler ingested pet chinchilla droppings. Molecular analysis of the gdh gene from both hosts classified the G. intestinalis cysts into the assemblage B genetic group, which has been previously shown to be characteristic of both human and chinchilla giardiasis. Both Giardia sub-assemblages BIII and BIV were present in the chinchilla droppings, whereas only the sub-assemblage BIV was isolated from the child's stool sample. To the best of our knowledge, this is the first report of a true zoonotic transmission of giardiasis, supporting the zoonotic potential of assemblage B.

Fifteen Mongolian gerbils were inoculated with 10 × 106 viable trophozoites of Giardia intestinalis. Their faeces were examined daily by flotation method and the number of shed cysts was counted. Two animals (male and female) were euthanised at 4- to 5-day intervals (9, 14, 18 days post-infection (DPI)). The remaining nine gerbils were sacrificed and dissected at the end of the experiment (23 DPI). Their small intestinal tissues were processed for examination using histological sectioning and scanning electron microscopy and their complete blood count (CBC) was examined. The highest number of trophozoites at the total was observed in the duodenum in gerbils sacrificed on 14 DPI. Number of shed cysts was positively correlated with number of trophozoites rinsed from the intestine. Infected gerbils had lower body weight gain in comparison with control group and in three male gerbils; diarrhoea occurred during infection. Cyst shedding was negatively correlated with values of mean corpuscular haemoglobin concentration. Females showed another pattern in cyst shedding than males. This information needs to be taken into account while planning the experiments.

Mean nitrate content in the morning urine of 117 patients with giardiasis comprises 155.3 mg/l, which exceeds about 4 fold the normal value. Maximum concentrations of 1000 mg/l were observed. Under treatment with metronidazol (Vagimid) nitrate content at first increased and then restored to normal. Endogenously synthesized nitrate with giardiasis 3-4 fold exceeds the daily nitrate intake by food and, therefore, plays an important role in total nitrate load of the patients.

The clinical, pathological and parasitological features of giardiasis resulting from experimental inoculation with 3 x 10(6) Giardia cysts were studied in goat kids. All experimentally inoculated goat kids given Giardia cysts became infected. Three of the eight inoculated kids had decreased appetite, formless feces and become slightly depressed beginning 7 or 8 days post inoculation. The mean duration of the appearance of abnormal feces was 6 days. Irregular and intermitted cysts shedding started after prepatent periods of 6-10 days and lasted throughout this study (10 weeks). The evidence of high infectivity and fast transmission of Giardia were observed under standard zoohygienic conditions. The characteristics of intestinal lesions were similar to those found in other hosts infected with Giardia. The most severe lesions were seen in the duodenum and proximal jejunum, and consisted of moderate villus atrophy, villus blunting, crypt hyperplasia and inflammatory infiltration in the lamina propria. Scanning electron microscopy revealed ultrastructural alterations in the microvillus border of enterocytes. Mucosal smears and histological sections of the gall bladder displayed Giardia trophozoites and gall bladder epithelium hyperplasia together with bile ductular proliferation in the liver tissue in two kids.

The authors hospitalized and treated in 1990-1994 at their Clinic 127 patients where giardiasis was the main or secondary diagnosis. They wanted to draw attention to the increasing prevalence of the disease, to clinical manifestations and problems associated with treatment. Clinicians should consider the possible presence of this disease in different gastrointestinal manifestations, chronic hepatitis, hepatopathies as well as in acute IgM anti-HAV, HBsAg and HCV negative hepatitis.

In subjects with positive giardiasis tests examinations of contacts were made not only in all members of the family but also in dogs and cats living with them in the same household. Attention was paid to the possible transmission of giardias. A total of 174 families were examined, i.e. 1140 subjects incl. 156 positive ones. Of 123 examined dogs 9 were positive and of 56 cats 8 were positive. Forty-three dog keepers were negative, of their 222 dogs 60 were positive. Although no association was proved between the infection of domestic animals and humans in the examined group, it is recommended to treat not only humans but also domestic animals.
