Until now, few cases of coincidental giardiasis and pancreatic tumors have been described. Among these cases, three described giardiasis cases coincided with confirmed pancreatic cancer. We present another case of Giardia infection coexisting with pancreatic cancer in a 67-year-old man who suffered from stenosis of the distal ductus choledochus combined with a hypoechoic mass in the head of the pancreas. The diagnostic conclusion of suspicious adenocarcinoma was based on endoscopic ultrasound fine-needle aspiration (EUS-FNA) biopsy and confirmed by a partial duodenopancreatectomy. On bloody cytology smears prepared from the EUS-FNA specimen, trophozoites of Giardia intestinalis accompanying an inflammatory background and features that fulfilled the morphological criteria of malignancy were observed. In histological sections from the duodenopancreatectomy specimens, the parasites were observed attached to the epithelium, but individual Giardia parasites were also observed beneath the epithelial lining. According to conventional genotyping, the infecting Giardia belonged to sub-assemblage AII.

Fifteen Mongolian gerbils were inoculated with 10 × 106 viable trophozoites of Giardia intestinalis. Their faeces were examined daily by flotation method and the number of shed cysts was counted. Two animals (male and female) were euthanised at 4- to 5-day intervals (9, 14, 18 days post-infection (DPI)). The remaining nine gerbils were sacrificed and dissected at the end of the experiment (23 DPI). Their small intestinal tissues were processed for examination using histological sectioning and scanning electron microscopy and their complete blood count (CBC) was examined. The highest number of trophozoites at the total was observed in the duodenum in gerbils sacrificed on 14 DPI. Number of shed cysts was positively correlated with number of trophozoites rinsed from the intestine. Infected gerbils had lower body weight gain in comparison with control group and in three male gerbils; diarrhoea occurred during infection. Cyst shedding was negatively correlated with values of mean corpuscular haemoglobin concentration. Females showed another pattern in cyst shedding than males. This information needs to be taken into account while planning the experiments.

Endogenous development of a pure strain of Eimeria media and of a precocious line derived from this strain was studied in specific pathogen-free (SPF) rabbits. Endogenous development of the parental strain comprised three generations and the gamogony began 76 h post-inoculation (pi). Two types of meronts were observed in each generation. The type A meronts gave rise to large, polynucleated merozoites present in low numbers. Multiplication was carried out by endomerogony. Within type B meronts, merozoites arose from ectomerogony. These were slender and more numerous than those of type A. The sporozoite refractile body was divided and distributed into the first and second generation merozoites but not into the third. The endogenous development of the precocious line was similar to that of the parental strain except that no refractile body was observed and the last merogony was absent. Gamogony appeared 60 h pi.

An infection with Septata intestinalis was diagnosed in a 35-year-old AIDS patient without diarrhoea. The diagnosis was based on morphological examinations of a duodenal biopsy specimen. Serum antibodies were detected reacting with spores of Encephalitozoon cuniculi. Spores of S. intestinalis and E. cuniculi stained with Brown Hopps Gram stain showed a red colour (Gram negative) and not a blue/black colour which was described for microsporidian spores in tissue.

Adult females of Strongyloides robustus Chandler, 1942, a parasite of sciurids in North America, were found in the duodenal mucosa of 30 of 32 red squirrels (Tamiasciurus hudsonicus (Erxleben)) collected in Cape Breton Island, Nova Scotia, Canada. The parasitic female is illustrated and redescribed; characteristics include: body 3.8-8.0 mm long, cephalic extremity with X-shaped mouth and 8 circumoral lobes, ovaries spiralling around intestine, and tail bluntly rounded. Eggs in fresh feces contained tadpole-stage larvae. In fecal cultures, eggs hatched and larvae invariably developed to the filariform infective third stage; i.e. a free-living generation did not occur and is probably absent in S. robustus in Cape Breton and possibly other parts of North America. It is hypothesized that homogonically developing S. robustus might be more fecund or more efficiently transmitted than species of Strongyloides that exhibit both homogonic and heterogonic development. Larvae of S. robustus in fecal cultures, i.e. homogonic larvae, are described in detail. Intestinal walls of second- and third-stage larvae, as well as the lateral chords of young third-stage larvae, contained numerous round bodies, likely nutrient stores. Third-stage larvae were present within 2 days in cultures maintained at 30 degrees C, 4 days at 20 degrees C, and 7 days at 15 degrees C. They lived for at least 33 and 30 days at 15 degrees C and 20 degrees C, respectively. Third-stage larvae probably die when their nutrient stores are exhausted.

A protocol for the handling of small intestinal biopsies from HIV-infected patients is presented. This protocol includes the Warthin-Starry stain for the detection of microsporidia. This stain has proved a reliable and sensitive diagnostic technique for microsporidial infections as it stains both Enterocytozoon bieneusi and Septata intestinalis in duodenal enterocytes. Because the stain demonstrates Septata intestinalis in lamina propria macrophages as well as enterocytes, it allows for the practical differentiation of these two microsporidial infections. The Warthin-Starry stain has also demonstrated Septata intestinalis in nasal and colonic biopsies in some of these patients. Since the completion of an earlier study, a further 40 cases of Enterocytozoon bieneusi and three cases of Septata intestinalis have been diagnosed in just over 240 consecutive duodenal biopsies from HIV positive patients presenting with diarrhoea and other gastrointestinal complaints. Other opportunistic infections include cytomegalovirus in four cases, mycobacteria in eight cases, cryptosporidia in nine cases, giardia in four cases and Isospora belli in one case. Since the ratio of these opportunistic infections has remained much the same as in the previous study of 180 consecutive duodenal biopsies, we suggest that these rates may reflect the actual prevalence of microsporidial infections in AIDS patients in Sydney, Australia.
It was confirmed, after experimental infection of 24 weaned pigs with different doses (200,000 and 4 mil.) of Eimeria debliecki oocysts that the developmental cycle of E. debliecki occurs in the anterior jejunum and after a high dose of oocysts also in the duodenum and anterior parts of the middle jejunum. Pathological changes characterized by a light atrophy of the villi, scarcely dispersed minute erosions of the epithelium in upper parts of the mucosa and an inflammatory response in the propria of the anterior jejunum were found in the area of the largest occurrence of developmental stages of E. debliecki (from 50 cm to 100 cm from the pylorus). An inflammatory infiltrate in the propria of the anterior jejunum from 3 DPI to 5 DPI contained a conspicuously large number of plasma cells with Russell bodies. Cellular changes were detected only in enterocytes with developmental stages of E. debliecki. Changes of the microvillous zone were observed in infected enterocytes; dilated mitochondria, free ribosomes and an increased number of residual bodies were found in the cytoplasm of enterocytes. The degree of the cellular changes of enterocytes was dependent on the maturity of the developmental stage of E. debliecki. Based on pathological changes observed by the use of light and electron microscopy, the coccidium E. debliecki is considered to be pathogenic for weaned pigs in spite it does not provoke a clinical infection.

In a gnotobiotic piglet infected with 50.10(6) oocysts of Cryptosporidium parvum, developmental stages of the parasite were found in the duodenum, anterior jejunum and middle jejunum on the first day post infection (1 DPI). During the studies of the ultrastructure, trophozoites and meronts of C. parvum were encountered in the microvillous zone of enterocytes, and the outer membrane of parasites was a continuation of the membrane of microvilli. Unusual was the attachment of C. parvum trophozoite on the opening of a goblet cell, as well as a meront localized in the cytoplasm in the enterocyte of anterior jejunum. These findings show that C. parvum is very adaptive and capable of developing in a still undescribed location.

Compared was the detection of Giardia intestinalis in duodenal contents and in the stool. During endoscopic examination duodenal contents were recovered by means of a tube attached to the mucosa and by aspiration from the region of pars verticalis. In 102 persons, duodenal and stool examinations revealed four and five cases of Giardia infection, respectively. The results showed that both diagnostic methods are equivalent and demonstrated that parasitologic examinations of patients suffering from dyspeptic disorders are very useful.
