BACKGROUND: Fatigue after breast cancer treatment affects one in four women. Feeling tired affects womens daily activities, coping with the disease and quality of life. After active treatment, women want to return to their working lives, where fatigue is a barrier. Despite the fact of fatigue being such a common consequence of the treatment, it seems to be often overlooked. MATERIALS AND METHODS: This article is a substudy of the research focused on the complex experience of women with breast cancer. In this substudy, we focus on breast cancer patients perceptions of fatigue and its impact on daily life. A qualitative methodology was used to collect the data. The interviews were structured into two consecutive parts. The first, narrative part was followed by a semi-structured interview. The research population was recruited to meet the conditions of maximum variation sample (different regions of the country, age categories, education, stage of the disease, and treatment modality). Fifty-three interviews were collected and transcribed verbatim. The data from which the categories were created were analysed using NVivo10 software and were open coded. The codes were then grouped into themes for thematic analysis. RESULTS: Most women experienced fatigue as a consequence of the disease. Fatigue often forced them to change their lifestyle. The women perceived great limitations when returning to work. They did not feel ready to return to work and often postponed it or considered changing their working hours or a type of job. CONCLUSION: Fatigue seems to be an underestimated consequence of the treatment in breast cancer patients. It often leads to a forced change in womens lifestyle, which has a negative impact on their mental health. Methods to monitor fatigue and early interventions are needed to improve womens quality of life after the treatment.

The aim was to study the time-of-day variations of cortisol, fatigue and sleep disturbances in sound engineers in relation to job task and shift work. The concentration of saliva cortisol and feeling of stress, sleepiness and fatigue were followed at three hour intervals in 21 sound engineers: 13 sound engineers, aged 45.1 +/- 7.3 years, broadcasting in a life show during fast forward rotating shifts and 8 sound engineers, aged 47.1 +/- 9.8 years, making records in a studio during fast rotating day shifts. Cortisol concentration was assessed in saliva with radioimmunological kits. The participants reported for stress symptoms during the shifts and filled sleep diary. The data were analyzed by tests of between-subjects effects (SPSS). A trend for higher cortisol was found with the group broadcasting in a life show. The sound engineers broadcasting in a life show reported higher scores of stress, sleepiness and fatigue, but no significant differences concerning the sleep disturbances between the groups were found. In conclusion our data show moderate level of stress and fatigue with the studied sound engineers, higher with the subjects broadcasting in a life show. The quality of sleep showed no significant differences between the studied groups, an indication that the sound engineers were able to tolerate the fast forward rotating shifts.

The fatigue properties of thermo-mechanically treated and machined aluminum alloy 7475-T7351 have been studied. The applied advanced machining strategy induced intensive plastic deformation on the machined surface under defined cutting conditions. Therefore, a detailed study of 3D surface topography was performed. Advanced characterization of the material structure and electron back scattered diffraction mapping of selected chemical phases were performed, as well as energy dispersive X-ray analysis of the surface. Advanced mechanical properties of the material were investigated in situ with a scanning electron microscope that was equipped with a unique tensile fixture. The fatigue results confirmed an evident dispersion of the data, but the mechanism of crack nucleation was established. Fracture surface analysis showed that the cracks nucleated at the brittle secondary particles dispersed in the material matrix. The surface topography of samples that had been machined in wide range of cutting/deformation conditions by milling has not proved to be a decisive factor in terms of the fatigue behavior. The incoherent interface and decohesion between the alumina matrix and the brittle secondary phases proved to significantly affect the ultimate strength of the material. Tool engagement also affected the fatigue resistance of the material.

PURPOSE: To synthesize the body of knowledge on the factors influencing the quality of life (QoL) after ischemic stroke (IS) in young adults. METHODS: Guidelines regarding the scoping review methodology developed by the Joanna Briggs Institute, and the PRISMA-ScR checklist for a scoping review was used in this paper. A total of 1197 studies were identified through a bibliographic search in Web of Science, MEDLINE, PsycInfo, ScienceDirect, Scopus, and ProQuest Science Database. Articles published between the years 2000-2021 were included. RESULTS: A total of nine papers were finally selected to respond to the research question. Three studies were prospective longitudinal studies compared QoL between young stroke and age-matched controls from the general population. Across all the analysed studies, 14 variables potentially associated with QoL were identified. QoL in young patients is mainly affected by clinical outcomes after IS (scored by the modified Rankin scale and the Barthel index-favourable initial functional status and higher independence in ADL leads to higher QoL) and psychological factors (post-stroke fatigue and depression-higher levels of fatigue and depression lead to lower QoL). The reviewed studies emphasized the importance of functional outcomes, post-stroke depression, fatigue and anxiety and early return to work. CONCLUSION: Further longitudinal studies are needed to identify the trajectory of post-stroke psychosocial symptoms over time and other potential predictors of unfavourable long-term QoL, thus specific young stroke rehabilitation and stroke self-management support programmes should be developed (address physical, psychological factors which influence the psychosocial adaptation post-stroke and the perception of the QoL).

This research is aimed at evaluating the effect of low-cycle fatigue on a newly designed hybrid sandwich ski structure to determine the changes that may occur due to cyclic loading and thus affect its use. This is primarily concerned with the fatigue behavior of the tested ski over different time intervals simulating its seasonal use and its effect on the mechanical properties of the ski, i.e., the durability and integrity of the individual layers of the sandwich ski structure. The ski was subjected to 70,000 deflections by moving the crossbar by 60 mm according to the ski deflection calculation in the arch. The results of the cyclic tests of the engineered ski design showed no significant changes in the ski during loading. The average force required to achieve deflection in the first 10,000 cycles was 514.0 ± 4.2 N. Thereafter, a secondary hardening of the structure occurred during relaxation and the force required increased slightly to 543.6 ± 1.7 N. The required force fluctuated slightly during the measurements and in the last series the value was 540.4 ± 0.8 N. Low-cycle fatigue did not have a significant effect on the mechanical properties of the ski; there was no change in shape or visual delamination of the individual layers of the structure. From the cross-section, local delamination was demonstrated by image analysis, especially between the Wood core and the composite layers E-Glass biaxial and Carbon triaxial.

BACKGROUND: Persisting cancer-related fatigue impairs health-related quality of life (HRQoL) and social reintegration in patients with Hodgkin's lymphoma (HL). The GHSG HD18 trial established treatment de-escalation for advanced-stage HL guided by positron emission tomography after two cycles (PET-2) as new standard. Here, we investigate the impact of treatment de-escalation on long-term HRQoL, time to recovery from fatigue (TTR-F), and time to return to work (TTR-W). PATIENTS AND METHODS: Patients received European Organisation for Research and Treatment of Cancer Quality of Life Questionnaire C30 (EORTC QLQ-C30) and life situation questionnaires at baseline, interim, end of treatment, and yearly follow-up. TTR-F was defined as time from the end of chemotherapy until the first fatigue score <30. TTR-W was analyzed in previously working or studying patients and measured from the end of treatment until the first documented work or education. We compared duration of treatment on TTR-F and TTR-W using Cox proportional hazards regression adjusted for confounding variables. RESULTS: HRQoL questionnaires at baseline were available in 1632 (83.9%) of all randomized patients. Overall, higher baseline fatigue and age were significantly associated with longer TTR-F and TTR-W and male sex with shorter TTR-W. Treatment reduction from eight to four chemotherapy cycles led to a significantly shorter TTR-F [hazard ratio (HR) 1.41, P = 0.008] and descriptively shorter TTR-W (HR 1.24, P = 0.084) in PET-2-negative patients. Reduction from six to four cycles led to non-significant but plausible intermediate accelerations. The addition of rituximab caused significantly slower TTR-F (HR 0.70, P = 0.0163) and TTR-W (HR 0.64, P = 0.0017) in PET-2-positive patients. HRQoL at baseline and age were the main determinants of 2-year HRQoL. CONCLUSIONS: Individualized first-line treatment in patients with advanced-stage HL considerably shortens TTR-F and TTR-W in PET-2-negative patients. Our results support the use of response-adapted shortened treatment duration for patients with HL.

One-hundred and twelve patients with multiple sclerosis were selected as population-based sample and examined on impairment (Expanded Disability Status Scale), disability (Barthel Index), handicap (Environment Status Scale), the quality of life (Multiple Sclerosis Quality of Life), fatigue (Modified Fatigue Impact Scale), depression (Beck Depression Inventory Score), respiratory function (spirometric parameters on spirometry) and physical fitness (spiroergometric parameters on a bicycle ergometer). The aim of the study was to examine and analyse (descriptive statistics) spiroergometric and spirometric parameters in patients with multiple sclerosis. Firstly, we tested the hypothesis whether spiroergometric and spirometric parameters are decreased and whether there are any correlations between these parameters and measures of impairment, depression, disability, handicap and quality of life. Secondly, we tested the hypothesis whether there is any correlation between a possible deconditioning and fatigue, and between a possible respiratory dysfunction and fatigue in multiple sclerosis. It results from this study that many spiroergometric parameters in patients with multiple sclerosis are significantly lowered in comparison to the population norm. A link can be found between some spiroergometric parameters and neurological impairment, disability, handicap and quality of life. It is not possible to prove any correlation between spiroergometric parameters and depression. From the spirometric parameters, these are expiratory flows that are significantly lowered in MS patients. It is not possible to prove any correlation between spirometric parameters and fatigue, depression, neurological impairment, duration of the disease, disability, handicap and quality of life in multiple sclerosis.

The magnesium AZ31 alloy was swaged with rotary pressure with the aim of redefining the microstructure and improving mechanical and fatigue properties. The rotary swaging process and subsequent ageing improved the yield stress in tension and compression. In the present study, the investigation was focused on fatigue behaviour. The samples were cycled in a symmetric regime with a frequency of 35 Hz. A dependence of the stress amplitude on the number of cycles up to the fracture was estimated. The microstructure of the samples and fracture surfaces was analysed with a scanning electron microscope. The fatigue process was influenced by the pronounced texture formed in the swaging process. The fatigue properties of the swaged samples improved substantially-the endurance limit based on 107 cycles was approximately 120 MPa-compared to those of the cast alloy. The analysis of the fracture surfaces showed a transcrystalline fatigue fracture.

This study investigated the possibility of nitride NiTi instruments using low-temperature plasma nitriding technology in a standard industrial device. Changes in the properties and fatigue life of used NiTi instruments before and after low-temperature nitriding application were investigated and compared. Nontreated and two series of plasma-nitrided NiTi instruments, designed by Mtwo company with tip sizes of 10/.04 taper, 15/.05 taper, and 20/.06 taper, were experimentally tested in this study. All these instruments were used and discarded from clinical use. The instruments were tested in an artificial canal made of stainless steel with an inner diameter of 1.5 mm, a 60° angle of curvature, and a radius of curvature of 3 mm. A low-temperature plasma nitriding process was used for the surface treatment of dental files using two different processes: 550 °C for 20 h, and 470 °C for 4 h. The results proved that it is possible to nitride dental instruments made of NiTi with a low-temperature plasma nitriding process. Promising results were achieved in trial testing by NiTi instruments nitrided at a higher temperature. Plasma-nitrided files were found to have, in some cases, significantly higher values than nontreated files in terms of fatigue life. The results showed that the nitriding process offers promising possibilities for suitably modified surface properties and quality of surface layer of NiTi instruments. Within the limitations of the present study, the cyclic fatigue life of plasma-nitrided NiTi dental files can be increased using this surface technology.

OBJECTIVE: To assess the effect of overall, between- and within-day subjectively rated fluctuations in motor and non-motor symptoms in people with functional motor disorder (FMD) on the health-related quality of life (HRQoL). BACKGROUND: FMD is a complex condition characterized by fluctuating motor and non-motor symptoms that may negatively impact HRQoL. METHODS: Seventy-seven patients (54 females, mean age 45.4 ± 10.4 years) with a clinically established diagnosis of FMD, including weakness, completed symptom diaries, rating the severity of motor and non-motor symptoms (i.e., pain, fatigue, mood, cognitive difficulties) on a 10-point numerical scale three times daily for seven consecutive days. HRQoL was assessed using the SF-36 questionnaire. For the analysis, fluctuation magnitude was defined in terms of the variability in self-reported symptom scores. RESULTS: The mental component of SF-36 was jointly predicted by the overall severity scores (t(74) = -3.61, P < 0.001) and overall general fluctuations (t(74) = -2.98, P = 0.004). The physical SF-36 was found to be related only to the overall symptom severity scores (t(74) = -7.09, P < 0.001), but not to the overall fluctuations. The assessment of the impact of different components showed that the mental component of SF-36 was significantly influenced by the combined effect of average fatigue (t(73) = -3.86, P < 0.001), between-day cognitive symptoms fluctuations (t(73) = -3.22, P = 0.002), and within-day mood fluctuations (t(73) = -2.48, P = 0.015). CONCLUSIONS: This study demonstrated the impact of self-reported symptom fluctuations across multiple motor and non-motor domains on mental but not physical HRQoL in FMD and highlighted the importance of assessing and managing fluctuations in clinical practice.
