V úvodu je prezentováno současné postavení psychiatrie. Dále je podán přehled nových antidepresiv (AD) a jsou uváděny jednotlivé aspekty jejich volby. Proměnné související s volbou antidepresiva se rozdělují na proměnné související s lékem (účinnost, snášenlivost, bezpečnost) a proměnné související s nemocným (věk, pohlaví, ruzné podtypy deprese, symptomatologie, komorbidita se somatickým a psychickým onemocněním, genetické informace). Detailně je rozebírána skupina SSRI, které jsou nejčastěji předepisovány. Ke správné volbě antidepresiva pro konkrétního nemocného je nezbytná orientace v dostupných antidepresivech, detailní anamnéza včetně farmakologické anamnézy a pečlivé zhodnocení klinického stavu nemocného. Lze předpokládat, že v budoucnosti budou dostupná i objektivní kritéria volby.

The current position of psychiatry is presented in the introduction. The paper reviews new antidepressants (ADs) as well as several aspects for their choice. Variables connected with the choice of an antidepressant are recognized as drug related variables (efficacy, tolerability, safety) and patient related variables (age, gender, several depression sub-types, symptomatology, comorbidity of a somatic and mental illness, genetic information). SSRI class of antidepressants is reviewed in greater detail as this is the class of drugs which is prescribed most often. In order to make the right choice of an antidepressant for a specific individual, it is necessary to be well acquainted with the antidepressants available, to know the patient’s medical history including the pharmacological anamnesis and to carefully assess the clinical condition of the patient. It may be assumed that objective criteria for the choice of antidepressant will be available in the future.
