Souhrn: Dlouhodobě je znám vztah mezi obezitou a vyšším výskytem kardiovaskulárních onemocnění, včetně hypertenze. Výskyt hypertenze se zvyšuje úměrně s rostoucí tělesnou hmotností a představuje významný faktor v souvislosti mezi obezitou a úmrtností na kardiovaskulární choroby. Bylo prokázáno, že i v rámci normálního BMI (body mass index) dochází s nárůstem hmotnosti ke zvyšování krevního tlaku. Mezi hlavní mechanizmy, kterými obezita negativně ovlivňuje krevní tlak, patří zvýšený objem krve, změny v cévním systému, aktivace renin- -angiotenzinového systému, inzulinová rezistence, zánětlivé procesy a spánková apnoe. Na této kazuistice bychom chtěli demonstrovat vliv bariatrického zákroku, konkrétně plikace žaludku, na průběh hypertenze. Kazuistika ukazuje, že bariatrická operace u pacienta s obezitou I. stupně a obtížně kompenzovanou hypertenzí vedla k významné redukci tělesné hmotnosti a zároveň přispěla ke snížení polypragmazie.

Summary: The long-known relationship between obesity and a significant increase in cardiovascular diseases, including hypertension, is well established. The prevalence of hypertension increases proportionally with rising body weight and represents a significant factor in the connection between obesity and mortality from cardiovascular diseases. It has been demonstrated that even within a normal BMI range, an increase in weight leads to a corresponding rise in blood pressure. The main mechanisms by which obesity negatively affects blood pressure include increased blood volume, changes in the vascular system, activation of the renin-angiotensin system, insulin resistance, inflammatory processes, and sleep apnea. In this case study, we aim to demonstrate the effect of bariatric surgery, specifically gastric plication, on the course of hypertension. The case study shows that bariatric surgery in a patient with class I obesity and poorly controlled hypertension led to a significant reduction in body weight and also contributed to a decrease in polypharmacy.
