Trombóza VMS je méně častou příčinou ischemic střevní (5-15 % akutních ischemických příhod střevních). Jsou shrnuty pričiny venózní trombózy, klinický obraz, diagnostický postup a léčba. Popsána kazuistika 79 leté pacientky s trombózou VMS. Provedena operační revize brišní dutiny s peroperačním stanovením správné diagnózy. Pro gangrénu části tenkého střeva provedena resekce. Krátce podávána antikoagulační terapie, časná zátěž stravou. Pacientka bez potřeby dalších revizí brišní dutiny. Vyšetřeny trombofilní markery se stanovením APC rezistence. Recidíva TEN v podobě oboustranné masivní plieni embolizace s následnou trvalou antikoagulancií.
Thrombosis of the superior mesenteric vein (VMS) is a frequent cause of ischaemia of the gut (5% to 15% acute intestinal ischaemias). The authors summarize the causes of venous thrombosis, the clinical picture. diagnostic procedure and treatment. They describe the case-history of a 79-year-old patient with thrombosis of the VMS. They revised the abdominal cavity and assessed the correct diagnosis on operation. On account of gangrene of the small intestine resection was performed. For a brief period anticoagulant treatment was administered, food was administered soon. The patient does not require any further revisions of the abdominal cavity. The authors examined thrombophil markers and assessed APV resistance. Relapse of TEN in the form of bilateral massive pulmonary embolization with subsequent anticagulation.
Cíl studie: Analyzovat časový interval ERCP a laparoskopické cholecystektomie od začátku onemocnění u pacientů s biliární pankreatitidou se zřetelem k celkové době hospitalizace. Materiál a metodika: V období 1998-1999 bylo ošetřeno na chirurgickém oddělení ON ve Vsetíně 39 pacientů s biliární pankreatitidou, z toho 8 pacientů (20,5 %) mělo těžkou formu a 31 pacientů (79,5 %) lehkou formu akutní pankreatitidy podle APACHE II skóre. Výsledky: U 39 pacientů s biliární pankreatitidou bylo provedeno ERCP, které u 27 pacientů prokázalo choledocholitiázu, u těchto byla provedená papilosfinkterotomie a extrakce litiázy. Po ERCP byla provedená laparoskopická cholecystektomie průměrně 9. den od prijetí, u 5 pacientů klasická cholecystektomie průměrně 10. den od prijetí. Průměrná doba hospitalizace byla u laparoskopicky operovaných 15 dnů, u klasicky operovaných 22 dnů. Závěr: Endoskopické ošetření žlučových cest s následnou laparoskopickou cholecystektomií se jeví jako optimální řešení u pacientů s biliární pankreatitidou.
Object of the study: To analyse the time interval of ERCP and of laparoscopic cholecystectomy from the onset of the disease in the patients with biUary pancreatitis with regard to the whole time of the hospitalisation. Material and methodology: From 1998 to 1999 patients with biUary pancreatitis were treated in the surgical department of the District hospital at Vsetín, 8 patients (20.5 %) had a severe disease and 31 patients (79.5 %) had a mild form of biliary pancreatitis according to APACHE II score. Results: ERCP was performed in 37 patients with biliary pancreatitis which proved choledocholithiasis in 27 patients, in these patients there was performed PST (papilosphincterotomy) with the follow-up extraction of lithiasis, after ERCP higher in 30 patients there was performed laparoscopic cholecystectomy on the average till the 9th day after the admitting to the hospital, in 5 patients classic cholecystectomy till the 10th day after the admitting, the whole time of the hospitalisation in the patients with biliary pancreatitis who were operated laparoscopicaly, lasted on the average 15 days, in the patients who were operated classicaly, it lasted 22 days. Conclusion: Endoscopic treatment of the biUary tract with the follow-up LSK CHCE is recommended as treatment of choice in the patients with biliary pancreatitis.
