Masivní krvácení při středoušních operacích se vyskytuje naštěstí velmi vzácně. Autoři prezentují kazuistiku pacienta, operovaného pro suspektní mesotympanický chemodektom. V průběhu operace došlo k prudkému arteriálnímu krvácení ze středouší, které bylo zastaveno pevnou tamponádou zvukovodu a nosohltanu. Následná angiografie ukázala, že šlo o krvácení z a. carotis interna anomálně probíhající středouším. Vzniklé pseudoaneurysma tepny ve středouší bylo jedenáctý den po operaci ošetřeno endovaskulárně superselektivní embolizací spirálkami. V druhé době byl bubínek zpevněn chrupavčitým transplantátem. Pacient se zhojil bez jakýchkoliv zdravotních následků, přetrvává pouze převodní nedoslýchavost pravého ucha. Autoři připomínají vývoj tepenného zásobení středouší a vznik nejvýznamnějších cévních anomálií středního ucha, kterými jsou perzistující stapedeální artérie a aberantní artérie carotis interna. Zdůrazňují, že pro včasnou diagnózu této anomálie je rozhodující selektivní angiografie (DSA) a. carotis a HRCT spánkové kosti posouzené otologickým rentgenovým specialistou.
Massive intraoperative hemorrhage during middle ear surgery is unusually rare. The authors describe a case report of the patient operated on suspected glomus tumor. During surgery a significant arterial bleeding occurred when a probatory excision from the suspected tumor had been taken. It was stopped by firm gauze plugging of the external ear canal and nasopharynx. Subsequent angiography showed the anomalous internal carotide artery traversing the middle ear to be a source of bleeding. A pseudoaneurysm of internal carotid artery developed in the middle ear. It was treated by superselective endovascular embolization with coils eleven days after accident. In second stage the tympanic membrane was strengthened by chondroperichondral graft. The patient healed without sequelae. There is only moderate conductive hearing loss on the operated side. The authors submit an overview of the arterial supply of the middle ear, its embryologic development as well as formation of the most important vascular anomalies of the middler ear, namely persistent stapedial artery and abberant internal carotid artery. They stress the meaning of the selective angiography (DSA) of the internal carotid artery and HRCT of the temporal bone evaluated by otologically specialized radiographer.

V úvodní části článku se autoři zabývají obecně nazální likvoreou, její diagnostikoua léčbou. Hlavní roli v diagnostice nazální likvorei hraje rentgenologické vyšetření, CT cisternogra-fie. Následuje kazuistické sdělení o 53leté pacientce s třemi měsíci trvající spontánní netraumatic-kou likvoreou. Píštěl byla uzavřena z neurochirurgického intradurálního přístupu.
In the introduction the authors deal with nasal liquorrhoea in general, with its diagnosis and treatment. The main part in the diagnosis of nasal liquorrhoea is played by X-ray examination, CT cisternography. This is followed by the case-record of a 53-year-old female patient with a three-month spontaneous non-traumatic liquorrhoea. The fistula was closed using a neuro- surgical intradural approach.
Autoři prezentují soubor 120 nemocných, u kterých byla v rámci otorinolaryngologickékonziliární služby provedena tracheostomie mimo ORL oddělení na jednotkách intenzivní péčeVšeobecné fakultní nemocnice (VFN) v Praze, v období 1. 1. 1998 - 30. 4. 1999. Tracheostomiebyly provedeny pro 51 % interních, 23,3 % neurologických, 11,6 % onkologických, 6,6 % traumatologických základních diagnóz a dále u 5,8 % pacientů s psychiatrickým onemocněním, u jednohopacienta (0,8 %) v septickém stavu. Nejmladší nemocný byl 17letý muž a nejstarší byla 92letá ženas průměrným věkem souboru pacientů 58,6 let. Průměrná doba endotracheální intubace byla 7,3dne (od 1 do 17 dní). U pěti pacientů byla provedena tracheostomie urgentně, z toho u tří bezzajištění dýchacích cest endotracheální intubací. Za sledované období 16 měsíců bylo dekanylováno27 pacientů (22,5 %) a 27 pacientů (22,5 %) zůstává nadále nosičem tracheostomické kanyly. 58pacientů (48,3 %) během hospitalizace exitovalo pro komplikace základních onemocnění.Peroperační komplikací byla aspirace cizího tělesa. Časnými komplikacemi bylo u čtyř pacientůkrvácení, které vyžadovalo chirurgickou revizi rány. Jedenkrát byla diagnostikována tracheoesofageální píštěl, u šesti pacientů došlo k rozpadu rány. Dvěma retracheostomovaným pacientům sev pozdním období rozvinula anatomicky i funkčně významná stenóza trachey, která si u jednohoz nich vyžádala trvalé kanylonosičství.
The authors present a group of 120 patients where within the framework of otolaryngological consultant services tracheostomy was not performed in an ENT department but in units of intensive care of the General Faculty Hospital in Prague between Jan 1, 1998 and April 30, 1999. The tracheostomies were performed on account of the following basic diagnoses: 51% medical, 23.3% neurological, 11.6% oncological, 6.6% traumatological and in 5.8% patients with psychiatric disease, in 0.8% patients in a septic condition. The youngest patient was a 17-year-old man and the oldest a 92-year-old woman, the mean age of the group being 58.6 years. The mean period of endotracheal intubation was 7.3 days (1 to 17 days). In five patients urgent tracheostomy was performed, incl. three without safeguarding the airways by endotracheal intubation. During the 16-month investigation period 27 patients were decannulated (22.5%) and 27 patients (22.5%) still have the tracheostomic cannula. 58 patients (48.3%) died during hospitalization due to complications of the basic disease. A peroperative complication was aspiration of a foreign body. Early complications in four patients included haemorrhage which called for surgical revision of the wound. In one patient a tracheooesophageal fistula was diagnosed, in six patients the wound disintegrated. Two re-tracheostomized patients developed during the late period an anatomically and functionally significant tracheal stenosis which in one patient called for a permanent cannula.
