Syndróm týraných, zneužívaných a zanedbávaných detí, známy pod skratkou CAN (child abuse and neglect), patrí k najviac prehliadaným a podceňovaným oblastiam pediatrie. Výskumy posledných rokov pritom ukazujú, že jeho závažnosť spočíva nielen v samotnom násilnom a traumatizujúcom správaní voči dieťaťu, ale aj v devastujúcich následkoch, ktoré takéto konanie zanecháva. Narastanie napätia a agresivity v spoločnosti, ako aj prehlbovanie chudoby a sociálnych rozdielov, predovšetkým v súvislosti s pandémiou a blízko prebiehajúcim vojenským konfliktom, robia problém ešte naliehavejším. Aj keď globálne riešenie problému presahuje naše možnosti a kompetencie ako pediatrov a zodpovednosť za prevenciu týchto prípadov leží na pleciach celej spoločnosti, naša úloha je nezastupiteľná. Veríme, že hlbšie pochopenie mechanizmov vedúcich k vzniku dlhodobých následkov, ako aj poukázanie na závažnosť vzniknutých zmien prinesie iný pohľad na problematiku a uľahčí prácu všetkým zainteresovaným.
Child abuse and neglect syndrome, known by its abbreviation as syndrome CAN, is one of the most overlooked and underestimated areas of pediatrics. Moreover, research in recent years has already showed, that its severity lies not only in the violent and traumatic behavior towards child, but also in the devastating consequences, that such behavior has. The growing tensions and aggression in society, the deepening of poverty and social disparities, especially in the context of the pandemic and the ongoing military conflict, make the problem even more pressing. Although the global solution of the problem goes beyond our possibilities and competencies as paediatricians and the responsibility for its prevention lies with society as a whole, our role is irreplaceable. We believe that deeper understanding of the mechanisms leading to the long-term consequences, as well as pointing out the severity of the resulting changes, will bring a different perspective of the issue and make work easier for all involved.
Milióny detských pacientov na celom svete podstupujú ročne chirurgické výkony v celkovej anestézii. Veľká väčšina týchto detí trpí predoperačnou úzkosťou, ktorej intenzita je v úzkej korelácii so vznikom maladaptívnych porúch po anestézii. Následky účinkov anestetík na vyvíjajúci sa mozog boli dlho prehliadané. Ukazuje sa, že celková anestézia môže ovplyvniť kognitívne funkcie na dlhšie obdobie. Dokonca môže mať vplyv na rozvoj kognitívnych funkcií nezrelého mozgu u veľmi mladých jedincov a môže urýchliť ich úpadok u starých ľudí.
Millions of paediatric patients per year, are undergoing the operations, all over the world. The most of these patients suffer from the preoperative anxiety, which intensity corelates with the development of maladaptive behavioral changes after the anesthesia. The influence of anesthetics to developing brain were omitted for a long time. But it has been shown that the general anaesthesia should influence cognitive functions for longer period. Even that it should have an impact to development on immature brain cognitive functions and should cause their deterioration at the adults.
