Trying to optimize protocol of ischemic tolerance we used 5 minutes of ischemia followed 2 days later by 30 minutes of ischemia. This resulted in significant (96%) protection of CA1 neurons. However, the 5-minute ischemia itself, without following lethal ischemia, leads to 45% degeneration of neurons in the CA1. These results indicate that: 1. the mechanizm which prevents degeneration of neurons is activated by the second pathophysiological stress, 2: process of delayed neuronal death is not irreversible and can be returned two days after ischemia. Logically, for the activation of protective mechanizm such brutal intervention like 30 minutes ischemia is not necessary. As an effective second pathophysiological stress we used 5 min. of ischemia, as well as 5 minutes of hypoxia with 4% of oxygen, while duration of initial ischemia that can be effectively treated in 4-vessel occlusion model of ischemia in rat was established at 10 minutes.
