In many ways methamphetamine has created something of a „speed“ hump in the road towards drug law reform and the dispensing of the war on drugs in favour of so called „health“ based models. There is an inconvenient reality to this most potent of central nervous system stimulant that tends to be glossed over in the conversations which tend to gravitate around the harmlessness of marijuana use and the perceived hypocrisy that exists with harm stemming from substances such as tobacco and alcohol. The modern history of methamphetamine is a minefield of uncomfortable truths about the outwardly pervasive harm that flows from the addiction this drug so readily creates. Indeed harm minimisation or reduction with crystal methamphetamine could be likened to giving someone a crash helmet to break their fall out of a ten story building. Methamphetamine has shattered the romantic notion of so called rec- reational drug use the all too familiar destruc- tive behaviour is something even the most ardent supporter of drug law reform finds more challenging to justify. If the primary justification for dispensing with prohibitionist laws is their limited efficacy with respect to drugs, should we also accept that similar laws have failed to address everything from burglary to rape and murder, and therefore we should dispense with these laws also? Should we also dispense with civil and common law rights, as breaches of these are so common? Like a family needs rules and guidelines, so does society. To dispense entirely with laws and rules which have been failing us could see the pendulum swing far beyond the balance that is required. Methamphetamine has put a timeline on our need to achieve the balance between health and the need for any society to have effective boundaries, as the harm that f lows from its use is not the users alone. Isn't it in achieving the right balance that our collective energies and resources be placed? Mike Sabin, is a former New Zealand Police Clan Lab specialist and founder of MethCon Group Ltd, a company specialising in educa- tion, research and policy related to meth- amphetamine. Mr. Sabin spent 12 years in the New Zealand Police, specialising early in his career in the area of drug enforcement. He worked on surveillance operations, elec- tronic interception and undercover opera- tions, before becoming one of New Zealand's first specialist Clandestine Drug Laboratory investigators responsible for investigating and dismantling meth labs. Early in 2006, Mr. Sabin left the Police hav - ing founded MethCon Group Ltd in late 2005, in the belief that law enforcement interventions have a disappointingly limited efficacy with regard to methamphetamine and a more balanced approach was needed to address what had become New Zealand's worst ever drug problem. MethCon Group Ltd is the only organisation of its kind in the world, and a multiple award winning organisation, having won business awards, a national leadership award and is currently in the running for a national entrepreneur of the year award. Mr. Sabin spent 18 months researching the methamphetamine problem in New Zealand and across the world looking to identify what works and what doesn‘t, to reduce the prevalence of the drug. The aim of this research was to inf luence more robust and effective policy and legislative responses while seeking to develop a more comprehensive and balanced approach to addressing methamphetamine use and prevalence. This research was compiled in a white paper, Solutions to the Methamphetamine Crisis in New Zealand: A Study of Supply and Demand- Side Interventions and their Efficacy. This was presented to New Zealand Members of Parliament and government officials in late May 2008.

Interní audity jsou nejen povinnou součástí systémů řízení kvality, ale je vhodné mít funkční systém zaveden i v takovém zdravotnickém zařízení, které neprochází externími ověřeními (akreditace, certifikace). Proč? Interní audit může být jednou z komunikačních cest v nemocnici, umí odhalit rizika při péči o pacienta, učí zaměstnance vnímat návaznost jednotlivých procesů a napomáhá k prohloubení spolupráce mezi zdravotnickými a nezdravotnickými odděleními nemocnice. Můžeme dosáhnout zakotvení dodržování požadovaných pravidel bez zpětného ověření? Kreativita lidí je široká a každý může nebo chce pochopit pravidla po svém. Často uslyšíme slova: „My jsme takto zvyklí, pracujeme tímto způsobem již x let a vyhovuje nám to.“ Máme jistotu, že se vždy vnitřními předpisy budou řídit všichni zaměstnanci? Audit je jedním z prostředků edukace, jak dostat nastavená pravidla mezi zaměstnance. Tak jako ve výchově dětí, i zde je nutná důslednost a rychlá zpětná vazba z konaných auditů. Provést audit, najít důkazy, pozitivní místa i činnosti k doladění, není pro některé auditory z jejich pohledu až tak těžké, jako sepsat následně zápis, který bude odpovídat zjištěné skutečnosti, bude postaven na pevných základech vyplývajících z vnitřních předpisů a legislativy a srozumitelně popsané důkazy potvrdí klady i nedostatky. Jaká je úloha manažera kvality v řízení auditů? Může se systém auditů obejít bez daných pravidel? Když se takto ptám, určitě ne. Přesto je zde na místě přínosem kreativita při tvorbě systémů auditů, „nakažení“ chutí do práce ostatních kolegů a schopnost naslouchat. Poznatky z auditů jsou, nebo by měly být, jedním z nástrojů řízení pro vedení nemocnice a zpětnou vazbou, jak se daří zakotvit potřebné postupy do běžného provozu.
Internal audits aren´t only an obligation of the system of quality management, but it´s useful to have a functional system even in such a health organization, which doesn´t go threw external proofs (acreditation, certification). Why? An internal audit can be one of the communication channels in hospital. It can reveal the risks in patient care. It teaches the staff to realize the connection among the process. It helps to strenghten the cooperation between health care departments and the administrative and supply department. Can we reach the fullfillment of required rules without retrospective certification? The creativity of every person is wide and they can or want to understand the rules differently. We often hear the words: „We are used to working this way for years and we are satisfied.“ Are we sure that all the employees will follow the internal rules? The audit is one of the means of education to get the set rules among the employees. As in the education of children, even here it´s necessary to be consistent and also to have an immediate feedback from the audits. Performing an audit, finding the evidence, positive aspects and weak aspects, isn´t as difficult from the side of the auditor, as it is to write a report, which will cover the actual facts and will be built on a strong basis arising from internal rules and legislative and comprehensible evidence will proove the positive and negative aspects. What is the function of the quality manager in directing an audit? Can the system of audits work without set rules? Definitely not. However, the creativity in forming the system of audits is a positive aspects. Another advantage is infecting your colleagues with the sense of happiness into work and the ability to listen. The audit findings are , or should be one of the managing instruments for the directory of the hospital and also the feedback, how we are able to establish the necessary procedures into basic operation.

The most common urological diseases in younger males are inflammatory diseases of small pelvis, as prostatitis, orchiepididymitis, urethritis and others. Their course significantly bothers the patient himself as well as his closest ones, encumbers account in Health Insurance Company and doesn't add any peace to the patient's attending physician either. Patient with chronic inflammatory disease comes to the outpatient's office 4-times more often than patient with BPH or prostate cancer. Part of clinical diagnosis in patients with inflammatory disease of small pelvis is infertility of unknown ethiology in up to 37% and erectile dysfunction in 12%. Uropathogenic flora (gram-negative or gram-positive microorganisms) is successfully proved as ethiologic agent only in small part of patients. Thorough laboratory diagnostics can discover inducer in major part of diseases, after all. Great majority of them belongs to the group of STIs (sexually transmitted infections). Besides that, even the above mentioned “uropathogens” can be included among STIs. The most common of these are Chlamydia infections, which form, with annual incidence over 90 million cases, a real worldwide problem. Yet, they do not reach the incidence of the most frequent STI, which is the HPV infection. For STI is characteristic polysymptomatic or asymptomatic course, and this is oftentimes in both sexual partners. During anti-microbial therapy of “banal” problems multi-resistance develops. Significant is a rapid increase of virus and Chlamydia infections that are curable however only with difficulties. Part of those unambiguously relates to the development of tumour diseases (HPV, HSV, and also Chlamydiosis). Tubular infertility of females is almost in 100% based on Chlamydia infection. The work presents view to the complex diagnostics of STIs and inflammatory diseases of small pelvis, findings of microbiological testing and examination of ejaculate. On the results of therapy of some of STIs we substantiate complicacy of treatment and inconsistency or total absence of therapeutic guidelines. It looks like that there could be possibly active prevention established by vaccination at least in one of the diseases. But how the virus shall act and what can we expect from the state economics (national vaccination) – these are the questions for the future. The main emphasis therefore has to be put on primary prevention.
