Nealkoholová tuková choroba jater (NAFLD) představuje spolu s alkoholovou chorobou jater nejčastější chronická onemocnění jater české populace. Obě jednotky mohou nabývat v závislosti na pokročilosti onemocnění celé spektrum klinických obrazů od prosté steatózy až po cirhózu jater s jejími závažnými komplikacemi – zejména portální hypertenzí a hepatocelulárním karcinomem. S rostoucí prevalencí diabetu mellitu 2. typu a obezity vzrůstá význam zejména první výše jmenované choroby, o jejíž diagnostice a léčbě pojednává tento článek. Současně si autoři dovolují čtenáře upozornit na komplexní změnu nomenklatury jaterních onemocnění, která klade mnohem větší důraz na kardiometabolická rizika s nimi spojená. Současně bylo reflektováno přání pacientů ve vztahu k vynechání stigmatizujících pojmů „(ne)alkoholová“ a „tuková“ v názvu onemocnění. Jako zastřešující termín byl zvolen název steatotické jaterní onemocnění (SLD; steatotic liver disease), který zahrnuje různé etiologie steatózy. Původní NAFLD se nyní nazývá MASLD (z angl. Metabolic Dysfunction-Associated Steatotic Liver Disease), tedy jaterní steatóza asociovaná s metabolickou dysfunkcí. MASLD zahrnuje pacienty, kteří mají steatózu jater, jeden z pěti kardiometabolických rizikových faktorů a prakticky nepijí alkohol. Termín steatohepatitida je stále považován za důležitý patofyziologický koncept a byl proto zachován. Preferovaný název je nyní MASH – steatohepatitida asociovaná s metabolickou dysfunkcí (z angl. Metabolic-Associated SteatoHepatitis).
Non-alcoholic fatty liver disease together with alcohol-related liver disease represent the most common chronic liver diseases in the Czech population. Both could - according to their severity - manifest as a whole spectrum of clinical pictures from simple steatosis to cirrhosis with its serious complications, especially portal hypertension and hepatocellular carcinoma. Due to increasing prevalence of diabetes mellitus type 2 and obesity, the first disease mentioned above gains importance and this article is discussig its diagnosis and treatment options. At the same time, the authors take the liberty of drawing the reader's attention to a significant change in the nomenclature of liver diseases placing much greater emphasis on the associated cardiometabolic risks. At the same time, the wishes of the patients were reflected and stigmatizing terms "non-alcoholic" and "fatty" were omitted. The name steatotic liver disease (SLD), which affects various etiologies of steatosis, was chosen as an umbrella term. The original NAFLD is now called MASLD (Metabolic Dysfunction-Associated Steatotic Liver Disease), i.e. liver steatosis associated with metabolic dysfunction. MASLD includes patients who have hepatic steatosis, one of the five cardiometabolic risk factors and drink minimal amounts of alcohol. The term steatohepatitis is still considered an important pathophysiological concept and has therefore been retained. The preferred name is now MASH - Metabolic-Associated SteatoHepatitis.

The pathogenesis of non-alcoholic fatty liver disease (NAFLD) is associated with abnormalities of liver lipid metabolism. On the contrary, a diet enriched with n-3 polyunsaturated fatty acids (n-3-PUFAs) has been reported to ameliorate the progression of NAFLD. The aim of our study was to investigate the impact of dietary n-3-PUFA enrichment on the development of NAFLD and liver lipidome. Mice were fed for 6 weeks either a high-fat methionine choline-deficient diet (MCD) or standard chow with or without n-3-PUFAs. Liver histology, serum biochemistry, detailed plasma and liver lipidomic analyses, and genome-wide transcriptome analysis were performed. Mice fed an MCD developed histopathological changes characteristic of NAFLD, and these changes were ameliorated with n-3-PUFAs. Simultaneously, n-3-PUFAs decreased serum triacylglycerol and cholesterol concentrations as well as ALT and AST activities. N-3-PUFAs decreased serum concentrations of saturated and monounsaturated free fatty acids (FAs), while increasing serum concentrations of long-chain PUFAs. Furthermore, in the liver, the MCD significantly increased the hepatic triacylglycerol content, while the administration of n-3-PUFAs eliminated this effect. Administration of n-3-PUFAs led to significant beneficial differences in gene expression within biosynthetic pathways of cholesterol, FAs, and pro-inflammatory cytokines (IL-1 and TNF-α). To conclude, n-3-PUFA supplementation appears to represent a promising nutraceutical approach for the restoration of abnormalities in liver lipid metabolism and the prevention and treatment of NAFLD.

BACKGROUND & AIMS: Severity of portal hypertension is usually quantified by measuring the hepatic venous pressure gradient (HVPG). However, due to its invasiveness, alternative markers are being sought. Bile acids (BA), being synthesized, metabolized, and transported by the liver, seem to have the potential to serve as endogenous markers. The aim of the present study was to determine whether serum BA reflect the severity of portal hypertension. METHODS: We correlated serum concentrations of individual BA with portal pressure (as HVPG) in an exploratory cohort of 21 cirrhotic patients with portal hypertension. The predictive potential of selected candidates was then confirmed in an independent validation cohort (n = 214). Additionally, nine previously published noninvasive markers were added to the stepwise logistic regression model to identify the most relevant ones, which were eventually used to create a prognostic index of portal hypertension. RESULTS: Serum levels of taurochenodeoxycholic acid (TCDCA) significantly correlated with HVPG and showed a high potential to predict clinically significant portal hypertension (HVPG ≥ 10 mm Hg: AUROC = 0.97 ± 0.06). This was confirmed in the validation cohort (AUROC = 0.96 ± 0.01). The predictive index (constructed based on AST/ALT, spleen diameter, and TCDCA concentration) was able to distinguish clinically significant portal hypertension with 95% sensitivity and 76% specificity. CONCLUSIONS: TCDCA seems to be a promising noninvasive marker of clinically significant portal hypertension. Its predictive potential may be further enhanced when it is combined with both the AST/ALT ratio and spleen diameter.
Nealkoholová tuková choroba jater (NAFLD) je v současnosti ve vyspělých zemích nejčastější chronickou jaterní nemocí. Vyskytuje se přibližně u čtvrtiny dospělé populace. Mezi nejrizikovější skupiny z hlediska NAFLD patří diabetici, obézní a pacienti s dyslipidémií, tedy jedinci s rozvinutým metabolickým syndromem. Prevalence tohoto onemocnění u nich přesahuje 50 %. U části těchto pacientů dojde k rozvoji zánětu, tzv. steatohepatitidě, a následné tvorbě vaziva, což ve svém důsledku může vést až ke vzniku jaterní cirhózy se všemi jejími komplikacemi, zejména jaterní insuficiencí, portální hypertenzí (ascitem, jícnovými varixy) a hepatocelulárním karcinomem. V poslední době je proto kladen velký důraz na aktivní vyhledávání pokročilých forem NAFLD v rizikových skupinách s cílem tyto pacienty včas rozpoznat a adekvátně léčit. V dnešní době jsou diagnostické metody široce dostupné a vývoj účinné terapie pokročilých forem NAFLD je v popředí výzkumného zájmu.
Non-alcoholic fatty liver disease (NAFLD) is currently the most common chronic liver disease in developed countries, where it affects up to a quarter of the population. The groups at most risk are diabetics, obese and patients with dyslipidemia, i. e. individuals with advanced metabolic syndrome, in whom the prevalence of NAFLD exceeds 50 %. Some of these patients develop inflammation, so-called steatohepatitis, and subsequent fibroproduction, which in turn can lead to liver cirrhosis with all the complications, especially liver failure, portal hypertension (ascites, esophageal varices) and hepatocellular carcinoma. Therefore recently great emphasis has been placed on actively searching for advanced forms of NAFLD in this population in order to identify and adequately treat these patients in a timely manner. Today, diagnostic methods are widely available and the development of effective therapies for advanced forms of NAFLD is at the forefront of research interest.

Jaterní testy patří mezi nejčastěji stanovované biochemické parametry a je nezbytné, aby jejich interpretace byla vždy prováděna správně. O přítomnosti jaterní choroby vypovídá nejen všem dobře známý základní panel jaterních testů (hodnoty bilirubinu, ALT, AST, GGT a ALP), ale také další biochemické parametry, zejména hladina albuminu, INR a počet trombocytů. Poslední jmenované analyty bývají často opomíjeny, stejně jako skutečnost, že normální hodnoty základního panelu jaterních testů ještě nevylučují pokročilé chronické jaterní onemocnění – fibrózu či cirhózu. Naproti tomu vysoké hodnoty jaterních testů automaticky neznamenají jaterní selhání. Ve světle vzrůstající incidence jaterních chorob stejně jako stoupající mortality z jaterních příčin v mnoha evropských zemích je žádoucí aktualizovat diagnostický algoritmus při záchytu patologických hodnot jaterních testů.
Liver function tests are among the most determined biochemical parameters, and it is essential that their interpretation is always performed correctly. The presence of liver disease is indicated not only by the well-known basic panel of liver function tests (bilirubin, ALT, AST, GGT and ALP), but also by other biochemical parameters, especially albumin, INR and platelet count. The latter analytes are often overlooked, as is the fact that normal values in the baseline liver test panel do not yet rule out advanced chronic liver disease - liver fibrosis or cirrhosis. In contrast, high levels of liver function tests do not automatically indicate liver failure. Considering the increasing incidence of liver disease as well as the rising mortality from liver causes in many European countries, it is desirable to update the diagnostic algorithm when pathological liver function tests are detected.

V současné době nemáme k dispozici kauzální medikamentózní léčbu nealkoholového ztukovatění jater (NAFLD) a její nalezení je jedním z hlavních výzkumných cílů hepatologie 21. století. Jeho léčba by měla vždy zahrnovat režimová opatření vedoucí k redukci tělesné hmotnosti, která jsou ve své podstatě velmi efektivní. V klinické praxi však s jejich pomocí dosáhne stanovených cílů pouze zlomek pacientů. V současnosti se terapie NAFLD opírá o screening a léčbu komponent metabolického syndromu, potřeba je rovněž minimalizovat příjem alkoholu. K nejslibnějším preparátům v léčbě NAFLD v právě probíhajících klinických studiích patří semaglutid a lanifibranor.
Finding an effective treatment of non-alcoholic fatty liver disease (NAFLD) remains one of main challenges in hepatological research for the 21st century, since there is no such a treatment yet. Lifestyle modifications leading to weight reduction are cheap and effective, however only a fraction of patients reaches significant weight loss goals. Current pharmacological treatment of NAFLD comprises screening and therapy of metabolic syndrome components and minimizing of alcohol intake. There are new substances being evaluated in clinical trials, the most promising ones are semaglutide and lanifibranor.
Úvod: Jaterní fibróza je průvodním jevem chronických jaterních chorob a často se vyvíjí po dlouhou dobu skrytě. Je rizikem i pro mimojaterní onemocnění včetně tzv. křehkosti. Cílem naší práce bylo určit riziko jaterní fibrózy u skupiny pacientů s traumatem proximálního femuru. Metodika: Pacienti operovaní pro zlomeninu proximálního femuru byli retrospektivně zhodnoceni pomocí neinvazivních indexů jaterní fibrózy (NAFLD fibrosis skóre, FIB-4 a APRI skóre). Kontrolní skupinu tvořili pacienti s diabetem. Výsledky: Bylo hodnoceno celkem 72 operovaných pacientů a 72 kontrol. Při využití nejvhodnějšího indexu (FIB-4 adjustovaného na věk) bylo hodnoceno 46 % pacientů bez jakéhokoli rizika jaterní fibrózy, 43 % pacientů se středním či neurčitým rizikem fibrózy a 11 % pacientů mělo s vysokou pravděpodobností dosud klinicky němou fibrózu. Zastoupení jednotlivých skupin se významně nelišilo od kontrol. Etiologicky se nejspíše jednalo o NAFLD či etylické postižení. Závěr: Významná jaterní fibróza může být přítomna u 1/10 pacientů operovaných pro trauma proximálního femuru. Detekce takových pacientů pomocí neinvazivních indexů je velmi jednoduchá a měla by být ke screeningu používána i v běžné praxi.
Introduction: Liver fibrosis is a consequence of chronic liver disease and often develops covertly over a long period of time. Fibrosis is also a risk factor for extrahepatic diseases, including frailty. The aim of our study was to determine the risk of liver fibrosis in a group of patients with hip trauma. Methods: Patients with surgical treatment of hip trauma were retrospectively evaluated using non-invasive liver fibrosis indices (NAFLD fibrosis score, FIB-4 and APRI score). The control group consisted of patients with diabetes. Results: A total of 72 patients and 72 controls were evaluated. Using the most appropriate index (FIB-4 adjusted for age), 46% of patients had no risk of liver fibrosis, 43% of patients had moderate or indeterminate risk of fibrosis, and 11% of patients were highly likely to have clinically significant fibrosis. Risk stratification did not differ significantly from the controls. The aetiology of liver fibrosis was most likely due to NAFLD or alcohol abuse. Conclusion: Significant liver fibrosis may be present in 1/10 of patients with hip trauma indicated for surgery. Detection of such patients using non-invasive indices is very simple and should be used for screening in common practice.
BACKGROUND AND AIMS: We previously identified subsets of patients with NAFLD with different metabolic phenotypes. Here we align metabolomic signatures with cardiovascular disease (CVD) and genetic risk factors. APPROACH AND RESULTS: We analyzed serum metabolome from 1154 individuals with biopsy-proven NAFLD, and from four mouse models of NAFLD with impaired VLDL-triglyceride (TG) secretion, and one with normal VLDL-TG secretion. We identified three metabolic subtypes: A (47%), B (27%), and C (26%). Subtype A phenocopied the metabolome of mice with impaired VLDL-TG secretion; subtype C phenocopied the metabolome of mice with normal VLDL-TG; and subtype B showed an intermediate signature. The percent of patients with NASH and fibrosis was comparable among subtypes, although subtypes B and C exhibited higher liver enzymes. Serum VLDL-TG levels and secretion rate were lower among subtype A compared with subtypes B and C. Subtype A VLDL-TG and VLDL-apolipoprotein B concentrations were independent of steatosis, whereas subtypes B and C showed an association with these parameters. Serum TG, cholesterol, VLDL, small dense LDL5,6 , and remnant lipoprotein cholesterol were lower among subtype A compared with subtypes B and C. The 10-year high risk of CVD, measured with the Framingham risk score, and the frequency of patatin-like phospholipase domain-containing protein 3 NAFLD risk allele were lower in subtype A. CONCLUSIONS: Metabolomic signatures identify three NAFLD subgroups, independent of histological disease severity. These signatures align with known CVD and genetic risk factors, with subtype A exhibiting a lower CVD risk profile. This may account for the variation in hepatic versus cardiovascular outcomes, offering clinically relevant risk stratification.
