BACKGROUND: Uterus transplantation (UTx) is a novel type of transplantation to treat infertility in women with an absent or nonfunctioning uterus. The International Society of Uterus Transplantation (ISUTx) has developed a registry to monitor worldwide UTx activities while serving as a repository for specific research questions. METHODS: The web-based registry has separate data fields for donor, recipient, surgeries, immunosuppression, rejections, pregnancies with live birth(s), and transplant hysterectomies. Data are prospectively registered. RESULTS: A total of 45 UTx procedures have been registered; the majority (78%) of those procedures were live donor (LD) transplants. Median age of the LDs, deceased donors, and recipients were 50 y (range 32-62), 38.5 y (19-57), and 29 y (22-38), respectively. The duration of LD surgery was approximately twice as long as the recipient surgery. Postoperative complications of any Clavien-Dindo grade were registered in 20% of LDs and 24% of recipients. Rejection episodes were more frequent (33%) early after transplantation (months 1-5) compared with later time points (months 6-10; 21%). Healthy neonates were delivered by 16 recipients, with 3 women giving birth twice. The total live birth rate per embryo transfer was 35.8%. Median length of pregnancy was 35 gestational weeks. Twelve uteri were removed without childbirth, with 9 transplant hysterectomies occurring during the initial 7 mo post-UTx. CONCLUSIONS: A mandatory registry is critical to determine quality and process improvement for any novel transplantation. This registry provides a detailed analysis of 45 UTx procedures performed worldwide with a thorough analysis of outcomes and complications.

Cieľ: Pneumocystis jirovecii patril v minulosti do skupiny prvokov, ale v súčasnosti je taxonomicky zaradený do ríše húb. P. jirovecii je oportúnny patogén, zodpovedný za pneumocystovú pneumóniu s častými komplikáciami u imunokompromitovaných pacientov. Oneskorené začatie vhodnej liečby zvyšuje riziko úmrtia u pacientov s oslabenou imunitou. Cieľom práce bolo zistiť a zhodnotiť spoľahlivosť metód laboratórnej diagnostiky pneumocystózy používaných v rutinných laboratóriách ako aj výskyt tohto ochorenia u pacientov zo Slovenska za 19 rokov. Materiál a metódy: Diagnostika je založená na mikroskopickom dôkaze (farbenie podľa Giemsa a Gram-Weigerta) a detekcii DNA parazita klasickou alebo real-time PCR v bronchoalveolárnej laváži a spúte. Výsledky: Pneumocysty boli zistené u 190 osôb (5,7 %) z celého súboru pacientov. Onkologickí pacienti predstavovali najrizikovejšiu skupinu z hľadiska infekcie pneumocystami, čo sme potvrdili ich najvyšším podielom (57,9 %) z jedincov s pneumocystózou. Na základe binárneho klasifikačného testu sme vyhodnotili 33,7 % citlivosť a 100 % špecifickosť mikroskopického dôkazu v porovnaní s PCR. Molekulárne metódy sú v porovnaní s mikroskopickým dôkazom citlivejšie v detekcii P. jirovecii a v súčasnosti predstavujú spoľahlivý detekčný systém v diagnostike pneumocystózy. Záver: Vzhľadom na narastajúci počet imunokompromitovaných osôb je diagnostika P. jirovecii u pacientov s pľúcnymi komplikáciami nevyhnutná. To sa potvrdilo aj v našej štúdii, kde v priebehu rokov stúpal počet vyšetrení a záchytov tohto oportúnneho patogénu.
Aim: In the past, Pneumocystis jirovecii belonged to the Protozoa group, but is currently taxonomically included in the kingdom Fungi. P. jirovecii is an opportunistic pathogen, responsible for pneumocystis pneumonia with frequent complications of immunocompromised patients. Delayed initiation of appropriate therapy increases the risk of death in immunocompromised patient. The aim of this work was to determine and evaluate the reliability of methods of laboratory diagnosis of pneumocystosis used in routine laboratories as well as the occurrence of this disease in patients from Slovakia during 19 years. Material and Methods: The diagnosis is based on microscopic examination (Giemsa- and Gram-Weigert-staining) and detection of parasite DNA by classical or real-time PCR in bronchoalveolar lavage and sputum. Results: Pneumocysts were detected in 190 persons (5.7%) from the whole group of patients. Cancer patients represented the riskiest group in terms of pneumocystosis, which was confirmed by the highest percentage (57.9%) of individuals infected with P. jirovecii. Compared with the PCR, 33.7% sensitivity and 100% specificity of microscopy was calculated by using a binary classification test. Molecular methods are more sensitive in the detection of P. jirovecii compared to microscopic evidence and currently represent a reliable detection system in the diagnosis of pneumocystosis. Conclusion: In view of the increasing number of immunocompromised persons, diagnostics of P. jirovecii in patients with pulmonary complications is essential. This was also confirmed in our study, where the number of examinations and detection of this opportunistic pathogen increased over the years.

[Infectious complications after kidney transplantation]
Infekce patří mezi hlavní komplikace po transplantaci ledviny se zásadním dopadem na přežití štěpů i pacientů. Infekční komplikace vznikají jako důsledek vztahu mezi expozicí patogenu, stavu imunitního systému, úrovní imunosupresivní terapie a komorbidit pacienta. Prevence, diagnostika a adekvátní terapie jsou klíčové pro zlepšení výsledků transplantace.
Infections are the main complications of kidney transplantation with the crucial impact on graft and patient surival. Infectious complications arise as a result of a relationship between patogen exposure, state of immune system, level of immunosuppressive therapy and patient's comorbities. Prevention, diagnosis and adequate therapeutical approach play the key role for amelioration of transplant outcomes.

INTRODUCTION: The indications for specific treatment in the cases of inflammatory cardiomyopathy are based on limited data from several small clinical trials. AIM: A comparison of the effect of two dose regimens of combined immunosuppressive therapy by adding them to conventional heart failure therapy and comparing them with conventional heart failure therapy alone in patients with inflammatory cardiomyopathy. METHODS AND STUDY POPULATION: We enrolled 20 patients; mean age 46.10±7.33 years, duration of symptoms <6 months, LVEF ≤40 %, NYHA class II–IV, with biopsy‐proven myocarditis. Patients were randomly separated into groups treated with immunosuppressive therapy in addition to conventional heart failure therapy or to a group treated with conventional heart failure therapy alone. Clinical and echocardiographic parameters were evaluated. RESULTS: The baseline values of LVEF in the group of immunosuppressive therapy (LVEF 22.3±4.7 %) were similar to those in the group treated with conventional heart failure therapy (LVEF 21.7±4.7 %; p=0.757). After twelve months there was no statistically significant difference in LVEF between the two studied groups (LVEF 33.7±9.5 % for the immunosuppressive therapy group and 41.3±13.0 % for the conventional therapy group; p=0.175). CONCLUSION: In our study population, we proved no positive effect of combined immunosuppressive therapy on the left ventricular function over 12 months. The main limitation of the study is the small number of enrolled patients (Tab. 4, Fig. 1, Ref. 35).

Účinnost nintedanibu na zpomalení poklesu plicních funkcí u pacientů s progredujícím fibrotizujícím (PF) fenotypem intersticiálního plicního procesu (IPP) byla ověřena klinickou studií INBUILD, jejíž výsledky byly publikovány v roce 2019. Zatímco v této studii bylo souběžné použití nintedanibu a imunosupresiv omezeno, v klinické praxi před rozhodnutím, zda nintedanibem imunosupresi nahradit, nebo jej ke stávající léčbě přidat, stojíme poměrně často. Zatím nebylo publikováno mnoho prací zabývajících se snášenlivostí kombinované imunosupresivní a antifibrotické léčby. Sdělení přináší přehled zkušeností s kombinovanou imunosupresivní a antifibrotickou léčbou u šesti nemocných, s dobrou snášenlivostí léčby.
The effectiveness of nintedanib in slowing the decline in lung function in patients with a progressive fibrotic (PF) phenotype of the interstitial lung process (IPP) was validated in the INBUILD clinical trial, published in 2019. While the concomitant use of nintedanib and immunosuppressants was limited in this study, the practice before deciding whether to replace nintedanib with immunosuppression or to add it to existing treatment is quite common. To date, many studies have been published on the tolerability of combined immunosuppressive and antifibrotic therapy. The communication provides an overview of the experience with combined immunosuppressive and antifibrotic treatment in six patients with good tolerance.

Léčba inhibitory imunitních kontrolních bodů (ICI) představuje relativně novou modalitu onkologické terapie, která je stále více využívána v léčbě řady maligních onemocnění, neboť její podání výrazně zvyšuje přežívání nemocných. Podávání ICI však může vést k řadě nežádoucích účinků včetně kardiotoxických, kdy nejčastějším a zároveň nejzávažnějším projevem je myokarditida asociovaná s ICI, které je věnován tento přehledový článek. Myokarditida navozená ICI má sice nízkou incidenci, její mortalita je však vysoká, dosahuje až 50 %. Důležitá je proto znalost existence této jednotky, jejích klinických manifestací, časná diagnostika a brzké nasazení imunosupresivní léčby, kdy terapií první volby jsou vysokodávkované kortikosteroidy.
Immune checkpoint inhibitor (ICI) administration represents relatively new modality of cancer therapy that is increasingly used in the treatment of many malignancies, as its administration significantly improves survival of the patients. However, ICI administration may lead to several side effects, including cardiotoxic, with the most common and severe manifestation being the subject of this review, ICI-associated myocarditis. Although ICI-associated myocarditis has a low incidence, its mortality is high, reaching almost 50%. It is therefore important to be aware of the existence of ICI-associated myocarditis, its clinical manifestations, timely diagnosis, and early administration of immunosuppressive treatment with high-dose corticoids representing the first-line therapy.
Imunosuprimovaní pacienti po transplantaci ledviny mají vysoké riziko infekčních komplikací. V této skupině pacientů se vyskytuje celá řada infekcí virové, bakteriální nebo mykotické etiologie včetně oportunních nákaz, ale za vysokou mortalitu jsou zodpovědné především infekce bakteriální. Z bakteriálních infekcí jsou nejčastější infekce močových cest a pneumonie, které mnohdy vyžadují hospitalizaci a přechodnou redukci imunosupresivní terapie, a to vždy po konzultaci transplantačního centra. Kromě symptomů daných lokalizací infekce se často vyskytuje horečka. Specifikem pacientů po transplantaci ledviny je diferenciální diagnostika horečky a/nebo vysoké zánětlivé aktivity. Kromě infekčních příčin zvažujeme polékovou horečku, rejekční epizodu, relaps systémového onemocnění nebo u pacientů se selháním funkce štěpu v dialyzační léčbě graft intolerance syndrom ("symptomatický štěp"). Záchrana života pacienta, která mnohdy vyžaduje významnou redukci imunosupresivní terapie, má vždy přednost před zabráněním rejekční epizody. Nedílnou součástí péče o transplantované pacienty je prevence infekcí, včetně doporučení očkování tam, kde je to možné.
Immunosuppressed kidney transplant recipients have a high risk of infectious complications. A variety of infections of viral, bacterial, or mycotic etiology, including opportunistic infections, occur in this group of patients, but bacterial infections are primarily responsible for the high infection-related mortality. Of the bacterial infections, urinary tract infections and pneumonia are the most common, often requiring hospitalization and temporary reduction of immunosuppressive therapy after consultation of the transplant center. In addition to symptoms due to the location of the infection, fever is often present. The differential diagnosis of fever and/or high inflammatory activity is specific to patients after kidney transplantation. In addition to infectious causes, we consider drug-related fever, a rejection episode, a relapse of systemic disease or in patients with graft failure in dialysis treatment graft intolerance syndrome ("symptomatic graft"). Saving the patient's life, which often requires a significant reduction in immunosuppressive therapy, always takes precedence over preventing a rejection episode. Infection prevention is an integral part of the care of transplant patients, including the recommendation of vaccination where possible.
