Pseudoaneuryzmy (nepravé aneuryzmy) aorty predstavujú zriedkavé, avšak život ohrozujúce stavy, ktoré sa vyskytujú najčastejšie po predchádzajúcich kardiochirurgických výkonoch. Príčiny ich vzniku sú multifaktoriálneho charakteru, ako je trauma hrudníka alebo infekcia. Klinický obraz môže byť variabilný. Manifestácia ochorenia môže byť asymptomatická, alebo na druhej strane až s katastrofickým priebehom. V našej kazuistike prezentujeme úspešnú chirurgickú liečbu veľkej pseudoaneuryzmy vzostupnej aorty u asymptomatického 17-ročného pacienta, ktorý pred 10 mesiacmi podstúpil náhradu aortálnej chlopne.
Aortic pseudoaneurysms are rare, but life-threatening conditions, that occur most commonly after previous cardiac surgery. The causes could be multifactorial such as chest trauma or infection. Clinical presentation can vary from asymptomatic to catastrophic outcomes. We report the successful surgical repair of a large ascending aortic pseudoaneurysm in an asymptomatic 17-year-old patient who had undergone aortic valve replacement 10 months ago.

INTRODUCTION: False aneurysms in the thoracic aorta are dangerous complications that can occur after cardiac surgery. They often result in high mortality rates. These aneurysms are caused by damage to all layers of the aortic wall. This study aimed to pinpoint the area of the experimental specimen (native vessel, anastomosis, or prosthetic graft) with the greatest deformation, to determine whether a false aneurysm is likely to develop in the anastomotic portion. METHODS: We conducted the inflation-extension test by performing eight cycles ranging from 0 to 20. The pressure sampling frequency was 100 Hz, and each cycle lasted approximately 34 seconds, resulting in a loading frequency of 0.03 Hz. During the experiment, each camera captured 3,000 frames. Based on the data collected, we evaluated and compared the loading stages of cycle 1 and cycle 8. RESULTS AND DISCUSSION: During loading, the native vessel experienced a dominant deformation of approximately 7% in the circumferential direction. The prosthetic graft, which had a longitudinal construction, deformed by approximately 8% in the axial direction. The prosthetic graft, on the other hand, only experienced a deformation of up to 1.5% in the circumferential direction, which was about 5 times smaller than the deformation of the native vessel. The anastomosis area was very stiff and showed minimal deformation. Additionally, there was little difference in the mechanical response between the first C1 and the eighth C8 cycle. CONCLUSION: Based on the available evidence, it can be inferred that aortic false aneurysms are more likely to form just behind the suture lines in the native aorta, which is more elastic compared to stiff sections of anastomosis and prosthetic graft. Numerous pulsations of the native vessel will likely cause the impairment of the aorta at the margin of the anastomosis. This will lead to disruption of the aortic wall and false aneurysm formation in the native vessel near the area of anastomosis.

Nahrazení infikované cévní protézy allograftem je možným řešením této komplikace díky dokumentovanému nízkému riziku reinfekce. Nejčastěji se allograft využívá v případech, kdy vlastní tkáň pacienta již není možné využít. Prezentujeme případ sedmdesátiletého pacienta, který podstoupil opakované cévní rekonstrukce na pravé dolní končetině. Na naši kliniku byl přijat pro progresivně se zvětšující rezistenci v pravém třísle. Byl zařazen na čekací listinu na čerstvý allograft. Podstoupil nahrazení infikovaného pseudoaneurysmatu a protetického materiálu femorální bifurkace tepenným allograftem. Původní femoro-popliteální autovenózní bypass byl reimplantován do allograftu. Během operace byly pozorovány známky sepse, hemokultura vyšla negativní. Kultivace z rány i drénu byly negativní. Pacient byl propuštěn sedmý pooperační den s profylaktickými antibiotiky. Časná ambulantní kontrola neodhalila žádné známky rekurence infekce a průchodnou cévní rekonstrukci. Sedm a půl měsíce po poslední operaci se femoro-popliteální bypass uzavřel, bylo rozhodnuto o konzervativním postupu. Dva roky po výkonu se vyvinulo malé, trombozované pseudoaneurysma allograftu z důvodu přechodné non-compliance pacienta k užívání imunosupresivní terapie. Postupovali jsme konzervativně. Dva a půl roku po operaci je allograft průchodný a končetina zachována.
Replacing an infected vascular prosthetic conduit with an allograft is a possible solution of this complication given the low recurrence of infection. It is most commonly utilized for cases where the use of autologous tissue is not an option. We present the case of a 70-year-old patient who had undergone repeated vascular reconstructions in the right lower limb. He was admitted to our department due to a progressively growing mass in the right groin and subsequently placed on the waiting list for a fresh allograft. The patient had the infected false aneurysm and prosthetic material of the femoral bifurcation replaced with an arterial allograft. The previous femoral popliteal autovenous bypass graft was reimplanted into the allograft. There were signs of sepsis during the operation; however, the blood culture was negative. Cultures from neither the wound nor the drain revealed the presence of any bacteria. The patient was discharged on the seventh post-operative day with prophylactic antibiotics. An early followup confirmed that there were no signs of recurrent infection and that the reconstruction remained patent. Seven and half months after the surgery, the femoral popliteal bypass graft became occluded and a conservative approach was chosen. A small thrombosed false aneurysm of the graft was revealed two years after the surgery due to transient non-compliance of the patient to immunosuppression therapy. It was treated conservatively. Two and a half years after the surgery, the allograft still remains open and the limb is preserved.

Pseudoaneurysma hrudní aorty je raritní diagnóza asociovaná s vysokou mortalitou. Prezentujeme případ pacienta, u kterého vznikla velká falešná výduť deset let po provedení Bentallovy operace a jednonásobného aortokoronárního bypassu. Zároveň s daným nálezem se u něho prokázala covid-19 pozitivita. Po zaléčení pneumonie byla úspěšně provedena operace, a to sešitím dvou defektů v aortálním konduitu. Jedná se o unikátní případ, kdy pacient přežil dva život ohrožující stavy.
Pseudoaneurysm of the thoracic aorta is a rare condition associated with high mortality. We present a case of a patient who developed a huge pseudoaneurysm 10 years after the Bentall procedure and one aortocoronary bypass while simultaneously being COVID-19 positive. After treating pneumonia, the surgery was successfully performed with suturing two defects in the aortic conduit. It is a unique case report where the patient survived two life-threatening conditions.

Úvod: Intramyokardiální disekující hematom (IDH) je dutina ve stěně srdce vyplněná krví, která vzniká oslabením stěny nekrózou po infarktu myokardu. IDH se může vytvořit ve stěně levé komory, pravé komory nebo interventrikulárního septa. Popis případu: Kazuistika popisuje pacienta s IDH jako vzácnou komplikaci po infarktu myokardu. Pacient byl hospitalizován pro trvající obtěžující kašel tři měsíce po prodělání infarktu myokardu řešeném implantací stentu. Dle echokardiografie bylo podezření na krytou rupturu volné laterální stěny s obrazem pseudoaneu- rysmatu, pro kterou bylo rozhodnuto o operačním řešení. Při operaci byl nález plošné dutiny ve stěně levé komory komunikující otvorem s levou komorou. Vnitřní i vnější stěna dutiny obsahovala srdeční svalovinu, a tak byla dutina diagnostikována jako IDH. Otvor i dutina byly uzavřeny stehy přes meshové podložky a suturou přes meshový proužek. Pooperační průběh se obešel bez komplikací a pacient byl propuštěn osmý den po operaci. Diskuse: Diagnostika IDH se zakládá na anamnéze infarktu myokardu a na nálezech na echokardiografii. Mezi známky echokardiografického obrazu IDH patří vytvoření nové dutiny uvnitř srdeční stěny s echolucentním středem. Diferenciální diagnostika IDH zahrnuje aneurysma, pseudoaneurysma, trombus uvnitř dutiny LK nebo trabekuly komor. Závěr: IDH je komplikace infarktu myokardu, na kterou je třeba pomýšlet při diferenciální diagnostice nově vzniklé dutiny ve stěně levé komory na ultrazvukovém vyšetření. Konzervativní řešení je možné u IDH malých rozměrů, u kterých nedochází k progresi. U hemodynamické nestability pacienta nebo u velkých nebo progredujících IDH je namístě chirurgické řešení.
Background: Intramyocardial dissecting hematoma (IDH) is blood-filled cavitation in the cardiac wall that can occur as a complication of myocardial infarction, chest trauma, or percutaneous intervention. It can develop in the left ventricular free wall, the right ventricle, or the interventricular septum. Case report: The case report describes a patient with IDH as a rare complication of myocardial infarction. The patient was hospitalized for ongoing irritating cough three months after ST elevated myocardial infarction treated with percutaneous coronary intervention. The echocardiography showed the image of a pseudoaneu- rysm of the left ventricle. The patient underwent surgery where a cavitation inside the left ventricle myocardial wall communicating with the left ventricle was found. The inner and outer border of the cavitation included myocardium, therefore diagnosis of IDH was established. The communication and the cavitation were closed with pledgeted sutures and a direct suture with a mesh stripe. Postoperative hospitalization was without complications and the patient was dismissed the eighth day after the surgery. Discussion: Diagnosis of IDH consists of anamnestic information of myocardial infarction and assessment on echocardiography. On echocardiography the IDH shows as a neocavitation within the heart wall with an echo-lucent center. The differential diagnosis of IDH includes aneurysm, pseudoaneurysm of the heart, thrombus of the ventricle and trabeculae. Conclusion: The IDH is a post-infarction complication important to include in the differential diagnosis of newly formed cavitation inside the ventricle wall seen on echocardiography. Conservative management is suggested when the IDH is small and not growing. Surgery is indicated if the patient is unstable or the IDH is large or growing.

Pseudoaneurysms are created by a traumatic or iatrogenic perforation of an artery, resulting in accumulation of blood between the two outermost layers of a blood vessel, the tunica media and tunica adventitia. Pedal artery pseudoaneurysms are an extremely uncommon complication of foot and ankle surgery; therefore, few cases have been reported in the literature. Early diagnosis is important to ensure timely treatment of this condition. Once clinical suspicion has been established, urgent referral to vascular surgery for prompt surgical evaluation is required to prevent potentially harmful sequalae. We present the case of a 70-year-old female who developed a pseudoaneurysm of the dorsalis pedis artery 33 days after undergoing open reduction internal fixation (ORIF) of a second metatarsal fracture. Her treatment included urgent referral to vascular surgery with subsequent surgical repair of the pseudoaneurysm via ligation of the medial dorsal branch of the dorsalis pedis artery. At 10-month follow-up, she denied any pain, sensory deficits, or functional disability and had returned to all preinjury activities with no recurrence of the pseudoaneurysm. Our case study demonstrates early diagnosis and successful treatment of a pseudoaneurysm of the dorsalis pedis artery that developed shorty after ORIF of a second metatarsal fracture.

We report the case of a 71-year-old female who presented with sudden onset of right cervical pain and ipsilateral arm hypoaesthesia. The diagnostic evaluation revealed a pathology of the origin of the innominate artery (IA). The differential diagnosis was among a spontaneous acute dissection and a ruptured pseudoaneurysm. The dilemma of the proper treatment emerged. Surgical or medical treatment? Open or endovascular approach? The patient was offered an open treatment under cardiopulmonary bypass and sort circulatory arrest. As less than ten cases of isolated IA dissection have been previously reported in the literature, we discuss the differential diagnosis difficulties and the treatment options.
Úvod: Anastomotická pseudoaneurysmata (APSA) jsou pozdní komplikací cévních rekonstrukcí. Přesná prevalence APSA není známa, protože mnoho případů není v literatuře publikováno. Ruptura APSA je však spojena s vysokou morbiditou a mortalitou. Metoda: Operační výkon byl proveden v celkové anestezii. Byl proveden řez v levém třísle současně s resekcí nekrotické kůže nad APSA. APSA bylo pečlivě vypreparováno s potvrzením nálezu CT angiografie (CTA) - masivní trombotizované APSA vystupující z distální anastomózy aortofemorálního bypassu (AFB). APSA bylo plně resekováno. AFB byl částečně resekován a za pomoci protetického interpozitu prodloužen. Společná stehenní tepna byla podvázána pod tříselným vazem. Výsledky: Pacient byl propuštěn 7. pooperační den s nízkými parametry zánětu. V současné době máme pa- cienta ve sledování jedenáct měsíců. Pacient má průchodnou cévní rekonstrukci bez známek anastomotického APSA na Dopplerově ultrasonografii. Závěr: Nejčastějším místem výskytu APSA po AFB je femorální oblast. Endovaskulární léčbu lze použít pouze ve velmi vybraných případech s velmi vysokým rizikem komplikací souvisejících s endovaskulárními intervencemi.
Introduction: Anastomotic pseudoaneurysms (APSA) are late complications of vascular reconstructions. The precise prevalence of APSA is not known, as many cases are not published in the literature. However, the rupture of APSA is associated with high morbidity and mortality. Method: The procedure was performed under full anesthesia. An incision was made in the left groin resect- ing the necrotic skin above APSA. APSA was carefully dissected, confirming the CT angiography finding of a massive thrombosed APSA arising from the distal anastomosis of the aorto-femoral bypass (AFB). The APSA was fully resected. An extension of the AFB with the use of prosthetic vascular graft was performed. The common femoral artery was ligated below the inguinal ligament. Necrotic skin from the groin was resected, and the wound was closed with a patent vascular reconstruction. Results: The patient was discharged on the 7th postoperative day with low infection's parameters. Currently we have eleven months of follow-up on the patient. The patient has a patent vascular reconstruction with- out any signs of anastomotic APSA on Doppler's ultrasonography. Conclusion: The most frequent site of APSA occurrence after AFB is the femoral region. Endovascular treat- ment can be used only in very selected cases with a very high risk of complications related to the endovascular devices.
Pseudoaneurysma aorty je vzácnou, avšak život ohrožující komplikací, s níž se lze setkat několik měsíců nebo i let po operaci na aortě. I když je kardiochirurgický výkon nejčastější příčinou, může k vzniku pseudoaneurysmatu vzácně dojít i po úrazu a infekci. Klinické projevy se mohou pohybovat v rozmezí žádné symptomy až katastrofální výsledek. Průměr a velikost pseudoaneurysmatu aorty se mohou postupem let zvětšovat, což tak může zvyšovat riziko jeho náhlé ruptury a krvácení. Popisujeme případ pseudoaneurysmatu vzestupné aorty, vzniklého 13 měsíců po koronárním bypassu a úspěšně řešeného chirurgicky. Diagnózu aneurysmatu je nutno stanovit v perioperačním i pooperačním období včas, protože jeho ruptura by měla katastrofální důsledky.
Aortic pseudoaneurysm is rare but life-threatening complication occurring several months or years after aortic surgery. Although cardiac surgery is the most common cause, it may rarely develop after trauma and infection. Clinical presentation can vary from asymptomatic to catastrophic outcomes. The diameter and size of the aortic pseudoaneurysm may increase over the years, which could consequently increase the risk of sudden rupture and hemorrhage. We present a case of an ascending aortic pseudoaneurysm developed 13 months after coronary artery bypass grafting that were successfully managed by surgical repair. It is essential to diagnose the aneurysm in a timely manner during the perioperative and postoperative periods because rupture of the aneurysm could be catastrophic.

The paper presents the results of treating 14 patients, namely eight patients with visceral artery aneurysms and six patients with visceral artery pseudoaneurysms. In 64.3% of the patients, the initial diagnosis was made based on the ultrasound examination. All the patients (100%) underwent CT angiography, while angiography was performed in 71.4% of the cases. Five (35.7%) patients with visceral artery pseudoaneurysms were emergently hospitalized; among them, the signs of bleeding were observed in 2 patients. In 9 patients, pathology was detected during tests for other conditions. Five (35.7%) patients underwent endovascular treatment, while 9 (64.3%) patients received surgical treatment. Endovascular interventions and open surgery demonstrated a nil mortality rate. After endovascular treatment, stent thrombosis was found in 1 patient. In the case of surgical treatment, visceral artery aneurysm was observed in 1 patient who underwent the resection of superior mesenteric artery pseudoaneurysm. Conclusions. The choice of the method of treating visceral artery aneurysms and visceral artery pseudoaneurysms depends on the location, size, anatomic features of the visceral arteries and the clinical course of the disease. Both endovascular and surgical treatment demonstrate good postoperative outcomes. Visceral ischemia is one of the most serious complications in the postoperative period, which can complicate both the diagnosis and the choice of treatment tactics.
