Úvod: Intramyokardiální disekující hematom (IDH) je dutina ve stěně srdce vyplněná krví, která vzniká oslabením stěny nekrózou po infarktu myokardu. IDH se může vytvořit ve stěně levé komory, pravé komory nebo interventrikulárního septa. Popis případu: Kazuistika popisuje pacienta s IDH jako vzácnou komplikaci po infarktu myokardu. Pacient byl hospitalizován pro trvající obtěžující kašel tři měsíce po prodělání infarktu myokardu řešeném implantací stentu. Dle echokardiografie bylo podezření na krytou rupturu volné laterální stěny s obrazem pseudoaneu- rysmatu, pro kterou bylo rozhodnuto o operačním řešení. Při operaci byl nález plošné dutiny ve stěně levé komory komunikující otvorem s levou komorou. Vnitřní i vnější stěna dutiny obsahovala srdeční svalovinu, a tak byla dutina diagnostikována jako IDH. Otvor i dutina byly uzavřeny stehy přes meshové podložky a suturou přes meshový proužek. Pooperační průběh se obešel bez komplikací a pacient byl propuštěn osmý den po operaci. Diskuse: Diagnostika IDH se zakládá na anamnéze infarktu myokardu a na nálezech na echokardiografii. Mezi známky echokardiografického obrazu IDH patří vytvoření nové dutiny uvnitř srdeční stěny s echolucentním středem. Diferenciální diagnostika IDH zahrnuje aneurysma, pseudoaneurysma, trombus uvnitř dutiny LK nebo trabekuly komor. Závěr: IDH je komplikace infarktu myokardu, na kterou je třeba pomýšlet při diferenciální diagnostice nově vzniklé dutiny ve stěně levé komory na ultrazvukovém vyšetření. Konzervativní řešení je možné u IDH malých rozměrů, u kterých nedochází k progresi. U hemodynamické nestability pacienta nebo u velkých nebo progredujících IDH je namístě chirurgické řešení.
Background: Intramyocardial dissecting hematoma (IDH) is blood-filled cavitation in the cardiac wall that can occur as a complication of myocardial infarction, chest trauma, or percutaneous intervention. It can develop in the left ventricular free wall, the right ventricle, or the interventricular septum. Case report: The case report describes a patient with IDH as a rare complication of myocardial infarction. The patient was hospitalized for ongoing irritating cough three months after ST elevated myocardial infarction treated with percutaneous coronary intervention. The echocardiography showed the image of a pseudoaneu- rysm of the left ventricle. The patient underwent surgery where a cavitation inside the left ventricle myocardial wall communicating with the left ventricle was found. The inner and outer border of the cavitation included myocardium, therefore diagnosis of IDH was established. The communication and the cavitation were closed with pledgeted sutures and a direct suture with a mesh stripe. Postoperative hospitalization was without complications and the patient was dismissed the eighth day after the surgery. Discussion: Diagnosis of IDH consists of anamnestic information of myocardial infarction and assessment on echocardiography. On echocardiography the IDH shows as a neocavitation within the heart wall with an echo-lucent center. The differential diagnosis of IDH includes aneurysm, pseudoaneurysm of the heart, thrombus of the ventricle and trabeculae. Conclusion: The IDH is a post-infarction complication important to include in the differential diagnosis of newly formed cavitation inside the ventricle wall seen on echocardiography. Conservative management is suggested when the IDH is small and not growing. Surgery is indicated if the patient is unstable or the IDH is large or growing.

The aim of this study was to evaluate the patency of the proximal and distal radial artery after coronary procedures performed via the distal radial artery (DRA). Ultrasound (US) as the most reliable method was used to diagnose radial artery occlusions (RAO). We evaluated 115 patients who underwent catheterization via distal radial access (dTRA). Following the procedure and after successful hemostasis (80 ± 36 min), arterial patency and diameter at conventional transradial access (cTRA) and distal puncture sites (either in the anatomical snuffbox or the dorsal distal RA) were assessed. No RAO were found in the proximal or distal RA and there were no significant other complications. The mean diameter of the radial artery at conventional puncture site was 2.86 ± 0.49 mm and at distal puncture site 2.31 ± 0.47 mm (p < 0.001). Postprocedural compression time of dTRA was very short. In conclusion distal radial access was associated with the absence of early arterial occlusion, significant local bleeding and other relevant complications.

Background: Despite the enormous benefits of radial access, this route is associated with a risk of radial artery occlusion (RAO). Objective: We compared the incidence of RAO in patients undergoing transradial coronary angiography and intervention after short versus prolonged hemostasis protocol. Also we assessed the efficacy of rescue 1-hour ipsilateral ulnar artery compression if RAO was observed after hemostasis. Material and Methods. Patients referred for elective transradial coronary procedures were eligible. After 6 F radial sheath removal, patients were randomized to short (3 hours) (n = 495) or prolonged (8 hours) (n = 503) hemostasis and a simple bandage was placed over the puncture site. After hemostasis was completed, oximetry plethysmography was used to assess the patency of the radial artery. Results: One thousand patients were randomized. Baseline characteristics were similar between both groups with average age 61.4 ± 9.4 years (71% male) and PCI performed on half of the patients. The RAO rate immediately after hemostasis was 3.2% in the short hemostasis group and 10.1% in the prolonged group (p < 0.001). Rescue recanalization was successful only in the short group in 56.2% (11/19); at hospital discharge, RAO rates were 1.4% in the short group and 10.1% in the prolonged group (p < 0.001). Conclusion: Shorter hemostasis was associated with significantly less RAO compared to prolonged hemostasis. Rescue radial artery recanalization was effective in > 50%, but only in the short hemostasis group.

Úvod: Indikace k léčbě výdutě podkolenní tepny jsou jasně dány. U výdutě s průchodnými přívodnými a odvodnými tepnami je riziko periferní embolizace z nástěnného trombu výdutě vysoké a léčba, většinou cévní intervence, je zaměřena především na prevenci této končetinu ohrožující komplikace. Není ale jasné, jak velké je riziko periferní embolizace, a jak tedy postupovat v případě průtočné výdutě podkolenní tepny plněné hlubokou stehenní tepnou při chronickém uzávěru povrchní stehenní tepny. Metody: V databázi II. chirurgické kliniky FN Olomouc byli vyhledáni nemocní, u kterých byla v letech 2015 až 2019 diagnostikována výduť podkolenní tepny. Z nich byli vybráni nemocní s průtočnou výdutí podkolenní tepny a současně chronickým uzávěrem povrchní stehenní tepny na stejnostranné končetině. Výsledky: Diagnostikovali jsme 66 nemocných s 85 výdutěmi podkolenních tepen. Čtyři nemocní měli průtočnou výduť podkolenní tepny a chronický uzávěr povrchní stehenní tepny na stejnostranné končetině. U těchto nemocných jsme po zjištění diagnózy postupovali konzervativně. U tří nemocných nedošlo ke klinicky zjevné komplikaci výdutě. U jednoho nemocného došlo po 21 měsících dispenzarizace k uzávěru výdutě trombózou s následným zkrácením klaudikačního intervalu, který nemocného limituje. Závěr: Naše zkušenosti z malého souboru nemocných ukazují na možnost postupovat u nemocných s průtočnou výdutí podkolenní tepny, nad kterou je povrchní stehenní tepna chronicky uzavřená, primárně konzervativně. Trombózu výdutě v dalším průběhu je možné předpokládat. K chirurgické intervenci jsou indikováni nemocní, u kterých trombóza výdutě vedla k limitujícím obtížím. Riziko periferní embolizace z nástěnného trombu výdutě nelze jednoznačně vyloučit. K přesnějšímu doporučení léčebného postupu jsou zapotřebí další studie na velkých souborech nemocných.
Introduction: The indications for popliteal artery aneurysm treatment are clear. In aneurysms with patent inflow and outflow arteries, the risk of peripheral embolisation from a mural thrombus is high and the treatment, mostly a vascular intervention, is focused on preventing this extremity-threatening complication. It is unclear, however, how high the risk of peripheral embolisation is and how to proceed with a patent popliteal artery aneurysm fed by the deep femoral artery when the superficial femoral artery is chronically occluded. Methods: All patients diagnosed with popliteal artery aneurysm between 2015 and 2019 were searched in the database of the Department of Surgery II of University Hospital Olomouc. Patients with a patent popliteal artery aneurysm and chronic superficial femoral artery occlusion in the ipsilateral extremity were selected. Results: We diagnosed 66 patients with 85 popliteal artery aneurysms. Four patients had a patent popliteal artery aneurysm and chronic superficial femoral artery occlusion in the ipsilateral extremity. In these patients, conservative treatment was indicated after the diagnosis was determined. In three patients, no clinically obvious complication of the popliteal artery aneurysm occurred. In one patient, popliteal artery aneurysm thrombosis occurred after a follow-up of 21 months, leading to a shortening of his calf claudication distance and limiting the patient. Conclusion: Our experience with this small group of patients shows the possibility of primary conservative treatment in patients with a patent popliteal artery aneurysm below the chronic superficial femoral artery occlusion site. Aneurysm thrombosis can be expected during follow-up. Patients in whom the thrombosis leads to limitations are indicated for surgical intervention. The risk of peripheral embolisation from the mural thrombus cannot be excluded with certainty. Further studies involving large groups of patients are needed to provide a more precise recommendation.

BACKGROUND: Ankle brachial index (ABI) is the principal screening method for peripheral arterial disease (PAD). In this study, we compare various types of Doppler-derived and oscillometric ABIs with results obtained through duplex ultrasonography. METHODS: 62 patients were enrolled in the study. For each limb, blood pressures for both ankle arteries and the arm were measured using Doppler and an automated oscillometric device. Duplex ultrasound was performed for all limbs and occlusions >50% were considered PAD-positive. ABI was calculated using both higher (HABP) and lower (LABP) arterial blood pressure on the individual limbs and the ability to predict duplex-detected stenoses was evaluated. RESULTS: LABP calculation provided results superior to the guideline-recommended HABP. Considering patients with ABI >1.4 or measurement failure as PAD-positive further enhanced the test parameters. The higher ABI cut-off of 1.0 resulted in somewhat better sensitivities (max 92%) and negative predictive values (max 87%) at the expense of a substantial increase in the number of false positives. Oscillometric method yielded poor sensitivities but very good specificities (max 94%) and positive predictive values (max 90%). CONCLUSIONS: Doppler-based LABP provides better results than the guideline-recommended HABP in diabetic patients, nevertheless even this method is not perfect. Increasing the cut-off value to 1.0 in these patients does not bring a substantial improvement of the test performance. Patients with high ABI should be automatically considered PAD-positive and referred for further investigation using imaging techniques.

Entrapment syndróm arteria popliteaje zriedkavá príčina ischémie hlavne u mladýchšportovcov. Spôsobený je atypickým vzťahom medzi popliteálnou artériou a svalmi vofossa poplitea, ktorý spôsobuje znížený tok krvi do postihnutej končatiny s klaudikáciami,bolesťami dolnej končatiny pri športe a tiež chladom končatiny. Práca prezentuje kazu-istiku mladého pacienta s akútnou končatinovou ischémiou na podklade entrapmentsyndrómu.
Popliteal artery entrapment syndrome is a rare cause of ischaemia mainly in young ath-letes. It is caused by an atypical relation between the popliteal artery and muscles inpopliteal fossa, which results in decreased blood flow to the affected leg with claudication,leg pain on exertion and coldness of the leg. This case report presents a young patientwith acute limb ischaemia based on popliteal artery entrapment.

Fibromuskulárna dysplázia je nezápalová arteriopatia s fokálnou prestavbou cievnej steny, ktorá vyúsťuje do stenóz, oklúzií, disekcií či aneuryziem. Ochorenie môže postihnúť prakticky každé arteriálne povodie. V dospelej populácii sa vyskytuje najmä v skupine mladých žien a postihuje prevažne renálne povodie. Autori prezentujú prípad dieťaťa so závažnou perinatálnou anamnézou, u ktorého sa rozvinul klinický obraz rýchlo progredujúceho ischemického postihnutia CNS, sprevádzaný farmakorezistentnou hypertenziou, stenózou renálneho, karotického aj vertebrálneho riečiska a veľmi pravdepodobne aj splanchnických a končatinových artérií. Ochorenie malo u dieťaťa malígny priebeh s fatálnym koncom, čo nie je typický obraz ochorenia u dospelých, ale býva prítomný v detskej populácii. U dospelých ide síce o raritné, ale dobre opísané ochorenie. V detskom veku však zriedkavosť ochorenia a jeho špecifiká môžu naznačovať, že by napriek rovnakým histopatologickým nálezom mohlo ísť o samostatnú nozologickú jednotku.
Fibromuscular dysplasia in a 2 years old child – casuistics Fibromuscular dysplasia is an idiopathic, non-inflammatory disease of small and middle-sized arteries at which the focal rebuilding of vessel walls eventuates into their stenoses, oclusias, dissections or aneurysms. The disease may affect virtually every arterial basin. In adult population it occurs mostly in the group of young females and affects mostly the renal basin. The authors present a case of a child with serious perinatal anamnesis that developed a clinical appearance of quickly progressing ischemic affected CNS. It was accompanied by uncorrectable hypertension, stenotic affection of renal, carotidal and vertebral basin and very probably splanchnic and extremital arteries. The disease of the child had malign course and ended fatally. That is not typical in adult population, however it occurs in child population. In adult population it is a rare, nevertheless well described disease. The rareness and specifics of the disease in kids might point to it being a separate unit despite identical histopathological findings.
