V přehledném článku je uvedena stručná historie objevení aterosklerózy a jejích rizikových faktorů. Současný pohled na patofyziologii vychází z nedávného konsenzu Evropské společnosti pro aterosklerózu (European Atherosclerosis Society, EAS), v němž je LDL cholesterol hlavním kauzálním rizikovým faktorem pro aterosklerózu a ateroskleróza velmi častým onemocněním, postihujícím ve středním věku 71 % mužů a43 % žen. Prevence aterosklerózy a její léčba se opírají o celkové kardiovaskulární riziko a ovlivnění všech přítomných rizikových faktorů, přičemž hlavním cílem je velké snížení hodnoty LDL chole-sterolu. Léčbu statinem samostatně nebo v kombinaci se zetimibem lze doplnit léčbou inhibitory PCSK9. Přestože existují důkazy z experimentálních, klinických i epidemiologických studií o prospěšnosti léčby hypolipidemiky, v poslední době se objevují tendence zpochybnit tuto léčbu včetně zpochybnění „cholesterolové hypotézy“ aterosklerózy.
Brief history of atherosclerosis discovery and its risk factors are presented in this summarizing article. Contemporary view on the atherosclerosis pathophysiology proceeds from the EAS consensus, in which is LDL‑cholesterol considered as the main causal risk factor for atherosclerosis and atherosclerosis as avery frequent disease occuring in middle aged 71 % male and 43 % female. Atherosclerosis prevention and treatment are based on the global cardiovascular risk assessment and on the influence of all present risk factors, esp. on the intensive reduction of LDL‑cholesterol as aprimary target. If is statin monotherapy or combination with ezetimibe not successful, PCSK9 inhibitors are recommended to add. Except of abig evidence from experimental, clinical and epidemiological studies about the lipid modifying therapy profit exists, some disputes to that therapy including the „cholesterol hypothesis“ of atherosclerosis are appearing.

Východiska: Počty pacientů s aterosklerózou a demencí narůstá a to zejména z důvodu prodlužování střední délky života a stárnutí populace.Obě dvě onemocnění spolu úzce souvisí a zároveň výrazně ovlivňují kvalitu života pacientů.Cíl: Cílem tohoto sdělení je seznámení s touto aktuální problematikou a to napříč odbornostmi.Metody: Data byla čerpána z aktuálních publikací, zejména z databáze PubMed. Dále byly využity další databáze a výsledky klinických pokusů a to zahraniční i české.Výsledky: Výzkumy naznačují, že aterosklerotické postižení karotid je možným prediktorem rizika rozvoje de-mence. Projevy aterosklerózy karotid jsou spojeny s projevy kognitivního deficitu a demencí. Lze tedy tedy usu-zovat, že jsou ateroskleróza a demence spolu úzce spjaty.Závěry: Střední délka života bude mít pravděpodobně nadále vzrůstající tendenci, proto bude narůstat i počet pacientů s aterosklerózou a demencí. Z tohoto důvodu je velmi důležitý epidemiologický výzkum, který se uve-denou problematikou zabývá. Na základě výzkumu pak lze formulovat efektivní prevenci, diagnostiku a léčbu těchto stavů.
Background: The number of patients with atherosclerosis and dementia is increasing, mainly due to the increase in life expectancy and population aging. Both diseases are closely related and at the same time significantly affect the quality of life of patients.Objective: The aim of this communication is to present this topical issue across the field of expertise.Methods: Data were taken from current publications, especially from PubMed database. Furthermore, other databases and results of clinical trials were used, both foreign and Czech.Results: Research suggests that atherosclerotic involvement of carotid arteries is a potential predictor of the risk of developing dementia. The manifestations of carotid atherosclerosis are associated with cognitive deficits and dementia. Identified risk factors for atherosclerosis often occur in patients with dementia. It can therefore be concluded that they are closely linked.Conclusions: Life expectancy is likely to continue to increase, so the number of patients with this disease will increase. That is why epidemiological research is needed to address this issue. Based on the research, effective prevention, diagnosis and treatment of these conditions can be formulated.

IL-1 sa zúčastňuje regulácie veľkého množstva proinflamatórnych stavov. Z rodiny cytokínu IL-1 kľúčovú úlohu zohráva predovšetkým prozápalový cytokín IL-1β, syntetizovaný v imflamazómoch. Kanakinumab je humánna monoklonálna protilátka proti IL-1β. uplatňuje sa v liečbe autoinflamačných stavov a určitých systémových ochorení spojiva. Z perspektívnych indikácii sa najsľubnejšou javí jeho aplikácia v kontexte aterosklerózy. Hoci v posledných desaťročiach sa viacerými štúdiami potvrdila dôležitosť lipidovej hypotézy, pretrvávajúca prie-merná 40% kardiovaskulárna mortalita naďalej zostáva nad pásmom očakávaného optima. To viedlo v poslednej dobe k ďalším intenzívnym výskumom. V tomto kontexte sa nedávno prezentovali výsledky štúdie CANTOS, prostredníctvom ktorej sa rehabilitovala zápalová hypotéza aterosklerózy. Štúdia CANTOS predstavuje v kontexte zápalovej hypotézy aterosklerózy historický miľník, ktorá otvorila úplne nový prístup k liečbe aterosklerózy – imunomoduláciu.
IL-1 participates in the regulation of many proinflammatory diseases. From the cytokine family of IL-1, the key role in proinflammatory dise-ases is attributed to IL-1β, produced in inflammasomes. Canakinumab is a human monoclonal antibody against IL-1β, applied in the treamtent of many autoinflammatory and systemic connective tissue diseases. Atherosclerosis seems to be the next perspective indication. Despite the outcomes of trials for atherosclerosis in the last decades confirmed the importance of lipid hypotesis, average 40% cardiovascular mortality still remains above the optimal goal. This was the main trigger to initiate new researches in this field. Recently presented outcomes of the CANTOS trial rehabilitated the inflammatory hypothesis of atherosclerosis. CANTOS tial represents in the context of inflammmatory hypothesis of athe-rosclerosis a historic checkpoint, which opened a new approach in the treatment of atherosclerosis – immunomodulation
