Cíl: Cílem studie bylo identifikovat a analyzovat faktory, které souvisí se zátěží a stresem rodičů handicapovaných dětí. Metody: Do systematického přehledu byly zahrnuty plnotextové přehledy primárních výzkumů, které byly publikovány v anglickém jazyce v období 2005-2021. K vyhledávání byly použity volně přístupné i licencované databáze CINAHL EBSCO, SCOPUS, Wiley Library Online, ProQuest a Web of Science. Výsledky: Z literárního přehledu vyplynulo, že během patnácti let byla publikována řada studií, které se zabývaly faktory způsobujícími stres a zátěž rodičů, kteří pečují o děti s handicapem. Bylo nalezeno celkem 325 studií. Ve čtyřech studiích byl potvrzen pozitivní vliv na snížení zátěže a stresu. V osmi studiích bylo identifikováno patnáct faktorů zvyšujících stres a zátěž rodičů dětí s handicapem. Mezi pozitivní faktory patřilo využívání respitní péče, pořízení psa, vládní finanční podpora a náboženství. Mezi negativní faktory patřila inkontinence dítěte, neúplná rodina, dětská deprese, poruchy učení, neobvyklé chování dítěte na veřejnosti, zhoršená kognitivní schopnost dítěte, Aspergerův syndrom, ženské pohlaví, prestižnější a lukrativnější povolání rodičů, nízká úroveň podpory příbuzných při péči o dítě, noční pomočování, obtížné chování dítěte a vysoké nároky na péči o dítě, pandemie covidu-19, obezita u dítěte. Závěr: Faktory působící na rodiče mohou mít pozitivní i negativní vliv na vývoj a výchovu dítěte. Velmi důležitá je včasná identifikace negativních faktorů, které mohou způsobit zátěž a stres rodičům handicapovaných dětí. Dětská sestra tvoří mezičlánek mezi rodiči, lékařem a dítětem. Proto by se měla podílet nejen na ošetřovatelské péči, ale také na včasném záchytu a identifikaci rizikových faktorů. Zdravotníci by měli poskytovat rodičům pomoc ve všech aspektech péče o děti.
Aim: The aim of the study was to identify and analyse factors which are related to burden and stress of parents with disabled children. Methods: The systematic review includes full-text primary research papers, published in English, in the period 2005-2021. Freely accessible as well as licensed databases CINAHL EBSCO, SCOPUS, Wiley Library Online, ProQuest a Web of Science have been used for the search. Results: The literature review shows that over a period of fifteen years, numerous studies have been published that address the factors causing stress and burden n parents taking care for children with disabilities. A total of 325 studies have been found. In four studies, a positive effect on the reduction of burden and stress was confirmed. In eight studies, fifteen factors increasing the stress and burden of parents of children with disabilities were identified. Positive factors included using respite care, acquiring a dog, governmental financial support and religion. Negative factors included the child incontinence, single-parent family, child depression, learning disorders, unusual behaviour of the child in public, impaired cognitive ability of the child, Asperger syndrome, female gender, more prestigious and lucrative occupation of parents, low level of support from relatives in taking care of the child, night enuresis, difficult behaviour of the child and high demands on child care, the COVID-19 pandemic, obese child. Conclusion: Factors affecting parents can have positive as well as negative influence on the education of the child. Early identification of negative factors that can cause burden and stress on parents of disabled children is very important. The paediatric nurse acts as a link between the parents, the doctor and the child. Therefore, it should be involved not only in nursing care, but also in the early detection and identification of risk factors. Healthcare professionals should help parents in all areas of childcare.
The development of attachment during the perinatal and/or postnatal periods between a child their mother, father and family as whole can be observed. This attachment provides the child with the safety and security feelings and may provide satisfaction to the mother and other family members. In children with physical and mental developmental disability, attachment may be challenged immediately after birth. It is typically recovered after a certain period. The authors describe the development of attachment of family members to children with somatic, mental or social disability based on an analysis of nine case reports. They conclude that the attachment of parents/mothers to their child with a disability is typically established after a certain period of time only. In the first period, there is a caregiving attitude of the mother even though the period of the origination of this tie takes place later. The mother is most frequently the person establishing attachment. However, this is not always the rule. In our cases, there were two fathers, who made the attachment earlier than respective mothers. Attachment was also evident in some grandparents and siblings. The parents, mostly the fathers, can lose the attachment established. Analysis of the degree of attachment to the child by parents may be complicated due to difficulty in assessing the extent and degree of the attachment of children with severe disabilities to their parents.

Cíl: Cílem výzkumu bylo zjistit souvislosti mezi kvalitou života dětí s chronickým onemocněním, kvalitou života rodičů a používanými copingovými strategiemi v rodině. Design: Výzkum byl realizován formou jednorázového průřezového šetření. Metody: Výzkum se uskutečnil formou dotazníkového šetření u chronicky nemocných dětí (n = 288) ve věku 2–12 let a jejich rodičů. Bylo využito dotazníků: Pediatric Quality of Life Inventory, Pediatric Quality of Life Inventory Family Impact Module a Family Crisis Oriented Personal Evaluation Scales. Výsledky: Souvislost mezi kvalitou života dětí s diabetem a kvalitou života rodičů, rodinným fungováním a dopadem nemoci na rodinu byla nízká. Děti s diabetem měli nejvyšší kvalitu života, ale jejich rodiče nejnižší z posuzovaných skupin onemocnění. Nejnižší celkové skóre kvality života dětí bylo zjištěno u dětí s JIA. Nejsilnější pozitivní asociace interních copingových strategií (emočně-kognitivní rekonstrukce a neaktivní řešení problému) a kvality života dětí a rodičů se ukázala u rodin dětí s astmatem. Závěr: Nízká asociace kvality života dětí a rodičů s copingovými strategiemi ukázaly jejich nízkou efektivitu. Tyto výsledky jsou východiskem pro volbu relevantních intervencí podporujících volbu nových copingových strategií.
Aim: The survey aimed at determining the associations between the quality of life of children with chronic diseases, the quality of life of their parents and coping strategies used in the family. Design: A cross-sectional study. Methods: The study was performed as quantitative research using a questionnaire survey in chronically ill children (n = 288) aged 2-12 years and their parents. The following instruments were used: Pediatric Quality of Life Inventory, Pediatric Quality of Life Inventory Family Impact Module and Family Crisis Oriented Personal Evaluation Scales. Results: The association between the quality of life of diabetic children and their parents' quality of life, family functioning and the impact of the illness on the family was weak. Children with diabetes had the highest quality of life but the quality of life of their parents was lowest among all disease groups in the study. The lowest overall quality of life of children was noted among those with juvenile idiopathic arthritis. The strongest positive associations between internal coping strategies (i.e. reframing and passive appraisal) and children's and parents' quality of life was observed in the families of children with asthma. Conclusions: The weak association between children's and parents' quality of life and coping strategies pointed to their low effectiveness. The results suggest that relevant interventions should be used that promote selection of new coping strategies.

Cíl: Cílem přehledové studie je analýza aktuálních literárních studií, které hodnotí potřeby dětí s onkologickým onemocněním. Design: Přehledová studie. Metodika: K vyhledávání potřebných dat byly využity volně dostupné a licencované databáze (Medline, Proquest, Science Direct, Scopus, Ebsco, Google Scholar, Web of Science, Willey, Cinahl). Výsledky: Literární analýzou bylo zjištěno, že problematikou hodnocení potřeb dětí s onkologickým onemocněním se v zahraničí zabývalo několik autorů. Konkrétně bylo vyhledáno 8 studií, publikovaných v letech 1993–2011. Potřeby dětí s onkologickou diagnózou byly hodnoceny jak kvalitativními, tak kvantitativními výzkumnými metodami. Jednotlivé oblasti bio-psycho-socio-spirituálních potřeb byly autory studií zkoumány vždy odděleně. Žádná z nalezených studií nehodnotila potřeby dětí komplexně ve všech cílových oblastech. Závěr: Problematika hodnocení potřeb dětí s onkologickým onemocněním je velmi často diskutována a zkoumána. Zahraniční studie se zabývají různými oblastmi potřeb, od potřeb biologických, až po potřeby spirituální. Přičemž největší důraz by měl být kladen na potřeby biologické v důsledku četných nežádoucích účinků onkologické léčby. Do budoucna by bylo vhodné vyvinout a validizovat hodnotící nástroj, který by prioritně zmapoval biologické potřeby dětí s onkologickým onemocněním. Nejvíce efektivní by byl ovšem bezesporu hodnotící nástroj, který by zmapoval potřeby dětí komplexně.
Aim: The aim of the survey study was to analyze published studies assessing the needs of children with cancer. Design: Literature review. Methods: Freely available and licensed databases (MEDLINE, ProQuest, ScienceDirect, Scopus, EBSCO, Google Scholar, Web of Science, Wiley and CINAHL) were searched for necessary data. Results: The literature analysis revealed that needs assessment of children with cancer was described by several authors. Specifically, eight studies were found that were published between 1993 and 2011. The needs of children with cancer diagnosis were evaluated by both qualitative and quantitative research methods. Individual areas of bio-psycho-socio-spiritual needs of the study were investigated separately by the authors. None of the studies evaluated children's needs comprehensively in all the areas. Conclusion: The issue of assessing the needs of children with cancer is often discussed and examined. Studies cover different needs, from biological to spiritual needs. The greatest emphasis should be placed on biological needs due to many side effects of cancer treatment. In the future, it would be appropriate to develop and validate an evaluation tool that would map biological needs of children with cancer. The most effective way would be an assessment tool which would map the needs of children in a comprehensive way.

Cíl: Cílem výzkumu bylo zjistit a popsat problémy a zkušenosti rodinných příslušníků pečujících o dítě s život ohrožující nebo život limitující nemocí a to od stanovení diagnózy až po případné úmrtí dítěte. Design: kvalitativní fenomenologická studie. Metodika: Výzkumný soubor tvořilo osm rodinných příslušníků dítěte s život limitujícím nebo život ohrožujícím onemocněním se zkušeností s přímou péčí o nemocné dítě. Pro sběr dat byla použita technika nestandardizovaného rozhovoru. Výsledky: Na základě analýzy dat byly stanoveny čtyři hlavní kategorie a podkategorie: 1. začátek nemoci (stanovení diagnózy, prognóza a informovanost), 2. terapeutická fáze (náročnost péče, dopad nemoci a léčby na rodinu), 3. přijetí života s nemocí (pocity a emoce, smíření) a 4. umírání a smrt (důstojné umírání, období po smrti). Závěr: Vážné onemocnění dítěte je pro rodiče náročnou životní situací, která je spojená se změnami v jejich osobním i rodinném životě. Pečující potřebují psychosociální podporu, která spočívá zejména v poradenské činnosti a vyžaduje uplatnění empatie a otevřené komunikace.
Aim: The aim of the research was to identify and describe the problems and experiences of family members caring for children with life-threatening or life-limiting diseases from diagnosis to the eventual death of the child. Design: A qualitative study using grounded theory method. Methods: The sample consisted of eight family members with experience of direct care of children with life-limiting or life-threatening disease. For data collection a technique of non-standardized interview was used. Results: Based on data analysis, four main categories and subcategories were defined: 1. onset of the disease (diagnosis, prognosis and awareness), 2. therapeutical phase (demands of care, the impact of disease and treatment on the family), 3. the acceptance of life with disease (feelings and emotions, coming to terms with disease) and 4. dying and death (dignified dying, the period after death). Conclusion: Serious disease in a child is a difficult life situation for parents, associated with changes in their personal and family life. Carers need psychosocial support, especially in the form of advice, and require the exercise of empathy and open communication.

The issue of behavioral disorders of intact children and their upward trend are well known. The discussion about behavioral disorders of children with mental disabilities is less common. Their cause is not a mental disability as such, but it is a separate disorder and therefore we can talk about multiple disabilities. Children with such disabilities need interdisciplinary and special approach, which can help to alleviate and eliminate consequences of disorders. This report is focused on some actual problems of care for children with mental disabilities and behavioral disordes. Here are some characteristics of children with mental disability and some opportunities for further development in order to enhancing scientific knowledge in this area. Our aim is also to present our views on the issue.
