INTRODUCTION: Trigeminal neuralgia (TN) secondary to tumors is encountered in up to 6% of patients with facial pain syndromes and is considered to be associated with tumors affecting the trigeminal nerve pathways. The most frequent are meningiomas and vestibular schwannomas (VS). Stereotactic radiosurgery (SRS) has emerged as a valuable treatment, with heterogeneity of clinical results. We sought to review the medical literature on TN treated with SRS for meningiomas and VS and investigate the rates of improvement of TN symptoms. METHODS: We reviewed articles published between January 1990 and December 2019 in PubMed. Pain relief after SRS, the maintenance of pain relief, and TN recurrence and complications were evaluated with separate meta-analyses, taking into account the data on individual patients. RESULTS: Pain relief after SRS was reported as Barrow Neurological Institute (BNI) pain intensity scores of BNI I in 50.5% (range 36-65.1%) of patients and BNI I-IIIb in 83.8% (range 77.8-89.8%). There was no significant difference in series discussing outcomes for tumor targeting versus tumor and nerve targeting. Recurrences were described in 34.7% (range 21.7-47.6; tumor targeting). Maintenance of BNI I was reported in 36.4% (range 20.1-52.7) and BNI I-IIIb in 41.2% (range 29.8-52.7; tumor targeting series). When both the nerve and the tumor were targeted, only 1 series reported 86.7% with BNI I-IIIb at last follow-up. Complications were encountered in 12.6% (range 6.3-18.8; tumor targeting series) of patients; however, they were much higher, as high as 26.7%, in the only study reporting them after targeting both the nerve and the tumor. The most common complication was facial numbness. CONCLUSION: SRS for TNB secondary to benign tumors, such as meningiomas and VS, is associated with favorable clinical course, but less favorable than in idiopathic TN. There was, however, heterogeneity among reports and targeting approaches. Although targeting both the nerve and the tumor seemed to achieve better long-term results, the rate of complications was much higher and the number of patients treated was limited. Future clinical studies should focus on the standard reporting of clinical outcomes and randomization of targeting methods.

V léčbě plicního karcinomu je základní modalitou radioterapie, která má prokázaný léčebný prospěch v radikálních i paliativních indikacích až u 76 % všech nemocných. V České republice dlouhodobě nepřekračuje podíl nemocných s bronchogenním karcinomem léčených radioterapií 25 %. V případě klinicky inoperabilního nemalobuněčného plicního karcinomu ve stadiu I je metodou volby stereotaktická radioterapie, která umožňuje lokální kontrolu až 90 % po třech letech a účinností je srovnatelná s operací, s lepší tolerancí. Pooperační radioterapie je vhodná v případě postižených mediastinálních uzlin. U lokálně pokročilého inoperabilního NSCLC je standardem konkomitantní chemoradioterapie podávaná souběžně s chemoterapií založené na dvojkombinaci cytostatik obsahující platinový derivát, neosvědčila se paušální eskalace dávky nad 60 Gy/6 týdnů pro vzrůst toxicity s vyšším rizikem úmrtí. Zlepšení výsledků léčby přinášejí technické inovace, jako jsou radioterapie s modulovanou intenzitou svazku, radioterapie řízená obrazem nebo sledování dýchacích pohybů v čase. Jednoznačný přínos léčby protonovým svazkem nebyl zatím prokázán. U malobuněčného karcinomu je ve stadiu limited disease nejúčinnější konkomitantní chemoradioterapie, se začátkem ozařování nejpozději od třetího cyklu chemoterapie, v dávce 66 Gy/33 frakcích nebo hyperfrakcionace 2* denně po dobu tří týdnů do dávky 45 Gy. Radioterapie se doporučuje i ve stadiu extensive disease (ED) jako konsolidační léčba po chemoterapii při velmi dobré léčebné odpovědi. V léčbě malobuněčných karcinomů je standardně doporučováno preventivní ozáření mozku, i když studie z poslední doby jako alternativu uvádějí pečlivé a časté sledování pomocí magnetické rezonance a včasnou léčbu v asymptomatickém stadiu. U nemalobuněčného plicního karcinomu snižuje preventivní ozáření podíl nemocných s rozvojem metastáz v mozku z 30 na 8 %, ale prodloužení přežití tím nebylo prokázáno.
In the treatment of pulmonary cancer, radiotherapy is a basic method that has demonstrated therapeutic benefit in both radical and palliative indications in up to 76% of all patients. In the Czech Republic, the proportion of patients with bronchogenic carcinoma treated with radiotherapy does not exceed 25%. In the case of clinically inoperable non-small cell lung cancer (NSCLC) in stage I, stereotactical radiotherapy is the method of choice which allows local control of the disease up to 90% of the patients after three years while it's efficiency is comparable to the surgery with better tolerance. Postoperative radiotherapy is appropriate for the affected mediastinal lymph nodes. In locally advanced inoperable NSCLC, the standard of concomitant chemoradiotherapy is co-administered with chemotherapy based on the two platinum-containing cytostatics. No flat-rate escalation over 60 Gy/6 weeks for increased toxicity with higher risk of death has been demonstrated. Flat-rate escalation over 60 Gy/6 weeks has not been proven useful as it increases the toxicity and the risk of death. Technical innovations, such as beam-modulated radiotherapy, image-based radiotherapy, or breathing movement monitoring over time, improve treatment outcomes. In small-cell carcinoma, at the stage of limited disease, the most effective is concomitant chemoradiotherapy, starting at the latest from the third cycle of chemotherapy, at a dose of 66 Gy/33 fractions or twice daily for 3 weeks to a dose of 45 Gy. Radiotherapy is also recommended at the extensive disease stage as a consolidation treatment after chemotherapy with a very good response. In the treatment of small cell carcinomas, preventive brain irradiation is recommended as standard, although recent studies, as an alternative, include careful and frequent magnetic resonance imaging (MR) and early treatment in an asymptomatic stage. In NSCLC, preventive irradiation reduces the proportion of patients with brain metastasis from 30% to 8%, but survival prolongation has not been demonstrated.

Kazuistika představuje případ pacienta s nemalobuněčným karcinomem plic (non-small cell lung cancer, NSCLC) klinického stadia IA3, který odmítl chirurgické odstranění tumoru. Místo operace byla pacientovi nabídnuta varianta stereotaktické radioterapie, se kterou pacient souhlasil a následně podstoupil bez komplikací. Při následných kontrolách byla pozorována na CT (výpočetní tomografie) snímcích parciální remise. Po jednom roce od terapie je pacient bez známek progrese či generalizace. Případ tak představuje alternativní možnost ke standardní operační léčbě pro pacienty, u kterých z různých důvodů tato modalita není indikovaná či preferovaná.
Case presents a patient with non-small cell lung cancer at clinical stage IA3, who refused surgical removal of the tumor. Instead of surgery, the patient was offered a variant with stereotactic radiotherapy that the patient agreed to and underwent without complications. Partial remission was observed in subsequent CT scans. One year after the therapy the patient is without signs of progression or generalization. The case therefore presents an alternative to standard surgical treatment for patients for whom, for various reasons, this modality is not indicated or preferred.

Mozkové metastázy představují důležitou a častou indikaci pro moderní radiochirurgii. Tato neinvazivní metoda nabízí dobrou lokální kontrolu s minimální toxicitou s možností opakování ozáření. CyberKnife je přístroj pro bezrámovou celotělovou radiochirurgii využívající obrazem řízenou robotickou technologii s lineárním akcelerátorem jako zdrojem fotonů. V naší prospektivní studii vyhodnocujeme velikost metastatických ložisek na MRI s využitím počítačově asistované volumetrie, přičemž srovnáváme prediktivní hodnotu objemových změn ozářených metastáz. Sledujeme efekt léčby v závislosti na histologickém typu nádoru, počtu mozkových metastáz, pohlaví a vstupním klinickém stavu pacientů. V našich podmínkách unikátní soubor pacientů léčených CyberKnife pro metastatické postižení mozku pak statisticky analyzujeme a výsledky srovnáváme s literárními daty pracovišť využívajících stejnou technologii i referenčními daty center rámové stereotaktické radiochirurgie (gama nůž).
Brain metastases are an important and frequently treated indication in modern radiosurgery. This non-invasive method offersgood local control with minimum toxicity and repeability of treatment. The CyberKnife is a device for frameless whole-bodyradiosurgery using image guided robotic technology and linear accelerator as a photon source. In our prospective study wemeasure the volume of brain metastases on MRI with use of computer-assisted volumetry and we evaluate the predictive valueof these volumetric changes. We track the effect of therapy according to histological type of tumor, number of brain metastases,sex and initial performance status of patients. The group of patients with CyberKnife treateted brain metastases, that is uniquein our conditions, is then statistically analyzed and results are compared to peer-reviewed articles of institutions using the sametechnology and reference data from frame-based (Gamma Knife) radiosurgery centers are matched.

BACKGROUND: Pineal region tumors represent a rare and histologically diverse group of lesions. Few studies are available to guide management and the outcomes after stereotactic radiosurgery (SRS). METHODS: Patients who underwent SRS for a pineal region tumor and for whom at least 6 months of imaging follow-up was available were retrospectively assessed in 5 centers. Data were collected from the medical record and histology level analyses were performed, including actuarial tumor control and survival analyses. RESULTS: A total of 70 patients were treated between 1989 and 2014 with a median follow-up of 47 months. Diagnoses were pineocytoma (37%), pineoblastoma (19%), pineal parenchymal tumor of intermediate differentiation (10%), papillary tumor of the pineal region (9%), germinoma (7%), teratoma (3%), embryonal carcinoma (1%), and unknown (14%). Median prescription dose was 15 Gy at the 50% isodose line. Actuarial local control and survival rates were 81% and 76% at 20 years for pineocytoma, 50% and 56% at 5 years for pineal parenchymal tumor of intermediate differentiation, 27% and 48% at 5 years for pineoblastoma, 33% and 100% at 5 years for papillary tumor of the pineal region, 80% and 80% at 20 years for germinoma, and 61% and 67% at 5 years for tumors of unknown histology. New focal neurological deficit, Parinaud syndrome, and hydrocephalus occurred in 9%, 7%, and 3% of cases, respectively. CONCLUSIONS: SRS is a safe modality for the management of pineal region tumors. Its specific role is highly dependent on tumor histology. As such, all efforts should be made to obtain a reliable histologic diagnosis.

Stereotaxe představuje aplikaci velmi vysokých dávek záření do nevelkých objemů v těle v několika málo frakcích se strmým úbytkem dávky do okolí. Preciznost zajišťuje přesná prostorová lokalizace objemu pomocí 3D zaměřovacího systému. Kybernetický nůž je neinvazívní alternativa chirurgického zákroku u některých zhoubných i nezhoubných ložisek v oblastech celého těla. Hlavní předností tohoto ozařovače je, že dokáže sledovat pozice nádoru během celé doby ozáření. Stereotaktickou radioterapií lze léčit maligní i benigní nádory, arteriovenózní malformace a rovněž některé funkční poruchy.
Stereotaxis refers to applying very high doses of radiation to locations of small volume in the body, in a couple of fractions, with a sharp cut-off of radiation dissipated into the surroundings. Precision is provided by accurate spatial location of the target area using a 3D targeting system. Cybernetic knife is a non-invasive alternative to surgery for certain both malignant and benign tumours in various areas of the body. The main advantage of this emitter is that it can track the location of the tumour during the whole time of irradiating it. Stereotactic radiotherapy can be used to treat both malignant and benign tumours, arteriovenous malformations and some functional disorders as well.

OBJECTIVE The purpose of this study was to evaluate long-term treatment results, radiation-related toxicity, and prognostic factors for the progression-free survival (PFS) of patients with pilocytic astrocytomas treated by means of stereotactic radiosurgery with a Leksell Gamma Knife. METHODS A total of 25 patients with pilocytic astrocytomas underwent Gamma Knife surgery during the period 1992-2002. The median target volume was 2700 mm(3) (range 205-25,000 mm(3)). The 18 patients treated with 5 daily fractions received a median minimum target dose of 25 Gy. Doses for the 2 patients treated with 10 fractions over 5 days (2 fractions delivered on the same day at least 6 hours apart) were 23 and 28 Gy. For the 5 patients treated with a single fraction, the minimum target dose ranged from 13 to 20 Gy (median 16 Gy). RESULTS Complete regression occurred in 10 patients (40%) and partial regression in 10 patients (40%). The 10-year overall survival rate was 96% and the 10-year PFS rate was 80%. Target volume appeared to be a significant prognostic factor for PFS (p = 0.037). Temporary Grade 3 toxicity appeared in 2 patients (8%), and these patients were treated with corticosteroids for 2 months. Permanent Grade 4 toxicity appeared in 2 patients (8%) and was associated with neurocognitive dysfunction. In these 2 individuals, the neurocognitive dysfunction was also felt to be in part the result of the additional therapeutic interventions (4 in one case and 6 in the other) required to achieve durable control of their tumors. CONCLUSIONS Radiosurgery represents an alternative treatment modality for small residual or recurrent volumes of pilocytic astrocytomas and provides long-term local control. Target volume appears to be the most important factor affecting PFS.

Nádory očnice tvoří relativně vzácnou a velmi heterogenní skupinu nádorových expanzí primárně uložených nebo sekundárně se propagujících do prostoru ohraničeného kostěnou orbitou. Primární orbitální nádory jsou vzácnější a většinou benigní, zatímco sekundární nádory mají spíše maligní charakter. Metastázy mohou být uloženy kdekoliv ve stěně anebo uvnitř orbity. Klinickými projevy tumorů jsou nejčastěji exoftalmus, poruchy zraku a diplopie. Nejpřínosnější vyšetření orbitálních patologií je kombinace CT a MR. Cílem léčby je dlouhodobá kontrola nádoru se snahou o uchování nebo zlepšení zraku. V terapii se využívá stejně jako u jiných nádorů observace, chirurgie, chemoterapie i radioterapie s ohledem na histologickou specifikaci a staging. Pokroky v radiodiagnostice, vzrůstající znalosti o přirozeném průběhu onemocnění, přesné radiochirurgické techniky a moderní chemoterapeutické protokoly změnily v posledních desetiletích terapeutické postupy i prognózu u celé řady nádorů očnice. Optimální je léčba vedená prostřednictvím multidisciplinárních týmů ve specializovaných centrech zabývajících se touto problematikou. Klíčová slova: orbita – nádor orbity – exoftalmus – orbitotomie
Orbital tumors constitute a relatively rare and heterogeneous group of lesions, either originating in the orbit or with secondary extension into the bony orbital margins. Primary orbital tumors are scarcer and usually benign, while secondary tumors are usually malignant. Metastases may be found anywhere in the wall of the orbit or inside the orbit. The most common symptoms include exophtalmus, visual impairment and diplopia. CT in combination with MRI are the most useful in the diagnostic algorithm. Therapy aims for long-term management of tumor growth with focus on visual improvement or preservation. Monitoring, surgery, chemotherapy and radiation with respect to histology and staging are all used in combination as it is common in other groups of tumors. Over the past few decades, advances in radiodiagnostics, improved knowledge of the natural course of tumors, more accurate radiosurgical techniques and improved chemotherapy protocols have changed therapeutic algorithms as well as prognosis in many orbital tambours. Multidisciplinary approach in specialized centers shall ensure optimal treatment. The radiosurgery of cavernomas remains a controversial issue. The only way to verify the positive effect of the treatment is by clinical observation during a longer follow-up period, where a decreased annual risk of rebleeding should be observed after the latent interval inherent to radiosurgery. Besides this, an improvement in clinical symptoms (e.g. secondary epilepsy) and regression of the treated lesion might also be observed. In a group of 112 patients with brain cavernomas treated in our centre between 1992 and 2000 with the marginal dose of a median 16 Gy, the risk of bleeding decreased from 2% before the treatment to 0.5% after 2 years' latent interval (median follow-up 84 months). A decrease in the extent of the cavernoma was observed in 53% of cases and an increase in 6.4%. Epilepsy, if present before the treatment, improved in 45% of cases. The risk of temporary and permanent morbidity caused by radiosurgery was 14.6 and 0.9%, respectively. This morbidity can be reduced by a lower marginal dose, and future studies should show if repeated radiosurgery decreases the risks from a natural course of the disease in those cases where the initial radiosurgery failed.
