INTRODUCTION: Electrophysiological diagnosis of cardiac autonomic neuropathy (CAN) is based on the evaluation of cardiovascular autonomic reflex tests (CARTs). CARTs are relatively time consuming and must be performed under standardized conditions. This study aimed to determine whether thermal quantitative sensory testing (TQST) can be used as a screening tool to identify patients with diabetes at a higher risk of CAN. METHODS: Eighty-five patients with diabetes and 49 healthy controls were included in the study. Neurological examination, CARTs, TQST, biochemical analyses, and neuropathy symptom questionnaires were performed. RESULTS: CAN was diagnosed in 46 patients with diabetes (54%). CAN-positive patients with diabetes had significantly higher warm detection thresholds (WDT) and significantly lower cold detection thresholds (CDT) in all tested regions (thenar, tibia, and the dorsum of the foot). CDT on the dorsum < 21.8°C in combination with CDT on the tibia < 23.15°C showed the best diagnostic ability in CAN prediction, with 97.4 % specificity, 60.9% sensitivity, 96.6% positive predictive value, and 67.3% negative predictive value. CONCLUSION: TQST can be used as a screening tool for CAN before CART.

SIGNIFICANCE: Both foveal and peripheral contour interactions are based on, as yet, unexplained neural mechanisms. Our results show that, unlike foveal contour interaction, peripheral contour interaction cannot be explained on the basis of the antagonistic structure of neural receptive fields. PURPOSE: Foveal contour interaction is markedly reduced for mesopic compared with photopic targets. This finding is consistent with an explanation based on the antagonistic structure of neural receptive fields. However, no reduction was found for low-luminance targets in the periphery, possibly because the luminances used previously remained substantially above peripheral scotopic detection thresholds. In this study, we compared foveal and peripheral contour interactions for long-wavelength photopic and mesopic targets, which would be expected to significantly elevate the peripheral retinal detection threshold. METHODS: Five normal observers viewed a randomly selected Sloan letter surrounded by four flanking bars at several edge-to-edge separations (min arc). Photopic and mesopic stimuli were viewed foveally and at 6° peripherally through a selective red filter that ensured that mesopic targets were within 1 log unit of detection threshold at both retinal locations. RESULTS: Whereas the magnitude of foveal contour interaction was substantially less at mesopic compared with photopic luminance (20 vs. 46% reduction of percent correct, on average), no significant difference was observed in peripheral contour interaction, which had average mesopic and photopic magnitudes of 38 and 40%. Moreover, confusion matrices representing photopic and mesopic contour interaction differed in the fovea but not in the periphery. The extent of contour interaction did not change with luminance at either retinal location. CONCLUSIONS: Our results indicate that, although the characteristics of foveal contour interaction can be accounted for by the antagonistic structure of neural receptive fields, the same mechanism is not compatible with the characteristics of peripheral contour interaction.

OBJECTIVE: To determine a minimum number of trials that preserve input-output (I-O) properties of duration and magnitude of exteroceptive EMG suppression (eEMGs). PATIENTS AND METHODS: eEMGs was recorded in 16 healthy subjects from thenar muscles following index finger stimulation at 2.5, 5, 10, and 20 times sensory threshold (xST). Individual trials were rectified and incrementally averaged in blocks of 5, 10, 20, 30, 40, 50, and 60. To determine if the block size affects I-O properties, the goodness of curve fit parameter R2 for each block was compared to R2 of the global function across all blocks combined. RESULTS: eEMGs was found in all subjects at 10xST and 20xST (100%, respectively) but less often at 5xST (63-75%) and 2.5xST (25-56%). A quadratic function best described both duration and magnitude of eEMGs. The quadratic R2 did not significantly differ between any individual block function (5-60) and the global function (eEMGs duration 0.647-0.704 vs 0.679; magnitude 0.525-0.602 vs 0.560, respectively). CONCLUSIONS: Averaging 5 trials consistently shows eEMGs at and above 10xST. I-O properties of eEMGs do not differ whether 5 or up to 60 trials are averaged. Clinical studies of eEMGs in thenar muscles are possible with as few as 5 trials averaged.

Úvod: Dynamické metody kvantitativního testování senzitivity (Quantitative Sensory Testing; QST) jsou relativně novou nadstavbovou aplikací QST umožňující objektivizovat funkci centrálních modulačních mechanizmů algické percepce, jejichž individuální nastavení může přispívat k rozvoji chronických bolestivých stavů. Testování těchto mechanizmů umožňuje např. predikci rozvoje pooperační bolesti. K nejčastěji hodnoceným dynamickým QST metodám patří tzv. podmíněná modulace bolesti (Conditioned Pain Modulation; CPM) a časová sumace percepce bolestivých podnětů (Temporal Summation; TS). Soubor a metodika: Vyšetření dQST (dynamic methods of Quantitative Sensory Testing) metod s využitím termoalgických podnětů a modulace bolestivým chladem bylo provedeno u 77 zdravých dobrovolníků (47 žen testovaných v ovulační fázi menstruačního cyklu, 30 mužů, věk 19–73 let). Hodnocen byl také vliv věku a pohlaví na výsledky obou dQST testů. Výsledky: Uspokojivá funkce podmíněné modulace bolesti byla prokázána u 84 % testovaných dobrovolníků. Časová sumace bolestivých podnětů byla zachycena u 92, resp. 98 % jedinců v závislosti na použité teplotě podnětů. Nefunkčnost mechanizmů u zbylé části souboru byla ve většině případů podmíněna nesprávně nastavenými technickými parametry testu. U žen testovaných v ovulační fázi menstruačního cyklu jsme prokázali signifikantně vyšší efektivitu CPM mechanizmu ve srovnání s muži. Funkce TS se mezi oběma pohlavími významně nelišila. Významný vliv věku na efektivitu CPM ani TS mechanizmu nebyl ve sledovaném souboru prokázán. Závěr: Provedená studie ověřila použitelnost zvolených algoritmů dynamického testování senzitivity a prokázala antinociceptivní nastavení žen v ovulační fázi menstruačního cyklu ve srovnání s muži, zatímco významný vliv věku na funkci centrálních modulačních mechanizmů algické percepce nebyl zachycen.
Background: So called “dynamic quantitative sensory methods” (dQST) represent relatively new QST application which enables to objectify certain mechanisms of pain processing, e.g. central integration or descending control. Their dysfunction may contribute to the development of chronic painful conditions. dQST methods proved the potential to predict the development of chronic postoperative pain. Conditioned pain modulation (CPM) and temporal summation (TS) are the most commonly used dQST tests. Patients and methods: The CPM and TS testing was performed in a group of 77 healthy volunteers (47 women tested in the ovulatory phase of the menstrual cycle, 30 men, age range 19–73) using thermal pain stimuli. Efficiency of CPM and TS mechanism including age and gender influence was evaluated. Results: Efficient CPM was found in 84% of healthy volunteers. In remaining 16%, the inefficiency was mainly due to low intensity of testing or modulatory stimuli. Based on the temperature used, efficient TS mechanism was found in 92% or 98% of healthy individuals. Women in the ovulatory phase of the menstrual cycle showed significantly more efficient CPM comparing to men. No sex differences of TS testing were found. Age had no significant effect on the CPM or TS mechanism in our group of healthy voluteers. Conclusions: Our study has verified the applicability of CPM and TS algorithms used. Women in ovulatory phase of the menstrual cycle show increased anti-nociceptive setting comparing to men. No significant effect of age on central modulation of pain perception has been found in pain-free healthy individuals. U řady neurodegenerativních chorob dochází ke změnám v čichovém vnímání, které lze vyšetřit psychofyzickými testy. Největší význam má vyšetření čichu u Alzheimerovy a Parkinsonovy choroby, u nichž se poruchy čichu objevují před ostatními klinickými příznaky. Pouhý anamnestický údaj o poruše čichu je u pacientů zcela nedostatečný. Pacienti si poruchu čichu často neuvědomují. Vyšetření čichu tak může včasně upozornit na přítomnost neurologické choroby či usnadnit diferenciální diagnostiku. Přehledový článek představuje povahu poruch čichu u vybraných neurodegenerativních onemocnění s důrazem na Alzheimerovu a Parkinsonovu chorobu, způsoby hodnocení kvanti‑ a kvalitativních poruch čichu se zaměřením na nejdostupnější psychofyzické testy, jimiž lze provádět rutinní vyšetření čichu v každodenní klinické praxi, a uplatnění vyšetření čichu v jejich diagnostice. U Alzheimerovy choroby je výrazněji postižena identifikace oproti detekčním prahům, zatímco u Parkinsonovy choroby je pokles napříč čichovými schopnostmi homogennější a prahy jsou zvýšeny více než u Alzheimerovy choroby.
In many neurodegenerative diseases, changes in olfactory perception have been identified that can be tested with psychophysical tests. Olfactory testing is of particular significance in Alzheimer's and Parkinson's disease, where olfactory decline precedes other clinical symptoms. Olfactory decline may indicate a neurological disorder, aid in differential diagnosis, or help to estimate disease prognosis. This review presents olfactory dysfunction profiles in selected neurodegenerative diseases with an emphasis on Alzheimer's and Parkinson's disease, assessment of quanti- and qualitative olfactory dysfunction, focusing on widely used psychophysical tests suitable for routine olfactory testing in everyday clinical practice, and contribution of olfactory testing to the diagnosis of the selected neurodegenerative diseases. In Alzheimer's disease, identification is more severely affected than detection thresholds, whereas in Parkinson's disease, decline is more homogeneous across various olfactory measures and increase in detection thresholds is more prominent. Úvod: Komplexní protokol kvantitativního testování senzitivity (QST), zahrnující hodnocení široké škály modalit termické, algické, taktilní a vibrační cítivosti, je často využívaným nástrojem pro kvantifikaci senzitivity a hodnocení tzv. senzitivního profilu, zejména u pacientů s neuropatickou bolestí. Protokol obsahuje složité slovní instrukce, které je pro dosažení standardizovaného výstupu nutné přesně následovat, což implikuje nutnost validace jeho jazykových mutací. Soubor a metodika: Pomocí české verze standardizovaných slovních instrukcí ke komplexnímu QST protokolu bylo vyšetřeno 75 zdravých dobrovolníků, 68 pacientů s periferní neuropatickou bolestí při diabetické neuropatii a 59 jedinců s centrální neuropatickou bolestí při roztroušené skleróze, a to vždy na HKK i DKK. Pro hodnocení spolehlivosti bylo u 26 jedinců testování provedeno opakovaně (4×) s přibližně 30denními rozestupy mezi jednotlivými vyšetřeními. Výsledky: Vyšetření pomocí české verze komplexního QST protokolu bylo pro pacienty nenáročné či málo náročné. V souboru zdravých dobrovolníků spadalo 94 % zjištěných hodnot jednotlivých senzitivních parametrů na HKK a 95 % na DKK do rozsahu fyziologických nálezů podle publikovaných normativních dat. Alespoň jedna abnormita z hodnocených senzitivních modalit byla zachycena u 90 % pacientů s neuropatickou bolestí. Shluková analýza umožnila identifikaci dvou symptomových profilů, z nichž jeden většinově odpovídal bolestivé diabetické neuropatii a druhý centrální bolesti při roztroušené skleróze. Reliabilita jednotlivých modalit hodnocená pomocí Cronbachova alfa se pohybovala v rozmezí 0,593–0,926. Závěr: Česká jazyková verze komplexního QST protokolu prokázala velmi dobrou praktickou použitelnost pro hodnocení širokého spektra senzitivních abnormit a nízkou subjektivní náročnost vyšetření. Jedná se o spolehlivý test s vysokou vnitřní konzistencí a dobrou opakovatelností všech hodnocených parametrů.
Background: Quantitative sensory testing (QST) following the protocol of the German Research Network on Neuropathic Pain (DFNS) is frequently used for the evaluation of sensory profi les in neuropathic pain patients. It covers a broad spectrum of sensory modalities using both nociceptive and non-nociceptive thermal and mechanical stimuli. The protocol requires the use of standardized instructions and thus necessitates validation of each language version. Material and methods: The standardized instructions for DFNS-QST protocol were translated using the forward-backward translation method and validated in a group of patients suffering from neuropathic pain of peripheral (n = 68) or central origin (n = 59) due to painful diabetic polyneuropathy or multiple sclerosis, and in a group of healthy volunteers (n = 75). The testing was repeated four times (with one-month interval between examinations) in 26 individuals to determine test-retest reliability. Results: In a group of healthy volunteers, 95% of values across all the parameters were within the physiological range based on published normal data. At least one sensory abnormity was found in 90% of neuropathic pain patients. Cluster analysis identified two distinctive pain profiles, the first mostly consisted of patients with painful diabetic neuropathy and the second of patients with pain in multiple sclerosis. Repeated administration confirmed an acceptable test-retest reliability (Cronbach´s alpha of 0.59–0.93 for the different sensory modalities). Conclusion: The Czech version of DFNS-QST protocol instructions displayed a good practical applicability in evaluating sensory abnormities in neuropathic pain patients, acceptable reliability and high internal consistency of the findings. The protocol provides results comparable to the original language version and can be recommended as an easy-to-apply and suitable testing method. Standard pattern-reversal visual evoked potentials (VEPs) and motion-onset VEPs (M-VEPs) were tested in 19 dyslexics and 19 normal readers aged 7-13 years in order to evaluate the feasibility of M-VEPs for the objective diagnostics of a visual subtype of dyslexia, in which a dysfunction of the magnocellular subsystem/dorsal stream of the visual pathway is suspected. The set of VEPs consisted of the pattern-reversal VEPs with check sizes of 20', two types of translational motion (with low and high contrast) and two types of radial motion (in the full field or the periphery). While the P100 peak parameters in pattern-reversal VEPs did not differ between the group of dyslexics and controls, the group of dyslexics displayed significantly longer N2 latencies in all types of M-VEPs. Abnormal N2 latencies were found in 35-56% of dyslexics in different types of M-VEPs, with translational motion with high contrast being the most sensitive stimulation. A receiver operating characteristic analysis showed that the latencies of M-VEPs displayed higher discrimination potential than M-VEPs amplitudes. The study confirms a "magnocellular pathway/dorsal stream deficit" in approximately half of dyslexics.

Even brief acoustic trauma during the critical period of development that results in no permanent hearing threshold shift may lead to altered auditory processing in adulthood. By monitoring the acoustic startle response (ASR), we examined the development of auditory function in control rats and in rats exposed to intense noise at the 14th postnatal day (P14). First ASRs appeared on P10-P11 to intense low-frequency tones. By P14, the range of sound intensities and frequencies eliciting ASRs extended considerably, the ASR reactivity being similar at all frequencies (4-32 kHz). During the subsequent two weeks, ASR amplitudes to low-frequency stimuli (4-8 kHz) increased, whereas the ASRs to high-frequency tones were maintained (16 kHz) or even decreased (32 kHz). Compared to controls, noise exposure on P14 (125 dB SPL for 8, 12, or 25 min) produced transient hyper-reactivity to startle stimuli, manifested by a decrease of ASR thresholds and an increase of ASR amplitudes. ASR enhancement occurred regardless of permanent hearing loss and was more pronounced at high frequencies. The hyper-reactivity of ASRs declined by P30; the ASR amplitudes in adult exposed rats were lower than in controls. The histological control did not reveal loss of hair cells in adult exposed rats, however, the number of inner hair cell ribbon synapses was significantly decreased, especially in the high-frequency part of the cochlea. The results indicate that early acoustic trauma may result in complex changes of ASRs during development.

OBJECTIVE: The cutaneous silent period (CSP) is a spinal inhibitory reflex mediated by small-diameter afferents (A-delta fibers) and large-diameter efferents (alpha motoneurons). The effect of limb temperature on CSPs has so far not been assessed. METHODS: In 27 healthy volunteers (11 males; age 22-58 years) we recorded median nerve motor and sensory action potentials, median nerve F-wave and CSPs induced by noxious digit II stimulation in thenar muscles in a baseline condition at room temperature, and after randomly submersing the forearm in 42 °C warm or 15 °C cold water for 20 min each. RESULTS: In cold limbs, distal and proximal motor and sensory latencies as well as F-wave latencies were prolonged. Motor and sensory nerve conduction velocities were reduced. Compound motor and sensory nerve action potential amplitudes did not differ significantly from baseline. CSP onset and end latencies were more delayed than distal and proximal median nerve motor and sensory latencies, whereas CSP duration was not affected. In warm limbs, opposite but smaller changes were seen in nerve conduction studies and CSPs. CONCLUSION: The observed CSP shift "en bloc" towards longer latencies without affecting CSP duration during limb cooling concurs with slower conduction velocity in both afferent and efferent fibers. Disparate conduction slowing in afferents and efferents, however, suggests that nociceptive EMG suppression is mediated by fibers of different size in the afferent than in the efferent arm, indirectly supporting the contribution of A-delta fibers as the main afferent input. SIGNIFICANCE: Limb temperature should be taken into account when testing CSPs in the clinical setting, as different limb temperatures affect CSP latencies more than large-diameter fiber conduction function.

BACKGROUND: In adults, motion-onset visual evoked potentials (M-VEPs) with a dominant N2 peak represent a useful diagnostic tool. However, it is difficult to use this type of VEP in children because of the long maturation (up to 18 years) of M-VEPs, which is characterised by a gradual decrease in N2 peak latency and shape development. Moreover, in some children, M-VEPs are difficult to identify with standard stimuli. METHODS: We tested features of M-VEPs in 30 children (7-12 years) with the following set of standard stimuli used in our lab for examining adults ( https://web.lfhk.cuni.cz/elf ): low-contrast translation motion (TM) and expansion/contraction motion (ExCoM) in full field and in periphery (with central 20° masked). In 16 children, a high-contrast TM was also tested. RESULTS: With standard (low-contrast) stimuli, a common M-VEP to TM and to ExCoM was detected in 77 and 83 % of children, respectively. The M-VEPs to ExCoM in the periphery were detected in only 43 % of children. An abnormal dominant P1 peak was found in 9 % of VEPs to TM, 12 % of VEPs to full-field ExCoM and 14 % of VEPs to peripheral ExCoM. The M-VEPs to all low-contrast stimuli displayed large inter-individual latency variability (N2 peak latency differed for more than 100 ms). High contrast (more suitable for the non-mature magnocellular pathway) shortened M-VEP latencies and improved amplitudes. CONCLUSIONS: Our findings show that the maturation of motion perception in children is inter-individually variable, which limits the diagnostic use of M-VEPs.
