Perioperační anafylaxe je vzácná, ale závažná komplikace chirurgických zákroků prováděnýchv celkové nebo lokální anestezii. Nejčastějšími spouštěči jsou periferní myorelaxancia a an-tibiotika. Příčinou perioperační anafylaxe však nemusí být jen léky, ale také tzv. skryté alergenyjako latex nebo chlorhexidin. Autoři představují kazuistiku pacientky, u které se anafylaxerozvinula krátce po úvodu do celkové anestezie u elektivní hysterektomie pro karcinom en-dometria. Alergologické vyšetření odhalilo pomocí laboratorních a kožních testů překvapivěhned několik suspektních příčin. Před novou operací bylo nutno provokačním testem vyloučitalergii na periferní myorelaxans. Skutečnou příčinou reakce byla silná alergie na dezinfekčnípřípravek chlorhexidin.

Perioperative anaphylaxis is a rare but serious complication of surgical procedures performedunder general or local anaesthesia. The most common triggers are neuromuscular blockingagents and antibiotics. However, not only a drug but also a hidden allergen such as latex orchlorhexidine can be the cause of perioperative anaphylaxis. Authors present a case reportof a patient with anaphylaxis that emerged shortly after a general anaesthesia induction inelective hysterectomy for an endometrial carcinoma. Allergy testing by laboratory and skintests surprisingly revealed several suspected causes. A provocation test with an alternativeneuromuscular blocking agent was necessary before the surgery. The real cause of the indexreaction was a strong allergy to the disinfectant chlorhexidine.
