Diabetes mellitus je asociovaný s vyššou celkovou mortalitou i morbiditou pacientov zároveň trpiacich fibriláciou predsiení a srdcovým zlyhávaním. Jedným z faktorov je vyššie riziko náhlej cievnej mozgovej príhody a systémového embolizmu. Preto je dôsledné stanovenie rizika a následná účinná antikoagulačná liečba u týchto pacientov kľúčová. Za posledné desaťročie sa do popredia dostali priame perorálne antikoagulanciá, ktoré postupne vytláčajú z bežnej praxe warfarín. V našom článku sumarizujeme doterajšie informácie o použití dabigatranu špecificky u pacientov s diabetom. Zameriavame sa na najdôležitejšie charakteristiky tohto liečiva a zároveň zdôrazňujeme niektoré špecifiká v rámci ostatných liečiv zo skupiny nových antikoagulancií – antidotum idarucizumab, liekové interakcie a jeho použitie u pacientov s pokročilou chronickou chorobou obličiek.

Diabetes mellitus is associated with higher overall mortality and morbidity in patients with both atrial fibrillation and heart failure. One factor is the higher risk of stroke and systemic embolism. Therefore, consistent risk assessment and subsequent effective anticoagulant therapy in these patients is crucial. In the last decade, direct oral anticoagulants have come to the fore, which are gradually displacing warfarin from common practice. In our article, we summarize the existing information on the use of dabigatran specifically in patients with diabetes. We focus on the most important characteristics of this drug and emphasize some specifics within other drugs from the group of new anticoagulants – antidote idarucizumab, drug interactions and its use in patients with advanced chronic kidney disease.
