Vaskulopatie je častým nálezem u systémové sklerodermie (SSc) a překryvných (overlap) syndromů s SSc včetně smíšené nemoci pojiva (MCTD). Manifestuje se zejména plicní arteriální hypertenzí (PAH) a výskytem digitálních ulcerací (DU). Léčba obou projevů je svízelná a dlouhodobá. Popisujeme případ 31leté nemocné s MCTD doprovázenou PAH a DU. Nemoc byla charakterizována Raynaudovým fenoménem, pleuritidou, perikarditidou, Coombs pozitivní hemolytickou anémií, intersticiálním postižením plic a pozitivitou autoprotilátek proti jadernému antigenu U1-RNP. Byla naplněna diagnostická kritéria MCTD podle Sharpa a spol. 1987. Terčovým orgánem nemoci byly plíce. Imunosupresivní léčba cyklofosfamidem i. v., cyklosporinem A p. o., azatioprinem p. o. byla provázena opakovanými septickými stavy a paralelním rozvojem tří DU na prstech dolní končetiny. Vyšetření pro dušnost posléze vedla ke katetrizačnímu stanovení diagnózy PAH (dokumentovanou středním tlakem v plicnici 40 mmHg). Zahájení léčby bosentanem vedlo k plnému zhojení DU, zlepšení echokardiografických nálezů (vymizení dilatace pravé komory) a zlepšení funkční třídy NYHA z II na I. Nemocná s MCTD je dlouhodobě v remisi. Dokumentujeme léčbu PAH a DU u nemocné s MCTD. Rovněž zaznamenáváme zhojení DU při léčbě bosentanem, nejen prevenci vývoje dalších DU.
Vasculopathy is a frequent complication of systemic sclerosis (SSc) and many overlap syndromes with SSc, including mixed connective tissue disease (MCTD). It manifests itself mainly by pulmonary arterial hypertension (PAH) and by the presence of digital ulcers (DUs). The treatment of both events is difficult and prolonged. We describe a case of a 31-year-old female with MCTD accompanied by PAH and DUs. The disease was characterized by Raynaud's phenomenon, pleurisy, pericarditis, Coombs' positive haemolytic anaemia, interstitial lung disease and positive autoantibodies against the nuclear antigen U1-RNP. She met the diagnostic criteria for MCTD by Sharp et al. (1987). The target organ for MCTD was the lung. Immunosuppressive therapy usually selected for this complication (cyclophosphamide, cyclosporin A and azathioprine) was discontinued due to recurrent septic states. Three DUs developed in the lower limbs. An examination for dyspnoea led to the diagnosis of PAH (mean pulmonary artery pressure of 40 mm Hg documented by heart catheterisation). The drug of choice for this PAH patient, bosentan, resulted not only in cardiac improvement (echocardiographic finding and NYHA), but also complete healing of the DUs. Our MCTD patient is in long-term remission. We report the treatment of PAH and DUs in a patient with MCTD. The healing of DUs was noted during bosentan treatment, in addition to preventing the development of new DUs.

Metotrexát patří k základním lékům používaným při léčbě revmatoidní artritidy (RA). Efektivita léčby revmatoidní artritidy metotrexátem (MTX) se pohybuje mezi 46 % a 65 % (hodnoceno podle kritéria ACR 20). Při léčbě metotrexátem se může vyskytnout celá řada nežádoucích účinků. Alespoň jeden z nich se objeví až u 72,9 % pacientů, závažné projevy toxicity se zjišťují až u 30 % pacientů. Farmakogenetika je nový moderní směr klinické farmakoterapie umožňující personalizaci terapie na základě zjištění genetické predispozice pacienta pro odpověď na danou léčbu. Výzkum farmakogenetiky MTX u revmatoidní artritidy se soustředí na souvislost mezi terapeutickým účinkem či toxicitou MTX a mutacemi genů metabolických drah, transportérů MTX, genů podílejících se na účinku adenosinu, genů HLA-G antigenů a enzymů metabolizmu bází nukleových kyselin. Mezi nejvíce studované patří gen kódující enzym 5,10-methylentetrahydrofolát reduktázu, jehož mutace má patrně souvislost s vystupňovanými nežádoucími gastrointestinálními účinky MTX. Nadějné výsledky přinesly studie zkoumající polymorfizmy související s metabolizmem adenosinu. Byla nalezena statisticky významná souvislost mezi některými těmito polymorfizmy a klinickým účinkem MTX dle DAS28 skóre nebo s výskytem nežádoucích účinků MTX. Wessels a spol. nalezli souvislost mezi polymorfizmy souvisejícími s metabolizmem adenosinu a dobrou odpovědí na MTX u pacientů s RA a navrhli prediktivní diagnostický model pro terapii RA metotrexátem na základě genotypizace. Souhrnně lze říci, že výsledky studií naznačují možnou diagnostickou hodnotu genotypizace pacientů s RA pro predikci úspěšnosti terapie MTX, schopnost tolerovat vyšší dávky MTX v terapii či naopak možnost určit pacienty primárně rezistentní na terapii a s dispozicí k závažným nežádoucím účinkům. V budoucnosti je nutné upřesnění dostupných výsledků ve studiích s větším počtem pacientů a delší dobou sledování.
Methotrexate is one of the essential medicines used in the treatment of rheumatoid arthritis (RA). The efficacy of treatment of rheumatoid arthritis with methotrexate (MTX) is between 46 % and 65 % (as assessed by ACR 20). A number of undesirable effects are associated with methotrexate treatment. At least one of them occurs in up to 72.9 % of patients, and severe toxicities are identified in up to 30 % of patients. Pharmacogenetics is a new modern way to enable personalization of clinical drug therapy based on the findings of the genetic predisposition of the patient to respond to the treatment. Research into MTX pharmacogenetics in rheumatoid arthritis focuses on the relationship between the therapeutic effect and toxicity of MTX, mutations in the genes of metabolic pathways, MTX transporters, and genes involved in the effect of adenosine, HLA-G antigens and enzymes of the metabolism of nucleic acids. One of the most widely studied genes is the one encoding the enzyme 5,10-methylenetetrahydrofolate reductase, whose mutation is probably related to an extension of adverse gastrointestinal effects of MTX. There have been promising results from studies examining polymorphisms associated with the metabolism of adenosine. There was a significant correlation between some of these polymorphisms and the clinical effect of MTX, according to both DAS28 score and the occurrence of adverse effects of MTX. Wessels et al found a correlation between polymorphisms associated with the metabolism of adenosine and a good response to MTX in RA patients, and have proposed a diagnostic predictive model for RA methotrexate therapy based on genotyping. In summary, the results of the studies suggest a possible diagnostic value for genotyping of patients with RA for prediction of MTX therapy and their ability to tolerate higher doses of MTX therapy, or conversely to determine patients primarily resistant to the therapy by reason of a predisposition to serious side effects. In the future, it will be necessary to analyze the available results in studies with larger numbers of patients and longer follow-up.
