Úvod: Diabetes mellitus 2. typu (DM2T) představuje zvýšené riziko rozvoje mikro- imakroangiopatických komplikací. Nová anti- diabetika mají vyšší účinnost ibezpečnost, méně nežádoucích účinků ařadu protektivních charakteristik užití v mnoha směrech. Pacienti a metodika: V letech 2011–2021 bylo trvale v naší diabetologické ordinaci léčeno okolo 2000 DM2T. V roce 2021 celkem 2 562 všech evidovaných diabetiků. Ztohoto počtu bylo T2DM 2398 (1203 mužů a 1 195 žen). Preparáty antidiabetiky, inkretinovými mimetiky nebo inzulinem bylo vroce 2021 léčeno celkem 2110 diabetiků 2. typu (88%) průměrného věku 69,7 ± 11,6 r. Vsouboru jsme analyzovali s ohledem na užití nových antidiabetik (NAD): věk, trvání diabetu, obezitu, komorbidity, počet užívaných léků a diabetické komplikace, zejména s cílem vyhodnotit vzestup užití NAD mezi 2011–2021. Výsledky: V roce 2021 v naší diabetologické ambulanci z nemocných s DM2T byly užity: pouze diabetická dieta 12 %; perorální antidiabetika (PAD) 53 %; monoterapie inzulinem či inzulinovými analogy 17 % a kombinací inzulinu a PAD (18 %). NAD (gliptiny, glifloziny i inkretinová mimetika) byly užity buď v monoterapii nebo v kombinacích u nemocných s DM2T v roce 2021 celkem u 35 %. Náš retrospektivní rozbor dat ukázal trvalý trend v nárůstu užívání NAD (nejvíce gliptinů) jak v monoterapii, tak i v kombinaci s metforminem. Díky novým léčebným možnostem jsme zaznamenali také významné zlepšení kompenzace diabetiků 2. typu. Závěr: Naše sdělení poukazuje na masivní posun a změnu přístupu v terapii DM2T v posledním desetiletí v běžné ambulantní praxi v užití NAD, což nepochybně souvisí s jejich benefity na kardiovaskulární a renální systém.
Introduction: Type 2 diabetes mellitus (DM2T) constitutes an increased risk of developing micro- and macroangiopathic complications. New antidiabetics have higher efficacy and safety, fewer side effects and a number of protective characteristics of use in many ways. Patients and methods: In the years 2011-2021, about 2000 DM2Ts were permanently treated in our diabetological surgery. In 2021 it was a total of 2,562 of all registered diabetics. Of this number, there were 2,398 cases of T2DM (1,203 men and 1,195 women). A total of 2110 type 2 diabetics (88%) of a mean age of 69.7 ± 11.6 years were treated with antidiabetic drugs, incretin mimetics or insulin in 2021. In the cohort, we analyzed regarding the use of new antidiabetics (NAD): age, diabetes duration, obesity, comorbidities, number of drugs used and diabetic complications, in particular to evaluate the increase in NAD use between 2011-2021. Results: In 2021 in our diabetological outpatient clinic of patients with DM2T we used: diabetic diet only 12%; oral antidiabetics (PAD) 53%; monotherapy with insulin or insulin analogues 17% and a combination of insulin and PAD (18%). NAD (gliptins, gliflozins and incretin mimetics) were used either within monotherapy or in combination in patients with DM2T in 2021 in a total of 35%. Our retrospective data analysis showed a steady trend in increasing the use of NAD (most gliptins) both as monotherapy and in combination with metformin. Thanks to new treatment options, we have also seen a significant improvement in the compensation of type 2 diabetics. Conclusion: Our communication points to a massive shift and change in approach to DM2T therapy in the last decade in routine. outpatient practice in the use of NAD, which is undoubtedly related to their benefits for the cardiovascular and renal systems.

Úvod: Diabetes mellitus (DM) v každé m věku významně ovlivňuje zdravotní stav a další perspektivy (jako pozdní komplikace) života jedince. Pacienti a metodika: Během roku 2019 bylo léčeno v naší diabetologické ambulanci DIASTO P 2 332 nemocných DM typu 2 (DM2T). Z nich bylo 1 562 (69 %) léčeno pouze per orálními antidiabetiky (PAD) a / nebo dietou. V souboru jsme analyzovali: věk, trvání diabetu, obezitu, diabetické komplikace a druh terapie, zejména jsme se zaměřili na měnící se trendy léčby DM2T během uplynulých 12 let (t.j. 2008-2019). Výsledky: Analýza terapie DM2T během 12 let ukázala významný pokles užívání pouhé diabetické diety (37,5 % v roce 200 8 až 13 % v roce 2019 ) a sulfonylmočoviny (z 42,8 % v roce 200 8 na 24 % v roce 2019), zatímco užívání metforminu v léčbě DM2T výrazně vzrostlo ( z 19,5 % v roce 200 8 na 65 % v roce 2019), a to jak v monoterapii, tak i v kombinaci s jinými léky. Rovněž jsme zaznamenali trvalý trend v nárůstu užívání nových PAD během uvedených roků s tím, že v roce 2019 byly užity: pioglitazon u 11,0 % DM2T; gliptiny (inhibitory DPP4 ve 43 %) a glifloziny (inhibitory SGLT2 - 4 %) . Díky novým léčebným možnostem jsme zaznamenali také významné zlepšení kompenzace diabetiků 2. typu. Závěr: Naše sdělení poukazuje na masivní posun a změnu přístupu v terapii DM2T v posledním desetiletí v běžné ambulantní praxi v užití PAD.
Introduction: Diabetes mellitus (DM) at any age significantly affects the health status and other perspectives (such as late complications) of an individual's life. Patients and methods: During 2019, 2,332 patients with type 2 DM (DM2T) were treated in our diabetological outpatient clinic DIASTOP. Of these, 1,562 were treated with oral antidiabetic drugs (OAD) and/or diet alone. In the sample, we analyzed: age, duration of diabetes, obesity, diabetic complications and type of therapy, in particular we focused on the changing trends in DM2T treatment over the past 12 years (i.e. 2008-2019). Results: Analysis of DM2T therapy over 12 years showed a significant decrease in the use of mere diabetic diet (37,5 % in 200 8 to 13 % in 2019) and sulphonylurea (from 42,8 % in 2008 to 24 % in 2019), while the use of metformin in the treatment of DM2T increased significantly (from 19,5 % in 200 8 to 65 % in 2019), both as monotherapy and in combination with other drugs (especially gliptins). We also observed a steady trend in the increase in the use of new oral hypoglycaemic drugs (OADs) during those years, with the following being used in 2019: pioglitazone in 11,0 % DM2T; gliptins (DPP4 inhibitors in 43 %) andgliflozins (SGLT2inhibitors 4 %). Conclusion: Our communication points to a massive shift and change of approach in DM2T therapy in the last decade in common outpatient practice in the use of OAD.
Úvod: Na jedné straně neexistuje jednoznačný konsensus stran zahájení inzulinoterapie u diabetu 2. typu (DM2T), nicméně ani ve vyšším věku by nemělo být váhání s její iniciací, případně intenzifikací nadměrně odkládáno ani oddalováno, a to ani pacientem, ani lékařem. Pacienti a metodika: Během roku 2019 bylo léčeno v naší diabetologické ambulanci DIASTO P 2 332 nemocných DM2T. Z nich bylo 770 (31 %) léčeno injekčními preparáty, tj. GLP1 agonisty, konvenční (KIL) nebo intenzifikovanou inzulinoterapií (IIL), případně v kombinaci s perorálními antidiabetiky (PAD) a dietou. V souboru jsme analyzovali: věk, trvání diabetu a zejména jsme se zaměřili na měnící se trendy léčby DM2T během uplynulých 12 let (t.j. 2008-2019 ) v užití inkretinů a inzulínu. Výsledky: Doba trvání DM2T 15,7 ± 9,lr. Analýza terapie DM2T během 12 let ukázala významný vzestup při užití inzulinoterapie v rozmezí 2008-2019 ( z 12,2 % na 29 %). KIL - kombinace inzulínu s PAD byla použita u 17,1 %. Samotný inzulin intenzifikované byl užit u 12 % DM2T; inkretiny v roce 2019 u 2 % diabetiků. Uvedená intenzifikace terapie DM2T vedla i k lepší kompenzaci. Závěr: Naše výsledky ukazují na podstatný vzestup užití konvenční i intenzifikované inzulinoterapie a nově i užití inkretinů v terapii DM2T.
Introduction: On the one hand, there is no clear consensus on the initiation of insulin therapy in type 2 diabetes (DM2T), however, even in old age, its initiation or intensification should not be excessively delayed or postponed, either by patients or physicians. Patients and methods: During 2019,2,332 DM2T patients were treated in our diabetological outpatient clinic DIASTOP. Of these, 770 (31 % ) were treated with injectable preparations, i.e. GLP1 agonists, conventional or intensified insulin therapy, possibly in combination with oral antidiabetics (OAD ) and the diet. In the cohort, we analyzed: age, duration of diabetes, and in particular, we focused on the changing trends in DM2T treat ment over the past 12 years (i.e., 2008-2019) in the use of incretins and insulin. Results: Duration of DM2T 15,7 ± 9,lr. Analysis of DM2T therapy over 12 years showed a significant increase in the use of insulin therapy in the period 2008-201 9 (from 12,2% to 29 %) . KIT - combination of insulin with OA D wa s used in 17,1 %. Insulin alone wa s intensively used in 12% DM2T; incretins in 2019 in 2 % of the diabetics. This intensification of DM2T therapy also led to better compensation. Conclusion: Our results show a significant increase in the use of conventional and intensified insulin therapy and, more recently, the use of incretins in DM2T therapy.
