Úvod: Prediktory úspěšné terapeutické intervence u obsedantně-kompulzivní poruchy nejsou dostatečně známy. Metoda: Cílem naší studie bylo posoudit účinnost komplexního léčebného programu (KBT a antidepresiva) u farmakorezistentních pacientů trpících obsedantně-kompulzivní poruchou a zjistit, zda některé demografické či klinické charakteristiky pacienta nebo zlepšení po prvnich dvou týdnech léčby mohou předpovědět úspěch či neúspěch léč¬ by na jejím konci. Pacienti s farmakorezistentní OCD, kteří byli přijati do KBT programu, byli dále léčeni psychofarmaky v nezměněné podobě a k této léčbě byla přidána systematická kognitivně-behaviorální terapie ve skupině. Stav pacientů byl na počátku a na konci léčby hodnocen pomoci Y-BOCS, CGI, BDI, BAI, stupnice disociativní zkušenosti (DES). Výsledky: Do léčby vstoupilo 37 pacientů. V průběhu léčby došlo ke statisticky významnému zlepšení ve specifické škále Y-BOCS, v hodnocení depresivity v BDI a v celkovém klinickém dojmu v CGI, nikoliv však v hodnocení úzkosti v BAI. Relativní zlepšení ve Y-BOCS korelovalo s věkem niaiiJKsiisii bylo statisticky významně více pacientů s pozitivní rodinnou zátěží. Zlepšení za prvni dva týdny léčby vysoce významně predikuje celkové zlepšení V průběhu ótýdenního pobytu.
Introduction: Predictors of successful therapeutic intervention for obsessive-compulsive disorder are not well known. Method: The aim of our study was to assess the effectiveness of a comprehensive treatment program (CBT and antidepressants) pharmacoresistant in-patient with obsessive compulsive disorder, and to determine whether certain demographic and clinical characteristics of the patient, or improvement after the first two weeks of treatment could predict the success or failure of the treatment at the end. The patients were evaluated using Y-BOCS, CGI, BDI, BAI, and DES before and after the treatment. Results: 37 patients entered treatment program. During the treatment there was a statistically significant improvement in a specific sca- le Y-BOCS, in the evaluation of depression in the BDI and in the overall clinical impression in CGI, but not in the assessment of anxiety in BAI. The relative improvement in Y-BOCS correlated with the age of the di sorder onset, but not with the severity of OCD, duration of OCD, the dose of pharmacs used. Among the non-responders was statistically significantly more pat ients with a positive family history of psychiatric disorders. Improveme nt over the first two weeks of treatment highly sig- nificantly predicts overall improvement over the 6-week stay
