BACKGROUND: Myasthenia gravis (MG) is a rare autoimmune disorder with significant clinical implications, including life-threatening myasthenic crises and exacerbations. Understanding real-world treatment patterns, especially associated direct medical costs, is essential for the effective management of healthcare delivery. METHODS: We conducted a descriptive cohort study using health administrative claims data from the Czech Republic covering more than 1,500 prevalent MG patients. Data were analysed for healthcare resource utilization, medication costs, and hospitalization rates related to MG and its complications. RESULTS: Acetylcholine inhibitors and corticosteroids were widely prescribed, with 91.1% and 75.2% of patients receiving them at least once, respectively. Immunosuppressive therapy was given to 45.2% of patients. Myasthenic crises occurred in 2% of patients, with a mean hospitalization cost of 21,020 EUR, while exacerbations occurred in 9.2% of patients, with lower costs (5,951 EUR per hospitalization). Outpatient intravenous immunoglobulin and plasma exchange therapies incurred additional costs of 20,700 EUR and 18,206 EUR per person-year, respectively. The mean total cost per patient-year was 1,271 EUR, with significant cost differences among patients with different treatment patterns. CONCLUSION: This study offers real-world insights into the treatment patterns and associated direct medical costs of MG in the Czech Republic. Myasthenic crises and exacerbations pose considerable cost burdens, while outpatient therapies and common pharmacotherapies are less costly. These findings are vital for healthcare planning, economic evaluation, and resource allocation, potentially leading to enhanced patient care and outcomes.

Young adults are a substantial driver of lagging vaccination against COVID-19 worldwide. We aimed to understand what vaccine or vaccination environment attributes may affect young adults' vaccine inclination. We contacted a convenience sample of 1415 students to recruit a minimum of 150 individuals for a web-based discrete choice experiment. The respondents were asked to choose one of two hypothetical vaccines, defined by six attributes-vaccine efficacy, risk of mild side effects, protection duration, administration route, recommender, and travel time to the vaccination site. Individual preferences were calculated with the Markov chain Monte Carlo hierarchical Bayes estimation. A total of 445 individuals (mean age 24.4 years, 272 (61.1%) women) completed the survey between 22 March and 3 May 2021. Vaccine protection duration (28.3 (95% CI, 27.0-29.6)) and vaccine efficacy in preventing COVID-19 (27.5 (95% CI, 26.3-28.8)) were the most important, followed by the risk of vaccine side effects (17.3 (95% CI, 16.2-18.4)). Individuals reluctant or unsure about vaccination (21.1%) prioritized the potential for mild side effects higher and vaccine efficacy lower than the vaccine-inclined individuals. New vaccination programs that target young adults should emphasize the protection duration, low risk of vaccine side effects, and high efficacy.

Tento článek prezentuje p řístupy použité ke sledování nákupů zdravotnických přístrojů v České republice. Vytvo řený systém umožňuje sledování a hodnocení efektivity nákupů a poskytuje objektivní informace všem osobám zainteresovaným v plánování nákupů zdravotnických přístrojů. Byl navržen a otestován proces sběru dat. Byly vytvořeny reporty, které jsou založené na multikriteriálním porovnání v rámci komplexního datového modelu. Tento model formalizuje data o nákupech a umožňuje formulovat hypotézy o efektivitě nákupů. Multikriteriální přístup je založen na hodnocení nákupů zdravotnických přístrojů na základě sady kritérií specifikovaných uživatelem. Mezi tato kritéria patří smluvní podmínky nákupu, cena zdravotnického přístroje a technické parametry. Navržený systém umožňuje uživatelům porovnávat kupní smlouvy, identifikovat jejich slabiny a plánovat nákupy zdravotnické techniky.
This article presents the approaches used for tracking purchases of medical equipment in the Czech Republic. Developed tracking system allows monitoring and evaluation of the procurement effectiveness and provides objective information to all parties involved in planning of medical equipment acquisition. The data collection process was designed and tested. Reports were designed and created based on multi-criteria comparisons within a complex data model. This model is based on formalization of procurement information and is designed to formulate hypotheses about the procurement effectiveness. Multi-criteria approach is based on the evaluation of medical devices purchases based on a set of criteria specified by the user. These criteria include the terms and conditions of purchase, the price of medical equipment and technical parameters. Designed system allows users to compare the purchase orders, identify their weaknesses and use this information to plan further acquisitions of medical equipment.
