Priame (direktné) perorálne antikoagulanciá (DOAK) sú v súčasnosti jednou z alternatív antikoagulačnej liečby, ktorá nevyžaduje rutinné laboratórne monitorovanie, je relatívne bezpečná, efektívna a svojím spôsobom dávkovania výhodná aj pre pacienta. Existujú však určité klinické situácie, v ktorých je laboratórna kontrola účinnosti týchto liekov nevyhnutná (napr. u pacientov s obličkovým alebo pečeňovým poškodením, pri podozrení na nespoluprácu pacienta či jeho predávkovanie). V článku sa zaoberáme mechanizmom pôsobenia DOAK, farmakologickými vlastnosťami a s tým súvisiacou detekciou ich vplyvu na hemostázu. Tieto informácie dopĺňa časť zaoberajúca sa klinickými aspektami užívania DOAK vrátane život ohrozujúcich stavov a možností zvrátenia účinku týchto liekov.
Direct oral anticoagulants (DOACs) are currently one of the alternatives of anticoagulant treatment that does not require routine laboratory monitoring, is relatively safe, effective and thanks to the dosage scheme also convenient for the patient. However, there are some clinical situations, in which the laboratory control of their effectiveness is inevitable (e.g. in patients with renal or liver dysfunction, in the case of the suspicion of non-compliance of the patient or his overdose). In the article, we describe mechanism of action of DOACs, their pharmacological properties and related detection of their influence on hemostasis. This information is enriched by the part about clinical aspects of the use of DOACs including life-threatening states and possibilities of the reversal of these agents.

Kardiovaskulárne ochorenia sú vedúcou príčinou mortality u pacientov s diabetes mellitus (DM) 2. typu. DM je jedným z rizikových faktorov vzniku venózneho tromboembolizmu (VTE). DM je známym rizikovým faktorom ischemickej choroby srdca vyplývajúcej z aterosklerózy, ale jeho úloha v etiopatogenéze VTE nebola doposiaľ celkom upresnená. Veľa odhadov z predchádzajúcich štúdií totiž nezohľadnilo potenciálny skresľujúci účinok obezity. Keďže klinické prejavy a diagnostika oboch chorobných stavov sú veľmi dobre známe, budeme sa v článku venovať predovšetkým etiopatogenézou a prevenciou tejto život ohrozujúcej komplikácie DM. Zároveň pripájame kapitolu o VTE pri DM a CoronaVirus Disease 19 (COVID-19).
Cardiovascular diseases are the leading cause of mortality in patients with diabetes mellitus (DM) type 2. DM is one of the risk factors of the development of venous thromboembolism (VTE). DM is well-known risk factor of ischemic heart disease associated with atherosclerosis, but its role in the etiopathogenesis of VTE has not been fully clarified. Many estimates from previous studies did not took into account potentially confounding effect of obesity. As the clinical manifestations and diagnostics of both clinical states are described elsewhere, in this article, we will focus predominantly on etiopathogenesis and prevention of this life-threatening complication of DM. Simultaneously, we add the chapter about VTE in DM and CoronaVirus Disease 19 (COVID-19).
