24hodinový profil pohybového chování může hrát významnou roli u pacientů s kardiovaskulárními onemocněními, mezi které patří také srdeční selhání. Zároveň je vyšší výskyt nežádoucích srdečních příhod spojován s nižší úrovní fyzické zdatnosti. Cílem studie bylo proto popsat 24hodinový pohybový profil pacientů se srdečním selháním s ohledem na jejich fyzickou zdatnost. Pohybové chování bylo zjišťováno pomocí tří akcelerometrů umístěných na odlišných částech těla (zápěstí, pas, stehno) u 20 pacientů (67,7 ± 6,34 roku) se srdečním selháním třídy NYHA II a II-III po dobu sedmi po sobě jdoucích dní. Fyzická zdatnost byla hodnocena na základě výsledků dosažených v krátké baterii pro testování fyzické zdatnosti seniorů. Pro popis výsledků byla použita deskriptivní statistika a Hottelingův test pro porovnání kompozic (intenzita a postura) pohybového chování mezi skupinami pacientů s odlišnou fyzickou zdatností. Pacienti se sníženou zdatností měli v průměru vyšší objem sedavého chování a méně zdraví prospěšné pohybové aktivity. Při srovnání pohybových profilů skupin pacientů s odlišnou fyzickou zdatností nebyly nalezeny žádné statisticky významné rozdíly (intenzita: p = 0,855; postura: p = 0,261). Interkvartilové rozpětí týdenní pohybové aktivity střední až vysoké intenzity (IQR = 351,81 min) ukazuje na vysokou míru variability mezi sledovanými pacienty. Výsledky pilotní studie naznačují, že bez ohledu na fyzickou zdatnost a stejné zařazení ve třídě NYHA se 24hodinové profily pohybového chování mohou u pacientů se srdečním selháním velmi lišit a je třeba k nim přistupovat individuálně při úpravě jejich 24hodinového pohybového režimu s cílem splnit mezinárodní doporučení. Využití akcelerometrů se jeví jako vhodné a v klinických podmínkách reálné jak pro popis pohybového chování, tak i pro případnou kontrolu jeho změny.
24-hour physical behavior profile may take part in an important preventive role in patients with cardiovascular disease, including heart failure. A higher risk of adverse cardiac events is also associated with a lower level of physical fitness. Therefore, the aim of this study was to describe the 24-hour physical behavior profile of patients with heart failure regarding their different levels of physical fitness. Physical behavior was assessed using three accelerometers placed on different parts of the body (wrist, hip, and thigh). The measurement concerned 20 patients (67.7±6.34 years) with NYHA class II and II-III heart failure for seven consecutive days. Physical fitness was assessed using the results of a short physical performance battery for physical fitness testing in the elderly. Descriptive statistics were used to describe the results, and the Hotteling's test was used to compare the components (intensity and postural) of physical behavior between groups of patients with different levels of physical fitness. On average, patients with lower physical fitness had more sedentary behavior and less health-promoting physical activity. No statistically significant differences were found when comparing the physical behavior profiles of groups of patients with different levels of physical fitness (intensity: p = 0.855; postural: p = 0.261). The interquartile range in weekly physical activity of moderate to vigorous intensity (R = 351.81 min) suggests a high degree of variability among the patients studied. The results of the pilot study suggest that regardless of physical fitness and the same NYHA class classification, 24-hour physical behavior profiles of heart failure patients may vary considerably and should be treated individually when modifying their 24-hour movement regimen to meet international recommendations. The use of accelerometers seems to be appropriate and feasible in clinical settings, both for describing physical behavior and for possible control of its change

Zvyšující se incidence pádů je celosvětovým problémem, který ovlivňuje nejen kvalitu života zainteresovaných osob, ale má i rozsáhlý ekonomický dopad, a to jak přímý – zahrnující náklady na zdravotní péči, tak i nepřímý – zahrnující ztrátu produktivity osob postižených pádem a jejich rodinných příslušníků. Je známo, že u seniorů se incidence pádů zvyšuje, a proto se nabízí otázka, k jakým změnám, které mohou ovlivňovat riziko a výskyt pádů, ve stáří dochází. Z biomechanického hlediska jsou zásadní změny, které se týkají stavebních článků systému (svaly, kosti, klouby, vazy, fascie, potažmo smysly), ty však nelze separovat od změn neurofyziologických, tedy od fungování centrálního nervového systému (vč. receptorů). Ve stáří se mění posturální nastavení, dochází k omezení rozsahu pohybu v kloubech, zhoršuje se kvalita měkkých tkání a kostí (sarkopenie, osteopenie), snižuje se svalová síla, ztrácí se zraková ostrost a zhoršuje se sluch a fungování vestibulárního aparátu. Počet receptorů a jejich citlivost se snižuje, zpomaluje se vedení nervových vzruchů, pomalejší je i proces vyhodnocování informací, tvorba odpovědi a nalezení adekvátního pohybového schématu. Ovlivněn je také přenos vzruchu na nervosvalovou ploténku a mění se pohybové stereotypy. Změna stavby jedné struktury se zákonitě projeví i ve změně funkce dané struktury a ovlivňuje funkci i stavbu dalších struktur. Všechny změny, ke kterým dochází postupně v průběhu života člověka, mají pravděpodobně za následek zhoršení schopnosti lidského organizmu pružně a efektivně reagovat na zátěžové situace, a zabránit tak pádu.
The growing incidence of falls is a global problem that not only affects the quality of life of those involved, but also has a far-reaching economic impact, both direct – including health care costs, and indirect – involving the loss of productivity of those affected and their families. It is known that the incidence of falls increases in the elderly, and therefore the question arises as to what changes occur in old age that may affect the risk and incidence of falls. From a biomechanical point of view, there are fundamental changes that affect the building blocks of the system (muscles, bones, joints, ligaments, fascia, and also the senses), but these cannot be separated from neurophysiological changes, i.e. from the functioning of the central nervous system (including receptors). In old age, the postural setting changes, the range of motion in the joints is reduced, the quality of soft tissues and bones (sarcopenia, osteopenia) deteriorates, muscle strength decreases, visual acuity is lost and hearing and the vestibular system function deteriorate. The number of receptors and their sensitivity is decreasing, the conduction of nerve impulses is slowing down, the process of evaluating information, creating responses and finding adequate movement patterns is also slower. Neuromuscular junction is also affected and movement stereotypes change. A change in the construction of one structure will naturally result in a change in the function of the given structure and affects the function and construction of other structures. All the changes that occur gradually during a person’s life are likely to result in a deterioration of the human body’s ability to respond flexibly and effectively to stressful situations and thus prevent a fall.
Nowadays, gait assessment in the real life environment is gaining more attention. Therefore, it is desirable to know how some factors, such as surfaces (natural, artificial) or dual-tasking, influence real life gait pattern. The aim of this study was to assess gait variability and gait complexity during single and dual-task walking on different surfaces in an outdoor environment. Twenty-nine healthy young adults aged 23.31 ± 2.26 years (18 females, 11 males) walked at their preferred walking speed on three different surfaces (asphalt, cobbles, grass) in single-task and in two dual-task conditions (manual task-carrying a cup filled with water, cognitive task-subtracting the number 7). A triaxial inertial sensor attached to the lower trunk was used to record trunk acceleration during gait. From 15 strides, sample entropy (SampEn) as an indicator of gait complexity and root mean square (RMS) as an indicator of gait variability were computed. The findings demonstrate that in an outdoor environment, the surfaces significantly impacted only gait variability, not complexity, and that the tasks affected both gait variability and complexity in young healthy adults.

Inerciálne senzory prešli v poslednom období rýchlym vývojom umožňujúcim ich bezproblémové využitie v klinickej praxi. Vďaka svojej prenositeľnosti a malým rozmerom je možné s nimi pracovať nie len v kontrolovaných laboratórnych podmienkach, ale i v domácom prostredí. Cieľom tohto textu bolo preto zhrnúť aktuálne dostupné možnosti týkajúce sa využitia inerciálnych senzorov v klinickej praxi, a to nie len z pohľadu pohybových činností, ktoré je možné skúmať, ale i z pohľadu najčastejšie využívaných metodologických prístupov. Podrobne sme sa venovali hodnoteniu posturálnej stability, inštrumentálnej verzii chodeckých klinických testov, Timed Up and Go a Sit-to-Stand testu, ale taktiež hodnoteniu pohybovej aktivity.
Inertial sensors with their rapid development in the recent years have become useful tools in clinical practice. They can be used in a controlled laboratory environment as well as home environment because of their portability and small size. The aim of this manuscript was to summarise topical possibilities for the use of inertial sensors in clinical practice, including information about suitable activities that can be studied and methodological approaches for their quantification. The assessment of postural stability, instrumented versions of clinical walking tests, Timed Up and Go or Sit-to-Stand tests as well as physical activity monitoring are discussed in detail.
