Úvod: Práce hasičů patří mezi rizikové z hlediska možného ohrožení zdraví. Během zásahů jsou hasiči vystavováni nebezpečným zplodinám hoření, které mohou mít akutní i chronický účinek. Cílem tohoto projektu bylo zjistit, k jakým změnám ve ventilačních parametrech dochází u hasičů časně po zásahu u požáru. Metodika: Projektu se účastnilo 24 hasičů (průměrný věk 37,1 let), kteří byli v rámci výcviku exponováni zplodinám hoření při hašení v zařízení simulujícím reálné prostředí požáru (flashover kontejner). Před zásahem, ihned po něm a druhý den byla provedena spirometrie. Před expozicí bylo provedeno měření vydechovaného oxidu dusnatého (FeNO) k posouzení možného alergického zánětu v dýchacích cestách. Výsledky: Průměrná hodnota FeNO byla u sledovaných osob 18,8 ppb (rozmezí 5–59). Při měření klidové spirometrie před zásahem byla u jedné osoby snížená hodnota FEV1 (usilovně vydechnutý objem za 1. sekundu), snížené hodnoty obstrukce v periferních dýchacích cestách (MEF – maximální průtoky vydechnutého vzduchu v různých úrovních usilovné vitální kapacity) byly nalezeny u 18 osob, zvýšená hodnota odporů v dýchacích cestách (Rocc) u 12 osob. Vitální kapacita (VC) plic byla u všech osob v normě. Ihned po cvičném zásahu došlo ve skupině sledovaných osob ke zlepšení průměrné hodnoty všech sledovaných ventilačních parametrů. Změny byly významné v parametrech FEV1, MEF25 (maximální průtok vydechnutého vzduchu v 25 % usilovné vitální kapacity), AEX (plocha pod výdechovou částí křivky) (p ≤ 0,001), FVC (usilovná vitální kapacita) a Rocc (p < 0,01). Druhý den došlo naopak ke zhoršení sledovaných ventilačních parametrů, a to jak vzhledem k hodnotám ihned po zásahu, tak i ke vstupním hodnotám. Změny druhý den po zásahu v porovnání s hodnotami před zásahem byly významné pro parametr VC, Rocc (p < 0,001), FEV1 (p < 0,01). Závěr: Sledování ventilačních parametrů během simulovaného požárního zásahu ukázalo, že ke zhoršení došlo až s odstupem – druhý den, a to v dynamických i statických parametrech. Z hlediska regenerace organismu hasičů je tedy velmi důležitá rekonvalescence ve dnech následujících po zásahu.
Introduction: The work of firefighters is one of the most risk in terms of potential health hazards. Firefighters are exposed to hazardous combustion products, which can have both acute and chronic effects. The aim of this project was to investigate what changes in ventilation parameters occur in firefighters early after a fire exposure. Methodology: Twenty-four firefighters (mean age 37.1 years) participated in the project and were exposed to combustion products during training in a device simulating a real fire exposure (flashover container). Spirometry was performed before, immediately after and the next day. Before exposure, exhaled nitric oxide (FeNO) measurement was taken to assess possible allergic inflammation in the airways. Results: The mean FeNO value in the subjects was 18.8 ppb (range 5–59). When measuring resting spirometry before the intervention, one person had a reduced FEV1 value (forced expiratory volume in 1 second), reduced values of peripheral airway obstruction (MEF-maximal expiratory flow rates at different levels of forced vital capacity) were found in 18 persons, increased values of airway resistance (Rocc) in 12 persons. The vital capacity (VC) of the lungs was normal in all persons. Immediately after the training intervention, the mean value of all the monitored ventilatory parameters improved in the group of subjects. The changes were significant in the parameters FEV1, MEF25 (maximum expiratory flow rate at 25 % of the forced vital capacity), AEX (area under the expiratory part of the curve) (p ≤ 0.001), FVC (forced vital capacity) and Rocc (p < 0.01). On the second day, there was a deterioration of the monitored ventilatory parameters, both with respect to the values immediately after the exposure and to the initial values. Changes on the second day after the exposure compared to pre-exposure values were significant for the VC parameter, Rocc (p < 0.001), FEV1 (p < 0.01). Conclusion: The monitoring of ventilation parameters during the simulated fire incident showed that the deterioration occurred only after the second day, both in dynamic and static parameters. Thus, from the point of view of the recovery of the firefighters’ organism, recovery in the days following the intervention is very important.

OBJECTIVES: Bilirubin is a potent endogenous antioxidant and immunomodulating substance, which is also implicated in both cell signalling and various metabolic pathways. Mild elevation of systemic bilirubin concentrations provides substantial protection against many diseases of civilization. Rare published reports have suggested that serum bilirubin might also be relevant to sports performance. The purpose of the current study was to evaluate serum bilirubin concentrations and the prevalence of Gilbert syndrome (GS) in elite athletes. METHODS: The study was carried out in 536 consecutive healthy elite athletes and in 2594 individuals of the Czech post-MONICA study representing the general Czech population. Serum bilirubin concentrations, the prevalence of benign hyperbilirubinemia > 17 μmol/L (1 mg/dL, a phenotypic sign of GS), and a variant of the UGT1A1 gene promoter responsible for GS manifestation in Caucasians (rs81753472) were evaluated in study subjects. RESULTS: Compared to the general Czech population, significantly higher serum bilirubin concentrations were found in elite athletes (9.6 vs. 11.6 μmol/L, p < 0.001), both in men (11.3 vs. 12.6 μmol/L, p < 0.001) and women (8.3 vs. 10.5 μmol/L, p < 0.001). Furthermore, the prevalence of GS was also significantly higher in elite athletes (9.6 vs. 22%, p < 0.001) together with the tendency to higher frequencies of the genotypes (TA)7/7 and (TA)6/7 UGT1A1. CONCLUSION: Elite athletes have significantly higher concentrations of serum bilirubin, the most potent endogenous antioxidant substance known. Simultaneously, the prevalence of GS syndrome is also much higher in elite athletes, suggesting that a mild elevation of serum bilirubin might predispose to better sports performance.

Souhrn: Kazuistické sdělení pojednává o náhodném nálezu laterální periodontální cysty. Upozorňuje na některá úskalí diagnostiky cystických afekcí čelistních kostí, založené na podrobném klinickém a rentgenovém vyšetření. Popisuje možnosti terapie této cysty a prevenci vzniku její recidivy, spočívající v kompletní exstirpaci cystického vaku. Definitivní diagnózu laterální periodontální cysty potvrdí či spíše stanoví histologické vyšetření.
Summary: Case report deals with an accidental finding of the lateral periodontal cyst. It warns about some pitfalls of the diagnostics of cystis lesions of jaws based on the detailed clinical and X-ray examination. There are described possibilities of the treatment and prevention of the recurrence of the cyst, consisting of the complete removal of the cystic saccule. Definitive diagnosis of the lateral periodontal cyst is verified or rather determined by histological examination.

The Dothideomycete Leptosphaeria maculans, a worldwide fungal pathogen of oilseed rape (Brassica napus), secretes a broad spectrum of molecules into the cultivation medium during growth in vitro. Here, candidate elicitor molecules, which induce resistance in B. napus to L. maculans, were identified in the cultivation medium. The elicitation activity was indicated by increased transcription of pathogenesis-related gene 1 (PR1) and enhanced resistance of B. napus plants to the invasion of L. maculans. The elicitation activity was significantly lowered when the cultivation medium was heated to 80°C. Active components were further characterized by specific cleavage with the proteolytic enzymes trypsin and proteinase K and with glycosidases α-amylase and β-glucanase. The elicitor activity was eliminated by proteolytic digestion while glycosidases had no effect. The filtered medium was fractionated by either ion-exchange chromatography or isoelectric focusing. Mass spectrometry analysis of the most active fractions obtained by both separation procedures revealed predominantly enzymes that can be involved in the degradation of plant cell wall polysaccharides. This is the first study searching for L. maculans-specific secreted elicitors with a potential to be used as defense-activating agents in the protection of B. napus against L. maculans in agriculture. © 2016 American Institute of Chemical Engineers Biotechnol. To achieve host colonization, successful pathogens need to overcome plant basal defences. For this, (hemi)biotrophic pathogens secrete effectors that interfere with a range of physiological processes of the host plant. AvrLm4-7 is one of the cloned effectors from the hemibiotrophic fungus Leptosphaeria maculans 'brassicaceae' infecting mainly oilseed rape (Brassica napus). Although its mode of action is still unknown, AvrLm4-7 is strongly involved in L. maculans virulence. Here, we investigated the effect of AvrLm4-7 on plant defence responses in a susceptible cultivar of B. napus. Using two isogenic L. maculans isolates differing in the presence of a functional AvrLm4-7 allele [absence ('a4a7') and presence ('A4A7') of the allele], the plant hormone concentrations, defence-related gene transcription and reactive oxygen species (ROS) accumulation were analysed in infected B. napus cotyledons. Various components of the plant immune system were affected. Infection with the 'A4A7' isolate caused suppression of salicylic acid- and ethylene-dependent signalling, the pathways regulating an effective defence against L. maculans infection. Furthermore, ROS accumulation was decreased in cotyledons infected with the 'A4A7' isolate. Treatment with an antioxidant agent, ascorbic acid, increased the aggressiveness of the 'a4a7' L. maculans isolate, but not that of the 'A4A7' isolate. Together, our results suggest that the increased aggressiveness of the 'A4A7' L. maculans isolate could be caused by defects in ROS-dependent defence and/or linked to suppressed SA and ET signalling. This is the first study to provide insights into the manipulation of B. napus defence responses by an effector of L. maculans.

Interaction of a plant with a fungal pathogen is an encounter with hundreds of molecules. In contrast to this, a single molecule often decides between the disease and resistance. In the present article, we describe the defense responses triggered by AvrLm1, an avirulence gene from a hemibiotrophic ascomycete, Leptosphaeria maculans, responsible for an incompatible interaction with Brassica napus. Using multiple hormone quantification and expression analysis of defense-related genes, we investigated signaling events in Rlm1 plants infected with two sister isolates of L. maculans differentiated by the presence or absence of AvrLm1. Infection with the isolate carrying AvrLm1 increased the biosynthesis of salicylic acid (SA) and induced expression of the SA-associated genes ICS1, WRKY70, and PR-1, a feature characteristic of responses to biotrophic pathogens and resistance gene-mediated resistance. In addition to SA-signaling elements, we also observed the induction of ASC2a, HEL, and CHI genes associated with ethylene (ET) signaling. Pharmacological experiments confirmed the positive roles of SA and ET in mediating resistance to L. maculans. The unusual cooperation of SA and ET signaling might be a response to the hemibiotrophic nature of L. maculans. Our results also demonstrate the profound difference between the natural host B. napus and the model plant Arabidopsis in their response to L. maculans infection.
