Background von Willebrand disease (VWD) is a genetic bleeding disorder caused by defects of von Willebrand factor (VWF), quantitative (type 1 and 3) or qualitative (type 2). The laboratory phenotyping is heterogenic making diagnosis difficult. Objectives Complete laboratory analysis of VWD as an expansion of the previously reported cross-sectional family-based VWD study in the Czech Republic (BRNO-VWD) and Slovakia (BRA-VWD) under the name "Heart of Europe," in order to improve the understanding of laboratory phenotype/genotype correlation. Patients and Methods In total, 227 suspected VWD patients were identified from historical records. Complete laboratory analysis was established using all available assays, including VWF multimers and genetic analysis. Results A total of 191 patients (from 119 families) were confirmed as having VWD. The majority was characterized as a type 1 VWD, followed by type 2. Multimeric patterns concordant with laboratory phenotypes were found in approximately 83% of all cases. A phenotype/genotype correlation was present in 84% (77% type 1, 99% type 2, and 61% type 3) of all patients. Another 45 candidate mutations (23 novel variations), not found in the initial study, could be identified (missense 75% and truncating 24%). An exon 1-3 gene deletion was identified in 14 patients where no mutation was found by direct DNA sequencing, increasing the linkage up to 92%, overall. Conclusion This study provides a cross-sectional overview of the VWD population in a part of Central Europe. It is an addition to the previously published BRNO-VWD study, and provides important data to the International Society of Thrombosis and Haemostasis/European Association for Haemophilia and Allied Disorders VWD mutation database with identification of novel causal mutations.

Inhibítory faktora VIII (FVIII) predstavujú závažnú komplikáciu liečby hemofílie. Prioritou liečby pacientov s inhibítormi je eradikácia inhibítorov a indukcia imunitnej tolerancie (ITI), ktorá umožní opätovnú liečbu faktorom VIII. Úspešnosť ITI je 60–85 % čo znamená, že časť pacientov s inhibítormi je stále odkázaná na menej účinnú liečbu prípravkami s "bypassing" aktivitou. V súčasnosti sú vo vývoji nefaktorové lieky, ktoré umožnia prevenciu krvácaní aj u pacientov s inhibítormi. Prvým liekom uvedeným do praxe je bišpecifická protilátka proti faktorom IXa a X – emicizumab, ktorý bol registrovaný pre profylaxiu nielen u pacientov s inhibítormi, ale aj bez inhibítorov. Dostupnosť nefaktorovej liečby môže zásadne zmeniť doterajšiu paradigmu liečby hemofílie, vrátane prístupu k včasnej profylaxii a manažmentu ITI. V práci diskutujeme význam ITI, prezentujeme výsledky ITI v našom centre a prvé skúsenosti s nefaktorovou liečbou emicizumabom u troch pacientov vo veku 3,5–7 rokov. U dvoch pacientov sme emicizumab indikovali po zlyhaní ITI a u jedného pacienta po úspešnej ITI pre objektívne prekážky farmakokineticky riadenej profylaxie s FVIII. Pri absencii krvácania počas 7–13 mesiacov hodnotíme profylaxiu bišpecifickou protilátkou ako vysoko efektívnu.
Factor VIII (FVIII) inhibitors are the most challenging complication of haemophilia treatment. The priority is to eradicate the inhibitors and induce immune tolerance (ITI) to allow resumption of therapy with FVIII. The success rate of ITI is 60-85%, which means that some patients with inhibitors remain dependent on less effective treatment with bypassing agents. New, non-factor drugs are currently under development for effective prophylaxis in inhibitor patients. The first non-factor agent already in use is emicizumab, a bispecific antibody against factors IXa and X, which has been authorized for prophylaxis not only in patients with inhibitors but also in those without inhibitors. The availability of non-factor treatment can fundamentally change the current paradigm of haemophilia treatment, including access to early prophylaxis and ITI management. We discuss here the importance of ITI and we present the results of ITI in our centre and our first experience with non-factor treatment using emicizumab in three patients aged 3.5-7 years. Emicizumab was indicated in two patients with ITI failure and in one patient after successful ITI because of objective obstacles to pharmacokinetic guided prophylaxis with FVIII. In the absence of bleeding for 7-13 months, we evaluate bispecific antibody prophylaxis as being highly effective.

Nedavne pokroky v molekulovej diagnostike hemofilie položili zaklady pre študium genotypovych a fenotypovych suvislosti a študium vplyvu genovych mutacii na vyskyt komplikacii choroby, akymi su inhibitory FVIII. V praci prezentujeme prve vysledky Narodneho genetickeho hemofilickeho programu, založeneho v r. 2014. Geneticky sme doteraz vyšetrili 173 pacientov s hemofiliou A, z nich 141 s ťažkym stupňom hemofilie, čo predstavuje 60 % populacie ťažkych hemofilikov na Slovensku. V subore ťažkych hemofilikov sme identifikovali 39 roznych mutacii, z nich 18 novych, doposiaľ neevidovanych v medzinarodnej mutačnej databaze. Zastupenie mutacii v našej populacii 141 pacientov s ťažkym stupňom hemofilie je v zhode s literaturou, inverzie intronov 22 a 1 u 59 (42 %), nonsense mutacie u 16 (11 %), veľke delecie u 4 (3 %), frame shift/stop kodon mutacie u 38 (27 %), splice site mutacie u 5 (4 %) a missense mutacie u 19 (13 %) pacientov. Analyza genovych mutacii u 34 pacientov s inhibitormi potvrdila porovnateľne riziko vzniku inhibitora pri inverziach (27 %), nonsense mutaciach (25 %) aj pri frame shift/stop kodon mutaciach (29 %), pričom vyššie riziko inhibitorov nesu mutacie ľahkeho oproti mutaciam ťažkeho reťazca – OR:2,57(0,85–7,77). Hodnotenie efektu imunotolerančnej indukcie (ITI) u 22 pacientov (26 ITI kur) podľa typu mutacie ukazalo kompletnu a parcialnu remisiu v 75 % pri inverziach a v 88–100 % pri ostatnych genovych mutaciach. Vysledky su ovplyvnene malou početnosťou suboru, problematika si vyžaduje ďalšie študium.
Recent advances in molecular diagnosis of haemophilia have enabled the study of genotypic and phenotypic relationships and of the effect of gene mutations on the occurrence of serious disease complications, such as inhibitors. We present the first results of the National Genetic Haemophilia Program, established in 2014. So far, we have investigated 173 haemophilia A patients, of which 141 have severe haemophilia, representing 60% of the population of severe haemophiliacs in Slovakia. In severe haemophiliacs, we identified 39 different mutations, including 18 new mutations not yet listed in the international mutation database. The proportion of mutations in our population of 141 severe haemophiliacs is consistent with literature: inversions of introns 22 and 1 in 59 (42%), nonsense mutations in 16 (11%), large deletions in 4 (3%), frame shift/stop codon mutations in 38 (27%), splice site mutations in 5 (4%) and missense mutations in 19 (13%) of patients. Analysis of gene mutations in 34 inhibitor patients confirmed a comparable risk of inhibitor development in patients with inversions (27%), nonsense mutations (25%) and frame shift/stop codon mutations (29%), with a higher risk of mutations localized in parts of the F8 gene encoding the light chain of FVIII molecule compared to heavy chain mutations – OR: 2.57 (0.85-7.77). Evaluation of the success rate of immune tolerance induction (ITI) in 22 patients (26 ITI courses) according to the gene defects in inhibitor patients showed complete and partial remission in 75% and 88-100% of patients with inversions and other gene mutations, respectively. These results are influenced by the small number of patients and this issue requires further study.

Manažment veľkých operácií u pacientov s hemofíliou predstavuje jednu z najzávažnejších oblastí substitučnej liečby hemofílie. Cieľom našej prospektívnej nerandomizovanej otvorenej štúdie bolo vyhodnotiť liečbu kontinuálnou infúziou (KI) a bolusovými injekciami (BI) FVIII u pacientov s ťažkou hemofíliou A, ktorí sa podrobili veľkým operáciám s rovnakými požiadavkami na intenzitu a trvanie liečby počas 13 dní. Zamerali sme sa aj na hodnotenie prínosu predoperačného vyšetrenia farmakokinetiky (PK) pred KI aj pred BI. Vyhodnotili sme 100 veľkých operácií vykonaných v období rokov 1998–2016 u 60 pacientov bez inhibítora, z nich 49 liečených KI a 51 BI. Medián minimálnych hladín FVIII bol porovnateľný v oboch skupinách [pri KI 68 % (IQR 59–90 %) a pri BI 70 % (57–77 %)], ale pri KI sme pozorovali nižšie, i keď nesignifikantne, krvné straty, pokles hemoglobínu a nižší výskyt závažných krvácaní ako pri BI. Nižší počet pacientov vyžadujúcich transfúziu pri KI (33 % vs. 55 %) dosiahol štatistickú významnosť (p < 0,05). Postupný pokles klírens FVIII počas KI viedol k signifikantne nižšej spotrebe FVIII: v 1. týždni 357 (308–378) IU/kg vs. 426 (375–467) IU/kg a celková spotreba 520 (474–585) IU/kg vs. 612 (554–625) IU/kg, p < 0,001, na čom sa podieľala signifikantná úspora FVIII pri totálnych artroplastikách liečených s KI. Vyšetrenie PK vykonané pred 60 operáciami (44 KI a 16 BI) odhalilo kratší biologický polčas < 10 h až u 17 pacientov (28 %), čo naznačuje, že vyšetrenie PK môže mať význam pre individuálne dávkovanie a časovanie pooperačnej substitučnej liečby nielen pri KI, ale aj pri BI s cieľom zvýšiť bezpečnosť veľkých chirurgických operácií u pacientov s hemofíliou.
Management of major surgery represents the most challenging issue of replacement therapy in severe haemophilia. The aim of our open, prospective, nonrandomised study was to evaluate continuous infusion (CI) of factor VIII (FVIII) and traditional therapy with bolus injections (BI) in severe haemophilia A patients undergoing major surgery with similar requirements regarding intensity and duration of replacement therapy, namely 13 days. We also focused on the benefit of preoperative pharmacokinetics (PK) studies either for CI or BI therapy. We evaluated a total of 100 major surgeries performed between 1998 and 2016 in 60 patients without inhibitors. Of these, 49 and 51 surgeries were performed using CI and BI respectively. Median trough levels of FVIII were comparable in both CI and BI groups [68% (interquartile range 59–90%) vs. 70% (57–77%)]. However, in the CI group we observed a lower, though non-significant, level of blood loss; fall in haemoglobin and number of serious bleedings compared with the BI group. In the CI group, there was a significantly lower number of patients requiring blood transfusions (33% vs. 55%, p < 0.05). Gradual decrease of FVIII clearance during CI resulted in lower week 1 and total FVIII consumption [357 (308–378) IU/kg vs. 426 (375–467) IU/kg and 520 (474–585) IU/kg vs. 612 (554–625) IU/kg, respectively, p < 0.001], especially due to significant FVIII cost reduction in patients with total arthroplasty treated with CI. PK evaluation performed before 60 surgeries (44 CI and 16 BI) revealed a shorter biological half time < 10 h in 17 (28%) patients tested, suggesting the potential of PK for individual tailoring of effective postoperative factor replacement either using CI or BI with the aim of further increasing the safety of major surgical procedures in haemophilia.

Inhibítory faktora VIII (FVIII), ktoré predstavujú závažnú komplikáciu hemofílie A spôsobujúcu neúčinnosť substitučnej liečby, sa vyšetrujú klasickou Bethesda metódou a modifikovanou Nijmegen metódou. V práci sme vyhodnotili koreláciu výsledkov inhibítorov zistených obidvomi metódami a sledovali sme vplyv hraničných titrov inhibítorov na farmakodynamiku (in vivo recovery a vzostup aktivity FVIII) a farmakokinetiku FVIII (biologický polčas – T1/2 a klírens) po jeho intravenóznej aplikácii. Porovnali sme 265 paralelných vyšetrení inhibítora Bethesda a Nijmegen metódou vykonaných u 77 pacientov s ťažkou hemofíliou A; z nich 60 pacientov bez inhibítora (súbor 1) a 17 pacientov s anamnézou inhibítorov (súbor 2) po absolvovaní imunotolerančnej liečby (ITT) s parciálnym (n = 4) alebo kompletným (n = 13) úspechom. Nijmegen metóda spoľahlivo vylúčila falošnú pozitivitu nízkych titrov inhibítora medzi 0,51–0,9 Bethesda jednotiek/ml (BU/ml) zistených Bethesda metódou. Potvrdili sme dobrú koreláciu medzi cut-off 0,7 BU/ml a 0,5 Nijmegen BU/ml (NBU/ml). Hodnotenie 120 vyšetrení farmakodynamiky FVIII v súbore 1 ukázalo pri aktuálnom titri inhibítora 0,2 ? 0,1 NBU/ml hodnoty in vivo recovery (IVR) 109 ? 19,4 % (rozptyl 60–160 %) a vzostup FVIII 2,2 ? 0,7 (1,4–3,5) %/1 IU/kg. V súbore 2 (72 vyšetrení) sme pri aktuálnom titri inhibítora 0,5 ? 0,15 NBU/ml zistili signifikantne nižšie hodnoty IVR 86 ? 32 % (rozptyl 44–136 %) a vzostupu FVIII 1,6 ? 0,6 (0,72–2,8) %/1 IU/kg (p < 0,05). Aj vyšetrenie farmakokinetiky ukázalo signifikantné rozdiely medzi súborom 1 (20 pacientov/20 vyšetrení) a súborom 2 (13 pacientov po ITT/26 vyšetrení): T1/2 11,5 ? 1,8 (8,2–16,2) h versus 8,14 ? 3,61 (2,9–16,4) h a klírens 3,7 ? 1,2 (2,6–6,5) ml/kg/h versus 6,48 ? 2,24 (2,9–11,9) ml/kg/h; (p < 0,05). Záver: Ako referenčnú metódu vyšetrenia inhibítorov odporúčame Nijmegen modifikáciu s hranicou negativity 0,5 NBU/ml. Keďže u časti pacientov po ITT negatívny výsledok inhibítora nekoreluje s výsledkami farmakodynamiky a farmakokinetiky FVIII, pre spoľahlivé hodnotenie úspechu ITT je potrebné ďalšie sledovanie a metódy s vyššou senzitivitou a špecificitou na detekciu inhibítorov.
Inhibitors of FVIII represent serious complication of hemophilia A resulting in the ineffectiveness of replacement therapy. Classical Bethesda method and Nijmegen modification are used for an inhibitor detection. We performed correlation analysis of inhibitors titres investigated by both methods and we evaluated an impact of marginal/low inhibitor titres on pharmacodynamics (in vivo recovery and incremental response of FVIII) and pharmacokinetics (half life-T1/2 and clearance) after intravenous administration of FVIII. We compared the results of 265 parallel inhibitor measurements by Bethesda and Nijmegen assay performed in 77 severe hemophilia A patients; out of them 60 patients with the negative history of inhibitors (Group 1) and 17 patients with history of inhibitors after immune tolerance induction (ITI) (Group 2) with partial (n = 4) and complete success (n = 13). Nijmegen method excluded the false positivity of low titer inhibitors between 0,51 and 0,9 BU/ml detected by Bethesda method and a good correlation between cut-off 0.7 Bethesda units/ml (BU/ml) and 0,5 Nijmegen BU/ml (NBU/ml) was confirmed. Comparison of 120 and 72 investigations of FVIII pharmacodynamics in Group 1 (inhibitor titre of 0,2 ? 0,1 NBU/ml) and in Group 2 (inhibitor titre 0,5 ? 0,15NBU/ml), respectively, showed in vivo recovery 109 ? 19,4 (range 60–160) % and 86 ? 32 (44–136) %, and incremental response 2,2 ? 0,7 (1,4–3,5) %/1IU/kg and 1,6 ? 0,6 (0,72–2,8) %/1 IU/kg, respectively. Twenty pharmacokinetics studies in 20 patients from Group 1 and 26 studies in 13 patients from Group 2 showed the half life 11,5 ? 1,8 h (8,2–16,2 h) and 8,14 ? 3,61 h (2.9–16.4 h), respectively, and a clearance 3,7?1,2 (2,6-6,5) ml/kg/h versus 6,48 ? 2,24 (2,9–11,9) ml/kg/h; (p < 0.05) respectively. Conclusion: We recommend Nijmegen assay as a reference method for inhibitor testing with negativity cut-off 0,5 NBU/ml. However, in several patients after ITI negative inhibitor titre does not correlate with pharmacodynamics and pharmacokinetics of FVIII. Definite evaluation of the ITI success requires further study and new methods of inhibitor detection with a higher sensitivity and specificity.
