INTRODUCTION AND OBJECTIVES: Multivessel primary percutaneous coronary intervention (pPCI) is still often used in patients with ST-elevation myocardial infarction (STEMI) and cardiogenic shock (CS). The study aimed to compare the characteristics and prognosis of patients with CS-STEMI and multivessel coronary disease (MVD) treated with culprit vessel-only pPCI or multivessel-pPCI during the initial procedure. MATERIAL AND METHODS: From 2016 to 2020, 23,703 primary PCI patients with STEMI were included in a national all-comers registry of cardiovascular interventions. Of them, 1,213 (5.1%) patients had CS and MVD at admission to the hospital. Initially, 921 (75.9%) patients were treated with culprit vessel (CV)-pPCI and 292 (24.1%) with multivessel (MV)-pPCI. RESULTS: Patients with 3-vessel disease and left main disease had a higher probability of being treated with MV-pPCI than patients with 2-vessel disease and patients without left main disease (28.5% vs. 18.6%; p < 0.001 and 37.7% vs. 20.6%; p < 0.001). Intra-aortic balloon pump, extracorporeal membrane oxygenation (ECMO), and other mechanical circulatory support systems were more often used in patients with MV-pPCI. Thirty (30)-day and 1-year all-cause mortality rates were similar in the CV-pPCI and MV-pPCI groups (odds ratio, 1.01; 95% confidence interval [CI] 0.77 to 1.32; p = 0.937 and 1.1; 95% CI 0.84 to 1.44; p = 0.477). The presence of 3-vessel disease and the use of ECMO were the strongest adjusted predictors of 30-day and 1-year mortality. CONCLUSIONS: Our data from an extensive all-comers registry suggests that selective use of MV-pPCI does not increase the all-cause mortality rate in patients with CS-STEMI and MVD compared to CV-pPCI.

Perkutánní koronární intervence chronického uzávěru koronární arterie (PCI CTO) patří k nejsložitějším intervencím na věnčitých tepnách s vysokou spotřebou zdrojů a zvýšeným rizikem komplikací. Se současně dostupnými strategiemi a technickým vybavením se úspěšnost výkonu pohybuje u zkušených operatérů nad hranicí 90 %. Úspěšný výkon zlepšuje kvalitu života, redukuje potřebu operační revaskularizace, snížením množství ischemického myokardu a zlepšením systolické funkce levé komory zlepšuje též dlouhodobé přežívání nemocných a zvyšuje toleranci nově vzniklé ischemie. Použití radiálního přístupu u komplexních lézí může snižovat počet vaskulárních komplikací a zlepšuje komfort nemocného. Intravaskulární ultrazvuk (IVUS) umožňuje detailní zobrazení lumen věnčité tepny, dále optimalizaci výsledku komplexních PCI. V případě intervence CTO může být použit k detekci rozsahu postižení tepny, správnému odhadu velikosti stentu, k rozlišení pravého lumen od subintimálního průběhu vodiče a v neposlední řadě k bezpečné punkci proximální části uzávěru v anatomicky nepříznivé situaci.
Percutaneous coronary intervention (PCI) of chronic total occlusions (CTO) is one of the most challenging procedures and complicated interventions on coronary arteries, needs high consumption of equipment and with high risk of complications. With concurrent strategies and technical equipment, the rate of success on hand of expertise is more than 90%. Successful procedure leads to improvement of symptoms, reduction of a need of coronary artery bypass graft (CABG) surgery, improvement of left ventricular systolic function, improved survival of the patients and more tolerance of new onset ischemia. The use of radial approach in complex lesions can reduce the rate of vascular complications and improve patient comfort. Intravascular ultrasound (IVUS) allows detailed visualization of the coronary artery lumen to further optimize the outcome of complex PCI. In the CTO intervention can be used to detect lesion length, enable appropriate stent sizing, verify guidewire location within an artery discriminating a true lumen from subintimal space and last but not least enable safely puncture the proximal cap of CTO with an ambiguous position.

Primární perkutánní koronární intervence (PCI) je v současné době široce preferovanou reperfuzní strategií pro terapii infarktu myokardu, a to vzhledem k superiorním výsledkům ve srovnání s trombolytickou terapií. Nicméně přítomnost masivního intrakoronárního trombu u pacientů s infarktem myokardu je často spojena s nepříznivými procedurálními výsledky, včetně selhání aspirace a neúspěšné reperfuze. Podání nízké dávky intrakoronární trombolýzy u pacientů s infarktem myokardu s ST elevacemi a masivním trombem po insuficientní PCI a trombektomii má za následek zmenšení trombotické nálože, zlepšení epikardiálního flow a zlepšení perfuze (1, 2).
Primary percutaneous coronary intervention (PCI) is currently a widely preferred reperfusion strategy for myocardial infarction therapydue to superior results compared to thrombolytic therapy. However, the presence of massive intracoronary thrombus in patients withmyocardial infarction is often associated with adverse procedural results, including failure of aspiration and failure of reperfusion. Administrationof a low dose of intracoronary thrombolysis in patients with myocardial infarction with ST elevations and massive thrombusafter insufficient PCI and thrombectomy resulted in decreased thrombotic burden, improvement of epicardial flow and perfusion.
