Hantaviruses in Europe cause human hemorrhagic fever with renal syndrome (HFRS) with various degree of severity. The most severe form is caused by the Dobrava/Belgrade virus (DOBV), associated with the rodent Apodemus flavicollis. During the last decade cases of infection caused by DOBV have been reported in Central Europe. The present study is a report on two Czech patients with severe HFRS who were infected during their stay in a mountain hut in Northern Slovakia. The two patients, combined with a third case observed in the same year in a nearby village in the Czech Republic, suggest that this region in Central Europe has to be considered as endemic for HFRS.

Hantaviry patří mezi tzv. emerging infections. Jsou to RNA viry rodu Hantavirus patřící do čeledi Bunyaviridae. Jejich genom je tvořen třemi segmenty RNA. Hantaviry kolují v přírodních ohniscích mezi hlodavci, se kterými se dlouhodobě vyvíjely v koevoluci a mají proto ke svým specifickým hostitelům těsný vztah a areál výskytu jednotlivých hantavirů se zpravidla překrývá s areálem výskytu druhu jejich hostitele. U člověka vyvolávají patogenní genotypy ve Starém světě hemoragickou horečku s renálním syndromem a V Severní a Jižní Americe hantavirový plicní syndrom. V práci je přehledně popsána virologie, biologie, etiopatogeneza, klinika a epidemiologie hantavirů a nákaz jimi vyvolávaných. V Evropě byly prokázány tři hlavní hantavirové genotypy: Puumala, Dobrava a Tula. Mimoto byl z genotypu Dobrava vyčleněn distinktní genotyp Saaremaa, jehož areál výskytu bude nutno ještě upřesnit. V areálu výskytu lumíků na severu Evropy je nutno počítat ještě s hantavirem Topografov. Nejzávažnější průběh onemocnění vyvolává virus Dobrava, virus Tula se pokládá za nepatogenní. Promořenost obyvatel hantaviry se v Evropě pohybuje kolem 1 %, ale v endemických oblastech a u exponovaných profesí dosahuje mnohem větších hodnot. V České republice byla prokázána existence všech 3 hlavních genotypů, promořenost dospělých obyvatel je v průměru kolem 1 %. U lidí bylo u nás prokázáno několik onemocnění, charakterizováných jako intersticiální nefritida. Problematika hantavirů je však dosud většinou na pokraji zájmu odborníků.
Hantaviruses belong to the so-called emerging infections. They are RNA viruses of the genus Hantavirus belonging to the Bunyaviridae family. Their genome is made up of three RNA segments. In natural foci they circulate among rodents, with whom they developed in long-lasting co-evolution and in consequence they have close links with their specific hosts. The area where individual hantaviruses are present usually coincides with the area of their host species. In humans in the Old World the pathogenic genotypes produce a haemorrhagic fever with a renal syndrome, while in North and South America we find a hantavirus pulmonary syndrome. The paper describes the virology, biology, etiopathogenesis, clinical aspects and epidemiology of hantaviruses and of the infections they cause. Three main hantavirus genotypes were demonstrated in Europe: Puumala, Dobrava and Tula. Moreover, a separate genotype Saaremaa was isolated from the genotype Dobrava - the area of its presence remains to be specified. In northern Europe, the home of lemmings, we also encounter the hantavirus Topografov. The most severe course of the disease is caused by the virus Dobrava, while the virus Tula is thought to be non-pathogenic. The immunity rate of the European population is believed to be 1 %; however, in endemic regions and in exposed professions this value is much higher. The existence of 3 main genotpes was confirmed in the Czech Republic with an immunity rate of adults approx. 1 %. In humans several disorders described as interstitial nephritis were diagnosed in this country. So far, however, hantaviruses are only of marginal interest to specialists.
