Autori retrospektívne podávajú správu o stave chirurgie katarakty na Slovensku v rokoch 2008 až 2010. Údaje sú v práci spracovávané na základe vypísaných dotazníkov z jednotlivých pracovísk vykonávajúcich operácie katarakty.

Authors in retrospective study give the results of cataract surgery in Slovak Republic since 2008 to 2010. There are compiled results of the filled forms from all ophthalmology centers where cataract surgery were done, in this study.

Cílem práce je zveřejnit výsledky očního oddělení Krajské nemocnice Pardubice v rámci celoevropské studie European Cataract Outcome Study probíhající na našem pracovišti od roku 2003 do roku 2005. Metodika: European Cataract Outcome Study je mezinárodní celoevropská studie monitorující dostupnost kataraktové chirurgie v různých částech Evropy, sleduje kvalitu operací katarakty na jednotlivých přihlášených pracovištích. Jejím cílem je podnítit zdokonalování operací katarakty a rozšířit nové technologie kataraktové chirurgie a využít poznatků k určování indikace a načasování operace pro ocenění užitku, který pacienti z operace katarakty mají. Tato studie hodnotí každoročně výsledky primárních operací katarakty v průběhu celého měsíce října. Probíhá od roku 1995, naše pracoviště se zapojilo od roku 2003 a spolupracuje dodnes. V první části studie se sledují základní demografická data se základním předoperačním vyšetřením a průběh operace katarakty, ve druhé části výsledky pooperačního vyšetření v průběhu 6 měsíců po operaci. Výsledky: Pacienti jsou na našem oddělení v naprosté většině odoperováni v lokální subkonjunktivální anestezii pomocí fakoemulzifikace s implantovanými měkkými čočkami, jejichž počet vzrostl v průběhu hodnocených let z 51 % na téměř 100 %. Evropským trendem je ambulantní pacient, na našem odd. jsou pacienti hospitalizovaní v 60–62,9 %. Během tří let jsme zaznamenali přítomnost přidružených vedlejších onemocnění ve 28–45 %. Pooperační nejlépe korigovaná zraková ostrost do 0,5 a lepší do 6 měsíců po operaci činí 86–90 %. Plánovaná refrakce se pohybovala od -0,40 do -0,48 dpt, konečná pooperační odchylka od plánované refrakce byla 1,2 dpt. Průměrný indukovaný astigmatismus se pohybuje na hladině 0,61dpt. Zjištěný rozdíl mezi operovaným a neoperovaným okem byl od 0,8–0,9 dpt, po operaci druhého oka 1,12–1,2 dpt. Závěr: Naše výsledky jsou přibližně srovnatelné se stavem oftalmologické péče v Evropě a pohybují se v evropském průměru.

The aim of this paper is to publish results from the Department of Ophthalmology, Regional hospital Pardubice, Czech Republic, E.U., obtained at our workplace during the period 2003 to 2005 for the all-European study with the title: European Cataract Outcome Study. METHODS: European Cataract Outcome Study is international all-European study monitoring availability of the cataract surgery in different parts of the Europe and it follows up the quality of the cataract surgeries in separately entered departments. Its goal is to incite the development of cataract surgery and extend new technologies of cataract surgery and use the findings for determining indication and timing of the surgery for the evaluation of the profit the patients have from cataract surgery. This study evaluates yearly the results of primary cataract surgery provided during one month of the year--October. The study has been proceeding since 1995; we have been participated in it since 2003 and cooperated up to the present day. In the first part of study, there are followed up the basic demographic data with pre-operative investigation and course of the cataract surgery, in the second part results of post-operative investigations during 6 months after the surgery. RESULTS: The patients in our department are in entire majority operated in local subconjunctival anesthesia by means of phacoemulsification with implanted foldable lenses, whose number rises along assessed years from 51% to almost 100% of cases. European tendency is out-patient cataract surgery; in our department, there're patients hospitalized in 60-62.9% of cases. During three years we have noted attendance of associated disorders in 28-45% of cases. The post-operative best-corrected visual acuity 0.5 and better up to 6 months after the surgery is in 86-90% of cases.The intended postoperative refraction was from -0.40 to -0.48 diopters (dpt), the final post-operative error from the planned refraction was 1.2 dpt. The average of induced astigmatism is 0.61 cylindrical diopters. The ascertained difference between operated and non-operated eye was 0.8-0.9 dpt, after the operation of the fellow eye it was 1.12-1.2dpt. CONCLUSION: Our results are approximately comparable with the condition of the ophthalmologic care in Europe and they correspond to the European average.

Jsou prezentovány výsledky dotazníkové studie, kdy všechna oční pracoviště v ČR, která operují šedý zákal, byla každoročně oslovena osmi, resp. patnácti dotazy týkajícími se četnosti operací, operační techniky a implantací nitroočních čoček. Vyhodnoceny byly výsledky za 10 let. Počet operací katarakty vzrostl ve sledovaném období z 11 339 na 46 175 očí. Počet implantací nitroočních čoček z 13,50 % na 98,80 %. Počet operací provedených fakoemulzifikační technikou se zvýšil od roku 1991 z 283 očí (1,80 %) na 36 543 očí (79,10 %) v roce 1997. Z nynějších 137 fakochirurgů jich 12 patří mezi tzv. high volume surgeons, tzn., že operují více než 50 katarakt měsíčně.

The author presents the results of a questionnaire survey where all ophthalmolo- gical departments in the Czech Republic which operate cataract were addressed every year with eight or fifteen questions resp. pertaining to the frequency of operations, surgical technique and implantation of intraocular lenses. The results covering 10 years were evaluated. The number of cataract operations increased during the follow-up period from 11 339 to 46 175 eyes. The number of implantations of intraocular lenses increased from 13.50 to 98.80% The number of operations made by the phacoemulsification technique was in 1991 283 eyes (1.80%) and increased to 36 543 eyes (79.10%) in 1997. Of the contemporary 137 phacosurgeons 12 are so-called high volume surgeons, i.e. they operate more than 50 cataracts per month.
