Po operacích se často vyskytují kognitivní poruchy, ve většině případů přechodného charakteru. Hovoříme o postoperativní kognitivní dysfunkci (POCD). Kognice může být postižena ve více složkách. Obvykle jsou postiženy exekutivní funkce, různé složky paměti, rychlost myšlení, pozornost. Vyšší výskyt této poruchy je v populaci seniorů. POCD je měřitelná psychometrickými testy. Nejsilnější projevy trvají zpravidla do týdne po anestézii a operaci, do tří měsíců výrazně slábnou až zanikají, výjimečně přetrvávají i jeden rok. Naprosto výjimečně může dojít anestézií a operací k spuštění rozvoje demence. To je možné především u lidí s preexistujícími změnami mozku vedoucímu ke vzniku demence, např. u lidí s dosud klinicky němou Alzheimerovou chorobou. POCD není zatím uznána jako samostatná diagnostická jednotka. Je zajímavé, že POCD se rozvíjí nejen po celkových, ale i po svodných anestéziích. Podobný klinický obraz může vzniknout po intenzivní péči na JIP, zejména spojené s resuscitací. Postoperativní delirium se vyskytuje nejčastěji u seniorů, především po ortopedických, srdečních a cévních operacích. Zpravidla se vyskytuje do 30 dnů od operace, častěji u pacientů, kteří byli léčeni na jednotkách intenzivní péče.
Cognitive disorders, in most cases of a temporary nature, often occur after surgery. Referring to them we use the term of Postoperative Cognitive Dysfunction (POCD). Cognition can be affected in its multiple functions. Usually, executive functions, various components of memory, speed of thinking and attention are affected. A higher incidence of this disorder is to be found in the elderly population. POCD is measurable by psychometric tests. The strongest manifestations usually last up to a week after anesthesia and surgery, they weaken significantly or disappear within three months, and they exceptionally persist for up to one year. Very rarely, anesthesia and surgery can trigger the development of dementia. This is especially true in people with pre-existing brain changes leading to dementia, such as people with clinically dumb Alzheimer´s disease. POCD is not yet recognized as a separate independent diagnostic unit. Interestingly, POCD develops not only after general anesthesia, but also after regional anesthesia. A similar clinical picture may arise in patients after intensive care in the ICU, especially associated with resuscitation. Postoperative delirium occurs most often in the elderly, especially after orthopedic, cardiac and vascular surgeries. It usually occurs within 30 days of surgery, more often in patients who have been treated in intensive care units.
INTRODUCTION: Stroke is one of the most feared complications during catheter ablation of atrial fibrillation (AF). While symptomatic thromboembolic events are rare, magnetic resonance imaging (MRI) may identify asymptomatic (ie, silent) cerebral lesions (SCLs) following pulmonary vein isolation (PVI) procedures. METHODS AND RESULTS: The REDUCE-TE Pilot was a prospective multicenter, single-arm observational study investigating the incidence of SCL in patients with symptomatic paroxysmal AF undergoing PVI with a novel gold-tip, externally irrigated ablation catheter. After ablation, cerebral diffusion-weighted MRI and a postablation follow-up were performed at 1 to 3 days after the ablation procedure. A neurocognitive test was done before and after ablation. The primary study endpoint was the occurrence of one or more new SCLs. Secondary study endpoints included neurocognitive status, procedural success rate, and periprocedural complications including symptomatic thromboembolic events. A total of 104 patients were enrolled (69% male, mean age: 61.5 ± 9.7 years, mean CHA2 DS 2 -VASc score: 1.7 ± 1.2). Postprocedural MRI examination was performed in 97 patients, and in nine of them (9.3%; 95% CI: 4.3-16.9%) a total of 11 SCLs were detected. Univariate analyses did not reveal any significant predictor for new SCLs. Nonsignificant trends were observed for low activated clotting time during ablation and for international normalized ratio value outside the range of 2 to 3 at ablation. There was no evidence of significant deterioration of neurocognitive function after PVI. In four patients, a pericardial tamponade was noted but all patients fully recovered during follow-up. CONCLUSIONS: Ablation of AF using a novel gold-tip, externally irrigated ablation catheter, resulted in SCLs in approximately one out of 10 patients without a measurable effect on neurocognitive function.
