Úvod: Poranění volární, flexorové ploténky proximálního interfalangeálního kloubu jsou často přehlédnutá poranění, která vedou k rozvoji deformity prstu typu swan neck, a pokud nejsou řešená, k rozvoji těžkých artrotických změn. Materiál a metodika: Autoři představují soubor 12 pacientů odoperovaných v letech 2006– 2012. Průměrný věk byl 29,6 roku. Odstup od úrazu byl 6 měsíců až 14 let. Výkon byl prováděn v lokální anestezii. Z volární incize nad PIP kloubem byla distální část ploténky 1 fixována dvěma vstřebatelnými stehy k bazi středního článku, při postavení v PIP kloubu 10–15° flexi. V jednom případě, kdy byla kost na bázi nekvalitní, jsme FP fixovali ke kosti pomocí kotvičky Mitek. Po operaci byla přiložena dorzální sádrová dlaha, která od druhého dne umožňuje aktivně cvičení flexe ve všech kloubech prstu, podle tolerance pacienta. Po 6 týdnech byla fixace odkládána a byla zahájena rehabilitace s prevencí hyperextenze v PIP kloubu. Pacienti byli zhodnoceni 6 měsíců od operace podle rozsahů pohybů a subjektivního hodnocení. Hodnotil se i návrat k sportovní či běžné činnosti. Výsledky: U všech pacientů došlo po operaci ke korekci swan neck deformity prstu ruky. Subjektivně hodnotilo výsledek jako výborný 11 pacientů. Jedna pacientka, u níž byl stav komplikován komplexním regionálním bolestivým syndromem (KRBS), s operací nebyla spokojena. Extenzi v PIP kloubu mělo 9 pacientů omezenou do 10° a 2 pacienti do 20°. U těchto dvou pacientů šlo o stav, kdy od úrazu do operace byl interval delší 10 let a bylo třeba výraznější uvolnění flexorové ploténky. Přesto tito pacienti hodnotili výsledek jako výborný. U jedné pacientky, kde byl stav komplikován KRBS, byla extenze v PIP kloubu omezena o 30°. Z celkového počtu 12 pacientů se jich 11 vrátilo ke sportovní aktivitě. Diskuze: Poranění flexorové ploténky omezují funkci ruky, proto její rekonstrukce je metoda, která může vést k její úpravě. Naše výsledky jasně ukazují, že i výkony s velkým odstupem od úrazu mohou mít dobré výsledky, pokud se nerozvinulo artrotické poškození kloubu.
Introduction: Injury to the volar, flexor plate proximal interphalangeal joint are often overlooked, leading to the development of deformity “swan neck” type deformities in the fingers, and if they are not addressed to the development of severe osteoarthritic changes. Material and Methods: The authors present a group of 12 patients who underwent surgery between 2006 and 2012. Their average age was 29.6. The time period between the injury and the surgery was 6 months to 14 years. The surgery was performed under local. The volar ZIG-ZAG incision on the PIP joint of the distal part of the plate was fixed two mA absorbable sutures to the base central section, while fixing the PIP joint 10 ° - 15 ° flexion. In one case, when the bone at the base was of poor quality, we fixed the FP to the bone using Mitek anchors. After the surgery, a dorsal plaster splint was applied, which allows the patient to actively exercise flexion in all joints of the fingers, right from the 2nd day after the surgery, according to the the patient individual condition. The fixation was removed after 6 weeks and the patients were started on a course of rehabilitation, while preventing hyperextension of the PIP joint. Patients were evaluated 6 months after surgery, evaluation the range of motion and their subjective experience. Whether the patient was able to fully return to sports or normal activities was also a part of the review. Results: The swan neck deformity of the fingers on their hands was corrected in all patients. 11 patients subjectively rated the result as excellent. One patient, where the condition was complicated by the complex regional pain syndrome (CRPS), operation was not satisfied with the results of the surgery. Extension of the PIP joint was limited to 10° in 9 patients and to 20° in 2 patients. In two of these patients the interval between the injury and the surgery was longer 10 years and the flexor release plate had to be loosened more, yet these patients rated the result as excellent. In one patient, where the condition was complicated by CRPS, the extension of the PIP joint is limited to 30°. Of the 12 patients, 11 have returned to playing sports. Discussion: Injuries to the flexor plate limit function of the hand, and because of that, reconstruction is a method that may lead to correction of its function. Our results clearly show that even surgeries performed a long time after the initial injury may have good results if the patient did not develop arthritic joint damage in the meanwhile.

PURPOSE OF THE STUDY: Can minimally invasive intramedullary osteosynthesis of distal radius fractures provide better therapeutic results than multidirectional locking plates. Retrospective study of 68 patients operated for distal radius fractures, 18 were treated with intramedullary X-screw (XSCR) fixation and 50 with the multidirectional angle-stable plate system (APTUS). The evaluation at 1-year follow-up included functional status of the wrist and hand, and radiographic findings. In the XSCR group, the functional outcomes of the treated extremity did not achieve values comparable with those of the uninjured side in any of the parameters measured. The radiographic findings did not meet the requirements of successful healing due to failure to restore an anatomical volar tilt in 22.2% cases. In the APTUS group, comparable values of the injured and the uninjured side were achieved in radial deviation, ulnar deviation, pronation, supination and grip strength. The radiographic criteria of successful healing were met by all fractures treated by locking plate osteosynthesis. Implant migration associated with secondary displacement of bone fragments was recorded in 33.3 % of the XSCR patients and only in 4.0 % of the APTUS patients. The overall evaluation show that intramedullary osteosynthesis does not produce better treatment outcomes compared with plate osteosynthesis in indicated types of fractures.
