STATEMENT OF PROBLEM: Accurate implant placement is essential for the success of dental implants. This placement influences osseointegration and occlusal forces. The freehand technique, despite its cost-effectiveness and time efficiency, may result in significant angular deviations compared with guided implantation, but the effect of angular deviations on the stress-strain state of peri-implant bone is unclear. PURPOSE: The purpose of this finite element analysis (FEA) study was to examine the effects of angular deviations on stress-strain states in peri-implant bone. MATERIAL AND METHODS: Computational modeling was used to investigate 4 different configurations of dental implant positions, each with 3 angles of insertion. The model was developed using computed tomography images, and typical mastication forces were considered. Strains were analyzed using the mechanostat hypothesis. RESULTS: The location of the implant had a significant impact on bone strain intensity. An angular deviation of ±5 degrees from the planned inclination did not significantly affect cancellous bone strains, which primarily support the implant. However, it had a substantial effect on strains in the cortical bone near the implant. Such deviations also significantly influenced implant stresses, especially when the support from the cortical bone was uneven or poorly localized. CONCLUSIONS: In extreme situations, angular deviations can lead to overstraining the cortical bone, risking implant failure from unfavorable interaction with the implant. Accurate implant placement is essential to mitigate these risks.
STATEMENT OF PROBLEM: Fatigue failure of implant components is a common clinical problem. Plasma nitriding, an in situ surface-strengthening method, may improve fatigue properties of dental implants. PURPOSE: The purpose of this in vitro study was to evaluate the effect of plasma nitriding on the fatigue behavior of implant systems. MATERIAL AND METHODS: The preload and friction coefficient of plasma nitrided abutment screws, as well as settlement of the implant-abutment interface, were measured. Then, the reverse torque values and pullout force were evaluated after cyclic loading. Finally, the fatigue properties of the implant system were investigated with static fracture and dynamic fatigue life tests, and the morphology of the fracture on the surface of the implant system was observed. RESULTS: The plasma nitriding treatment reduced the friction coefficient; increased the preload, settlement value, reverse torque values, pullout force, and static fracture load; and prolonged fatigue life. Furthermore, abutment screws with plasma nitriding treatment showed a different fatigue fracture mode. CONCLUSIONS: Plasma nitriding improved mechanical performance and may be a suitable way to optimize the fatigue behavior of dental implants.
STATEMENT OF PROBLEM: The bonding of light-activated adhesives to root canal dentin with an additional touch-polymerization activator has been insufficiently examined. PURPOSE: The purpose of this in vitro study was to investigate the effect of touch-polymerization activators and extended light-irradiation time on the microtensile bond strength (μTBS) of light-activated adhesives. MATERIAL AND METHODS: Post cavities were prepared in 50 extracted mandibular premolars and bonded using Prime&Bond Universal (PBU); PBU+Self Cure Activator (SCA); Clearfil SE Bond 2 (SEB); SEB+Clearfil DC Activator (DCA); or Clearfil Universal Bond Quick ER (UBQ). After light-irradiation for 10 or 20 seconds, the post cavities were filled with dual-activated resin core materials. Eight beams were prepared per specimen and subjected to the μTBS test. The μTBS data were analyzed by using 3-way ANOVAs with the Bonferroni correction (α=.05). RESULTS: The 3-way ANOVAs indicated that the use of touch-polymerization activators (SCA and DCA) significantly increased the μTBS of PBU and SEB in both the coronal (P=.015) and apical (P=.001) regions. The extension of light-irradiation time to 20 seconds significantly improved their μTBS in the apical region (P<.001), but not in the coronal region (P=.09). Light-irradiation for 20 seconds increased the μTBS of UBQ significantly in the coronal region (P=.014). CONCLUSIONS: Touch-polymerization activators improved the bond strength of light-activated adhesives to root canal dentin, especially when combined with an extended light-irradiation time.
A straightforward technique is presented for an interim or short-term definitive esthetic replacement of missing anterior teeth requiring no tooth preparation. Composite resins are injected into transparent silicone indices fabricated from 3-dimensional-printed casts of a digital waxing. The dentin core is formed of a durable short fiber-reinforced injectable composite resin and veneered with an enamel-shade composite resin for enhanced esthetics. Besides being noninvasive, this technique is more straightforward than traditional options, reducing chair time while providing an accurate outcome.

STATEMENT OF PROBLEM: Artificial denture teeth made of polymethyl methacrylate (PMMA) resin have good adhesion to the denture base but are relatively soft and have limited wear resistance during function. PURPOSE: The purpose of this in vitro study was to evaluate the influence of the addition of 2 inorganic nanofillers on the flexural strength, maximal displacement, elastic modulus, Isolde impact resistance, and Brinell hardness of acrylic resin denture teeth. MATERIAL AND METHODS: Heat polymerizing polymethyl methacrylate resin was mixed with silanized silica or silanized feldspar in concentrations of 5 wt%, 10 wt%, and 15 wt%. The first test was conducted after 24 hours of storage in laboratory conditions, and the second assessment was conducted after 3 months of storage in distilled water at 37 °C. The Brinell hardness was evaluated, and the elastic modulus and maximal displacement at fracture were calculated. The flexural strength and Isolde impact resistance were measured with a 3-point flexural test. Acrylic resin specimens without filler addition were used as a control group. Statistical analysis included 2-way ANOVA for independent variables (α=.05) and the Student t test for time-dependent changes (α=.05). These were performed with Statistica 12 software. RESULTS: The acrylic resin specimens modified with the addition of silanized feldspar had significantly higher Brinell hardness, elastic modulus, maximal displacement, and flexural strength and also had no adverse effect on Isolde impact resistance compared with the conventional acrylic resin. Silica filler increased the Brinell hardness and elastic modulus of acrylic resins but significantly reduced the flexural strength and Isolde impact resistance.

STATEMENT OF PROBLEM: The vertical marginal discrepancy of restorations can increase upon cementation, and poor marginal fit can lead to cement dissolution, marginal discoloration, microleakage, and secondary caries. The amount of increase is related to the type of luting cement used, but how lithium disilicate pressed crowns are affected by different resin cements is unclear. PURPOSE: The purpose of this in vitro study was to compare the effect of using different resin luting cements on the vertical marginal discrepancy of lithium disilicate pressed crowns. MATERIAL AND METHODS: A total of 18 intact extracted mandibular third molars were disinfected in a solution of 10% formalin for 7 days and were then prepared to receive a ceramic crown. Impressions were made with polyvinyl siloxane and lithium disilicate pressed crowns made and cemented with 1 of 3 resin luting cements. The marginal discrepancy was measured at 4 points on the finishing line of each tooth, with optical microscopy at ×200 magnification before and after cementation. Statistical analysis was done with the Kruskal-Wallis test to compare the median marginal increase among the 3 groups (a=.05). RESULTS: The least amount of marginal increase after cementation was with Harvard PremiumFlow cement, with an average marginal increase of 42 ±11 μm. RelyX Ultimate cement increased the margins by an average 45 ±29 μm. The highest marginal increase was found in the Enamel Plus HRi preheated composite resin group (116 ±47 μm). CONCLUSIONS: The marginal increase of pressed crowns cemented with preheated composite resin (Enamel Plus HRi) exceeded the clinically acceptable range of marginal discrepancy.

STATEMENT OF PROBLEM: Dentists frequently use electronic devices to determine tooth color. However, neither the instructions for these devices nor the accompanying brochures refer to the environmental conditions required at the point of measurement. PURPOSE: The purpose of this multicenter prospective clinical study was to reveal whether a change in the oral background influences tooth color determination. MATERIAL AND METHODS: Students (N=42) at the dental clinic in Berlin, Leipzig, Greifswald and Olomouc (women n=27, men n=15) participated in this study. It was their first contact with the spectral photometer (Easyshade Advance 4.0; VITA Zahnfabrik). After a short introduction on how to use the device, the students made 1-point measurements on the same patient on the maxillary central incisor. In the first measurement, the patient's mouth was open, and the palatal surface uncovered. In the second measurement, the patient's mouth was closed slightly, and the tongue pressed on the lingual surface of the maxillary central incisor. The mean ±SD and the 95% confidence interval (95 % CI) were calculated using the Student t test for each test series (α=.05). RESULTS: Statistical evaluation of the 2 measurements revealed changes in the L*a*b* values with a mean L*=0.204, a*=-0.351, and b*=0.02; a median of 0.4, -0.3, and -0.1, respectively; a ±SD of 2.37, 0.64, and 0.89, respectively; and 95% CIs of L*=-0.476 to 0.884, a*=-0.531 to -0.702, and b*=-0.23 to 0.52, respectively. These differences were not statistically significant (P>.05). The measurements of L*, a*, and b* with and without tongue coverage of the lingual surfaces of the maxillary teeth did not show any statistically significant differences (P=.663). CONCLUSIONS: The results demonstrate that the position of the tongue does not influence measurement accuracy during the application of the Easyshade Advance device.

STATEMENT OF PROBLEM: A valid system for assessing and classifying functional occlusion has not been established. The prevalence of anterior protected articulation is not known. PURPOSE: The purpose of this study was to quantify the prevalence of various functional occlusal contact patterns, including anterior protected articulation, among dental students. MATERIAL AND METHODS: Occlusal contacts were examined during lateral excursions from the maximal intercuspal position to the canine-to-canine position in 100 young adults. A combination of 3 common clinical methods was used: a visual examination, articulating paper, and feedback from the participants. RESULTS: Data from 3 classification systems were analyzed: (1) Occlusal contacts on the working side only – canine protected articulation was present in 25% of laterotrusions; anterior protected articulation was present in 18.5% of laterotrusions. Group function was present in 56% of laterotrusions. Other schemes were present in 0.5% of laterotrusions. (2) Contacts on both the working and the nonworking side. (3) Contacts on the working and nonworking side during both right and left laterotrusion. Nonworking side contacts were present in 33% of the participants. Nonworking side interference was present in 1 participant. CONCLUSIONS: The prevalence of anterior protected articulation found in this study was high enough to allow anterior protected articulation to be considered one of the fundamental working side occlusal contact patterns. More studies will be necessary to confirm this finding.

STATEMENT OF PROBLEM: Long-term practice-based clinical evaluations of various contemporary ceramic crown restorations from multiple practitioners are limited. PURPOSE: The aims of this study were to evaluate the clinical performance of ceramic single crowns and to identify factors that influence their clinical performance. MATERIAL AND METHODS: Ceramic single crowns that had been placed at the Mayo Clinic and in function since 2005 were identified and included in the study. The restorations were examined clinically, radiographically, and with photographs. Modified United States Public Health Services criteria were used for the clinical evaluation. The ceramic systems evaluated were bilayer and monolayer. RESULTS: Fifty-nine patients (41 women, 18 men) with 226 single teeth and implants restored with single ceramic crowns were identified. The mean duration from insertion date to study examination date was 6.1 years. Thirteen restorations (6%) were replaced at a mean 3.3 years after insertion date (range, 0.1-6.1 years). Estimated replacement-free survival rates (95% confidence interval [CI]; number of teeth/implants still at risk) at 5 years after insertion date were 95.1% (95% CI, 92.2-98.1; 153) and at 10 years were 92.8% (95% CI, 89.1-96.8; 8). The most common reason for replacement was fracture to the core of posterior layered ceramic crowns. The most commonly used luting agent was resin-modified ionomer cement. Most restorations exhibited clinically acceptable marginal integrity, shade, no caries recurrence, and no periapical pathology. CONCLUSIONS: The clinical performance of ceramic single crowns at 5 and 10 years supports their application in all areas of the mouth. With the majority of fractures to the core occurring early in the lifetime of layered ceramic posterior crowns, consideration of other monolithic ceramic systems for posterior crowns is advised.
