Zvyšující se výskyt antibiotických rezistencí patří k závažným problémům 21.století. Výskyt bakteriálních kmenů rezistentních k antibiotikům následně zužuje spektrum vhodných antibiotik použitelných pro léčbu i běžných bakteriálních infekcí nebo pro prevenci jejich výskytu, např. v chirurgii. Čistírny odpadních vod, nemocnice, ale i potravinový řetězec patří k ohniskům, kde nejčastěji dochází ke vzniku či šíření nových i stávajících kmenů bakterií rezistentních k antibiotikům a genů rezistence k antibiotikům. Ke stanovení antibiotických rezistencí se v laboratořích standardně používají fenotypové kultivační metody, které jsou však náročné na čas i práci a částečně i přesnou interpretaci výsledků. Z tohoto důvodu jsou hledány rychlejší alternativní metody detekce bakterií rezistentních k antibiotikům nebo přímo genů rezistence k antibiotikům. Příkladem alternativní metody detekce bakterií rezistentních k antibiotikům je například použití fenotypové metody využívající hmotnostní spektrometrie s laserovou desorpcí a ionizací za účasti matrice s průletovým analyzátorem pro stanovení producentů beta-laktamas. Zrychlení a zároveň větší přesnost detekce poskytují genotypové metody. Pomocí polymerasové řetězové reakce lze přímo detekovat a kvantifikovat geny rezistence k antibiotikům. Pro další zrychlení a vyšší specifitu detekce amplikonů z PCR lze použít mikročipy. Metody masivního paralelního sekvenování poskytují ucelenou informaci o rezistomu daného prostředí. Umožňují sekvenovat DNA amplikony či jednotlivé molekuly DNA pro detekci determinant antibiotické rezistence. Metody masivního paralelního sekvenování mají potenciál nahradit konvenční charakterizaci patogenů a umožňují detekci všech mikroorganismů ve vzorku (včetně obtížně kultivovatelných či nekultivovatelných mikroorganismů).
The increasing occurrence of antibiotic resistance is one of the major problems of the 21st century. The occurrence of bacterial strains resistant to antibiotics subsequently narrows the spectrum of suitable antibiotics usable for the treatment of common bacterial infections or for the prevention of their occurrence, e.g., in surgery. Wastewater treatment plants, hospitals, and also the food chain belong to the hotspots, where the emergence and spread of new or existing strains of antibiotic resistant bacteria and antibiotic resistance genes occur most frequently. Phenotypic culture methods are routinely used in laboratories to determine antibiotic resistance, but they are laborious and time-consuming and the interpretation of exact results is also difficult. For this reason, faster alternatives for the detection of antibiotic resistant bacteria or even antibiotic resistance genes are sought. Such an example of an alternative method for the detection of antibiotic resistant bacteria is the use of the matrix-assisted laser desorption/ionization time-of-flight mass spectrometry phenotypic method to identify the beta-lactamase producers. Genotype methods provide faster analysis and, at the same time, more accurate detection. Antibiotic resistance genes can be directly detected and quantified by polymerase chain reaction. Microarrays can be used to further speed up and increase the specificity of PCR amplicons detection. Massive parallel methods provide comprehensive information on the resistoma of the specific environment. They facilitate sequencing of individual DNA molecules or amplicons to detect determinants of antibiotic resistance. Massive parallel methods have the potential to replace conventional pathogen characterization and allow the detection of all microorganisms in a sample (including difficult-to-cultivate or noncultivable microorganisms).

Bakterie komplexu Burkholderia cepacia vyvolávají u pacientů s cystickou fibrózou (CF) závažné plicní infekce. Riziko epidemického šíření infekce přimělo centra CF zavést přísná izolační opatření. Navíc je nutné správně a včas mikrobiálního původce identifikovat, k čemuž slouží postupy molekulární mikrobiologie. V posledním desetiletí nebyl v ČR diagnostikován žádný nový případ infekce způsobené epidemickým kmenem ST-32 druhu Burkholderia cenocepacia, který stál za velkou epidemií 90. let 20. století.
Bacteria from the Burkholderia cepacia complex cause severe lung infections in people with cystic fibrosis (CF). The risk of epidemic spread of the infection led the CF centres to implement strict isolation precautions. Furthermore, it was imperative to identify the microbial agent correctly and timely which was made possible thanks to the methods of molecular microbiology. No new Burkholderia cenocepacia infections, caused by the ST-32 strain which had been responsible for a large outbreak in 1990s, were diagnosed in the Czech Republic in the last ten years.
rRNA gene Sanger sequencing is being used for the identification of cultured pathogens. A new diagnostic approach is sequencing of uncultured samples by using the commercial DNA extraction and sequencing platform SepsiTest (ST). The goal was to analyze the clinical performance of ST with a focus on nongrowing pathogens and the impact on antibiotic therapy. A literature search used PubMed/Medline, Cochrane, Science Direct, and Google Scholar. Eligibility followed PRISMA-P criteria. Quality and risk of bias were assessed drawing on QUADAS-2 (quality assessment of diagnostic accuracy studies, revised) criteria. Meta-analyses were performed regarding accuracy metrics compared to standard references and the added value of ST in terms of extra found pathogens. We identified 25 studies on sepsis, infectious endocarditis, bacterial meningitis, joint infections, pyomyositis, and various diseases from routine diagnosis. Patients with suspected infections of purportedly sterile body sites originated from various hospital wards. The overall sensitivity (79%; 95% confidence interval [CI], 73 to 84%) and specificity (83%; 95% CI, 72 to 90%) were accompanied by large effect sizes. ST-related positivity was 32% (95% CI, 30 to 34%), which was significantly higher than the culture positivity (20%; 95% CI, 18 to 22%). The overall added value of ST was 14% (95% CI, 10 to 20%) for all samples. With 130 relevant taxa, ST uncovered high microbial richness. Four studies demonstrated changes of antibiotic treatment at 12% (95% CI, 9 to 15%) of all patients upon availability of ST results. ST appears to be an approach for the diagnosis of nongrowing pathogens. The potential clinical role of this agnostic molecular diagnostic tool is discussed regarding changes of antibiotic treatment in cases where culture stays negative.

Mikrofluidika je inovativní obor, který se zabývá zpracováním malého množství kapaliny v mikrokanálech. V kombinaci s pokročilými analytickými technikami, jako je např. mikrofluidní PCR, nabízí významné výhody nejen pro analýzu genové exprese. Tato metoda využívá mikrokanály a mikroventily k přesnému dávkování a míchání činidel, čímž se minimalizuje spotřeba vzorku a činidla a také čas stráve‐ ný pipetováním. Tyto vlastnosti činí mikrofluidní PCR ideální pro analýzu genové exprese, kde je vyžadováno podrobné monitorování a kvantifikace mRNA. Jedním z přístrojů umožňujícím mikrofluidní PCR je Biomark X. Díky své schopnosti multiplexování a také díky své‐ mu mikrofluidnímu designu umožňuje analýzu mnoha vzorků současně. Tato pokročilá technologie má široké uplatnění v biologickém výzkumu, diagnostice a personalizované medicíně a nabízí nové příležitosti k objevování a pochopení genetických procesů.
Microfluidics is an innovative science that deals with the manipulation of small volumes of fluid in microchannels. In combination with advanced analytical techniques such as microfluidic PCR, it offers significant advantages not only for gene expression analysis. Microflui‐ dic PCR enables PCR reactions to be performed using very small sample volumes, as it utilizes microchannels and microvalves for precise reagent dispensing and mixing. This fact increases both sensitivity and accuracy of the analysis. The Biomark X instrument utilizes micro‐ fluidic PCR for gene expression analysis, as it is ideal for mRNA quantification. With its multiplexing capability and microfluidic design, it enables the analysis of multiple samples simultaneously. This advanced technology finds broad applications in biological research, diagnostics, and provides new opportunities for the discovery and understanding of genetic processes.
Although triple labeling of molecular beacons has been documented to improve quenching efficiencies and studies generally assume similar benefits at long TaqMan probes, a limited number of works have studied this issue in TaqMan probes. We therefore prepared a series of long triple-labeled oligodeoxynucleotide probes with 6-carboxyfluorescein as a fluorophore at the 5'-end and BlackBerry (BBQ-650) or azaphthalocyanine quenchers at the 3'-end and in the intrastrand position and systematically compared their quenching efficiencies with those of the corresponding double-labeled probes including important control probes. A model polymerase chain reaction (PCR) assay enabled the determination of the quenching efficiencies of static and Förster resonance energy transfer (FRET) quenching in the target probes. The type of probe had no effect on the static quenching ability. Importantly, FRET quenching of double-labeled probes with a quencher at the 3'-end showed a statistically insignificant difference from the control probe without any quencher, indicating the need to shift the quencher closer to the fluorophore in long probes. Shortening the distance between the fluorophore and the quencher played a key role in FRET quenching, whereas the introduction of an additional quencher only slightly improved the quenching efficiency. BBQ-labeled probes had lower quenching efficiencies than azaphthalocyanine probes. The methodologies and relationships described above seem, however, to be universal and applicable to any quencher.
Bartonella henselae je celosvětově rozšířená fakultativně intracelulární gramnegativní tyčkovitá bakterie z čeledi Bartonellaceae. Přirozeným rezervoárem jsou domácí zvířata, zejména kočky, které se bartonellou nakazí od nejčastějšího přenašeče – blechy kočičí. K přenosu infekce na člověka dochází při poškrábání nebo pokousání zvířetem, které má kontaminované drápky infekčním trusem blech. Za možný je též považován přenos kontaminací při mazlení kočky nebo při sání infikovaného klíštěte obecného. Nemoc z kočičího škrábnutí, Cat Scratch Disease (CSD), infekční onemocnění způsobené Bartonella henselae, postihuje děti i dospělé a má klinické projevy od mírných příznaků až po život ohrožující komplikace zejména u imunodeficientních pacientů. Článek je shrnutím dosavadních základních znalostí o tomto patogenu i o onemocnění, které způsobuje.
Bartonella henselae is a facultatively intracellular Gram-negative rod-shaped bacterium of the Bartonellaceae family that is widespread worldwide. The natural reservoir is domestic animals, especially cats, which become infected with bartonella from the most common vector – a cat flea. Transmission of the infection to humans occurs through a scratch or bite by an animal whose claws are contaminated with infectious flea feaces. Transmission of contamination when petting a cat or through an infected castor bean tick is also considered possible. Cat scratch disease (CSD), an infectious disease caused by Bartonella henselae, affects children and adults and has clinical manifestations ranging from mild symptoms to life-threatening complications, especially in immunodeficient patients. The article is a summary of the current basic knowledge of this pathogen and the disease it causes.
Národní referenční laboratoř pro enteroviry se zabývá surveillance akutních chabých paréz, environmentální a enterovirovou surveillance. Sérologickými metodami vyšetřuje protilátky proti enterovirům, poliovirům, hantavirům a SARS-CoV-2.
The National Reference Laboratory for Enteroviruses deals with acute flaccid paralysis surveillance, environmental and enterovirus surveillance. It examines antibodies against enteroviruses, polioviruses, hantaviruses and SARS-CoV-2 by serological methods.

Leptospiróza je onemocnění způsobené spirochétami rodu Leptospira, které je celosvětově rozšířené. Člověk se může infikovat přes půdu nebo vodu kontaminovanou močí rezervoárových hostitelů. Mezi projevy leptospirózy patří často nespecifické příznaky podobné chřipkovému onemocnění ale v 5–10 % případů může docházet k vážnému průběhu onemocnění s multiorgánovým selháváním. Diagnostika leptospirózy probíhá nepřímo pomocí sérologických metod nebo přímo pomocí kultivace nebo polymerázové řetězové reakce (PCR). Celosvětově nejpoužívanější je sérologická metoda MAT (mikroskopický aglutinační test). V NRL pro leptospiry využíváme k diagnostice leptospirové infekce metody MAT, PCR i kultivaci. V loňském roce jsme potvrdili celkem 15 pozitivních vzorků, z toho 10 pomocí MAT a 5 pomocí PCR. Celkový záchyt leptospirózy na celém území ČR za rok 2021 byl 31 pozitivních případů, což je lehce nadprůměrný výskyt onemocnění. Nejčastějším infikujícím sérotypem v letech 2001–2018 byla L. grippotyphosa.
Leptospirosis is a disease caused by spirochetes of the genus Leptospira, which is widespread worldwide. Humans can become infected through soil or water contaminated with the urine of reservoir hosts. The manifestations of leptospirosis often include non-specific flu-like symptoms, but in 5–10% of cases, a serious course of the disease with multi-organ failure may occur. Leptospirosis is diagnosed indirectly using serological methods or directly using culture-based method or polymerase chain reaction (PCR). The most used worldwide is the MAT (microscopic agglutination test) serological method. In the National Reference Laboratory for Leptospira, to diagnose leptospiral infections we use MAT, PCR as well as culture-based methods. Last year, we confirmed a total of 15 positive samples, 10 of them by MAT and 5 by PCR. The total detection of leptospirosis in the entire territory of the Czech Republic for the year 2021 was 31 positive cases, which is a slightly above-average incidence of the disease. The most common infecting serotype in 2001–2018 was L. grippotyphosa.

V NRL pro streptokokové nákazy byl v letech 2008–2020 metodou latexové aglutinace určován sérotyp u 1 038 vzorků Streptococcus agalactiae od 972 pacientů. U 43 izolátů (4,4 %) se určení sérotypu nepodařilo a byly označeny jako netypovatelné. Cílem této práce bylo u těchto netypovatelných izolátů určit genotyp metodou multiplexové polymerázové řetězové reakce (mPCR). Genotyp se podařilo určit u celého souboru 43 netypovatelných izolátů. Nejčastěji byl zjištěn genotyp V (41,9 %), následován genotypem Ia (20,9 %). U izolátů s latexaglutinací určeným sérotypem převažoval sérotyp III (29,2 %) a sérotyp V byl z hlediska výskytu na 2. místě (26,2 %). Byla získána kompletní data o výskytu sérotypů/genotypů S. agalactiae v České republice v letech 2008–2020. Monitorování výskytu sérotypů a genotypů je důležitou součástí zavádění případné vakcíny proti S. agalactiae do klinické praxe.
The NRL for Streptococcal Infections performed serotyping of 1038 isolates of Streptococcus agalactiae from 972 patients by the latex agglutination method in 2008–2020. Forty-three isolates (4.4%) whose serotyping failed were classified as non-typeable. The aim of the present study was to determine the genotype of these non-typeable isolates using multiplex polymerase chain reaction (mPCR). Genotyping was successful in the entire set of 43 non-typeable isolates. The most common genotype was V (41.9%), followed by Ia (20.9%). The isolates serotyped by latex agglutination were predominantly assigned to serotype III (29.2%) and V (26.2%). Complete data were obtained on the prevalence of S. agalactiae serotypes/genotypes in the Czech Republic in 2008–2020. Monitoring the serotype and genotype distribution of the pathogen is a prerequisite for the introduction of a potential vaccine against S. agalactiae into clinical practice.
