The strain Escherichia coli Nissle 1917 (EcN) is widely used as an efficient probiotic in therapy and prevention of human infectious diseases, especially of the intestinal system. Concurrently, small adult pigs are being used as experimental omnivore models to study human gastrointestinal functions. EcN bacteria were applied to 6 adult healthy female pigs in a 2-week trial. 6 Control animals remained untreated. Altogether, 164 and 149 bacterial strains were isolated from smear samples taken from gastrointestinal mucosa in the experimental and control group, respectively. Each individual E. coli strain was then tested for the presence of 29 bacteriocin-encoding determinants as well as for DNA markers of A, B1, B2 and D phylogenetic groups. A profound reduction of E. coli genetic variance (from 32 variants to 13 ones, P = 0.0006) was found in the experimental group, accompanied by a lower incidence of bacteriocin producers in the experimental group when compared to control (21.3 and 34.9%, respectively; P = 0.007) and by changes in the incidence of individual bacteriocin types. The experimental administration of EcN strain was not sufficient for stable colonization of porcine gut, but induced significant changes in the enterobacterial microbiota.

Syndrom bakteriálního přerůstání (SIBO – small intestinal bacterial overgrowth) představuje heterogenní skupinu chorobných stavů a poruch charakterizovaných zvýšeným počtem a/nebo abnormálním druhem bakterií v tenkém střevě. Trávicí trakt člověka je za fyziologických podmínek osídlen více než 400 druhy bakterií, logicky s maximálním výskytem v tlustém střevě. Vychýlení dynamické rovnováhy (ať už ve smyslu kvalitativním, nebo kvantitativním) může vést k důsledkům závažným pro makroorganismus (např. u dětí se syndromem krátkého střeva může vyústit v intestinální selhání). Pro stanovení diagnózy SIBO se podle většiny autorů považuje za dostačující nález více než 105 mikroorganismů/ml aspirátu z proximálního jejuna (norma u zdravého dospělého jedince je ? 104 bakterií/ml).
AIM: To evaluate bacteriocinogeny in short-term high-dose indomethacin administration with or without probiotic Escherichia coli Nissle 1917 (EcN) in experimental pigs. METHODS: Twenty-four pigs entered the study: Group A (controls), Group B (probiotics alone), Group C (indomethacin alone) and Group D (probiotics and indomethacin). EcN (3.5×10(10) bacteria/d for 14 d) and/or indomethacin (15 mg/kg per day for 10 d) were administrated orally. Anal smears before and smears from the small and large intestine were taken from all animals. Bacteriocin production was determined with 6 different indicator strains; all strains were polymerase chain reaction tested for the presence of 29 individual bacteriocin-encoding determinants. RESULTS: The general microbiota profile was rather uniform in all animals but there was a broad diversity in coliform bacteria (parallel genotypes A, B1, B2 and D found). In total, 637 bacterial strains were tested, mostly Escherichia coli (E. coli). There was a higher incidence of non-E. coli strains among samples taken from the jejunum and ileum compared to that of the colon and rectum indicating predominance of E. coli strains in the large intestine. Bacteriocinogeny was found in 24/77 (31%) before and in 155/560 (28%) isolated bacteria at the end of the study. Altogether, 13 individual bacteriocin types (out of 29 tested) were identified among investigated strains. Incidence of four E. coli genotypes was equally distributed in all groups of E. coli strains, with majority of genotype A (ranging from 81% to 88%). The following types of bacteriocins were most commonly revealed: colicins Ia/Ib (44%), microcin V (18%), colicin E1 (16%) and microcin H47 (6%). There was a difference in bacteriocinogeny between control group A (52/149, 35%) and groups with treatment at the end of the study: B: 31/122 (25%, P=0.120); C: 43/155 (28%, P=0.222); D: 29/134 (22%, P=0.020). There was a significantly lower prevalence of colicin Ib, microcins H47 and V (probiotics group, P<0.001), colicin E1 and microcin H47 (indomethacin group, P<0.001) and microcins H47 and V (probiotics and indomethacin group, P=0.025) compared to controls. Escherichia fergusonii (E. fergusonii) was identified in 6 animals (6/11 isolates from the rectum). One strain was non-colicinogenic, while all other strains of E. fergusonii solely produced colicin E1. All animals started and remained methanogenic despite the fact that EcN is a substantial hydrogen producer. There was an increase in breath methane (after the treatment) in 5/6 pigs from the indomethacin group (C). CONCLUSION: EcN did not exert long-term liveability in the porcine intestine. All experimental pigs remained methanogenic. Indomethacin and EcN administered together might produce the worst impact on bacteriocinogeny.

Cílem práce je publikace výsledků efektu samotné antibiotické terapie (ATB) a následně i její kombinace se systémovou enzymoterapií u akutních nekomplikovaných urogenitálních infekcí Chlamydia trachomatis. Již dříve jsme publikovali výsledky samotné terapie azitromycinem (AZT: 1 × 500 mg/den po 3 dny) a stejné terapie v kombinaci s Wobenzymem (W: 3 × 5 tbl/den po 14 dní). Nyní předkládáme i výsledky naší druhé studie, kde jsme sledovali prakticky totéž – tedy kombinaci stejné enzymoterapie s jiným ATB, s klaritromycinem (CLR: 2 × d 500 mg tbl. formy bez řízeného uvolňování po 14 dnů). Úspěšnost samostatné terapie CLR (opět dle výsledku kontrolního stěru 4 týdny po skončení ATB terapie; průkaz antigenu imunofluorescenční metodou) byla u mužů (n = 16) naprosto stejná jako s AZT, tedy 62,5 %, ale u kombinované terapie (n = 16) u nich vzrostlo procento úspěšnosti dokonce ze 75 % AZT + W na 87,5 % CLR + W. Oproti tomu úspěšnost terapie žen pouze CLR (n = 16) i CLR + W (n = 16) byla naprosto nulová a tímto problémem je nutné se i nadále zabývat.
Enzyme therapy for urogenital infections with Chlamydia trachomatis The aim of the paper is to publish the results of the effect of antibiotic therapy (ATB) alone as well as of its combination with systemic enzyme therapy in acute uncomplicated urogenital infections with Chlamydia trachomatis. We have previously published the results of the treatment with azithromycin alone (AZT: 1 × 500 mg/day for 3 days) and of the same treatment in combination with Wobenzym (W: 3 × 5 tablets/day for 14 days). Now, we present the results of our second study where virtually the same was investigated, i. e. a combination of the same enzyme therapy with another ATB, clarithromycin (CLR: 500 mg tablets without controlled release twice daily for 14 days). The success rate of the treatment with CLR alone (again based on the results of a follow-up smear 4 weeks after cessation of ATB treatment; antigen detection using the immunofluorescence method) in men (n = 16) was identical to that with AZT, namely 62.5 %; however, in combined therapy (n = 16), the success rate even increased from 75 % for AZT + W to 87.5 % for CLR + W. By contrast, in women the success rates of treatment with CLR alone (n = 16) and with CLR + W (n = 16) were zero and this issue needs to be addressed further. Wobenzym.

Cíle práce: Analýza případů laboratorně diagnostikované klíšťové encefalitidy (KE) na virologickém oddělení Ústavu klinické mikrobiologie Fakultní nemocnice v Hradci Králové (ÚKM FN HK) v letech 2003–2006 a jejich porovnání se sledováními provedenými v letech 1999–2002, respektive 1995–1998. Dalším cílem práce bylo zhodnocení rutinně používaných metod (enzyme-linked-immunosorbent-assay – ELISA) pro diagnostiku KE. Materiál a metodiky: Analyzovány byly vzorky vyšetřené virologickým oddělením ÚKM FN HK v letech 2003–2006, celkem od 3 284 pacientů. Základní diagnostickou metodou byl průkaz specifických IgM a IgG protilátek v séru a likvoru metodou ELISA. Používány byly sety firem Euroimmun, SRN a Test-Line, s. r. o. Clinical Diagnostics, ČR. Případy nespecifické reaktivity při stanovení IgM protilátek byly sledovány pouze v letech 200–2006. Výsledky: V letech 2003–2006 byla KE na našem pracovišti diagnostikována celkem u 163 pacientů ve věku 3,5 měsíce až 80 let, což bylo o 48 případů (tj. 29 %) více než v letech 1995–1998, respektive o 36 případů (tj. 22 %) více než v letech 1999–2002. Z prokázaných případů KE byli 94 muži (57,7 %) a 69 žen (42,3 %). Nadpoloviční většinu tvořili pacienti starší 41 roků (94/163, tj. 57,6 %), pacientů starších 51 roků bylo 60 (37 %). Bifázický průběh infekce byl zaznamenán u 93/136 případů (68,4 %). Klíště bylo anamnesticky zachyceno u 101/136 případů (74 %), pití nepasterizovaného mléka u jednoho pacienta. U 38 vzorků (1,4 %) byla detekována nespecifická reaktivita IgM protilátek. Nebyla zaznamenána izolovaná reaktivita likvoru bez reaktivity séra. Závěry: Na území východočeského regionu došlo v letech 2003–2006 k nárůstu počtu diagnostikovaných případů KE. Přibylo hlavně závažnějších klinických forem, které převažovaly nad lehčími. Zůstala zachována početní distribuce pacientů s prokázanou KE, co se týče věku a pohlaví, i sezonní forma výskytu infekce, ovšem s posunem vrcholu z letních do podzimních měsíců. Metoda ELISA je pro rutinní diagnostiku vyhovující. Úroveň komerčně používaných souprav je dobrá. Je však nutno počítat s možností nespecifické reaktivity IgM protilátek, a proto musí již u prvního vzorku v akutní fázi onemocnění být zároveň s IgM stanoveny i IgG protilátky v séru a na přítomnost protilátek musí být vyšetřena párová séra. Stanovení protilátek v likvoru se jeví pro stanovení diagnózy KE málo přínosným. Spolupráce ošetřujícího lékaře a klinického mikrobiologa je nezbytná.
Aim of the study: The analysis of cases of tick-borne encephalitis (TBE) laboratory diagnosed at the Department of Virology of the Institute of Clinical Microbiology of the University Hospital in Hradec Králové in 2003–2006 and comparison of the data from 2003–2006, 1999–2002 and 1995–1998. Another objective was to compare two routinely used ELISA (enzyme-linked immunosorbent assay) diagnostic kits for TBE. Material and Methods: The samples from 3,284 patients tested by the Department of Virology of the University Hospital in Hradec Králové in 2003–2006 were analyzed. The detection of the specific IgM and IgG antibodies in serum and cerebrospinal fluid specimens was performed using two ELISA diagnostic kits (Euroimmun, Germany, and Test-Line Ltd, Clinical Diagnostics, Czech Republic). Non-specific reactivity of IgM antibodies was only analyzed in 2004–2006. Results: In 2003–2006, TBE was diagnosed in 163 patients of the University Hospital in Hradec Králové aged from 3.5 months to 80 years. The number was by 48 cases (29 %) higher than that in 1995–1998 and by 36 cases (22 %) higher than that in 1999–2002. TBE was confirmed in 94 males (57.7 %) and 69 females (42.3 %). More than half of the patients were aged above 41 years (94/163, i.e. 57.6 %) and 60 patients (37 %) were older than 51 years. A biphasic course of infection was observed in 93/136 cases (68.4 %). As many as 101/136 patients (74 %) had a history of a tick bite, one patient reported the consumption of non-pasteurized milk. Thirty-eight (1.4 %) specimens showed non-specific reactivity of IgM antibodies. None of the cases showed cerebrospinal fluid reactivity in the absence of serum reactivity. Conclusions: Increase in the diagnosed TBE cases was reported in 2003–2006 in the East Bohemia Region. It was due mainly to more severe clinical forms of TBE that were more frequent than the milder ones. The distribution of the confirmed TBE cases remained unchanged in terms of age and sex, but the seasonal peak moved from summer to autumn. ELISA proved suitable for use in the routine diagnosis of TBE. Both of the tested commercial kits are of good quality. Nevertheless, in view of possible non-specific reactivity of IgM antibodies, IgG antibodies need to be tested in both the first acute serum specimen and paired sera. The detection of antibodies in cerebrospinal fluid is unlikely to be of diagnostic benefit for TBE. Close cooperation between the attending physician and clinical microbiologist is crucial.

Human intestinal microbiota create a complex polymicrobial ecology. This is characterised by its high population density, wide diversity and complexity of interaction. Any dysbalance of this complex intestinal microbiome, both qualitative and quantitative, might have serious health consequence for a macro-organism, including small intestinal bacterial overgrowth syndrome (SIBO). SIBO is defined as an increase in the number and/or alteration in the type of bacteria in the upper gastrointestinal tract. There are several endogenous defence mechanisms for preventing bacterial overgrowth: gastric acid secretion, intestinal motility, intact ileo-caecal valve, immunoglobulins within intestinal secretion and bacteriostatic properties of pancreatic and biliary secretion. Aetiology of SIBO is usually complex, associated with disorders of protective antibacterial mechanisms (e.g. achlorhydria, pancreatic exocrine insufficiency, immunodeficiency syndromes), anatomical abnormalities (e.g. small intestinal obstruction, diverticula, fistulae, surgical blind loop, previous ileo-caecal resections) and/or motility disorders (e.g. scleroderma, autonomic neuropathy in diabetes mellitus, post-radiation enteropathy, small intestinal pseudo-obstruction). In some patients more than one factor may be involved. Symptoms related to SIBO are bloating, diarrhoea, malabsorption, weight loss and malnutrition. The gold standard for diagnosing SIBO is still microbial investigation of jejunal aspirates. Non-invasive hydrogen and methane breath tests are most commonly used for the diagnosis of SIBO using glucose or lactulose. Therapy for SIBO must be complex, addressing all causes, symptoms and complications, and fully individualised. It should include treatment of the underlying disease, nutritional support and cyclical gastro-intestinal selective antibiotics. Prognosis is usually serious, determined mostly by the underlying disease that led to SIBO.
Cíl práce: Shrnutí vlastních poznatků o výskytu, klinickém obrazu a terapii chorob ústní sliznice vyvolaných HSV. Materiál a metody. Analýza souboru 135 pacientů Stomatologické khniky LF UK a FN v Hradci Králové za 11 let. Statistické hodnocení závislostí mezi sledovanými parametry (klinická diagnóza, věk, pohlaví, celkový zdravotní stav, způsob terapie) pomocí analýzy rozptylu (ANOVA), nepárového t-testu a Fisherova testu. Výsledky: Soubor pacientů (81 žen, 54 mužů) sestával ze 49 jedinců s HSV primoinfekcí v podobě primární herpetické gingivostomatitidy (37 %), z jedinců s projevy recidiv HSV infekce v podobě herpetické stomatitidy u 68 pacientů (50 %) a herpes labialis u 18 pacientů (13 %) ve věkovém rozmezí 1 až 90 let, věkovým průměrem 40,4 let a největším počtem pacientů ve 3. a 7. decenniu. Průměrný věk pacientů s herpetickou gingivostomatitidou byl statisticky významně nižší (p = 0,0001). Výskyt herpetické stomatitidy souvisel statisticky významně se závažnými komorbiditami (p = 0,0001). Lokální léčba byla indikována u 73 jedinců (54 %), kombinovaná u 58 pacientů (43 %), celková u 4 pacientů (3 %). U většiny jedinců s herpetickou gingivostomatitidou byla použita lokální terapie (80 %), u větsiny pacientů s herpetickou stomatitidou byl použit kombinovaný způsob terapie (62 %). Závěr: Věk jedinců s primární herpetickou gingivostomatitidou a herpetickou stomatitidou byl statisticky významně odlišný. Herpetická stomatitida byla častou oportunní virovou infekcí u celkově nemocných jedinců. K terapii se osvědčila antiseptika s protivirovými účinky, která lze kombinovat s celkově podávanými virostatiky.
Objectives: Recapitulation of our knowledge dealing with diagnosis and therapy of oral mucosal diseases caused by HSV. Material and Methods: Analysis of a sample of 135 patients coming from Department of Dentistry, Faculty of Medicine in Hradec Králové within 11 years. Statistical evaluation of the data and related associations (clinical diagnosis, age, gender, general health status, treatment mode) were realized by ANOVA analysis, t-test, and Fisher s exact test. Results: A total sample of 135 individuals (81 women and 54 men) consisted of 49 individuals with primary herpetic gingivostomatitis (37 %) and recurrences with clinical signs of herpetic stomatitis in 68 patients (50 %) and labial herpes simplex in 18 patients (13 %) aged from 1-90 years, mean age 40.4 year, with maximal ocurrence in the 3rd and 7th decadees. The mean age of individuals with herpetic gingivostomatitis was significantly lower (p = 0.0001). The ocurrence of herpetic stomatitis was associated with severe concomitant diseases (p = 0,0001). Topical treatment was applied in 73 individuals (54 %), combined mode in 58 individuals (43 %), only systemic therapy in 4 individuals (3 %). In most of individuals with herpetic gingivostomatitis only topical treatment was used (80 %), in most of herpetic recurrences combined therapy was applied (62 %). Conclusions: The age of individuals with primary infection and herpetic recurrences was significantly different. Herpetic stomatitis ocurred often as an opportunistic viral infection in seriously ill and older patients. Antiseptics with antiviral properties and their combinations with systemic administration of antiviral drugs proved successfuly in the treatment.
