BACKGROUND: Transthyretin amyloidosis, also called ATTR amyloidosis, is associated with accumulation of ATTR amyloid deposits in the heart and commonly manifests as progressive cardiomyopathy. Patisiran, an RNA interference therapeutic agent, inhibits the production of hepatic transthyretin. METHODS: In this phase 3, double-blind, randomized trial, we assigned patients with hereditary, also known as variant, or wild-type ATTR cardiac amyloidosis, in a 1:1 ratio, to receive patisiran (0.3 mg per kilogram of body weight) or placebo once every 3 weeks for 12 months. A hierarchical procedure was used to test the primary and three secondary end points. The primary end point was the change from baseline in the distance covered on the 6-minute walk test at 12 months. The first secondary end point was the change from baseline to month 12 in the Kansas City Cardiomyopathy Questionnaire-Overall Summary (KCCQ-OS) score (with higher scores indicating better health status). The second secondary end point was a composite of death from any cause, cardiovascular events, and change from baseline in the 6-minute walk test distance over 12 months. The third secondary end point was a composite of death from any cause, hospitalizations for any cause, and urgent heart failure visits over 12 months. RESULTS: A total of 360 patients were randomly assigned to receive patisiran (181 patients) or placebo (179 patients). At month 12, the decline in the 6-minute walk distance was lower in the patisiran group than in the placebo group (Hodges-Lehmann estimate of median difference, 14.69 m; 95% confidence interval [CI], 0.69 to 28.69; P = 0.02); the KCCQ-OS score increased in the patisiran group and declined in the placebo group (least-squares mean difference, 3.7 points; 95% CI, 0.2 to 7.2; P = 0.04). Significant benefits were not observed for the second secondary end point. Infusion-related reactions, arthralgia, and muscle spasms occurred more often among patients in the patisiran group than among those in the placebo group. CONCLUSIONS: In this trial, administration of patisiran over a period of 12 months resulted in preserved functional capacity in patients with ATTR cardiac amyloidosis. AIMS: Fabry disease (FD) is a rare X-linked genetic disorder caused by α-galactosidase A (AGALA) deficiency. Whereas 'classic' variant has multisystemic manifestation, the more recently described 'later-onset' variant is characterized by predominant cardiac involvement that often mimics hypertrophic cardiomyopathy (HCM). METHODS AND RESULTS: Consecutive unrelated patients with HCM were screened for FD in 16 (out of 17) cardiac centres in the Czech Republic covering specialized cardiology care from June 2017 to December 2018. AGALA activity and globotriaosylsphingosine (lyso-Gb3 ) levels were measured in all subjects using the dry blood spot method. FD was suspected in male patients with AGALA activity <1.2 μmol/h/L and in females with either low AGALA activity or lyso-Gb3 > 3.5 ng/mL. Positive screening results were confirmed by genetic testing. We evaluated 589 patients (390 males, 66%) with HCM (mean maximal myocardial thickness 19.1 ± 4.3 mm). The average age was 58.4 ± 14.7 years. In total, 17 patients (11 males, 6 females) had a positive screening result, and subsequently, six of them (four males and two females) had a genetically confirmed pathogenic GLA mutation (total prevalence of 1.02%). Five of these patients were carrying the p.N215S mutation known to cause a typical later-onset cardiac FD. CONCLUSIONS: We confirmed the prevalence of FD repeatedly reported in previous screening programmes (approximately 1% irrespective of gender) in a non-selected HCM population in Central Europe. Our findings advocate a routine screening for FD in all adult patients with HCM phenotype including both genders. The dry blood spot method used led to identification of clearly pathogenic variants.

Fukutinopatie patří do skupiny α-dystroglykanopatií, autosomálně recesivně dědičných myopatií kosterního svalu s variabilním postižením srdce. Onemocnění bylo objeveno v Japonsku, kde se endemicky vyskytuje ve formě Fukuyamovy vrozené svalové dystrofie. Vzácně se však vyskytuje také v jiných zemích s odlišnou genetickou architekturou. Prezentujeme případ fukutinopatie diagnostikovaný u mladého muže českého původu s dilatační kardiomyopatií, elevací kreatinkinázy a frustními známkami myopatie kosterního svalu při neurologickém vyšetření. Celoexomová sekvenace identifikovala na jedné alele patogenní variantu fukutinu (FKTN) His172Leu, která při rozsáhlé deleci exonů 1-9 na druhé alele FKTN působila jako homozygotní varianta. Kardiální i neurologický stav pacienta zůstává po čtyřech letech sledování stabilní. Recentně byla u pacienta na magnetické rezonanci (MR) mozku náhodně zjištěna rozsáhlá epidermoidní cysta čtvrté komory s útlakem mozkového kmene vyžadující neurochirurgický zákrok, která by mohla zapadat do vývojových anomálií mozku pacientů s fukutinopatiemi. V kontextu tohoto vzácného onemocnění rozebíráme diagnostiku a léčbu kardiomyopatií doprovázených onemocněním kosterního svalu.
Fukutinopathy belongs to α-dystroglykanopathies, autosomal recessive inherited skeletal muscle myopathies with a variable cardiac involvement. The disease was discovered in Japan with endemic occurrence as Fukuyama congenital skeletal dystrophy. However, it can be rarely diagnosed also in other countries with a different genetic architecture. We present a case of fukutinopathy in a young male of Czech origin with dilated cardiomyopathy and elevation of creatine phosphokinase. Neurologic assessment revealed just borderline signs of skeletal muscle myopathy. Whole-exome sequencing revealed in one allele a pathogenic missense variant of fukutin (FKTN) His172Leu, which was accompanied by a large deletion of exons 1-9 in the second allele of FKTN acting as a homozygous variant. Cardiac and neurologic status of the patient remained stable in the last four years. Recently, the patient has been diagnosed with a large epidermoid cyst of the fourth brain chamber causing a brainstem compression, which required a neurosurgical intervention. The finding could be related to congenital cerebral abnormalities seen in patients with fukutinopathies. In the context of this rare disease, we discuss diagnostics and management of cardiomyopathies associated with a skeletal muscle myopathy.

Úvod: Těhotenství u pacientek s preexistující kardiomyopatií se systolickou dysfunkcí levé komory srdeční nebo po ortotopické transplantaci srdce (OTS) je rizikové pro matku i dítě. Proto je ve většině případů nedoporučujeme. Avšak ve vybraných případech lze těhotenství nechat proběhnout s vědomím zvýšeného rizika. Cílem této práce je analyzovat soubor pacientek se srdečním selháním nebo po transplantaci srdce, které úspěšně absolvovaly těhotenství a porod živého plodu během sledování na našem pracovišti. Metody: Pacientky jsme retrospektivně identifikovali v klinickém informačním systému IKEM a zpracovali jsme jejich klinická data a výsledky pomocných vyšetření. Výsledky: Celkem jsme zaznamenali devět těhotenství u osmi žen s preexistující kardiomyopatií. Ve většině případů šlo o dilatační kardiomyopatii (šest pacientek, 75 %). Ejekční frakce levé komory (EF LKS) byla v mezích normy nebo lehce snížená v 56 % případů (pět těhotenství), středně snížená ve dvou případech (22 %) a v pásmu těžké dysfunkce ve dvou dalších případech (22 %), kde jsme těhotenství nedoporučovali. Nebylo doporučeno ani těhotenství u ženy s hypertrofickou kardiomyopatií a silně pozitivní rodinnou anamnézou zahrnující úmrtí bratra na srdeční selhání a OTS u matky. Později byla v rodině zjištěna Danonova nemoc. Během těhotenství s preexistující kardiomyopatií a v následujícím roce po porodu jsme zaznamenali dvě dekompenzace srdečního selhání, jednu dekompenzaci arteriální hypertenze a dvě tranzitorní ischemické ataky. Kardiovaskulární příhoda komplikovala pět těhotenství (55 %). Ve čtyřech případech (44 %) byl pozorován pokles EF LKS ≥ 10 % po roce sledování. V pozdějším období byla u ženy s Danonovou nemocí nutná srdeční transplantace, nedošlo k žádnému úmrtí. Dále jsme zaznamenali čtyři porody po OTS, které měly kromě jedné epizody preeklampsie nekomplikovaný průběh s narozením čtyř hypotrofických, ale jinak zdravých dětí. Medián od OTS do porodu byl 68 měsíců. Další sledování bylo bez významnějších komplikací, ženy jsou naživu se sledováním 49-118 měsíců po porodu. Závěry: Díky moderní léčbě srdečního selhání je možné nechat proběhnout těhotenství i u žen s neischemickými kardiomyopatiemi a dysfunkcí levé komory srdeční. Těhotenství je možné také u vybraných žen po OTS, nicméně vzhledem k složité biologické situaci a prognostickým aspektům jej u těchto pacientek nedoporučujeme.
Introduction: Pregnancy in females with pre-existing cardiomyopathy with left-ventricular systolic dysfunction and/or after heart transplantation is associated with risks for both the mother and the child. Thus, it is not recommended in the majority of patients. However, in selected cases, with awareness of the risks, pregnancy may be considered. We aimed to analyse a group of patients with heart failure and/or after heart transplantation (HTx) who were pregnant and gave birth to a living new-born during follow-up in our institution. Methods: Patients were found in the clinical database of IKEM retrospectively. Clinical and para-clinical data were analysed. Results: Nine pregnancies in eight females with pre-existing cardiomyopathy were identifi ed. Most patients had dilated cardiomyopathy (75%), their left-ventricular ejection fraction (LVEF) was normal or mildly reduced in 56%, moderately reduced in 22%, and severely reduced in 22%. Pregnancy was not recommended in females with severe left-ventricular systolic dysfunction and also in a female with hypertrophic cardiomyopathy and a strong family history with death due to heart failure in her brother and necessity of HTx in her mother, which was subsequently diagnosed with Danon disease. During pregnancies with pre-existing cardiomyopathy and twelve months postpartum we recorded two episodes of decompensated heart failure, one episode of decompensated arterial hypertension and two transient ischemic attacks. Taken together, cardiovascular events complicated 55% of these pregnancies. We observed a decrease in LVEF ≥10% in 44% of pregnancies after one year. Subsequently, the female with Danon disease required HTx. There was no maternal death. In addition, we recorded four deliveries after HTx, which were except of one episode of preeclampsia uncomplicated and gave birth to four hypotrophic, but healthy babies. Median time from HTx to delivery was 68 months, subsequent period was uneventful, all four females are alive with a follow-up of 49–118 months after delivery. Conclusions: Contemporary therapeutic modalities of heart failure lead in most patients with dilated cardiomyopathy to at least temporary improvement of left-ventricular systolic function outside the range of severe systolic dysfunction, which enables in selected cases birth of living fetus. Pregnancy is possible also in highly selected females after HTx. However, due to their complicated biological situation and prognostic aspects, we do not recommend pregnancy in this setting.

BACKGROUND: Variants in myosin heavy chain 7 (MYH7) are responsible for disease in 1% to 5% of patients with dilated cardiomyopathy (DCM); however, the clinical characteristics and natural history of MYH7-related DCM are poorly described. OBJECTIVES: We sought to determine the phenotype and prognosis of MYH7-related DCM. We also evaluated the influence of variant location on phenotypic expression. METHODS: We studied clinical data from 147 individuals with DCM-causing MYH7 variants (47.6% female; 35.6 ± 19.2 years) recruited from 29 international centers. RESULTS: At initial evaluation, 106 (72.1%) patients had DCM (left ventricular ejection fraction: 34.5% ± 11.7%). Median follow-up was 4.5 years (IQR: 1.7-8.0 years), and 23.7% of carriers who were initially phenotype-negative developed DCM. Phenotypic expression by 40 and 60 years was 46% and 88%, respectively, with 18 patients (16%) first diagnosed at <18 years of age. Thirty-six percent of patients with DCM met imaging criteria for LV noncompaction. During follow-up, 28% showed left ventricular reverse remodeling. Incidence of adverse cardiac events among patients with DCM at 5 years was 11.6%, with 5 (4.6%) deaths caused by end-stage heart failure (ESHF) and 5 patients (4.6%) requiring heart transplantation. The major ventricular arrhythmia rate was low (1.0% and 2.1% at 5 years in patients with DCM and in those with LVEF of ≤35%, respectively). ESHF and major ventricular arrhythmia were significantly lower compared with LMNA-related DCM and similar to DCM caused by TTN truncating variants. CONCLUSIONS: MYH7-related DCM is characterized by early age of onset, high phenotypic expression, low left ventricular reverse remodeling, and frequent progression to ESHF. Heart failure complications predominate over ventricular arrhythmias, which are rare.

Background: Pheochromocytoma is a neuroendocrine tumour originating from the chromaffin cells of adrenal glands or sympathetic paraganglia. It produces most frequently norepinephrine, epinephrine, and dopamine. As a result of non-specific and variable clinical presentation, pheochromocytoma is difficult to diagnose. Case summary: A 37-year-old female without medical history and 57-year-old male patient with diabetes, arterial hypertension, and aortic valve replacement with a mechanical prosthesis were admitted to the hospital after successful cardiopulmonary resuscitation due to ventricular fibrillation. In both patients, coronary angiography demonstrated a normal finding, and echocardiography showed left ventricular (LV) contractile dysfunction with improvement in the subsequent examination. In the first patient, the diagnosis of stress-induced cardiomyopathy was considered as the most probable cause. She was later admitted to hospital due to acute pulmonary oedema with hypertensive crisis. Echocardiography documented reversible LV systolic dysfunction with improvement after 3 days. The course of hospitalization of the male was complicated by multi-organ dysfunction syndrome comprising renal failure, paralytic ileus, and pancreatic irritation, which normalized after 2 weeks. The diagnosis of pheochromocytoma was confirmed by laboratory tests and imaging methods. After pharmacological pre-treatment with doxazosin in both patients and bisoprolol in the female, successful adrenalectomies were performed with no relapse of tumour. Discussion: We describe an atypical clinical presentation of pheochromocytoma with initial cardiac arrest due to ventricular fibrillation and reversible LV systolic dysfunction. Our cases underline that clinical suspicion of pheochromocytoma as a potentially correctable cause should be raised in unexplained cases of severe heart failure, ventricular arrhythmias, and cardiac arrest.
