Úvod: Jaterní testy jsou součástí rutinně prováděných biochemických vyšetření krve, která jsou bezpečná a levná a poskytují informace užitečné pro diagnostiku a léčbu jaterních dysfunkcí. V této studii jsme hodnotili význam faktoru R při rozlišování intrahepatických a extrahepatických příčin cholestázy u kojenců. Metody: Na základě analýzy operační charakteristiky přijímače (ROC) jsme hodnotili diagnostickou hodnotu faktoru R v každé skupině, v porovnání s biopsií jater, která je zlatým diagnostickým standardem. Data byla analyzována pomocí statistického softwaru SPSS a hodnota p < 0,05 naznačuje statisticky významný rozdíl. Výsledky: Do studie bylo zařazeno 37 pacientů s novorozeneckou hepatitidou (skupina A) a 25 pacientů s biliární atrezií (skupina B). Faktor R byl vypočten podle vzorce (ALT pacienta/ ULN) /(ALP pacienta/ ULN). Analýza ROC ukázala, že faktor R ≤ 0,09 s vysokou citlivostí (92 %) (p-hodnota = 0,001) ukazuje na diagnózu biliární atrezie. Specificita, pozitivní predikční hodnota (PPH), negativní predikční hodnota (NPH), diagnostická přesnost a oblast pod křivkou ROC (AUC) dosahovaly hodnot 75,7 %, 72 %, 93 %, 82 % a 0,87. Závěr: Studie ukázala, že faktor R je spolehlivý diagnostický marker pro odlišení nejčastějších intrahepatických a extrahepatických příčin novorozenecké cholestázy a v porovnání s biopsií jater, která je zlatým diagnostickým standardem, vykazuje dobrou diagnostickou přesnost, senzitivitu, specificitu, PPH i NPH. Introduction: Liver function test is a part of safe and affordable routine blood biochemical tests and provides useful information for the diagnosis and management of liver dysfunction. In this study we evaluated the value of the R factor in distinguishing between intrahepatic and extrahepatic causes of infant cholestasis. Methods: Receiver operating characteristic (ROC) analysis was performed to evaluate the diagnostic value of the R factor in each group in comparison with liver biopsy as the gold standard of diagnosis. The data were analyzed using SPSS statistical software and P < 0.05 indicated a significant difference. Results: The study included 37 neonatal hepatitis patients (group A) and 25 biliary atresia patients (group B). The R factor was calculated using the (ALT patient/ ULN) / (ALP patient/ ULN) formula; ROC analysis revealed that if the R factor was ≤ 0.09 the patient diagnosis would be biliary atresia with high sensitivity (92%) (p-value = 0.001). The corresponding specificity, positive predictive value (PPV), negative predictive value (NPV), diagnostic accuracy and area under the ROC curve (AUC) were 75.7%, 72%, 93%, 82% and 0.87, respectively. Conclusion: The study revealed that the R factor is a good diagnostic marker for differentiating between the most common intrahepatic and extrahepatic causes of infant cholestasis and has good diagnostic accuracy, sensitivity, specificity, PPV and NPV, in comparison with liver biopsy as the gold standard of diagnosis.

This report describes extrahepatic bile duct obstruction in two ferrets, which were presented with anorexia, chronic weight loss and general weakness. Physical examination revealed lethargy, cachexia, dehydration, abdominal pain and icterus. Marked haematological, serum chemistry and urinalysis abnormalities included hyperbilirubinaemia (65·5 and 114·2 μmol/L), high concentrations of alanine transaminase (1327·53 and 2578·88 IU/L) and biluribinuria. Abdominal ultrasonography revealed thickening of the bile duct together with the gall bladder wall. The common bile duct was obviously distended. Choledochotomy revealed inspissated bile with fragile green gelatinous plugs that was removed to allow the bile to flow into the duodenum. Cytology and culture of the biliary tract contents were negative for bacteria. Laboratory analyses of biliary plugs showed presence of only protein substances, without detectable mineral composition. Histopathological examination of the liver showed diffuse steatosis and non-specific cholangiohepatitis in both cases.

Objective. Endosonography (EUS) performed prior to endoscopic retrograde cholangiopancreatography (ERCP) or surgery in patients with a low to moderate probability of choledocholithiasis can reduce morbidity, mortality and costs. Aims. This study aimed at evaluating the sensitivity and specificity of EUS and transabdominal ultrasonography (TUS) compared to ERCP in a cohort of 100 patients diagnosed with extrahepatic biliary obstruction. Methods. There were four working groups. The first group processed the initial data including history, physical examination and assessment of laboratory markers of cholestasis. The second group examined TUS. The third group examined the pancreas and biliary tree using EUS, always prior to ERCP. The fourth working group carried out ERCP. Results. The sensitivity and accuracy of EUS in the diagnosis of common bile duct dilatation were 84% and 83%, respectively, i.e. significantly higher (p=0.0001) than TUS (46% and 66%, respectively). The specificities of methods (82%, 91%) were not significantly different (p=0.218). The sensitivity and accuracy of EUS in the diagnosis of pathological content of the common bile duct were 88% and 92%, respectively, i.e. significantly higher (p<0.0001) than TUS (33% and 64%). The specificities of the methods (96%, 94%) were not significantly different (p=0.641). The incidence of choledocholithiasis in the high-risk group was significantly higher than in the moderate-risk group (p=0.012). Conclusion. Radial EUS is a method with high sensitivity, specificity, positive and negative predictive values. It should therefore be preferred to ERCP in patients with low or moderate risk of choledocholithiasis.

Methotrexate (MTX), an important anticancer and immunosuppressive agent, has been suggested for the treatment of primary biliary cirrhosis. However, the drug's pharmacodynamics and toxicity is dependent on its concentrations in plasma which in turn are directly related to MTX's elimination in the liver and kidney. Therefore, the aim of this study was to evaluate changes in MTX biliary and renal excretion during either intrahepatic or obstructive cholestasis in rats. The steady state pharmacokinetic parameters of MTX were evaluated in rats one (BDO1) or seven (BDO7) days after bile duct obstruction (BDO) or 18 h after administration of lipopolysaccharide (LPS). In comparison to the respective control groups, biliary and total clearances of MTX were decreased to 12% and 49% in the BDO1 group, to 5% and 56% in the BDO7 animals, and to 42% and 43% in the LPS group, respectively. Renal clearance of MTX was unchanged in BDO groups, but decreased to 23% of controls in the LPS animals. The serum biochemistry and expression of main hepatic MTX transporters (Mrp2, Mrp3, Mrp4, Bcrp, Oatp1a1, Oatp1a4 and Oatp1b2) confirmed the pathological cholestatic changes in the liver and partly elucidated the cause of changes in MTX pharmacokinetic parameters. In conclusion, this study is the first describing marked alteration of MTX hepatic and renal elimination induced by cholestasis in rats. Moreover, the reported changes in MTX pharmacokinetics and respective transporter expression suggest important mechanistic differences between the two widely used cholestatic models.
