Asistovaná reprodukcia je pomerne rýchlo sa rozvíjajúci odbor medicíny, ktorý poskytuje liečbu neplodným párom. Od narodenia prvého dieťaťa zo „skúmavky“ uplynulo viac než 45 rokov, za ktoré obdobie boli vyvinuté rôzne metodiky za účelom zvýšenia úspešnosti liečby. Samotná spontánna koncepcia je podmienená mnohými faktormi, o ktorých máme aj v súčasnosti pomerne málo informácií. Vývoj asistovanej reprodukcie postupne odhaľuje jednotlivé významné procesy a stavy ktoré ovplyvňujú vznik a vývoj úspešnej tehotnosti. Diabetes mellitus a s ním spojené metabolické poruchy majú negatívny vplyv na kvalitu pohlavných buniek, fertilizáciu, nidáciu a vývoj tehotnosti. Identifikácia a úspešná liečba diabetu má preto dôležitý význam pred zahájením liečby neplodnosti.
Assisted reproduction is a relatively rapidly developing field of medicine focused on the treatment of infertile couples. More than 45 years have passed since the birth of the first “test- tube baby”. Since that time various methods were developed to increase the success rate of the treatment. Spontaneous conception itself is conditioned by many factors, about which we still have relatively little information. The development of assisted reproduction techniques gradually reveals important processes and conditions that influence the development of a successful pregnancy. Diabetes mellitus and associated metabolic disorders have a negative impact on the quality of gametes, fertilization as well as the development of pregnancy. Identification and successful treatment of diabetes plays a key role before starting infertility treatment.

The aim of the study was to describe the menstrual status and perception, risk of low energy availability (LEA) and the presence of orthorexia nervosa (ON) in youth female from a soccer team. Also, verify the possible effect of LEA and ON on physical performance. Data from 19 female players (14.6 ± 1.42 yr) belonging to a soccer team from Cyprus was taken during pre-season. The menstrual cycle status was evaluated by specific questions, LEA by the Low Energy Availability in Females Questionnaire (LEAF-Q), ON by the ORTO-R questionnaire and physical performance by jump, handgrip and cardiorespiratory exercise tests. Players were separated into the risk of LEA and ON. Comparison and correlation tests were performed with a significance set at p < 0.05. As the main results, 66.7% of players perceived that the menstrual period affects their performance in the game, 83.3% did not communicate with coaches about their menstrual cycle; the prevalence of risk of LEA was 26.3%; players with risk of LEA also presented higher scores ON; neither LEA nor ON presented a significant association with players performance. The findings highlighted that youth players perceived an impact of the menstrual period on performance, but did not communicate with the coach about it. Players with the risk of LEA and high values of ON seem not to be associated with a decrease in physical performance during the pre-season evaluation. Attention is required as the players were assessed once. Monitoring these parameters throughout the sports season is recommended to obtain better clarification about the topic.
Stafylokokový syndrom toxického šoku (STŠ) je závažné onemocnění, které může být život ohrožující. Původcem jsou kmeny Staphylococcus aureuss produkcí toxinu syndromu toxického šokuTSST-1 a/nebo enterotoxinu. Existují dvě formy této infekce, menstruální a nemenstruální. Za 40 let v období 1983–2022 jsme v NRL pro stafylokoky CEM SZÚ zaregistrovali 156 případů STŠ bez souvislosti s menstruací. V řadě propouštěcích zpráv byl uváděn těžký průběh včetně hospitalizace na oddělení ARO. Z celého souboru skončilo 29 případů úmrtím. STŠ nejčastěji komplikoval pyodermie (26,9 %) a zhnisané poúrazové rány (18,6 %). Na dalších místech byly zhnisané rány pooperační (14,7 %) a zhnisané popáleniny (10,2 %). V článku charakterizujeme toxinogenní kmeny S. aureus, které jsme mohli potvrdit jako původce těchto onemocnění. Nejčastěji se jednalo o producenty TSST-1 v kombinaci s enterotoxinem – obvykle typu A: 74 (47,3 %). Toxin TSST-1 produkovalo výlučně 21 kmenů (13,5 %). Celkem 61 kmenů (39,1 %) produkovalo pouze nějaký typ enterotoxinu (A, B, C, D a H), samostatně nebo i ve dvojkombinaci. Většina kmenů byla citlivá k methicilinu (MSSA), pouze 8 kmenů bylo MRSA. Zjištění klinické diagnózy a tedy i adekvátní terapie, může být u STŠ obtížné, protože kromě základních příznaků existuje řada další klinických projevů, které se u tohoto onemocnění mohou vyskytovat. Je tedy velice potřebné zjištění stafylokokové toxinové etiologie.
Staphylococcal toxic shock syndrome (TSS) is a serious condition that can be life-threatening. It is caused by Staphylococcus aureus strains with the production of toxic shock syndrome toxin-1 (TSST-1) and/or enterotoxin. There are two forms of this infection, menstrual and non-menstrual. In the 40 years between 1983 and 2022, we registered 156 cases of TSS unrelated to menstruation in the NRL for Staphylococci of the Centre for Epidemiology and Microbiology of the National Institute of Public Health. A number of discharge reports indicated a severe course of the disease including hospitalization at the Department of Anaesthesiology and Resuscitation. Out of the entire set, 29 cases ended in death. TSS most often complicated pyoderma gangrenosum (26.9%) and traumatic wounds (18.6%). Other places were festering surgical wounds (14.7%) and festering burns (10.2%). In the article, we characterize the toxigenic S. aureus strains that we were able to confirm as the causative agents of these diseases. The most common were TSST-1 producers in combination with enterotoxin – usually type A: 74 (47.3%). TSST-1 toxin was exclusively produced by 21 strains (13.5%). A total of 61 strains (39.1%) produced only some type of enterotoxin (A, B, C, D and H), alone or in a double combination. Most strains were susceptible to methicillin (MSSA), only 8 strains were MRSA. Establishing a clinical diagnosis and therefore an adequate therapy can be difficult in TSS, because in addition to the basic symptoms, there are a number of other clinical manifestations that can occur in this disease. It is therefore necessary to identify the staphylococcal toxin aetiology.

Východiska: Sexualita tvoří neodmyslitelnou součást každého jedince, a proto se řadí mezi elementární lidské potřeby. Se sexualitou úzce souvisí sexuální zdraví a gynekologické prohlídky v rámci gynekologické prevence, které by měly dívky (ženy) s mentálním postižením pravidelně podstupovat. Mají ale dívky o tělesných změnách v období puberty, o menstruaci, o těhotenství, o gynekologických prohlídkách adekvátní informace?Cíl: Cílem článku bylo zpracovat z dostupných českých a zahraničních zdrojů relevantní informace týkající se problematiky mentálně postižených dívek v oblasti sexuality, sexuálního zdraví a gynekologické prevence. Dalším cílem bylo zjistit, jak u této skupiny žen probíhá těhotenství a porod.Metody: Článek byl koncipován jako teoretická stať, ke které byla využita metoda obsahové analýzy dokumentů.Výsledky a diskuze: Na základě obsahové analýzy dokumentů bylo zjištěno, že mentálně postižené těhotné ženy pravidelně nedochází ke gynekologovi na prenatální prohlídky. Otázkou je, zda tyto ženy mají o těhotenství a o podstatných vyšetřeních nedostatečné informace anebo na prenatální kontroly jednoduše nedocházejí. Dále bylo zjištěno, že se u mentálně postižených žen v období těhotenství objevuje gestační diabetes mellitus, preeklampsie a předčasný porod. Pokud porod probíhá vaginální cestou je zde riziko dystokie ramének plodu. Po porodu se může objevit postpartální krvácení. Z neonatologických komplikací se u novorozenců objevuje nízké Apgar skóre, intrauterinní růstová retardace, zvýšená mrtvorozenost, perinatální úmrtí a vyšší frekvence hospitalizací novorozenců na jednotkách intenzivní péče.Závěr: Dívky s mentálním postižením potřebují podporu, aby byly schopné porozumět samy sobě a změnám, které nastávají v ženském těle v průběhu života. Podpora dívek v oblasti sexuality, sexuálního zdraví a gynekologické prevence by se neměla lišit od intaktní ženské populace. Je potřeba pouze zvolit vhodné metody k předání informací. Dívkám s mentálním postižením velmi často chybí praktické zkušenosti, proto je důležité edukovat dívky také v praktické rovině. Pouze verbální osvěta je nedostatečná, protože dívkám s mentálním postižením chybí abstrakce. Vzhledem k nedostatečné informovanosti mentálně postižených dívek na téma sexuality a sexuálního zdraví, se kterým je spojená také otázka těhotenství a porodu, jsme se zaměřili u mentálně postižených dívek na osvětu a edukaci právě této oblasti.
Background: Sexuality is an inherent part of every individual and ranks among the elementary human needs. Sexuality is closely related to sexual health and gynaecological examinations as part of gynaecological prevention, which girls (women) with intellectual disabilities should undergo regularly. But do girls have adequate information about the physical changes during puberty, about menstruation, about pregnancy, about gynaecological examinations?Aim: The aim of the article was to compile relevant informations from available Czech and foreign sources regarding the issues of mentally disabled girls in the field of sexuality, sexual health and gynaecological prevention. Another aim was to find out how pregnancy and childbirth progress in this group of women.Methods: The article was conceived as a theoretical essay for which the method of content analysis of documents was used.Results: Based on the content analysis of the documents, it was found that mentally challenged pregnant women do not regularly visit the gynaecologist for antenatal check-ups. The question is whether these women have inadequate information about pregnancy and essential examinations or whether they simply do not attend prenatal check-ups. It has also been found that gestational diabetes mellitus, pre-eclampsia and premature birth occur in mentally handicapped girls during pregnancy. If delivery is by the vaginal route there is a risk of fetal shoulder dystocia. Postpartum haemorrhage may occur after delivery. Among neonatal complications, low Apgar score, intrauterine growth retardation, increased stillbirth, perinatal death and higher frequency of neonatal intensive care unit admissions are seen in newborns.Conclusion: Girls with intellectual disabilities need support to be able to understand themselves and the changes that occur in a woman's body throughout her life. Support for girls in the areas of sexuality, sexual health and gynaecological prevention should be no different from the intact female population. It is only necessary to choose appropriate methods to convey the information. Girls with intellectual disabilities very often lack practical experience, so it is important to educate girls on a practical level as well. Verbal education alone is insufficient because girls with intellectual disabilities lack abstraction. Due to the lack of awareness among girls with intellectual disabilities on the topic of sexuality and sexual health, which is also linked to the issue of pregnancy and childbirth, we focused on sensitizing and educating girls with intellectual disabilities in this area.

The aim of this study was to rapidly review the literature on the prevalence of menstrual disorders in female athletes from different sports modalities. Articles were searched in the Web of Science and PubMed database in May 2022. A total of 1309 records were identified, and 48 studies were included in the final stage. The menstrual disorders described in the included studies were primary (in 33% of included studies) and secondary amenorrhea (in 73% of included studies) and oligomenorrhea (in 69% of included studies). The prevalence of menstrual disorders among the studies ranged from 0 to 61%. When data were pooled according to discipline (mean calculation), the highest prevalence of primary amenorrhea was found in rhythmic gymnastics (25%), soccer (20%) and swimming (19%); for secondary amenorrhea in cycling (56%), triathlon (40%) and rhythmic gymnastics (31%); and oligomenorrhea in boxing (55%), rhythmic gymnastics (44%) and artistic gymnastics (32%). Based on the results of this review, the study supports the literature of the higher prevalence of menstrual disorders in gymnastics and endurance disciplines. However, team sports modalities such as volleyball and soccer also presented a considerable percentage of menstrual disorders compared to the general population. It reinforces the importance of coaches and physicians paying attention to athletes' menstrual cycle as the occurrence of menstrual disorders can be associated with impairment on some health components.
