Phlebotomine sand flies (Diptera: Phlebotominae) are the principal vectors of Leishmania spp. (Kinetoplastida: Trypanosomatidae). In Central Europe, Phlebotomus mascittii is the predominant species, but largely understudied. To better understand factors driving its current distribution, we infer patterns of genetic diversity by testing for signals of population expansion based on two mitochondrial genes and model current and past climate and habitat suitability for seven post-glacial maximum periods, taking 19 climatic variables into account. Consequently, we elucidate their connections by environmental-geographical network analysis. Most analyzed populations share a main haplotype tracing back to a single glacial maximum refuge area on the Mediterranean coasts of South France, which is supported by network analysis. The rapid range expansion of Ph. mascittii likely started in the early mid-Holocene epoch until today and its spread possibly followed two routes. The first one was through northern France to Germany and then Belgium, and the second across the Ligurian coast through present-day Slovenia to Austria, toward the northern Balkans. Here we present a combined approach to reveal glacial refugia and post-glacial spread of Ph. mascittii and observed discrepancies between the modelled and the current known distribution might reveal yet overlooked populations and potential further spread.

Tularemie je infekční onemocnění způsobené drobnou gramnegativní bakterií Francisella tularensis. Patří mezi zoonotické infekce a v minulosti proběhlo v našich podmínkách několik významných epidemií. Diagnóza je často obtížná především pro horší dostupnost přímých vyšetřovacích metod. Sérologické vyšetření bývá v prvních fázích onemocnění nepřínosné. Základem je pečlivě odebraná epidemiologická anamnéza. Rozeznáváme několik klinických forem, přičemž plicní forma může být potenciálním následkem využití tularemie jako biologické zbraně.
Tularemia is an infectious disease caused by the tiny gram-negative bacterium Francisella tularensis. It belongs to zoonotic infections and in the past there have been several significant epidemics in our conditions. Diagnosis is often difficult mainly due to poorer availability of direct methods. Serological examination is usually unprofitable in the first stages of the disease. The basis for correct diagnosis is a carefully collected epidemiological anamnesis. We recognize several clinical forms, while the pulmonary form may be potentialy misused as a biological weapon.

Babesióza patří mezi méně běžné, ale významné infekční onemocnění přenášené klíšťaty. Během posledních 50 let byl zaznamenán zvyšující se počet případů po celém světě, zvláště v USA. Severní část USA je endemickou oblastí, kde počet případů za poslední dekádu stoupl na 2 000 ročně. Původcem většiny těchto nákaz je v tomto regionu Babesia microti, parazit drobných hlodavců. V Evropě bylo od roku 1957 zaznamenáno 56 autochtonních případů lidské babesiózy. Většina těchto onemocnění byla způsobena druhem Babesia divergens, parazitem hovězího dobytka. Od roku 1992 bylo 13 případů nákazy B. microti importováno ze Severní Ameriky do Evropy. Onemocnění je závažné zejména pro splenektomované a imunokomprimované pacienty. Ačkoli nejvýznamnějším přenašečem infekce babesií v Evropě je klíště Ixodes ricinus, byl přenos infekce u řady pacientů způsoben krevní transfuzí, která pro oslabené jedince může končit fatálně. Diagnóza babesiózy se opírá o identifikaci intraerytrocytárních parazitů v krevním roztěru, detekcí DNA babesie testy PCR a stanovením protilátek sérologickými metodami a imunofluorescencí. Onemocnění se léčí antibiotiky (azithromycin, při těžkém průběhu clindamycin) a chininem. Zvýšený nárůst humánní babesiózy je dán nejen změnami klimatu a aktivity klíšťat, pobytem lidí v přírodě a zvýšenou migrací lidí, ale významnou úlohu zde hraje i rozvoj molekulárních metod a rostoucí povědomí o tomto onemocnění.
Babesiosis is a less common but important tick-borne infectious disease. Over the last 50 years, an increasing number of cases have been reported worldwide, especially in the USA. The northern part of the US is an endemic area where the incidence has risen to 2,000 cases per year in the last decade. Babesia microti, a parasite of small rodents, is the cause of most of these infections in that region. In Europe, 56 autochthonous cases of human babesiosis have been reported since 1957. Most of them were caused by the species Babesia divergens, a parasite of cattle. Since 1992, 13 cases of B. microti infection have been imported from North America into Europe. The disease is serious especially for splenectomised and immunocompromised patients. Although the most important vector of babesiosis in Europe is the tick Ixodes ricinus, infection was transmitted through blood transfusion in number of patients, which can be fatal for immunosuppressed patients. The diagnosis of babesiosis is based on the identification of intraerythrocytic parasites in a blood smear, PCR detection of Babesia DNA, and determination of antibodies by serology and immunofluorescence assays. The disease is treated with antibiotics (azithromycin or clindamycin in a severe course of the disease) and quinine. The increase in human babesiosis is not only due to climate change and tick activity, outdoor leisure activities, and increased human migration, but an important role is also played by improved molecular methods and growing awareness of the disease.

Článek pojednává o riziku přenosu infekce SARS-CoV-2 (Severe Acute Respiratory Syndrome-related coronavirus) pro plod v děloze. Transplacentární přenos viru je možný, ale je vzácný díky speciálnímu ochrannému vybavení placenty. Ve vlastním souboru rodiček, který autoři sestavili, bylo riziko nákazy plodu a novorozence nízké. U nemocných rodiček vyšetřovali histologické změny placenty a popsali obraz placentitidy způsobený infekcí SARS-CoV-2. Autoři přidali ilustrační kazuistiku nedonošeného dítěte s vrozenou infekcí SARS-CoV-2. V diskuzi pak autoři popisují možnosti a podrobnosti přenosu SARS-CoV-2 na plod a novorozence, onemocnění novorozenců a prognózu na základě literárních referencí.
This article discusses the risk of transmission of SARS-CoV-2 (Severe acute respiratory syndrome-related coronavirus) infection to the fetus in utero. Transplacental transmission of the virus is possible but is rare due to the special protective equipment of the placenta. In the authors' own cohort of parturients, the risk of fetal and neonatal infection was low. They examined histological changes in the placentas of the affected mothers and described a picture of placentitis caused by SARS-CoV-2 infection. The authors added an illustrative case report of a premature infant with congenital SARS-CoV-2 infection.In the discussion, the authors then describe the possibilities and details of SARS-CoV-2 transmission to the fetus and newborn, neonatal disease, and prognosis based on literature references.

The COVID-19 pandemic highlighted some potential limitations of transmission-based precautions. The distinction between transmission through large droplets vs aerosols, which have been fundamental concepts guiding infection control measures, has been questioned, leading to considerable variation in expert recommendations on transmission-based precautions for COVID-19. Furthermore, the application of elements of contact precautions, such as the use of gloves and gowns, is based on low-quality and inconclusive evidence and may have unintended consequences, such as increased incidence of healthcare-associated infections and spread of multidrug-resistant organisms. These observations indicate a need for high-quality studies to address the knowledge gaps and a need to revisit the theoretical background regarding various modes of transmission and the definitions of terms related to transmission. Further, we should examine the implications these definitions have on the following components of transmission-based precautions: (i) respiratory protection, (ii) use of gloves and gowns for the prevention of respiratory virus infections, (iii) aerosol-generating procedures and (iv) universal masking in healthcare settings as a control measure especially during seasonal epidemics. Such a review would ensure that transmission-based precautions are consistent and rationally based on available evidence, which would facilitate decision-making, guidance development and training, as well as their application in practice.

BACKGROUND: Vector-borne zoonotic diseases are a concerning issue in Europe. Lyme disease and tick-borne encephalitis virus (TBEV) have been reported in several countries with a large impact on public health; other emerging pathogens, such as Rickettsiales, and mosquito-borne flaviviruses have been increasingly reported. All these pathogens are linked to wild ungulates playing roles as tick feeders, spreaders, and sentinels for pathogen circulation. This study evaluated the prevalence of TBEV, Borrelia burgdorferi sensu lato, Rickettsia spp., Ehrlichia spp., and Coxiella spp. by biomolecular screening of blood samples and ticks collected from wild ungulates. Ungulates were also screened by ELISA and virus neutralization tests for flaviviral antibody detection. RESULTS: A total of 274 blood samples were collected from several wild ungulate species, as well as 406 Ixodes ricinus, which were feeding on them. Blood samples tested positive for B. burgdorferi s.l. (1.1%; 0-2.3%) and Rickettsia spp. (1.1%; 0-2.3%) and showed an overall flaviviral seroprevalence of 30.6% (22.1-39.2%): 26.1% (17.9-34.3%) for TBEV, 3.6% (0.1-7.1%) for Usutu virus and 0.9% (0-2.7%) for West Nile virus. Ticks were pooled when possible and yielded 331 tick samples that tested positive for B. burgdorferi s.l. (8.8%; 5.8-11.8%), Rickettsia spp. (26.6%; 21.8-31.2%) and Neoehrlichia mikurensis (1.2%; 0-2.4%). TBEV and Coxiella spp. were not detected in either blood or tick samples. CONCLUSIONS: This research highlighted a high prevalence of several tick-borne zoonotic pathogens and high seroprevalence for flaviviruses in both hilly and alpine areas. For the first time, an alpine chamois tested positive for anti-TBEV antibodies. Ungulate species are of particular interest due to their sentinel role in flavivirus circulation and their indirect role in tick-borne diseases and maintenance as Ixodes feeders and spreaders.

Hlodavci jsou významnými přenašeči onemocnění a jejich přítomnost představuje riziko pro veřejné zdraví. Proto je provádění deratizace důležitým nástrojem v boji proti jejich šíření. V současné době je regulace hlodavců celosvětově do značné míry závislá na používání antikoagulačních rodenticidů. A to i přesto, že tyto účinné látky způsobují otravy necílových živočichů, znečišťují životní prostředí, hlodavci se vůči nim stávají rezistentní a jsou řazeny na seznam účinných látek, které mají být v souladu s nařízením EP a Rady (EU) č. 528/2012 o dodávání biocidních přípravků na trh a jejich používání nahrazeny. Evropská biocidní politika, která klade zvýšený důraz na udržitelné a bezpečné užívání biocidních přípravků, zavedla již před časem z hlediska aplikací a používání antikoagulačních rodenticidů řadu významných specifikací a podmínek. Nicméně tyto podmínky nebyly v České republice zatím dostatečně implementovány. A to především širokou veřejností, jejíž chybějící znalosti o této problematice vedou často k nešetrným aplikacím deratizačních přípravků nebo k vytváření chybných poptávek po službě speciální ochranné deratizace. V mnoha případech jsou podmínky výběrových řízení soukromých, ale i státních institucí v rozporu s etiketami (štítky) biocidních přípravků, a tedy i v rozporu s biocidní politikou. Cílem tohoto příspěvku je zdůraznit, že prosté vyložení antikoagulačních rodenticidu situaci výskytu hlodavců nemusí vyřešit. K deratizační praxi je nutné přistupovat komplexně a rodenticidy používat po důkladném zvážení s cílem dosáhnout maximálního efektu za předpokladu dodržení bezpečnosti.
Rodents are important disease vectors and their presence poses a risk to public health. Therefore, the implementation of rodent control is an important tool in the fight against their spread. Currently, rodent control worldwide is largely dependent on the use of anticoagulant rodenticides. This is in spite of the fact that these active substances cause poisoning of non-target animals, pollute the environment, rodents become resistant to them and are included in the list of active substances to be replaced in accordance with Regulation (EU) No 528/2012 of the European Parliament and of the Council concerning the making available on the market and use of biocidal products. European biocidal policy, which places increased emphasis on the sustainable and safe use of biocidal products, has introduced a number of important specifications and conditions for the application and use of anticoagulant rodenticides some time ago. However, these conditions have not yet been sufficiently implemented in the Czech Republic. This is especially the case with the general public, whose lack of knowledge on this issue often leads to unfriendly applications of extermination products or to the creation of erroneous demands for special protective rodent control services. In many cases, the terms of tenders of private and public institutions are in contradiction with the labels of biocidal products and therefore in contradiction with the biocidal policy. The aim of this paper is to emphasize that simply putting out an anticoagulant rodenticide may not solve the rodent situation. Rodent control practices must be approached in a comprehensive manner and rodenticides must be used after careful consideration in order to achieve maximum effect, provided safety is maintained.
