Pancracine, a montanine-type Amaryllidaceae alkaloid (AA), is one of the most potent compounds among natural isoquinolines. In previous studies, pancracine exhibited cytotoxic activity against diverse human cancer cell lines in vitro. However, further insight into the molecular mechanisms that underlie the cytotoxic effect of pancracine have not been reported and remain unknown. To fill this void, the cell proliferation and viability of cancer cells was explored using the Trypan Blue assay or by using the xCELLigence system. The impact on the cell cycle was determined by flow cytometry. Apoptosis was evaluated by Annexin V/PI and by quantifying the activity of caspases (-3/7, -8, and -9). Proteins triggering growth arrest or apoptosis were detected by Western blotting. Pancracine has strong antiproliferative activity on A549 cells, lasting up to 96 h, and antiproliferative and cytotoxic effects on MOLT-4 cells. The apoptosis-inducing activity of pancracine in MOLT-4 cells was evidenced by the significantly higher activity of caspases. This was transmitted through the upregulation of p53 phosphorylated on Ser392, p38 MAPK phosphorylated on Thr180/Tyr182, and upregulation of p27. The pancracine treatment negatively altered the proliferation of A549 cells as a consequence of an increase in G1-phase accumulation, associated with the downregulation of Rb phosphorylated on Ser807/811 and with the concomitant upregulation of p27 and downregulation of Akt phosphorylated on Thr308. This was the first study to glean a deeper mechanistic understanding of pancracine activity in vitro. Perturbation of the cell cycle and induction of apoptotic cell death were considered key mechanisms of pancracine action.

The search for novel and effective therapeutics for Alzheimer's disease (AD) is the main quest that remains to be resolved. The goal is to find a disease-modifying agent able to confront the multifactorial nature of the disease positively. Herewith, a family of huprineY-tryptophan heterodimers was prepared, resulting in inhibition of cholinesterase and neuronal nitric oxide synthase enzymes, with effect against amyloid-beta (Aβ) and potential ability to cross the blood-brain barrier. Their cholinesterase pattern of behavior was inspected using kinetic analysis in tandem with docking studies. These heterodimers exhibited a promising pharmacological profile with strong implication in AD.

Plants of the Amaryllidaceae family are promising therapeutic tools for human diseases and have been used as alternative medicines. The specific secondary metabolites of this plant family, called Amaryllidaceae alkaloids (AA), have attracted considerable attention due to their interesting pharmacological activities. One of them, galantamine, is already used in the therapy of Alzheimer's disease as a long acting, selective, reversible inhibitor of acetylcholinesterase. One group of AA is the montanine-type, such as montanine, pancracine and others, which share a 5,11-methanomorphanthridine core. So far, only 14 montanine-type alkaloids have been isolated. Compared with other structural-types of AA, montanine-type alkaloids are predominantly present in plants in low concentrations, but some of them display promising biological properties, especially in vitro cytotoxic activity against different cancerous cell lines. The present review aims to summarize comprehensively the research that has been published on the Amaryllidaceae alkaloids of montanine-type.

Biktegravir (BIC) je nový inhibitor integrázy HIV 1 i HIV 2 druhé generace. V únoru 2018 byl schválen americkým Úřadem pro kontrolu potravin a léčiv (FDA) v kombinaci s emtricitabinem a tenofovir alafenamidem (BIC/F/TAF – 50/200/25 mg, Biktarvy®) pro léčbu dospělých osob s infekcí HIV 1. Podává se jedna tableta jednou denně. Jeho účinnost, bezpečnost, snášenlivost a lékové interakce byly hodnoceny ve čtyřech (GS-US-380-1489, 1490, 1844 a 1878) studiích s použitím srovnatelného komparátoru – dolutegraviru či inhibitoru proteázy u předléčených pacientů. Indikací BIC/F/TAF je infekce HIV 1 u osob dosud neléčených nebo u dříve léčených bez známek selhání před-chozího režimu a bez prokázané rezistence na některou komponentu. Bictegravir (BIC) is a novel, second generation HIV 1 and HIV 2 integrase strand transfer inhibitor. In February 2018, it has been approved by the Food and Drug Administration (FDA) in combination with emtricitabine and tenofovir alafenamide (BIC/F/TAF – 50/200/25 mg, Biktarvy®) for the treatment of adult patients with HIV 1 infection. It is administered with one tablet once daily. Its efficiency, safety, tolerability and drug interactions were elucidated in four (GS-US-380-1489, 1490, 1844 and 1878) studies using a comparator ‒ dolute-gravir, or protease inhibitor in naïve patients. The indications of BIC/F/TAF include HIV 1 infection in treatment naïve patients, or in previously treated patients with no signs of failure of the previous regimen and without a proven resistance to any component. In the period 96 weeks, no resistance to BIC was found. Other studies, including the post marketing ones, are underway.

V přehledovém článku je proveden rozbor základních farmakologických vlastností, léčebných výsledků i indikací fixní kombinace sofosbuviru a velpatasviru (Epclusa®). Jde o kombinaci inhibitoru kofaktoru polymerázy (produktu NS5A oblasti virového genomu) - velpatasviru a inhibitoru RNA dependentní RNA-polymerázy (produktu NS5B oblasti virového genomu) - sofosbuviru. Protože se jedná o první kombinaci virostatik účinnou proti všem základním genotypům viru hepatitidy C, je její indikační pole velmi široké. Primární výhodou preparátu je jeho vysoká bezpečnost, minimální nežádoucí účinky, téměř 100% účinnost a především jednoduchost jeho podání z pohledu pacienta.
The review article analyzes the basic pharmacological properties, the therapeutic results and the indication of the fixed combination of sofosbuvir and velpatasvir (Epclusa®). It is a combination of a cofactor polymerase inhibitor (virus NS5A domain) - velpatasvir and a RNA-dependent RNA-polymerase inhibitor (NS5B viral genome) - sofosbuvir. Because it is the first combination of virostatics effective against all the major genotypes of the hepatitis C virus, its indicating field is very wide. The primary benefit of the preparation is its high safety, minimal side effects, almost 100% efficacy and, above all, its simplicity of use from the patient's point of view.

Tacrine (THA), the first clinically effective acetylcholinesterase (AChE) inhibitor and the first approved drug for the treatment of Alzheimer's disease (AD), was withdrawn from the market due to its side effects, particularly its hepatotoxicity. Nowadays, THA serves as a valuable scaffold for the design of novel agents potentially applicable for AD treatment. One such compound, namely 7-methoxytacrine (7-MEOTA), exhibits an intriguing profile, having suppressed hepatotoxicity and concomitantly retaining AChE inhibition properties. Another interesting class of AChE inhibitors represents Huprines, designed by merging two fragments of the known AChE inhibitors-THA and (-)-huperzine A. Several members of this compound family are more potent human AChE inhibitors than the parent compounds. The most promising are so-called huprines X and Y. Here, we report the design, synthesis, biological evaluation, and in silico studies of 2-methoxyhuprine that amalgamates structural features of 7-MEOTA and huprine Y in one molecule.

Sirtuin activating compounds (STACs) attenuate various type of liver insults through mechanisms which are not fully understood. In the present study, we investigated the ameliorative potential of quercetin (natural polyphenol) and SRT1720 (synthetic SIRT1 activator) against D-galactosamine/lipopolysaccharide-induced hepatotoxicity (an experimental model of acute liver failure). Moreover, we compared and contrasted the roles of stress responsive enzymes, sirtuin 1 (SIRT1) and heme oxygenase 1 (HO-1) in hepatoprotection/ hepatotoxicity. Liver injury was induced in male Wistar rats by intraperitoneal injection of D-galactosamine (400 mg/kg) and lipopolysaccharide (10 microg/kg). Some animals were pretreated with quercetin (50 mg/kg i.p.) or SRT1720 (5 mg/kg i.p.). Twenty-four hours later, the effects of these treatments were evaluated by biochemical studies and Western blot. D-GalN/LPS treatment upregulated HO-1 expression, downregulated SIRT1 expression, decreased AST: ALT ratio and markedly increased bilirubin, catalase and conjugated diene levels. Pretreatment of D-GalN/LPS rats with either quercetin or SRT1720 returned SIRT1 expression, HO-1 expression and all the aforementioned markers towards normal. Collectively, these findings suggest that elevated HO-1 and low SIRT1 expressions are involved in the pathogenesis of D-GalN/LPS-induced hepatotoxicity. Drugs that downregulate HO-1 and/or upregulate SIRT1 seem to have antihepatotoxic effects and need further exploration.
