The placenta provides a significant physical and physiological barrier to prevent fetal infection during pregnancy. Nevertheless, it is at times breached by pathogens and leads to vertical transmission of infection from mother to fetus. This review will focus specifically on the Zika flavivirus, the HIV retrovirus and the emerging SARS-CoV2 coronavirus, which have affected pregnant women and their offspring in recent epidemics. In particular, we will address how viral infections affect the immune response at the maternal-fetal interface and how the placental barrier is physically breached and discuss the consequences of infection on various aspects of placental function to support fetal growth and development. Improved understanding of how the placenta responds to viral infections will lay the foundation for developing therapeutics to these and emergent viruses, to minimise the harms of infection to the offspring.

AIMS: Coronavirus disease 2019 is responsible for a worldwide increase in morbidity and mortality. The relationship of this infection to mother-to-child vertical transmission has not been elucidated yet. However, recent reports indicate a foetal death rate of up to 3%. METHODS: We report a case of sudden pre-term foetal demise in a woman positive for SARS-CoV-2 but asymptomatic, with physiological course of pregnancy. RESULTS: One of the possible explanations of sudden foetal death may be acute placental insufficiency caused by a SARS-CoV-2 placental infection or the development of foetal inflammatory response syndrome (FIRS). CONCLUSION: Considering the potential risk of foetal demise, questions remain regarding foetal monitoring and the timing of labour and delivery in the second and third trimesters, particularly in asymptomatic or mild maternal SARS-CoV-2 infection. A relevant multidisciplinary team must also be aware of these risks associated with possibly fatal consequences.

An estimated 1.3 million pregnant women were living with HIV in 2018. HIV infection is associated with adverse pregnancy outcomes and all HIV-positive pregnant women, regardless of their clinical stage, should receive a combination of antiretroviral drugs to suppress maternal viral load and prevent vertical fetal infection. Although antiretroviral treatment in pregnant women has undoubtedly minimized mother-to-child transmission of HIV, several uncertainties remain. For example, while pregnancy is accompanied by changes in pharmacokinetic parameters, relevant data from clinical studies are lacking. Similarly, long-term adverse effects of exposure to antiretrovirals on fetuses have not been studied in detail. Here, we review current knowledge on HIV effects on the placenta and developing fetus, recommended antiretroviral regimens, and pharmacokinetic considerations with particular focus on placental transport. We also discuss recent advances in antiretroviral research and potential effects of antiretroviral treatment on placental/fetal development and programming.

Klinický průběh onemocnění covid‑19 u dětí byl od začátku šíření infekce většinou popisován jako mírný, méně závažný než u dospělých, s malým rizikem nutnosti hospitalizace či intenzivní péče. S šířením viru do různých části světa také přibylo studií a dat o dětských pacientech, které potvrdily výše uvedené. V dubnu byly reportovány první případy Kawasaki‑like syndromu z Velké Británie v návaznosti na infekci covid‑19 u dětí, později pojmenované jako multisystémový zánětlivý syndrom u dětí (MIS‑C). Děti větši‑ nou vyžadují jen podpůrnou terapii, v případě vážného průběhu je možné zahájit terapii remdesivirem a dexamethasonem. Průběh u novorozenců je většinou příznivý, jen s minimem symptomů. Těhotné ženy s covid‑19 mají vyšší riziko vážného průběhu. Vertikální přenos z matky na plod je velmi vzácný.
The clinical course of COVID‑19 in children has been described as mild, less severe than in adults, with little risk of hospitalization or intensive care. With the spread of the virus to different parts of the world, there have also been more studies and data on pediatric patients that have confirmed the above mentioned facts. In April, the first cases of Kawasaki‑like syndrome from the United Kingdom were reported following COVID‑19 infection in children, later referred to as multisystem inflammatory syndrome in children (MIS‑C). Children usually require only supportive therapy, in case of a severe course, it is possible to start treatment with remdesivir and dexamethasone. The course in newborns is usually favorable, with only minimal symptoms. Pregnant women with COVID‑19 have a higher risk of a severe illness compared with nonpregnant peers. Vertical transmission from mother to fetus is very rare.

Objectives To evaluate the strength of association between maternal and pregnancy characteristics and the risk of adverse perinatal outcomes in pregnancies with laboratory confirmed COVID-19. Methods Secondary analysis of a multinational, cohort study on all consecutive pregnant women with laboratory-confirmed COVID-19 from February 1, 2020 to April 30, 2020 from 73 centers from 22 different countries. A confirmed case of COVID-19 was defined as a positive result on real-time reverse-transcriptase-polymerase-chain-reaction (RT-PCR) assay of nasal and pharyngeal swab specimens. The primary outcome was a composite adverse fetal outcome, defined as the presence of either abortion (pregnancy loss before 22 weeks of gestations), stillbirth (intrauterine fetal death after 22 weeks of gestation), neonatal death (death of a live-born infant within the first 28 days of life), and perinatal death (either stillbirth or neonatal death). Logistic regression analysis was performed to evaluate parameters independently associated with the primary outcome. Logistic regression was reported as odds ratio (OR) with 95% confidence interval (CI). Results Mean gestational age at diagnosis was 30.6±9.5 weeks, with 8.0% of women being diagnosed in the first, 22.2% in the second and 69.8% in the third trimester of pregnancy. There were six miscarriage (2.3%), six intrauterine device (IUD) (2.3) and 5 (2.0%) neonatal deaths, with an overall rate of perinatal death of 4.2% (11/265), thus resulting into 17 cases experiencing and 226 not experiencing composite adverse fetal outcome. Neither stillbirths nor neonatal deaths had congenital anomalies found at antenatal or postnatal evaluation. Furthermore, none of the cases experiencing IUD had signs of impending demise at arterial or venous Doppler. Neonatal deaths were all considered as prematurity-related adverse events. Of the 250 live-born neonates, one (0.4%) was found positive at RT-PCR pharyngeal swabs performed after delivery. The mother was tested positive during the third trimester of pregnancy. The newborn was asymptomatic and had negative RT-PCR test after 14 days of life. At logistic regression analysis, gestational age at diagnosis (OR: 0.85, 95% CI 0.8-0.9 per week increase; p<0.001), birthweight (OR: 1.17, 95% CI 1.09-1.12.7 per 100 g decrease; p=0.012) and maternal ventilatory support, including either need for oxygen or CPAP (OR: 4.12, 95% CI 2.3-7.9; p=0.001) were independently associated with composite adverse fetal outcome. Conclusions Early gestational age at infection, maternal ventilatory supports and low birthweight are the main determinants of adverse perinatal outcomes in fetuses with maternal COVID-19 infection. Conversely, the risk of vertical transmission seems negligible.

Počet osob s infekcí způsobené lidským virem imunodeficience (Human Immunodeficiency Virus, HIV) v České republice stále stoupá. Narůstající počet HIV pozitivních gravidních žen není tolik výrazný a zásluhou profylaktického programu se riziko vertikálního přenosu výrazně snížilo. Profylaktický program je rozdělen do 4 částí (antiretrovirová léčba během gravidity, porod císařským řezem a nahrazení kojení mléčnou stravou, užívání antiretrovirové suspenze během prvních šesti týdnů života novorozence). Dispenzarizační program je do 18 měsíců věku dítěte, kdy je dítě po finálním negativním PCR testu (polymerázová řetězová reakce) vyřazeno z evidence.
A number of people with human immunodeficiency virus infection (Human Immunodeficiency Virus, HIV) are still rising in theCzech Republic. The increasing number of HIV-positive pregnant women is not so significant due to the prophylactic program,the risk of vertical transmission has significantly decreased. The prophylactic program is divided into 4 parts (antiretroviral treatmentduring pregnancy, birth by caesarean section and replacing breastfeeding with a milk diet, using antiretroviral suspensionduring the first six weeks of newborn life). The dispensation program is up to 18 months of child age when the child is removedfrom the evidence after the final negative PCR test (polymerase chain reaction).
