The precise method of action of dienogest on the production and development of endometriosis lesions is unknown, and its controversial effect on endometrial thickness has been under investigation. In the following study the Dienogest's effects on the target animal's histology of female reproductive organs, including the tissues from the Fallopian tubes, ovaries, and uterus, as well as the impact of drug administration on the liver's enzymes and the drug's effects on triglycerides, body weight, and HbA1c, have all been studied. The findings of the following experiment indicated that there was no significant elevation of liver enzymes. The little to no elevation of the liver enzymes indicated that the drug did not induce stress on the hepatic cells and according to the subsequent experiment it is safe for clinical use. Moreover, after 10, 20, and 30 days of blood administration, the level of blood TG significantly decreased, and after 30 days of intake, the level of blood sugar significantly decreased. However, there were no appreciable changes after 10 and 20 days. After 30 days of treatment, the rats' weight also showed a very minor drop. In addition to it, the results of histological changes in the tissue in the following study represented that there were evident changes in the tissues which comprised of decline in blood circulation, fibrosis in tissues, and degeneration of follicles.

Jaterní testy patří mezi nejčastěji stanovované biochemické parametry a je nezbytné, aby jejich interpretace byla vždy prováděna správně. O přítomnosti jaterní choroby vypovídá nejen všem dobře známý základní panel jaterních testů (hodnoty bilirubinu, ALT, AST, GGT a ALP), ale také další biochemické parametry, zejména hladina albuminu, INR a počet trombocytů. Poslední jmenované analyty bývají často opomíjeny, stejně jako skutečnost, že normální hodnoty základního panelu jaterních testů ještě nevylučují pokročilé chronické jaterní onemocnění – fibrózu či cirhózu. Naproti tomu vysoké hodnoty jaterních testů automaticky neznamenají jaterní selhání. Ve světle vzrůstající incidence jaterních chorob stejně jako stoupající mortality z jaterních příčin v mnoha evropských zemích je žádoucí aktualizovat diagnostický algoritmus při záchytu patologických hodnot jaterních testů.
Liver function tests are among the most determined biochemical parameters, and it is essential that their interpretation is always performed correctly. The presence of liver disease is indicated not only by the well-known basic panel of liver function tests (bilirubin, ALT, AST, GGT and ALP), but also by other biochemical parameters, especially albumin, INR and platelet count. The latter analytes are often overlooked, as is the fact that normal values in the baseline liver test panel do not yet rule out advanced chronic liver disease - liver fibrosis or cirrhosis. In contrast, high levels of liver function tests do not automatically indicate liver failure. Considering the increasing incidence of liver disease as well as the rising mortality from liver causes in many European countries, it is desirable to update the diagnostic algorithm when pathological liver function tests are detected.

PURPOSE: The poly(ADP-ribose) polymerase inhibitor rucaparib is approved for the treatment of patients with recurrent ovarian and metastatic castration-resistant prostate cancer; however, limited data are available on its use in patients with hepatic dysfunction. This study investigated whether hepatic impairment affects the pharmacokinetics, safety, and tolerability of rucaparib in patients with advanced solid tumors. METHODS: Patients with normal hepatic function or moderate hepatic impairment according to the National Cancer Institute Organ Dysfunction Working Group (NCI-ODWG) criteria were enrolled and received a single oral dose of rucaparib 600 mg. Concentrations of rucaparib and its metabolite M324 in plasma and urine were measured. Pharmacokinetic parameters were compared between hepatic function groups, and safety and tolerability were assessed. RESULTS: Sixteen patients were enrolled (n = 8 per group). Rucaparib maximum concentration (Cmax) was similar, while the area under the concentration-time curve from time 0 to infinity (AUC0-inf) was mildly higher in the moderate hepatic impairment group than in the normal control group (geometric mean ratio, 1.446 [90% CI 0.668-3.131]); similar trends were observed for M324. Eight (50%) patients experienced ≥ 1 treatment-emergent adverse event (TEAE); 2 had normal hepatic function and 6 had moderate hepatic impairment. CONCLUSION: Patients with moderate hepatic impairment showed mildly increased AUC0-inf for rucaparib compared to patients with normal hepatic function. Although more patients with moderate hepatic impairment experienced TEAEs, only 2 TEAEs were considered treatment related. These results suggest no starting dose adjustment is necessary for patients with moderate hepatic impairment; however, close safety monitoring is warranted.

BACKGROUND: Natural history models for primary sclerosing cholangitis (PSC) are derived from adult patient data, but have never been validated in children. It is unclear how accurate such models are for children with PSC. METHODS: We utilized the pediatric PSC consortium database to assess the Revised Mayo Clinic, Amsterdam-Oxford, and Boberg models. We calculated the risk stratum and predicted survival for each patient within each model using patient data at PSC diagnosis, and compared it with observed survival. We evaluated model fit using the c-statistic. RESULTS: Model fit was good at 1 year (c-statistics 0.93, 0.87, 0.82) and fair at 10 years (0.78, 0.75, 0.69) in the Mayo, Boberg, and Amsterdam-Oxford models, respectively. The Mayo model correctly classified most children as low risk, whereas the Amsterdam-Oxford model incorrectly classified most as high risk. All of the models underestimated survival of patients classified as high risk. Albumin, bilirubin, AST, and platelets were most associated with outcomes. Autoimmune hepatitis was more prevalent in higher risk groups, and over-weighting of AST in these patients accounted for the observed versus predicted survival discrepancy. CONCLUSIONS: All 3 models offered good short-term discrimination of outcomes but only fair long-term discrimination. None of the models account for the high prevalence of features of autoimmune hepatitis overlap in children and the associated elevated aminotransferases. A pediatric-specific model is needed. AST, bilirubin, albumin, and platelets will be important predictors, but must be weighted to account for the unique features of PSC in children.

Cílem článku je shrnout nejpodstatnější informace souvisejí s interpretací zvýšených jaterních testů v klinické praxi, upozornit na některá úskalí a na příkladech ukázat časté typy jaterních lézí. V úvodu jsou zmíněny základní teoretické informace týkající se jaterních testů, dále určité specifické příznaky a údaje z anamnézy napomáhající v interpretaci jaterních testů a diferenciální diagnóze. V poslední části jsou uvedeny příklady konkrétních pacientů, jejich jaterní testy a diagnostický závěr.
The aim of the article is to summarize essential information concerning the interpretation of elevated liver tests in the clinicalpractice, highlight some of the pitfalls, and use examples to demonstrate common types of hepatic lesions. The introductionsection deals with basic theoretical information regarding liver tests as well as certain specific signs and medical history data thataid in interpreting liver tests and performing differential diagnosis. The final section presents examples of particular patients,their liver tests, and the diagnostic conclusions.

Sodium glucose co-transporter type 2 inhibitors (SGLT-2 inhibitors) are a class of antidiabetics, recently approved for the treatment of patients with T2DM. They feature cardioprotective and renoprotective action, while they exert beneficial effects on metabolic parameters. Non-alcoholic fatty liver disease (NAFLD) is a frequent co-morbidity in diabetic patients. Its prevalence reaches up to 70%. Since there is no specific treatment approved for NAFLD, both experimental and clinical studies have been recently conducted highlighting the efficacy and safety of SGLT-2 inhibitors mainly in animal models and secondarily in patients with T2DM and NAFLD. This class of antidiabetics seems very attractive, improving both glycemic control and liver function tests, while inhibiting NAFLD progression. However, further investigation is required to establish them as a first-line treatment option in T2DM patients with NAFLD, after thorough assessment of their efficacy and safety in clinical practice.

Porovnání vzorků testovaných pacientů s dvěma kontrolními vzorky se zvýšenou aktivitou AST bylo hodnoceno na základě výpočtu procenta precipitační aktivity polyethylenglykolu (PEG-precipitable activity, % PPA) dle Davidsona a Watsona. Séra pacientů a kontrol byla dále skladována při 4°C po dobu 144 hodin a monitorování aktivity enzymu AST bylo prováděno každých 24 hodin. Výsledky a závěr: U obou vyšetřovaných pacientů bylo po precipitaci s PEG 6000 potvrzeno podezření na výskyt makroenzymové formy AST v séru (% PPA 92 a 79), na rozdíl od techniky skladování vzorku při 4°C, kde pokles aktivity AST nebyl prokázán. Použití poměrně jednoduchého a finančně nenáročného laboratorního průkazu makroenzymové formy AST mohou zamezit dalším nákladným invazivním vyšetřením a diagnostickým chybám.
Design: Original paper Settings: Institute of Clinical and Experimental Medicine, Vídeňská 1958/9, 140 21 Praha 4 Materials and Methods: Sera of two tested patients were precipitated with polyethylene glycol 6000 (PEG 6000, Sigma Aldrich) in a volume ratio of 1:1. After centrifugation, AST enzyme activity in the supernatant was determined (Architect ci16200, Abbott). AST enzyme activity values were compared to those of the patient sera, which were diluted with saline (NaCl 9 g/L) in a volume ratio of 1:1. The results of comparing of the tested patient samples with the two control samples were evaluated as a percentage of PEG-precipitable activity (%PPA). The patient and control sera were stored at 4°C for 144 hours and AST enzyme activity was monitored after each 24 hours. Results and Conclusions: The presence of macro-AST as the most probable cause of permanently increased activity of AST in serum was confirmed. PEG-precipitable activities in tested patients were 92 and 79 %, respectively. In contrast, the comparative method of storing samples at 4°C did not reveal any decline of AST activity. These relatively simple and inexpensive laboratory methods of macro-AST detection can save on unnecessary examinations as well as prevent diagnostic errors.
