INTRODUCTION: Toxic epidermal necrolysis (TEN) is a rare, life-threatening autoimmune disease predominantly manifested in the skin and mucous membranes. Today, infectious complications have the dominant share in mortality of TEN patients. Due to the nature of the therapy and administration of immunosuppressive medications, a wide range of potentially pathogenic microorganisms, which cause infectious complications in different compartments in these patients, is not surprising. MATERIAL AND METHODOLOGY: This is a multicentric study, which included all patients with TEN hospitalized between 2000-2015 in specialized centres in the Czech Republic and Slovakia. The total catchment area was over 12.5 million inhabitants. The actual implementation of the project was carried out using data obtained from the registry CELESTE (Central European LyEll Syndrome: Therapeutic Evaluation), when specific parameters relating to epidemiological indicators and infectious complications in patients with TEN were evaluated in the form of a retrospective analysis. RESULTS: In total, 39 patients with TEN were included in the study (12 patients died, mortality was 31%), who were hospitalized in the monitored period. The median age of patients in the group was 63 years (the range was 4-83 years, the mean was 51 years), the median of the exfoliated area was 70% TBSA (total body surface area) (range 30-100%, mean 67%). SCORTEN was calculated for 38 patients on the day of admission. Its median in all patients was 3 (range 1-6; mean 3). Any kind of infectious complication in the study group was recorded in 33 patients in total (85%). In total, 30 patients (77%) were infected with gram-positive cocci, 27 patients (69%) with gram-negative rods, and yeast cells or fibrous sponge were cultivated in 12 patients (31%). A total of 32 patients (82%) were found to have infectious complications in the exfoliated area, 15 patients (39%) had lower respiratory tract infections, 18 patients (46%) urinary tract infections and 15 patients (39%) an infection in the bloodstream. The most common potentially pathogenic microorganism isolated in our study group was coagulase neg. Staphylococcus, which caused infectious complications in 24 patients. Enterococcus faecalis/faecium (19 patients), Pseudomonas aeruginosa (17 patients), Staphylococcus aureus (11 patients) and Escherichia coli (11 patients) were other most frequently isolated micro-organisms. CONCLUSION: The published data were obtained from the unique registry of TEN patients in Central Europe. In the first part, we have succeeded in defining the basic epidemiological indicators in the group of patients anonymously included in this registry. The study clearly confirms that infectious complications currently play an essential role in TEN patients, often limiting the chances of survival. The study also shows a high prevalence of these complications in the period after 15days from the start of hospitalization, when most patients already have completely regenerated skin cover.

Centrální venózní katetry jsou u hematologických pacientů vyžadujících intenzivní léčbu nebo podávání parenterální chemoterapie často využívaným přístupem pro zajištění žilního vstupu. Přes četné výhody centrálních venózních katetrů může být jejich používání spojeno s mnohými komplikacemi. Mezi nejzávažnější a nejčastější komplikace patří rozvoj katetrových infekcí, které zvyšují morbiditu a mortalitu pacientů. Pod záštitou České leukemické skupiny – pro život (CELL) byla formou dotazníků provedena retrospektivní analýza používání centrálních žilních katetrů a výsledků mikrobiologického vyšetření těchto katetrů na hematologických centrech České a Slovenské republiky. V analýze byla hodnocena data ze 7 pracovišť dospělého věku a 4 pracovišť pediatrických za rok 2015. Celkově bylo na pracovištích zavedeno 3 195 centrálních venózních katetrů, kdy krátkodobé centrální venózní katetry byly používány 4krát častěji než dlouhodobé. Preferovaným podtypem krátkodobého katetru podle místa zavedení byly katetry zavedené cestou vena subclavia (49,7 %), ale mezi jednotlivými centry byly významné rozdíly dané především zvyklostí pracoviště. K mikrobiologickému vyšetření bylo odesláno průměrně 45 % extrahovaných katetrů, z dětských pracovišť byl zaslán k vyšetření významně vyšší podíl katetrů než z dospělých pracovišť (65,3 % vs. 33,5 %). Průměrně u 20 % z celkového počtu odeslaných katetrů k mikrobiologickému vyšetření byl prokázán pozitivní kultivační záchyt. Nejfrekventnějším kultivačním záchytem byly grampozitivní koky, zastoupené koaguláza negativními stafylokoky a dále Staphylococcus aureus. V případě eliminace koaguláza negativních stafylokoků převažovaly v mikrobiologických záchytech gramnegativní bakterie, především enterobakterie Pseudomonas aeruginosa, Escherichia coli a Klebsiella pneumonie. Při srovnání výsledků analýzy s daty ze světových prací a doporučení se situace na hematologických centrech v České a Slovenské republice významně neliší. I tak analýza ukázala mezi jednotlivými centry významné rozdíly a nezbytnost důsledného dodržování platných mezinárodních doporučení pro prevenci a diagnostiku katetrových infekcí, jejichž souhrn v práci předkládáme.
Central venous catheters are routinely used as a venous access in haematological patients requiring intensive care or parenteral chemotherapy. Despite its many benefits, the use of central venous catheters can be associated with several complications. The most serious and most common complications are catheter related bloodstream infections that significantly increase morbidity and mortality of patients with haematological malignancy. On behalf of the Czech leukaemia group for Life (CELL), a retrospective analysis of the use of central venous catheters and the results of microbiological examination of extracted central venous catheters at haematological centres in Czech Republic and Slovakia was performed. A short questionnaire was used for this purpose. Data from 2015 from 7 adult and 4 paediatric centres were evaluated. A total of 3195 central venous catheters were inserted in the centres involved, whereby short-term catheters were used 4 times more often than long-term catheters. The preferred subtype of short-term catheter according to the site of insertion were catheters inserted via the subclavian vein (49.7%), however there were significant differences between centres. On average – 45% of the extracted catheters were sent for microbiological examination. A significantly higher proportion of catheters were thus examined in paediatric centres compared to adult centres (65.3% vs. 33.5%). Positive culture results were found on average in 20 % catheters examined microbiologically. Most frequently, catheter tip cultures involved gram positive cocci, mostly coagulase negative staphylococci and Staphylococcus aureus. When coagulase negative staphylococci were excluded, gram negative bacteria namely Pseudomonas aeruginosa, Escherichia coli, and Klebsiella pneumoniae, were the most prevalent findings. No significant differences were found in Czech and Slovak haematological centres when compare to international data and recommendations. Nevertheless, significant differences between individual centres were observed and thus we believe that more strict compliance with current international recommendations for the prevention and diagnosis of catheter related infections is required. These guidelines are also part of this manuscript.

Acinetobacter seifertii is a recently described species that belongs to the Acinetobacter calcoaceticus-Acinetobacter baumannii complex. It has been recovered from clinical samples and is sometimes associated with antimicrobial resistance determinants. We present here the case of three A. seifertii clinical isolates which were initially identified as Acinetobacter sp. by phenotypic methods but no identification at the species level was achieved using semi-automated identification methods. The isolates were further analysed by whole genome sequencing and identified as A. seifertii. Due to the fact that A. seifertii has been isolated from serious infections such as respiratory tract and bloodstream infections, we emphasize the importance of correctly identifying isolates of the genus Acinetobacter at the species level to gain a deeper knowledge of their prevalence and clinical impact.

Koaguláza negativní stafylokokoky jsou podmíněnými patogeny, které mohou způsobit u oslabených pacientůzávažná onemocnění. V humánním klinickém materiálu bývá nalézána asi polovina popsaných druhů. Vzácně lze zachytit i některé další druhy, které se spíše nacházejí ve veterinárním materiálu nebo v potravinách. V příspěvku prezentujeme kazuistiku 71letého muže, u kterého byl jako příčina sepse izolován kmen Staphylococcus condimenti. Tento druh se obvykle nachází v sójové omáčce a dosud byl jeho záchyt z humánního materiálu popsán velice vzácně.
Coagulase-negative staphylococci are opportunistic pathogens which may cause serious conditions in debilitatedor compromised patients. About half of the described species have been found in human clinical specimens. Some other species, typically found in veterinary specimens or in food, are rarely detected in humans. A case report ispresented of a 71-year-old man diagnosed with sepsis caused by Staphylococcus condimenti. This species has been detected in soy sauce, and its isolation from human clinical specimens appears to be very rare.

BACKGROUND: Catheter-related bloodstream infection (CRBSI) is a common complication in patients receiving home parenteral nutrition (HPN). Data regarding catheter salvage after a CRBSI episode are limited. We aimed to determine the incidence of CRBSI and rates of catheter salvage in adult patients receiving HPN. MATERIALS AND METHODS: We retrospectively searched our prospectively maintained HPN database for the records of all adult patients receiving HPN from January 1, 1990, to December 31, 2013, at our tertiary referral center. Data abstracted from the medical records included demographics, diseases, treatments, and outcomes. The incidence of CRBSI and rates of catheter salvage were determined. RESULTS: Of 1040 patients identified, 620 (59.6%) were men. The median total duration on HPN was 124.5 days (interquartile range, 49.0-345.5 days). Mean (SD) age at HPN initiation was 53.3 (15.3) years. During the study period, 465 CRBSIs developed in 187 patients (18%). The rate of CRBSI was 0.64/1000 catheter days. Overall, 70% of catheters were salvaged (retained despite CRBSI) during the study period: 78% of infections with coagulase-negative staphylococci, 87% with methicillin-sensitive Staphylococcus aureus, and 27% with methicillin-resistant S aureus. The percentage of catheters salvaged was 63% from 1990 to 1994, 63% from 1995 to 1999, 61% from 2000 to 2004, 72% from 2005 to 2009, and 76% from 2010 to 2013. CONCLUSION: Catheter salvage is possible after a CRBSI episode. Since most episodes of CRBSI are caused by skin commensals, effective treatment without removal of the central venous catheter is possible in most cases.

Východiska: Infekcie krvného prúdu súvisiace s centrálnym venóznym katétrom (catheter-related bloodstream infections – CRBSI) patria k život ohrozujúcim komplikáciám. Sú spojené so zvýšenou morbiditou, mortalitou pacientov, ako aj zvýšenými nákladmi na zdravotnú starostlivosť. Onkologickí pacienti predstavujú obzvlášť rizikovú skupinu, a to najmä pre imunodeficientný stav podmienený samotným ochorením ako aj onkologickou liečbou. Hoci incidencia infekcií spojených s centrálnym venóznym katétrom za posledných 10 rokov výrazne klesla, ich mortalita ostáva stále vysoká na úrovni 12–25 %. Kým incidencia CRBSI sa v populácii pacientov na jednotkách intenzívnej starostlivosti (intensive care unit – ICU) v Európe pohybuje okolo 3,0 epizódy/1 000 katétrových dní (ICU IQR 0,5–4,1), u onkologických pacientov je udávaná incidencia 1,1–7,5 epizódy/1 000 katétrových dní. Jedným z problémom pri hodnotení incidencie týchto infekcií ostáva nejednotnosť jednotlivých definícií ako aj zamieňanie pojmov určených pre klinický výskum s pojmami používanými v „dohľade nad infekciami“. Cieľ: Tento prehľadový článok okrem spomínaných definícií jednotlivých typov katétrových infekcií, patogenézy či rizikových faktorov sumarizuje epidemiologické údaje a poznatky z oblasti prevencie, diagnostiky a liečby za posledných 10 rokov. V roku 2011 vydalo Center for Disease Control (CDC) ve spolupráci s Prevention and Healthcare Infection Control Practices Advisory Committee (HICPAC) Odporučenia pre prevenciu intravaskulárnych katétrových infekcií zamerané na správne zavádzanie a starostlivosť o vaskulárne katétre. Pomocou uvedených postupov za posledných 10 rokov podarilo znížiť incidenčnú denzitu katétrových infekcii až o > 50 %. Nakoľko centrálne venózne katétre patria k bežnej súčasti onkologickej liečby, s problematikou katétrových infekcií by preto mal byť oboznámený každý onkológ a dodržiavanie uvedených stratégií spolu s dohľadom nad incidenciou CRBSI by malo patriť k štandardom v starostlivosti o onkologického pacienta. Záver: Dodržiavanie odporúčaných „evidence-based“ postupov zameraných na prevenciu katétrových infekcií a prísny dohľad nad ich výskytom v jednotlivých zdravotníckych zariadeniach je základným krokom k redukcii ich incidencie. Ich včlenenie do nemocničných smerníc v písomnej forme, spätná kontrola ich dodržiavania ako aj pravidelné hlásenie ich incidencie vyšším inštitúciám odporúčajú viaceré popredné zdravotnícke organizácie. Takýmto postupom sa v západných krajinách podarilo znížiť incidenciu pod 1 epizódu CRBSI/1 000 katétrových dní, čo by malo byť cieľom každého pracoviska využívajúceho centrálne venózne katétre starostlivosti o onkologického pacienta.
Background: Catheter-related bloodstream infections (CRBSI) represent a life-threatening complication. They are associated with high morbidity, mortality and healthcare costs. Cancer patients are at particular risk due to the nature of the disease and the therapy-associated immunodeficiency. Although the incidence of catheter-related infections decreased in the past decade, the mortality rate remains high at 12–25%. The incidence of CRBSI differs among patients. While the incidence in ICU patients across the Europe was reported to be 0.5–4.1 episodes per 1.000 catheter-days, the incidence in cancer patients was 1.1–7.5 episodes per 1.000 catheter-days. One of the factors negatively influencing the incidence is the lack of uniformity in CRBSI definitions, etiology and risk factors for surveillance purposes. Purpose: This preview not only presents the definitions of catheter-related infections and etiology and risk factors for developing CRBSI, it also it also summarizes epidemiologic data, diagnostic techniques, and the prevention and treatment strategies for CRBSI according to knowledge acquired over the last 10 years. Following the implementation of the prevention strategies guidelines for CRBSI published in 2011 by Center for Disease Control and Prevention and Healthcare Infection Control Practices Advisory Committee, the incidence of CRBSI has decreased by more than 50%. As central venous catheters are a common part of cancer treatment, every oncologist should be aware of the risk of catheter-related infections. The prevention and treatment guidelines with surveillance of CRBSI should be the gold standard in the care of cancer patients as well. Conclusion: Adherence to evidence-based guidelines for prevention of catheter-related infections and surveillance of CRBSI are the basic steps required to reduce the rate of CRBSI. Implementation of these strategies in hospital healthcare policy, particularly in written form, and control of adherence and reporting of the incidence rate to higher authorities are strongly recommended. When these interventions were successfully implemented, they reduced the incidence rate under 1 episode of CRBSI per 1.000 catheter-days in Western countries. Each healthcare facility using central venous catheters should be advised to implement these strategies, particular when treating cancer patients.

BACKGROUND: Hemodialysis (HD) in children with a concomitant ventriculoperitoneal shunt (VPS) is rare. Registry data suggest that peritoneal dialysis with a VPS is safe, but little is known about HD in the presence of a VPS. METHODS: We performed a 10-year survey to determine the prevalence of a VPS, complications and outcome in children with a VPS on HD in 15 dialysis units from the 13 countries participating in the European Pediatric Dialysis Working Group. RESULTS: Eleven cases of HD with a VPS were reported (prevalence 1.33 %; 328 patient-months) and compared with prospective Registry data. The median age at start of dialysis was 9.6 [inter-quartile range (IQR) 1.0-15.0] years and median HD vintage was 2.4 (IQR 1.7-3.0) years. Dialysis was performed through a central venous line (CVL) and through an arteriovenous fistula in six and five children, respectively. Three CVL infections occurred in two children, but these children did not develop VPS infections or meningitis. Symptoms of hemodynamic instability were reported in six (55 %) children at least once per week, with hypotension or hypertension occurring in four of these children and nausea, vomiting and headaches occurring in two; four other children reported less frequent symptoms. Seizures on dialysis occurred in two children, at a frequency of less than once per month, with one child also experiencing visual disturbances. During follow-up (median 4.0; IQR 0.38-7.63 years), three children remained on HD and eight had a functioning transplant. No patients were switched to PD. CONCLUSIONS: Hemodialysis in children with a VPS is safe, but associated with frequent symptoms of hemodynamic instability. No episodes of VPS infection or meningitis were seen among the children in the survey, not even in those with CVL sepsis.
