This article aims to present the differential diagnostics of benign and malignant eyelid tumours. The most common malignant eyelid tumour is basal cell carcinoma, followed by squamous cell carcinoma. The common signs of malignity are loss of lashes, ulceration, and infiltration of the lesion. Often the clinical appearance is various and therefore only a histological analysis gives the proper diagnosis. For most tumours, surgical resection is the gold standard of therapy. The reconstruction of the defects should be performed by an experienced oculoplastic surgeon. In malignant tumours that require large safety margins, the defect can be easily very large, and the reconstruction must then be performed with advanced ophthalmic plastic reconstruction techniques.
Dieser Artikel zielt darauf ab, die Differenzialdiagnostik von gutartigen und bösartigen Augenlidtumoren darzustellen. Der am häufigsten bösartige Augenlidtumor ist das Basalzellkarzinom, gefolgt vom Plattenepithelkarzinom. Die häufigsten Anzeichen von Malignität sind Verlust von Wimpern, Ulzeration und Infiltration der Läsion. Das klinische Erscheinungsbild ist vielfältig und daher nur ein histologisches. Die Analyse liefert die richtige Diagnose. Bei den meisten Tumoren ist die chirurgische Resektion der Goldstandard der Therapie. Die Rekonstruktion sollte von einem erfahrenen ophthalmoplastischen Chirurgen durchgeführt werden. Bei bösartigen Tumoren, die großen Sicherheitsabstand erfordern, kann der Defekt leicht sehr groß werden. Die Rekonstruktion muss dann mit fortgeschrittenen Techniken der plastischen Rekonstruktion durchgeführt werden.
Retrobulbar haematoma (RBH) is a rare complication that may affect vision after a trauma or a surgical procedure. The diagnosis must be made promptly, as only early surgical intervention can adequately prevent irreversible visual impairment. Because of the bony orbital walls, there is hardly any room for the increasing intraorbital volume due to the retrobulbar haemorrhage. This leads to an increase in intraorbital pressure and subsequently to compression of the optic nerve. Symptoms include disorders in ocular motility, ophthalmoplegia, diplopia, conjunctival chemosis, subconjunctival haemorrhage, proptosis, increased intraocular pressure, deterioration in visual acuity, decreased direct pupillary reflex, and a relative afferent pupillary defect. If the cause is traumatic or iatrogenic, prompt lateral canthotomy with cantholysis is the treatment of choice, and successfully lowers pressure in most cases. It can be performed in the emergency room by an ophthalmologist and may even be indicated without previous imaging. As the reconstruction of cantholysis is generally uncomplicated, we recommend performing the procedure when RBH is suspected. If canthotomy with cantholysis does not lead to adequate improvement, surgical orbital decompression must be performed. Supportive treatment should always include systemic steroids.
Das retrobulbäre Hämatom (RBH) ist eine seltene, aber potenziell visusbedrohende Komplikation nach einem Trauma oder auch nach einem chirurgischen Eingriff. Die Diagnose muss dabei zeitnah gestellt werden, da nur eine rasche chirurgische Intervention irreversible Visusminderungen adäquat verhindern kann. Da es aufgrund der knöchernen orbitalen Begrenzung kaum Raum für das zunehmende, intraorbitale Volumen durch die retrobulbäre Blutung gibt, kommt es zu einer Erhöhung des intraorbitalen Drucks und konsekutiv zu einer Kompression des N. opticus. Mögliche Symptome umfassen insbesondere Motilitätsstörungen bis zur Ophthalmoplegie, eine Diplopie, eine Bindehautchemose, subkonjunktivale Blutungen, einen Exophthalmus, einen erhöhten intraokulären Druck, eine Verschlechterung der Sehschärfe sowie eine reduzierte direkte Lichtreaktion und ein relatives afferentes Pupillendefizit. Wenn die Ursache traumatisch oder iatrogen bedingt ist, ist eine zeitnahe, laterale Kanthotomie mit Kantholyse das Mittel der Wahl und sorgt in den meisten Fällen für eine erfolgreiche Druckentlastung. Die Kanthotomie mit Kantholyse kann auch sehr gut in der Notaufnahme durch den Augenarzt durchgeführt werden und ist ggf. sogar ohne eine vorherige Bildgebung indiziert. Da die Rekonstruktion der Kantholyse meist unkompliziert möglich ist, empfehlen wir die Durchführung dieser Entlastung bereits, wenn klinisch der hochgradige Verdacht auf eine RBH gestellt wird. Sollte die Kanthotomie mit Kantholyse nicht für eine ausreichende Entlastung sorgen, muss eine chirurgische Orbitadekompression durchgeführt werden. Therapiebegleitend sollte immer eine pharmakologische Therapie mit Steroiden stattfinden.

Since the beginning of 2020, SARS-CoV-2, the pathogen of COVID-19, has led to a global pandemic that also affects ophthalmology. Ophthalmologists can be confronted at any time with potentially COVID-19 associated ocular symptoms or manifestations in patients and also become infected through close patient contact. Even without systemic infection, the ocular surface can come into direct contact with aerosols or liquids containing SARS-CoV-2 particles. A smear infection through hand-to-eye contact is also possible. A purely isolated ocular infection has not yet been shown. Rather, it seems that ocular complications occur in the context of a systemic infection. However, ocular symptoms can also be the first symptom of COVID-19. The most common ocular complication of COVID-19 is mild follicular conjunctivitis. Haemorrhagic conjunctivitis, dry eye disease, episcleritis, or retinal involvement can also occur less frequently. There are currently no evidence-based therapy recommendations for COVID-19 associated diseases of the ocular surface. Artificial tears might be helpful for symptom relief. There is no evidence for antiviral, antibiotic, or anti-inflammatory therapies, but these medications might be used in individual cases. Potential intraocular complications include retinal artery occlusions and haemorrhages, as well as cotton wool spots caused by complement-mediated thrombotic angiopathy. Neuro-ophthalmological complications including Miller-Fisher syndrome or infarct-related central blindness can also occur in very rare cases. Knowledge of potential transmission routes and personal protective equipment is just as essential for each ophthalmologist as a basic knowledge of potential ocular symptoms and complications.
SARS-CoV-2, der Erreger von COVID-19, führt seit Anfang 2020 zu einer weltweiten Pandemie, von der auch die Augenheilkunde betroffen ist. Dabei kann ein Ophthalmologe aktuell jederzeit mit potenziell COVID-19-assoziierten okulären Symptomen oder Manifestationen bei Patienten konfrontiert werden und sich aber auch selber durch engen Patientenkontakt anstecken. Auch ohne systemische Infektion kann die Augenoberfläche direkt mit Aerosolen und Flüssigkeiten, die SARS-CoV-2-Partikel enthalten, in Kontakt kommen. Auch eine Schmierinfektion durch Hand-zu-Augen-Kontakt ist möglich. Eine rein isolierte okuläre Infektion ist bisher nicht nachgewiesen. Vielmehr scheint es so, dass okuläre Komplikationen im Rahmen einer systemischen Infektion auftreten können, wobei okuläre Symptome auch Erstsymptom sein können. Die häufigste okuläre Komplikation einer COVID-19-Infektion ist die milde follikuläre Konjunktivitis. Seltener können auch hämorrhagische Konjunktivitiden, eine Keratoconjunctivitis sicca, eine Episkleritis sowie retinale Beteiligungen auftreten. Aktuell gibt es keinerlei evidenzbasierte Therapieempfehlungen für COVID-19-assoziierte Erkrankungen der Augenoberfläche. Tränenersatzmittel könnten zur Symptomlinderung hilfreich sein, eine Evidenz für antivirale, antibakterielle oder antientzündliche Therapien liegt nicht vor, sie können im Einzelfall aber angewendet werden. Zu den potenziellen intraokulären Komplikationen gehören Arterienverschlüsse und retinale Hämorrhagien sowie Cotton-Wool-Spots, verursacht durch eine komplementvermittelte thrombotische Angiopathie. Sehr selten können auch neuroophthalmologische Komplikationen wie das Miller-Fisher-Syndrom oder infarktbedingte zentrale Erblindungen auftreten. Das Wissen um potenzielle Übertragungswege und über die persönliche Schutzausrüstung ist für den Ophthalmologen genauso unerlässlich wie ein Grundwissen über potenzielle okuläre Symptome und Komplikationen.

The aim of this work is to draw attention to possible injuries of the eye and the facial skeleton caused by firefighting sport. There was a group of 9 patients presented who were treated from 2006 to 2015 in the Department of Ophthalmology at the University Hospital in Pilsen and diagnosed with severe eyeball contusion after being hit by a jet of water and/or a water pipe. Three cases are presented in detail.
Ziel dieser Arbeit ist es, auf mögliche Verletzungen des Auges und des Gesichtsschädels beim Feuerwehrsport aufmerksam zu machen. Vorgestellt wird eine Gruppe von 9 Patienten, die von 2006 bis 2015 in der Augenklinik des Universitätsklinikums in Pilsen mit der Diagnose Augapfelprellung behandelt wurden, nachdem sie von einem Wasserstrahl und/oder einem Strahlrohr getroffen wurden. Ausführlich vorgestellt werden dann 3 Fälle.

Changes in the biomechanical properties of the human cornea play an important role in the pathogenesis of corneal ectatic diseases. Many different pathological conditions in the cornea may reduce its biomechanical resistance. Corneal collagen cross-linking (CXL) has emerged as a promising technique to slow or even to stop the progression of ectasia. In this procedure, riboflavin (vitamin B2) is administered in conjunction with ultraviolet A light (UVA, 365 nm). This interaction causes the formation of reactive oxygen species, leading to additional covalent bonds between collagen molecules, with consequent biomechanical stiffening of the cornea. Although this method is not yet accepted as an evidence-based treatment of corneal ectasia, the results of prospective, randomised studies of CXL used in the treatment of this pathological entity show significant changes in the properties of corneal tissue. This procedure is currently the only aetiopathogenetic treatment of ectatic eyes that can delay or stop the process of cornea destabilisation, reducing the necessity for keratoplasty. Despite promising results, CXL is associated with issues that include long-term safety and duration of the stabilising effect.

Corneal cross-linking can halt the progression of keratoconus, but what is the best approach for treatment? There are a number of treatment options for keratoconus, but only corneal cross-linking (CXL) appears to halt the progression of the disease. To guarantee effective cross-linking, CXL treatment involves removal of the corneal epithelium prior to riboflavin application and ultraviolet light illumination - "epi-off" CXL. Several methods of "epi-on" (transepithelial) CXL have been proposed, such as keeping the corneal epithelium intact which should be less painful and help avoid other CXL-associated adverse events. The evidence so far is that epi-off CXL remains the most effective method of strengthening the cornea and slowing keratoconus progression - but transepithelial methods are gaining ground.

Classically, the pupillary pathway is considered as a simple reflex arc comprising retinal ganglion cells, midbrain interneurons, oculomotor nerve and short ciliary nerves. However, there are some specialties in the construction of the pupillary pathways that have to be kept in mind when dealing with diseases involving pupillary disorders. This may help to localise lesions. Additionally, studies in patients with lesions of the retrogeniculate pathways have shown that pupillary disorders are possible even with lesions not involving the classical reflex arc. The pupil is therefore not only controlled subcortically, some components are influenced by the visual cortex. The aim of this article is to clarify various findings and terms such as relative afferent pupillary defect and pupillary hemihypokinesia.

BACKGROUND: We evaluated visual outcomes after vitrectomy for diabetic cystoid macular edema and factors possibly influencing final visual acuity. MATERIALS AND METHODS: Studied prospectively, pars plana vitrectomy was performed on 72 consecutive eyes of 61 patients with diabetic cystoid macular edema not responsive to laser photocoagulation therapy. Vitreomacular traction was observed on biomicroscopy, B-scan ultrasonography, and optical coherence tomography in 21 eyes, 15 eyes had a complete posterior vitreous detachment (PVD) and 36 eyes had partial peripheral PVD. Vitrectomy with releasing vitreomacular tangential and axial tractional forces was performed. All patients were followed up for at least 6 months. RESULTS: The anatomical results were satisfactory in 69 of 72 eyes (96 %), the final visual acuity improved by 2 or more lines in 45 of 72 eyes (63 %), remained unchanged in 22 of 72 eyes (31 %), and deteriorated after surgery in 5 of 72 eyes (7 %), due to residual cystoid macular edema and massive macular hard exudates. The best results were obtained in edema with tractional predominance and in eyes with a duration of cystoid macular edema shorter than 6 months. CONCLUSIONS: Vitrectomy for diabetic cystoid macular edema is an effective procedure for reducing the edema and improving visual acuity. Surgery was beneficial not only in eyes with vitreomacular traction but also in eyes with complete PVD. Visual improvement after vitrectomy is related to duration of edema, therefore the timing of the surgery is an important prognostic factor.

BACKGROUND: Angiolymphoid hyperplasia with eosinophilia (ALHE) and Kimura's disease are two rarely occurring disorders very similar to each other; however, they are individual nosological entities. For a long time they were considered as a single disease due to the frequency of similar characteristics. The majority of authors have recently considered these diseases as two individual entities with some reciprocal specifications, both clinical as well as morphological. PATIENT: We report on the case of a 76-year old male white European who had suffered for more than six years from subcutaneous tumour formations in different parts of the body. Eyelid oedema on the right side with palpable resistance under the upper temporal edge of the orbit initially occurred five years after the first symptoms of the disease. A similar finding occurred on the left side after eight months. The tumours on both sides were surgically removed and sent for histological analysis. A unilateral recurrence of the finding appeared after one year, followed by surgical intervention and histological examination. RESULTS: Tumour infiltrate in the first two orbita excisions was topically related to lacrimal gland structures; these structures were not found in the third excision. The case was histologically diagnosed as ALHE despite some similar characteristics with Kimura's disease. The diagnosis of ALHE was histologically supported, especially by the absence of lymphatic follicle formations and fibrotisation in the infiltrate, and the identification of the appearance of epitheloid up to histiocytoid of proliferating endothelium, which forms small lumens. The diagnosis of ALHE was additionally supported by the fact that the incidence of Kimura's disease in white Europeans is very rare. CONCLUSIONS: According to our findings, both entities overlap one another, even in some characteristics considered to be distinguishing. The question arises, therefore, whether the strict separation of these nosological entities can indeed be determined with one hundred percent accuracy.
