Studie analyzuje příčiny a důsledky současné nepříznivé situace v dostupnosti geriatrických ambulancí v ČR, zabývá se možnostmi rozšíření jejich sítě a zlepšení organizace systému geriatrických služeb. Mezi 230 geriatry registrovanými v České lékařské komoře bylo provedeno dotazníkové šetření. Ambulantní praxi vykonává pouze 48 geriatrů, většina navíc pouze v části svého pracovního úvazku. Velmi kritické bylo vyjádření geriatrů zejména k připravenosti ČR na demografické stárnutí, k dostupnosti ambulantní geriatrické péče, k návaznosti na další služby a informovanosti laické i odborné veřejnosti. Zdravotní pojišťovny nemají v současné době zájem o uzavírání smluv s novými poskytovateli ambulantní geriatrické péče. Kromě problematického financování se jako další významné překážky rozvoje ambulantní geriatrie jeví přetrvávající nepropojenost zdravotních a sociálních služeb a nízká úroveň poptávky zapříčiněná zejména nedostatkem relevantních informací o možnostech a přínosech geriatrické péče. Budoucnost ambulantní geriatrie je úzce spojena s rozvojem komunitní péče o seniory a musí být založena především na spolupráci s praktickými lékaři a ostatními odborníky v oblasti zdravotních a sociálních služeb.
The study analyses the causes and consequences of the current unfavourable situation in the availability of geriatric outpatient services in the Czech Republic, dealing also with the possibilities of extending their network and improving the organization of the geriatric care system. A survey was carried out among the 230 geriatricians registered in the Czech Medical Chamber. Only 48 geriatricians provide outpatient care services and, moreover, most of them only as a part of their week workload. The geriatricians’ opinion was highly critical above all concerning the readiness of the Czech Republic for the demographic aging process, the availability of outpatient geriatric care, follow-up of other services, and awareness of general public and also experts. Health insurance companies are currently not interested in contracting new providers of outpatient geriatric care. In addition to the problematic funding, persisting separation of health and social services and a low level of demand mainly due to the lack of relevant information on the possibilities and benefits of the geriatric care seem to be other major obstacles of the development of outpatient geriatrics. The future of outpatient geriatrics is closely connected with the development of community care for elderly, and must be based mainly on cooperation with general practitioners and other professionals in the field of health and social services.
Článek informuje o možnostech uplatnění principů moderního marketingu při řízení lékařské posudkové služby sociálního zabezpečení. Posudková služba se nachází desetiletí v obtížné situaci, kdy na jedné straně se zvyšují nároky na kvalitu a objem posudkové činnosti, na druhé straně však dlouhodobě přetrvávají personální problémy v důsledku neatraktivity lékařské posudkové činnosti, stárnutí lékařů pracujících v lékařské posudkové službě a nedostatečného počtu lékařů s odbornou způsobilostí v oboru posudkového lékařství. Posudková služba stojí na okraji zájmu odborné i laické veřejnosti, ačkoliv důsledky práce několika set posudkových lékařů vykazují dopad do státního rozpočtu cca 90 mld. Kč ročně. Kumulace všech zmíněných vlivů předurčila systém LPS k nastolení účinných opatření v podobě marketingového mixu 4C, nemají-li pokračovat další problémy. Výsledkem by měla být samostatná, nezávislá, finančně zabezpečená lékařská posudková služba s vysoce kvalifikovanými odborníky a s významnými prvky organizačně procesní stability, postavená na základech interního a externího marketingu a komunikace.
The article presents the means of implementing principles of modern marketing into the management of medical assessment service. For several decades, medical assessment service has been in a difficult position, with increasing requirements regarding the quality and amount of assessment service on the one hand, and long-term staffing problems on the other. These problems have been caused by the unattractiveness of medical assessment service, aging of current medical assessment physicians, and an overall lack of medical assessment specialists. Although the outcomes of the work of a few hundred medical assessment physicians represent an annual burden for the state budget of approximately 90 billion CZK, medical assessment service is of marginal interest to both professionals and general public. The cumulation of the above-mentioned aspects will require the medical assessment service to implement effective measures embodied by the 4C marketing medley, in order to prevent further complications. The result of this process should be an independent and financially secure medical assessment service, staffed by highly qualified professionals, operating on clearly defined and stable organisational processes, and based on principles of internal and external marketing and communication.
