Práce popisuje fyziologický dechový stereotyp a uvádí metody a techniky pro jeho vyšetření, které je možné využít v běžné klinické praxi. Je zde hodnocen dechový stereotyp chronických plicních onemocnění s ohledem na přítomnost obstrukční nebo restrikční ventilační poruchy a jsou zde shrnuty dostupné rehabilitační postupy pro úpravu změněné dechové mechaniky.
The article reports on normal mechanics of breathing as well as methods and techniques that can be used for its evaluation in common clinical practice. Additionally, it focuses on breathing mechanics in chronic respiratory diseases with respect to the presence of either obstructive or restrictive ventilatory defects. Finally, it suggests rehabilitation approaches that can be used in the treatment.
Rehabilitační léčba se řadí mezi doporučované léčebné postupy u pacientů s chronickým respiračním postižením. Cílem práce bylo vyhodnotit efekt rehabilitační léčby na zdravotní stav a kvalitu života pacientů s plicní formou sarkoidózy. Sedmnáct pacientů s plicní sarkoidózou stadia II (FVC 99 %, DLCO 84 %) absolvovalo 12týdenní rehabilitační léčbu. Ze sledovaných parametrů došlo k významnému zlepšení síly nádechových (33 %, p<0,01) i výdechových (16 %, p<0,01) svalů, rozvíjení hrudníku (29 %, p<0,01), tolerance zátěže (6 %, p<0,01) a kvality života (9 %, p<0,01), zatímco plicní funkce zůstaly nezměněny. Rehabilitační léčba má pozitivní efekt na zdravotní stav a kvalitu života pacientů s mírnou ventilační poruchou při plicní formě sarkoidózy a je vhodnou komponentou komplexní léčebné péče o pacienty.
Rehabilitation is one of treatment approaches recommended in patients with chronic respiratory diseases. The aim of this study was to evaluate the effect of rehabilitation treatment on the health status and health-related quality of life in patients with pulmonary sarcoidosis. Seventeen patients with stage II pulmonary sarcoidosis (FVC 99 %, DLCO 84 %) attended a 12-week rehabilitation program. The assessments showed significant improvements in maximum inspiratory (33 %, p < 0.01) and expiratory (16 %, p < 0.01) pressure, chest expansion (29 %, p < 0.01), six-minute walk test (6 %, p < 0.01) and health-related quality of life (9 %, p < 0.01), whilst lung function tests showed no significant changes. In conclusion, rehabilitation treatment has a beneficial effect on the health status and health-related quality of life in patients with mild pulmonary sarcoidosis and it represents a relevant component of comprehensive care for patients with this disease.

Program plicní rehabilitace tvoří komplexní léčebnou péči o nemocné s dechovými obtížemi. Zahrnuje mezioborovou spolupráci lékařů, fyzioterapeutů, psychologů a dalších zdravotnických pracovníků. Komplexní léčba zahrnující jednotlivé techniky plicní rehabilitace umožňuje pacientovi se aktivně podílet na své léčbě a rychleji se přiblížit úrovni kvality života, které dosahoval před započetím nemoci.
A pulmonary rehabflitation programme is an important part of a comprehensive treatment programme for patients with breathing disorders. It includes multidisciplinary cooperation between physicians, physioterapists, psychologists and other health care workers. The aim of this progrcimme is to provide the patient with opportunity to participate in his or her treatment and to approach the previous level of quality of life as fast as possible
