AIM: To compare the 12-month outcomes of visual performance and patient satisfaction of a higher-order aspheric monofocal intraocular lens (IOL) and a conventional monofocal IOL. METHODS: Prospective, randomized, comparative, double-blinded study including 22 patients who underwent bilateral cataract surgery with implantation of the Tecnis Eyhance ICB00 IOL in one eye (ICB00 group) and the Tecnis ZCB00 IOL in the fellow eye (ZCB00 group). Uncorrected distance (UDVA), corrected distance (CDVA), uncorrected intermediate (UIVA), distance-corrected intermediate (DCIVA), uncorrected near (UNVA), and distance-corrected near visual acuities (DCNVA) were evaluated during a 12-month follow-up. Contrast sensitivity, defocus curves, and reading text size were also evaluated. Patient satisfaction was assessed with a questionnaire at the 6-month postoperative visit. RESULTS: Mean postoperative logMAR UDVA was 0.01±0.12 and -0.02±0.10 in ICB00 and ZCB00 groups, respectively (P=0.37). Mean logMAR UIVA was 0.32±0.19 and 0.45±0.16 in ICB00 and ZCB00 groups, respectively (P=0.010). Differences between groups in UNVA did not reach statistical significance (P=0.16). The intermediate reading acuity at 66 cm (P=0.02) and 80 cm (P=0.04) was significantly better in the ICB00 group. Postoperative contrast sensitivity results did not differ significantly between groups (P>0.05). Patients reported high overall satisfaction, with 62% of patients using spectacles for reading in everyday life. CONCLUSION: The eyes of patients implanted with the enhanced monofocal IOL evaluated have significantly better visual acuity for intermediate distances with the same contrast sensitivity as a conventional monofocal IOL.

Cíl: Porovnání objektivní kvality vidění u pacientů podstupujících fakoemulzifikaci s oboustrannou implantací segmentované multifokální nitrooční čočky (SMIOL). Metody: Do retro-prospektivní studie bylo zařazeno 110 očí (55 pacientů), kteří podstoupili operaci katarakty s bilaterální implantací SMIOL. Pacienti byli rozděleni dle typu implantované nitrooční čočky do skupiny 1 (SBL-2, 62 očí) a skupiny 2 (SBL-3, 48 očí). Pooperačně byla měřena a hodnocena monokulární i binokulární nekorigovaná zraková ostrost do dálky (UCDVA), korigovaná zraková ostrost do dálky (BCDVA), nekorigovaná zraková ostrost na střední vzdálenost (UCIVA, při 66 cm), nekorigovaná zraková ostrost na blízko (UCNVA, při 40 cm), korigovaná zraková ostrost na blízko (BCNVA) a defokusační křivka. Sledovací období činilo 6 měsíců. Výsledky: Průměrná hodnota UCDVA ve skupině 1 (SBL-2) byla 0,10 ±0,15 LogMAR monokulárně, 0,01 ±0,10 LogMAR binokulárně a ve skupině 2 (SBL-3) byla 0,02 ±0,11 LogMAR monokulárně a -0,07 ±0,09 LogMAR binokulárně. Defokusační křivky ukázaly, že skupina SBL-3 dosahovala při vergenci -1,50 D odpovídající 66 cm (střední vzdálenost) lepších hodnot než čočka SBL-2, a to průměrně 0,03 ±0,11 LogMAR, zatímco SBL-2 0,12 ±0,14 LogMAR (p = 0,01). Vergenci -2,50 D charakterizující vidění na blízko (40 cm) dosáhla čočka SBL-2 v naší studii 0,33 ±0,15 LogMAR a čočka SBL-3 0,00 ±0,11 LogMAR (p = 0,00). Závěr: Obě SMIOL poskytovaly 6 měsíců po operaci velmi dobré vidění na všechny testované vzdálenosti. Čočka SBL-2 vykazovala lepší výsledky v UCIVA, zatímco čočka SBL-3 vynikala v UCDVA a UCNVA.
Purpose: To compare objective quality of vision in patients undergoing phacoemulsification with implantation of a bilateral segmented multifocal intraocular lens (SMIOL). Methods: A retro-prospective study included 110 eyes of 55 patients who underwent cataract surgery with bilateral SMIOL implantation. Patients were divided according to the type of implanted intraocular lens into group 1 (SBL-2, 62 eyes) and group 2 (SBL-3, 48 eyes). Postoperatively, monocular and binocular uncorrected distance visual acuity (UCDVA), corrected distance visual acuity (BCDVA), uncorrected intermediate visual acuity (UCIVA, at 66 cm), uncorrected near visual acuity (UCNVA, at 40 cm), corrected near visual acuity (BCNVA) and defocus curve were measured and evaluated. The follow-up period was 6 months. Results: Mean UCDVA in group 1 (SBL-2) was 0.010 ±0.15 LogMAR monocularly, 0.01 ±0.10 LogMAR binocularly, and in group 2 (SBL-3) was 0.02 ±0.11 LogMAR monocularly and -0.07 ±0.09 LogMAR binocularly. Binocular defocus curves showed that the SBL-3 group performed better than the SBL-2 lens at a vergence of -1.50 D corresponding to 66 cm (center distance), averaging 0.03 ±0.11 LogMAR, while the SBL-2 group averaged 0.12 ±0.14 LogMAR (p = 0.01). The -2.50 D vergence characterizing near vision (40 cm) was achieved by the SBL-2 lens in our study at 0.33 ±0.15 LogMAR and by the SBL-3 lens at 0.00 ±0.11 LogMAR (p = 0.00). Conclusion: Both SMIOLs provided very good vision at all tested distances 6 months postoperatively. The SBL-2 lens performed better in UCIVA, while the SBL-3 lens excelled in UCDVA and UCNVA.
